    Bombardier Ski-Doo Recall #2

    October 1, 2004
    Bombardier is recalling several Ski-Doo models because the starter ring gear may crack, causing fragmentation and debris, which may act as a projectile at high speeds. A projectile could cause serious injury or death.

    The affected models are the Ski-Doo 2004 MX Z (E) 800 HO and GSX LTD (E) 800 HO snowmobiles.

    BRP has received one report of debris from the ring gear, causing a minor injury to the drivers hand.

    BRP snowmobiles come in black, blue, gray, white, and yellow. Recalled models are listed below.

    The units were sold at Ski-Doo dealers nationwide between August 2003 and August 2004 for about $8,500 to $9,800.

    Owners of affected units will be notified directly. Consumers should contact an authorized Ski-Doo dealer to schedule an appointment for repair, free of charge.

    Bombardier Ski-Doo Recall #2...

    Honda Recalls Accord, Prelude, Acura Models

    February 2, 2005
    Honda has issued a voluntary recall of nearly half a million automatic transmission Honda and Acura models to repair ignition switch interlocks.

    Some interlock levers may be improperly dimensioned, the company said, possibly allowing a driver to switch the ignition off and remove the key without putting the shift lever in the "Park" position. No injuries have been reported.

    The recall includes some 1999 to 2002 Honda Accord models and all 1997 to 2001 Honda Prelude models. Some 1999 and 2000 Acura TL sedans are also included in the recall.

    Details regarding mailing dates and affected vehicles will be available on the Honda and Acura Web sites on February 4, 2005, the company said.

    Menards Southern Breezes Carpeting

    March 9, 2005
    Shaw Industries is recalling about 5,000 square yards of its Southern Breezes wall-to-wall carpet because of a flammability hazard.

    Due to a manufacturing error, an uneven application of a latex pre-coat treatment containing a flame retardant was applied to the carpet backing. As a result, the carpet does not meet flammability standards as required under the federal Flammable Fabrics Act. The carpet could ignite, posing a serious risk of burn injuries to consumers.

    The recalled wall-to-wall carpet was manufactured by Shaw Industries exclusively for sale by Menard Inc. in its Menards stores. The recalled carpet was sold under the Menard style number 75123 and style name Southern Breezes in the colors Toast, Midnight and Clove.

    The carpeting was sold at certain Menards stores in Illinois, Indiana, Iowa, Michigan, Minnesota, Nebraska, North Dakota, South Dakota and Wisconsin from April 2004 through June 1, 2004 for $0.88 per square foot.

    Consumers with recalled carpet should contact Shaw Industries to arrange for free replacement carpeting and installation. The list of stores where the recalled carpeting was sold is attached.

      Robertshaw Controls Recalls TS-11 Thermal Safety Control Gas Valves

      November 1, 2004
      Robertshaw Controls is recalling its TS-11 thermal safety xontrol gas valve and the magnet head that accompanies it. If the pilot light goes out, the gas valve can stick in an open position, permitting gas to continue to flow, possibly causing an explosion.

      The firm has received 12 reports of flash fires, including 9 reports of injuries. The injuries involved first, second, and in one instance third degree burns.

      The TS-11 Thermal Safety Control Gas Valves are installed in commercial cooking equipment with pilot lights, including ranges, griddles, fryers, and warming trays.

      The recalled gas valves were produced between February 2003 and August 2004. The magnet heads on the gas valves were made during the same range of dates. The recalled products can be identified by date codes 0306 through and including 0432. The TS-11 gas valve with different manufacturing dates and a different hazard was recalled in 2002.

      The units were sold through original equipment manufacturers and food service equipment dealers to commercial food service providers (such as restaurants). The recalled products were sold from February 2003 through August 2004.

      Robertshaw Controls Company will arrange for free repair or replacement of the recalled gas valves. If you smell gas near the appliance or in the building, immediately leave the area and call your gas company or a certified gas technician to investigate the cause. If you do not smell gas, check the pilot lights on your gas appliances. If any pilot lights are out, do not attempt to relight. Have the appliance examined by the gas company or a qualified technician. Have the date-code of your TS-11 gas valve ready when you contact Robertshaw.

