    American Girl Jewelry Recalled for Lead Poisoning Hazard

    Children's jewelry sold at American Girl stores is being recalled because it contains high levels of lead. Lead is toxic if ingested by young children and can cause adverse health effects.

    The recall includes American Girl necklaces, bracelets, earrings and hair accessories for girls. The recall does not include jewelry for American Girl dolls. The jewelry's packaging features a red and white cardboard backing with "American Girl" and "Made in China" written on the front.

    The jewelry was sold at Only American Girl Place in Chicago and New York, and the American Girl Outlet in Oshkosh, Wis., from May 1999 through February 2006 for between $8 and $12. American Girl jewelry sold through its catalogue or Web site is not included in the recall.

    Consumers should immediately take recalled jewelry away from children and return the items for a full refund (including applicable tax and return postage costs) to:

    American Girl Jewelry Recalled for Lead Poisoning Hazard...

    Jordache Recalls Hooded Sweatshirts

    March 16, 2006
    Jordache Ltd. is recaling about 30,000 youth hooded sweatshirts because the drawstring presents a strangulation hazard.

    The recall is for "U.S. Polo Association" sweatshirts.

    The garments have a drawstring through the hood, posing a strangulation hazard to children. In February 1996, CPSC issued guidelines to help prevent children from strangling or getting entangled on the neck and waist by drawstrings in upper garments, such as jackets and sweatshirts.

    The recalled youth hooded fleece garments have drawstrings. They were sold in a variety of colors and have "U.S. Polo Assn." written on them. A sewn-in tag also reads, "U.S. Polo Assn". The style number for these garments is 12-5169.

    The sweatshirts were sold at JC Penney stores nationwide from September 2005 through December 2005 for about $40.

    Consumers should remove or cut the drawstrings to eliminate the hazard, or return the garment to the store where purchased for a refund.

    Olympus Expands Infinity Film Camera Recall

    Olympus is adding two camera models to a March recall intended to address a defect in the flash circuit. The defect can cause the camera to smoke and overheat when it is turned on.

    The additional models bring the total recall to about 1.2 million cameras.

    Olympus Imaging America Inc. has received 21 reports of camera or flash circuitry overheating in the U.S. with no reports of injuries.

    The original recall announced March 14, 2006 included the Infinity Twin, AF-1 Twin, Infinity Zoom 200 series, AZ 200 series, and Quantary Infinity Zoom 222 Olympus-brand 35mm film cameras. Olympus has now added the Infinity Zoom 76 and the Promaster Infinity Twin model cameras to the recall. The model name is printed on the face of the camera. These cameras have a built-in flash.

    The cameras were sold at department, electronic and camera stores, and mail-order retailers nationwide from January 1989 through December 1995 for between $220 and $365.

    Consumers should stop using these cameras and contact Olympus for information on receiving a free repair or, if necessary, replacement.

      Syntace USA Recalls Bicycle Handlebar Stems

      Syntace USA is recalling about 2,300 F99 (Force 99) Bicycle Handlebar Stems. The stems can crack or break under normal conditions, causing the rider to fall and suffer serious injury.

      The recalled bicycle handlebar stem is black. Stems with production codes 01 03 through 45 04, and stems with no production code are included in this recall. The production code is the bottom set of numbers located on the end of the stem.

      The stems were sold by bicycle dealers, mail order magazines and on the Internet from May 2003 through July 2005 for about $90.

      Consumers should stop using bicycles equipped with the recalled handlebar stem immediately and contact Syntace for instructions to receive a free replacement stem and installation.

      Delta Water Heaters Recalled

      September 7, 2006
      Delta Combination Water Heaters are being recalled. The burner plate and flue hood seal on the water heaters can fail due to an improper seal causing a leak of flue gases and carbon monoxide (CO). This poses the risk of CO poisoning to consumers inside of the house.

      Triangle Tube has received one report of a flue leak with the recalled Combination Water Heater during installation. No injuries have been reported.

      The recalled Delta Performance or Delta Performance Plus Series Combination Water Heaters operate on natural or propane gas. The water heaters are white and rectangular. Triangle Tube is written on the silver-colored control panel. Delta is printed on the front of the water heater. The burner is located under a white or gray cover that protrudes outward at the bottom front of the heater. The models and serial numbers included in this recall are listed below. The model and serial numbers are located on the rating plate on either the front or the right side of the water heater.