      GM Recalls Nearly 200,000 Vehicles

      February 10, 2005
      GM is recalling more than 193,000 vehicles in the United States to fix a variety of safety defects. Accoring to the National Highway Traffic Safety Administration, the recalls includring 155,465 pickup trucks, SUVs and vans that may suffer hydraulic fluid leaks, causing increased steering and braking effort and potential crashes.

      Models affected, all from the 2004-2005 model years, include the Avalanche, Express, Kodiak, Silverado and Suburban from GM's Chevrolet division. GMC's Savana, Sierra, Topkick, Yukon XL and the Hummer H2 were also affected, NHTSA said.

      In a separate recall, the agency reports 19.924 Cadillac SRX and Cadillac XLRs, along with Pontiac Grand Prix models from the 2004 model year, are being recalled because of potential problems with their accelerator pedals.

      GM is also recalling 17,815 Buick Rainier, Chevrolet TrailBlazer, GMC Envoy and Isuzu Ascender SUVs from the 2005 model year because their windshields may not have been mounted properly.

      NHTSA officials suggest that if a crash occurs, the windshield may not be retained, increasing the risk of injury to a vehicle occupant.

      Martha Stewart Full Moon Lantern

      January 20, 2005
      Martha Stewart Living Omnimedia is recalling about 1,700 Full Moon Lantern sets. The lantern can overheat and melt or ignite, posing a burn hazard to consumers.

      There have been 11 consumer complaints including reports of overheating, melting and fire. No injuries have been reported.

      The recall involves Full Moon Lanterns sold by Martha Stewart Living Omnimedia, Inc. The lanterns were sold in the following three different Halloween themes: Bat (Model # XPM 012A), Flying Witch (Model # XPM 012B), and Jack O-Lantern (Model # XPM 012C).

      Martha Stewart Living Omnimedia, Inc. sold the recalled Full Moon Lanterns through its on-line and print catalog, Martha Stewart: The Catalog for Living, from September 1, 2004 through October 31, 2004 at $19 for a set of two lanterns.

      Consumers should stop using the recalled lamps immediately. Martha Stewart Living Omnimedia, Inc. is directly contacting all affected consumers to arrange for a full refund.

      Tai Tung Toys Recalled

      October 21, 2004
      Tai Tung Toys is recalling about 128,000 toys including its toy ice cream car, police car and washing machine. The multicolored beads inside the toys can become accessible to children, posing a choking hazard.

      The recall includes an ice cream car, police car and a toy washing machine. The multicolored cars are about 5-inches long and contain various stickers. The pink, white, or red toy washing machines are 4.5-inches high.

      The toys make animal and music sounds. When the music and animal sounds play, lights flash and the toys multicolored beads can be seen moving inside. CE and MADE IN CHINA are written on the battery cover.

      The toys were sold at toy and discount stores nationwide from June 2000 through September 2004 for about $3.

      Consumers should immediately take the recalled toys away from young children and return them to the store where purchased for a refund.

      Tai Tung Toys Recalled...

      Target Birch and Bark Candles

      June 25, 2005
      Target is recalling about 230,000 Birch and Bark Candles. The birch and bark surrounding the candles can ignite, posing a fire and burn hazard.

      There have been 18 reports of the birch and bark candles catching fire and five reports of property damage. No injuries have been reported.

      The recalled candles were sold individually wrapped in a ribbon on a plate. The candles are cream in color and were sold in sizes 3-by-3 inches, 3-by-6 inches, 4-by-6 inches and 6-by-6 inches.

      The candles were sold at Target stores nationwide from September 2004 through January 2005 between $6 and $15.

      Consumers should return the candles to the nearest Target store to receive a gift card from Target in the amount of $15, plus applicable state taxes.

      Target Birch and Bark Candles...