      The units were sold by plumbing and heating wholesale companies to independent plumbing and heating contractors from August 2001 to January 2006 for about $3,500.

      Decorative snaps and metal clips manufactured by Provo Craft of Spanish Fork, Utah, are being recalled. They contain high levels of lead, which is toxic if ingested by young children and can cause adverse health effects.

      The recall involves the "Rob and Bob Studio Clip-Ease" metal clips with model numbers 28-1084, 28-1085 and 28-1086 and the "Rob and Bob Studio Snap-Ease" snaps with model number 28-1080. The product name and model number can be found on the packaging.

      The items were sold at Scrapbook stores and craft retailers nationwide from August 2005 through October 2006 for about $6.

      Consumers should stop using these products immediately and contact Provo Craft for instructions on how to obtain a full refund.

      First Years Liquid-Filled Teethers

      The First Years is voluntarily recalling liquid filled teethers due to possible bacterial contamination, the U.S. Food and Drug Administration announced.

      The liquid inside the teethers may contain pseudomonas aeruginosa and pseudomonas putida which can cause serious illness in children if the teether is punctured and the liquid from the teether is ingested.

      No illnesses have been reported to date in connection with this problem.

      The teethers are sold nationwide including major retailers, grocery, drug and specialty stores from July 2005 to January 2006 for $2.99 to $3.99.

      DaimlerChrysler Recalls Chrysler & Dodge Models for Faulty Wiper Motor

      March 20, 2006
      DaimlerChrysler Group is recalling 268,800 vehicles from the 2005-2006 model years to replace a faulty front windshield wiper motor, according to the company.

      The recall includes the Dodge Durango, Dodge Caravan and Grand Caravan, and Chrysler Town & Country. The wiper motor armature shaft on the vehicles can break resulting in a loss of wiper function.

      The company has notified the National Highway Traffic Safety Administration of the voluntary recall. Owners will be notified when parts are available.

      No accidents or injuries have been reported related to the problem, according to Chrysler.

      York Weightlifting Bars Recalled

      York Barbell Co. is recalling 3,300 sets of Olympic weightlifting bars. Due to a defect in the sleeve mechanism, the bar can break under the pressure of significant weight, posing a hazard to weight lifters and bystands.

      York Barbell has received two reports of bars breaking under the pressure of significant weight. No injuries have been reported.

      This recall involves all 7' Int'l Hard Chrome 1000 lb. and 1500 lb. test Olympic bars with model number 32012 and 32013, 7' Int'l Black Oxide 1000 lb. and 1500 lb. test Olympic bars with model number 32020 and 32021, 7' International Men's Olympic Needle-bearing bars with model number 32010 and 6 ' International Women's Olympic Needle-bearing bars with model number 32011. The bars have split sleeves and YORK is printed on the sleeve assemblies of each bar.

      The bars were sold at fitness wholesalers and retailers nationwide from November 2005 through July 2006 for between $200 and $500.

      Consumers should stop using the recalled bars immediately and return them to York Barbell for a full refund.

      NHTSA Investigates Chrysler Pacifica

      June 28, 2006
      The Chrysler Pacifica is the focus of a National Highway Traffic Safety Administration investigation because the engines might stall in some models.

      The probe covers approximately 147,000 vehicles and began in February after federal regulators received 60 complaints.

      Both computer software and defective fuel pumps are thought to be the likely cause of the stalling. Software is suspected to be the cause of stalling in 131,000 2005 and 2006 models,

      Defective fuel pumps are thought to be the cause in 17,000 2005 models, according to NHTSA.

      Road Rage Stunt Machine Toy Trucks Sold by QVC Recalled

      March 29, 2006
      Thunder Spin R.C. Road Rage Stunt Machine radio-controled toy trucks sold by QVC are being recalled. Electrical circuits within the truck can overheat causing the toy to overheat, posing a risk of fire.

      There have been 14 reports of overheating, smoking, melting and fire, including two reports of minor property damage to a countertop and electrical receptacle. No reports of injuries have been received.

      The Thunder Spin R.C. Road Rage Stunt Machine is 10 inches by 11 inches and was sold in blue or green. The radio-controlled Stunt Machine comes with one charger, one 9.6 V rechargeable battery pack, and one 9 volt battery. The model number, T23624, is written only on the product's packaging, located above the Universal Product Code. The Stunt Machine comes with two orange flame decals on each side of the vehicle, one of which includes Stunt Machine in orange block lettering. A Thunder Spin logo can be found on the bottom of the vehicle.