      Halcyon SCUBA Buoyancy Compensator (BC) Inflators

      September 2, 2004
      About 4,000 Halcyon SCUBA Buoyancy Compensator (BC) Inflators are being recalled. The bladder can have a slow leak because of imperfections within the machining of the stainless air barrel of their inflators. This can cause unexpected buoyancy problems with divers, possibly resulting in decompression sickness.

      Halcyon has reports of 10 incidents of BCs leaking. No injuries or deaths have been reported.

      Halcyon is recalling all stainless steel power inflators. They have a stainless steel oral and power inflate button. Inflators made of plastic are not included in this recall. The repaired inflators have a groove at the base of their retainer nut. The BCs sold with these inflators are black and the Halcyon logo is on the front and collar of the BCs.

      Dive stores nationwide sold these inflators individually from October 2003 through May 2004 for about $46. The inflators also were sold as part of BC units ranging from $270 to $600.

      Consumers should stop using these BCs immediately and contact Halcyon for information on receiving a free repair.

      Family Dollar Spinning Christmas Tree Topper

      Family Dollar Stores Inc. is recalling about 13,000 spinning star Christmas tree toppers. The tree topper can melt or smoke near the on/off switch, which could pose a fire hazard.

      Family Dollar has received eight reports of the tree topper melting or smoking. There are no reports of fires, injuries or property damage.

      The recalled tree topper is a plastic star with a gold or silver metallic finish. The center of the tree topper is clear plastic center with a spinning disk with lights. Xmas L.E.D. Spinning Tree Top is written on the box.

      The units were sold at Family Dollar Stores nationwide from September 2005 through December 2005 for about $8.

      Consumers should immediately stop using the spinning star tree topper and return it to a Family Dollar store for a full refund.

      Gap Recalls babyGap Children's Coats

      Gap is recalling about 25,000 babyGap children's coats sold in the U.S. and Canada. The coats have toggle fasteners that could break and detach from the coat, posing a choking hazard to young children.

      This recall involves three different children's coat styles with toggles including Rose Toggle Hoodie (style 600298), Pink Duffle Coat (style 600344), and Blue Plaid and Mojave Toggle Coat (style 600942). The coats were sold in infant sizes up to 24 months and are made of cotton, polyester, or polyester/acrylic. The toggles are light tan plastic and about 1 3/4 inches long. They were made in China and Indonesia.

      The coats were sold at babyGap, GapKids and Gap Stores nationwide and at www.gap.com from October 2008 through April 2009 for between $20 and $50.

      Consumers should immediately take the recalled coats away from children and return them to any babyGap, GapKids, or Gap store for a full refund. Consumers who return the coats before August 31, 2009 will also receive a 20 percent off coupon for a regular priced item. Contact Gap to return by mail if purchased online.

      Bottle Sippers Recalled

      September 15, 2005
      About 500,000 Bottle Sippers pull-up bottle caps are being recalled by the distributor, Bradshaw International Inc. The pull-up valve can detach from the cap, posing a choking hazard to young children.

      Bradshaw International has received one report of a pull-up valve detaching from the screw-on cap. The incident involved a young child who began to choke after a pull-up valve detached in her mouth while she was drinking.

      The recalled bottle sippers have white screw-on bases with dark blue, green or red pull-up valves. They are sold in packages of three. The package is labeled Bottle Sippers. Bradshaw International Inc. and #70118 are written on the back of the package.

      The bottle sipper caps were sold at grocery stores nationwide from January 2002 through August 2005 for about $2 per package.

      Consumers should stop using these recalled bottle sippers immediately and return them to the store where purchased for a refund.

      Bottle Sippers Recalled...

      OkiData Fax Machines Recalled

      Oki Data is recaling several models of fax machines because a mechanical malfunction can pose a shock hazard to users.

      The recalled fax machines include the OKIFAX 5780, 5980, 5980DL models, and Konica 9880 model (manufactured by Oki Data Americas Inc.), with serial numbers beginning with 3110A, 311A, 312A, 401A and 402A. The model number is found on the top right hand side on the front of the unit and the serial numbers can be found on the back of the unit, bottom of case, centered, on the black and white bar code label.

      Oki Data independent dealers and Konica Business Technologies U.S.A. Inc. sold the fax machines between October 2003 and February 2004 for between $2,800 and $4,000 (depending on the model).

      Consumers should stop using the fax machines immediately, unplug the unit, and contact the company for a free repair.

      OkiData Fax Machines Recalled...

      New Safety Warning for Paintball Guns

      March 24, 2004
      Based on its investigation of two deaths caused by carbon dioxide (CO2) canisters flying off paintball guns, the U.S. Consumer Product Safety Commission (CPSC) is issuing a new warning.

      Paintball gun users should be advised that the brass or nickel-plated valve that connects directly to the canister must not be unscrewed from the canister when removing the canister assembly from a paintball gun. This valve must stay secured and rotate with the CO2 canister.

      In both of the deaths investigated by CPSC, the brass valve unscrewed from the canister, turning the pressurized canister into a deadly projectile.

      In June 2003, a 15-year-old boy died after being struck in the head by a paintball CO2 canister as he was removing the canister from the gun. In February 2004, a female bystander was killed by a CO2 canister that was expelled as someone else was removing the canister from a paintball gun. While these investigations are not complete, CPSC is issuing this new safety warning to help people avoid the hazard.

      We are very concerned about the recent deaths associated with paintball gun canisters, said CPSC Chairman Hal Stratton. We are actively investigating these cases. Even though our investigation is not complete, we believe it is urgent to provide this new safety message about the valve detaching from the canister while unscrewing it from the paintball gun.

      CPSC also recommends that people make sure that any modifications to the paintball gun or the CO2 canister are done properly. For example, installing anti-siphon tubes involves removing and re- installing the canister valve. It is critical that the valve be re- installed with the appropriate adhesive and the proper torque.

      • Make sure the brass or nickel-plated canister valve is securely attached to the canister, rotates with the canister, and does not unscrew from the canister.
      • The canister assembly should unscrew from the paintball gun in about three or four full turns; if you finish the 4th full turn and the canister is not unscrewed from the gun, stop! Take it to a professional.
      • Some people have used paint or nail polish to mark the brass valve and the CO2 canister so they can see that the valve and the canister rotate together while being removed from the gun.

      New Safety Warning for Paintball Guns...

      Makita Chain Saw Recall

      Makita is recalling about 3,400 chain saws. The flywheels on some of the chain saws can come apart during use, which could cause serious personal injury.

      Makita has received three reports of the flywheel coming apart. There have been no reports of injury.

      The recall involves Makita DCS6401 and DCS7901-model chain saws in which the last five digits of the serial numbers are within the following ranges:

      The chain saws housing is teal with the name Makita written on it. The chain saws serial number can be found on the right hand side of the silver nameplate which is located on the back of the fuel tank. No other Makita chain saws are involved in the recall. Any DCS6401 or DCS7901 chain saws with the letter N preceding the serial number on the nameplate and a blue dot on the shipping carton have been repaired and are not involved in the recall.

      Outdoor power equipment distributors and industrial contractor supply houses nationwide sold these chain saws from October 2004 through August 2005 for between $520 and $730.

      Consumers should stop using these recalled chain saws immediately and contact Makita to arrange for a free repair. Known purchasers were sent direct mail notification of this recall.

      Old Navy Recalls Childrens Zippered Outerwear

      Old Navy is recalling more than 600,000 children's zippered coats. The coats have a plastic zipper pull that can detach from the zipper, posing a choking hazard to young children.

      Old Navy has received 13 reports of the zipper pulls detaching, including one report of a child mouthing the object. No injuries have been reported.

      This recall involves childrens outerwear garments with a clear, oval-shaped zipper pull. The garments were sold in sizes 6 months to 4T. Style numbers are printed on a white label sewn into the garment. The label is located under the garment-care tag.