      The trucks were sold by QVC Inc. nationwide from November 2005 through December 2005 for between $40 and $44.

      Consumers should stop using the product immediately. QVC will send a letter to all purchasers of the product notifying them of the product's recall and instructing them to return the battery pack and battery charger to obtain a full refund.

      Pilates Balls Recalled

      January 10, 2006
      About 34,600 pilates balls are being recalled. The plastic black clips at the end of the rubber tubing can come apart when in use. In addition, the grommet used to hold the black rubber tubing could separate from the nylon webbing.

      Consumers using the pilates balls could fall and be hit by pieces that separate.

      The manufacturer has received four reports of minor bodily injuries.

      The inflatable pilates balls are blue with black handles. They are about 26-inches in diameter when inflated. They are sold with a blue air pump.

      The balls were sold by catalogs nationwide from August 2005 through September 1, 2005 for about $9.

      Consumers should immediately stop using these pilates balls and contact the firm to obtain instructions on how to receive free replacement handles. All consumers who purchased these Pilates balls were notified directly about the recall.

      Pilates Balls Recalled...

      Mosquito Magnet Cleaning Tool

      January 11, 2006
      American Biophysics Corp. is recalling about 335,000 "L"-Shaped Quick-Clear Valve Adapters used with Mosquito Magnet Insect Traps.

      The maintenance accessory is an "L"-shaped, Quick-Clear Valve Adapter that can leak or break apart during use, possibly striking and injuring nearby persons or releasing pressurized carbon dioxide (CO2) that can freeze exposed skin.

      American Biophysics has received 11 reports of persons struck by broken adapters and three reports of minor skin irritations caused by the CO2 gas.

      The recall involves a maintenance accessory used to clear obstructions in the fuel delivery system of certain Mosquito Magnet insect traps. The Quick-Clear Valve Adapter is a black and gold, "L"-shaped, brass and plastic device.

      The adapter was included as a maintenance accessory to clean fuel lines in newly manufactured and retrofitted Mosquito Magnet insect traps sold only under the following Mosquito Magnet model names: Defender, Liberty, Freedom and Commercial Pro. Only "L"-shaped adapters are affected by this recall. Mosquito Magnet products made since November 2003 contain only straight adapters that do not need to be replaced. Adapters for Mosquito Magnet traps that use rechargeable batteries are not affected by this recall.

      The units were sold at home centers, hardware stores, and industrial suppliers nationwide, as well as online and catalog retailers, provided the "L"-shaped Quick-Clear Valve Adapter as an accessory with Mosquito Magnet insect trap products manufactured from October 2002 through November 2003. The adapters were sold separately for about $8.

      Consumers should not use the recalled "L"-shaped Quick-Clear Adapter on the Mosquito Magnet insect trap. Contact the firm to receive a free replacement adapter.

      Nordstrom Egg-Shaped Candle Holders Recalled

      Nordstom is recalling about 5,300 egg-shaped candle holders. The candle holder is constructed of plastic and could ignite if exposed to flame, posing a fire hazard to consumers.

      Nordstrom has received one report of an incident involving the holder igniting. Minimal smoke damage occurred. No injuries have been reported.

      The recalled product is an egg-shaped tea light candle holder sold under style numbers 2C0755A, 2C0755B, and 2C0755C. The tea light holder came in the colors sky blue, pool blue and lavender. Both the style number and the color can be found on a sticker at the bottom of the candle holder.

      The candle holders were sold at Nordstrom stores nationwide from February 2005 through July 2005 for between $3 and $6.

      Consumers should stop using these candle holders immediately and return them to a Nordstrom store for a full refund.

      Pottery Barn is recalling about 68,000 gourd candles. The candles can have excessive flame height and could fail to self-extinguish when burning down to the bottom of the candle, posing fire and burn hazards to consumers.

      The recall involves five styles of individual gourd-shaped candles and a set of eight mini-gourd candles. The style number for the candles can be found on the price ticket, receipt or shipping invoice.

      Shimano Recalls Quick-Release Bike Device

      Shimano Inc. is recalling about 8,500 of its quick-release devices for bicycles. The quick-release skewers can unexpectedly fail or break when locked in position. When this happens the rider could lose control and fall.