      The Robert Bosch Tool Corp. is recalling about 120,000 Model 3400 table saws because the blade drive mechanism may loosen or the motor can separate from the tool.

      Loosening of the blade drive mechanism can result in kickback of the item being sawed, resulting in possible laceration. Motor unit separation can cause the coasting saw blade to damage the saw wiring resulting in possible electric shock, or the separated motor could strike the user and cause injury.

      Robert Bosch Tool Corporation has received eleven reports of loose or broken motors. No injury or property damage has been reported.

      Only Skil table saws with model number 3400 printed on the front side of the table base with the date codes listed below are included in the recall. Date codes are printed on the upper right corner of the table base and include 2002 date codes 28501-28831, 2003 codes 38101-39231 and 2004 codes 48101-48811. The table saw holds a 10-inch blade and is made of metal tabletop with a red plastic base.

      The saws were sold at Home Depot, Lowes and Menards as well as independent hardware retailers nationwide from July 2002 through October 2004 for between $149 and $199.

      Consumers should contact Robert Bosch Tool Corporation to receive a repair kit. The kit includes hardware and instructions for installation.

      Abalone, Venetian Carnevale Necklace Kits Recalled

      Action Products Inc. is recalling about 2,900 children's Abalone and Venetian Carnevale Necklace craft kits because of a risk of lead exposure.

      The lobster clasps in both craft kits contain high levels of lead. The Abalone Necklace's pendant also contains high levels of lead. Lead can be toxic if ingested by young children and can cause adverse health effects.

      The recall includes the children's jewelry craft kits 'Abalone Necklace' (item #67117) and 'Venetian Carnevale Necklace' (item #67118). Both kits contain components to assemble necklaces including charms, beads, wire and clasps. The kit's name, model number, and 'Curiosity Kits' are printed on the product's packaging.

      The kits, made in China, were sold at various retailers in the U.S. and Canada from June 2007 through April 2009 for about $6.

      Consumers should immediately take the recalled children's craft necklaces away from children and contact Action Products International to arrange for the necklace's return and to receive a free replacement. Shipping will be free for consumer returns.

      Toyota Tundra Pickups Recall

      Toyota is launching a voluntary Special Service Campaign with the National Highway Traffic Safety Administration (NHTSA) involving its Tundra pickups. The term Special Service Campaign allows Toyota to avoid the word recall.

      The campaign involves about 2,500 Tundra Access Cab pickup trucks currently in operation in the U.S.

      A Front Passenger Occupant Classification System (FPOCS) is installed in certain 2005 model year Tundra Access Cab pickups equipped with front captain's chairs. This system monitors the weight and load on the front passenger seat, as well as the seatbelt buckle switch status, to determine whether to activate or deactivate the front passenger airbag.

      A small number of mid-2005 model year Tundra Access Cab vehicles equipped with the optional front captain's chairs may have been manufactured with the incorrect FPOCS indicator light lens, which may not allow the front passenger the ability to verify the status of the front airbag.

      Toyota will send owners of the involved vehicles a Special Service Campaign notification via first class mail beginning in early-August. Owners are requested to contact their local Toyota dealer upon receiving their notification for replacement of the FPOCS indicator light lens. Replacement will be done at no charge to the owners of the vehicle.

      Electrolux Recalls Room Air Conditioners/Heaters

      Electrolux is recalling about 5,800 room air conditioning and heating units. The heating coil in the units can be damaged during assembly, causing an electrical short and overheating, which poses a fire hazard to consumers.

      Electrolux has received two reports of incidents, though there have been no injuries.

      The recalled air conditioning/heating units are installed thru-the-wall and include the Frigidaire FAH12EM21 model and Carrier models 52FED31201AA.. The model numbers can be found on a serial plate on the side of the unit. The beige AC/heating units are about 14 high, 24 wide, and 20 in depth.

      The units were sold by independent dealer, builders, and remodelers between June 2003 and September 2003 for about $500.

      Consumers should stop using the units immediately and contact Electrolux for a free inspection and repair, if needed.