      Shimano has received one report of the quick release breaking. No injuries have been reported.

      This recall involves quick releases supplied after November 1, 2005 with Shimano front hubs and front wheels on the following road racing and MTB bicycles. Model numbers included in the recall are: HB-5501, HB-5600, HB-6600, HB-7800, HB-HF-08, HB-M756, HB-M760, HB-M760, HB-M960, HB-M965, WH-7801, WH-7801C, WH-7801C50, WH-7801SL, WH-R600, WH-M965, DH-2N71, DH-3N71 and DH-3D71. Only quick releases with silver skewers and without a round sticker on the back of the quick release lever are involved in this recall.

      The devices were sold at bicycle specialty stores and dealers nationwide from November 2005 through March 2006 for between $13 and $44.

      Consumers should stop riding immediately and take the quick release device into their local bike dealer or retailer for a free inspection and repair.

      Shimano Recalls Quick-Release Bike Device...

      Four Seasons Sunroom Glass Recalled

      February 2, 2006
      Four Season Solar Products LLC is recalling the glass used in about 6,000 sunrooms. Sunlight reflecting off of certain sunroom roof glass and skylights onto adjacent cedar shingles or cedar shakes could pose a fire hazard.

      Cardinal IG Company of Eden Prairie, Minn. manufactured the reflective glass units. Four Seasons Solar Products, LLC, of Holbrook, N.Y., through its dealers and franchisees, sold and constructed sunrooms that used these glass units.

      Cardinal IG and Four Seasons are aware of four fires that could be attributed to this scenario. There are no reported injuries. The damage ranged from minor damage to shingles and underlying sheathing to incidents that caused some structural damage to roofs and walls.

      The repair program involves only the roof glass in sunrooms (including skylights installed as part of the sunroom) that reflect sunlight onto nearby cedar shingles or cedar shakes. It involves reflective Code 77 roof insulating glass units. It includes new sunrooms sold from 1996 through 2002, but possibly constructed later, and those sunrooms, regardless of when built, that had roof glass or skylights replaced with Code 77 glass from 1996 to 2005.

      The glass was sold through franchisees and Four Seasons retail stores in the United States and Puerto Rico from January 1996 through October 2005.

      Owners of Four Seasons Sunrooms described above should call the toll free number to determine if their units are affected and if so, to obtain a free repair. Cardinal IG and Four Seasons working together will repair the roof glass through installation of a capillary tube. The repair can be completed from the exterior of the sunroom except for certain types of skylights. There is no need to replace the glass.

      Liberty Mountain Recalls Climbing Harnesses

      December 12, 2006
      Liberty Mountain is recalling its Edelweiss Challenge Climbing Harnesses. The buckles on the harness' leg loops could fail, posing a fall hazard for climbers.

      Liberty Mountain has received five reports of the leg buckle failing. No injuries have been reported.

      The sit harnesses are worn around climber's waist and legs and attach to climbing ropes to protect climbers from falling. The harnesses have three buckles including one on the waist belt and one on each leg loop. "Edelweiss" is printed on the waist belt. Only harnesses using the para buckle system are included in this recall. Para buckle harnesses have a single bar installed in the center of each of the three buckles.

      The harnesses were sold by climbing equipment retailers, Liberty Mountain's catalog, and by various Web retailers from January 2005 through September 2006 for about $47.

      Consumers should immediately stop using the recalled harnesses and contact Liberty Mountain for instructions on returning the harnesses including a return authorization number.       Dollar Tree Stores Recall Counterfeit Extension Cords

      Dollar Tree Stores is recalling about 600,000 counterfeit extension cords because of a shock hazard. The counterfeit extension cords could have undersized wire and substandard insulation, which can cause overheating, resulting in a possible shock hazard.

      The recalled cords are green, measure 6 feet long, are intended for indoors and have a three-outlet extension. Attached to the cord is a silver counterfeit UL holographic label marked "09/01 E157848 UL LISTED CORD SET BW-3045 13A 125V 1625W".

      The cords were sold at Dollar Tree Stores, Dollar Bill$, and Dollar Express nationwide from November 2005 through May 2006 for $1.

      Consumers should stop using the recalled extension cords immediately and return them to any Dollar Tree store for a refund.
