    WASHINGTON, D.C., April 29, 1999 -- The Walgreen Company is recalling about 40,000 Kid's Sipper Bottles. The bottles' caps can be pulled off, creating a potential choking hazard.

    The announcement was made in cooperation with the Consumer Product Safety Commission.

    CPSC and Walgreen said they were not aware of any injuries or incidents involving these bottles and said the recall was being conducted to prevent the possibility of injury.

    The Kid's Sipper Bottle is a 9-ounce rectangular bottle with one of four animal characters on the cap - a fish, frog, chicken (duck) or whale. The bottle includes a plastic belt clip on the back.

    The bottles' packaging includes the writing, "KID'S Sipper Bottle with belt clip," and "For Ages 18 Mos. & Up." The bottom of the bottle has a sticker reading, "MADE IN CHINA."

    Walgreens nationwide sold these bottles between March 1999 and April 1999 for $1.69 or two for $3.

    Consumers should stop using these bottles immediately, and return them to the store where purchased for a refund. For more information, consumers should call (800) 934-4768 anytime, or go to the company's website at www.walgreens.com.

    Kubota Recalls Riding Mowers Due to Fire Hazard

    Kubota is recalling about 9,500 Zero Turn riding mowers. The carburetor can fail allowing fuel to leak, posing a fire hazard.

    The recalled riding mowers are propelled by gasoline V-twin engines and have 48-inch, 54-inch or 60-inch wide mower decks. The mowers come with adjustable roll bars and have 22 or 27 horsepower engines and are bright orange with KUBOTA decals on both sides behind the drivers seat.

    This recall includes the following model numbers: ZG222(serial numbers 10002 through 13673), ZG227 (serial numbers 10002 through 12957), ZG327 (serial numbers 10004 through 13318) which can be identified by the model designation and serial number on the data plate located on the right side frame, either before the mower deck or immediately after the mower deck.

    The mowers were sold by authorized Kubota dealers nationwide from February 2008 through December 2008 for between about $7,600 and $10,000. They were made in the U.S.

    Owners of these machines should immediately stop using them until they have been inspected by an authorized dealer and a free replacement carburetor has been installed.

    Thornwood Toy Chests Recalled


    WASHINGTON, Feb. 3, 2000 -- Thornwood Furniture Manufacturing, of Phoenix, Ariz., is recalling about 400 toy chests for replacement.

    The lids on these toy chests could fall onto a child's head or neck. An investigator for the Georgia Office of Consumer Affairs discovered the problem with the toy chest while conducting retail surveillance for the Consumer Product Safety Commission (CPSC).

    So far, no injuries have been reported.

    The oak toy chests have model number 841LQ or 1814LQ located on the rear panel. They measure 35-inches long, 19-inches high and 19-inches deep. A 1-inch-wide ventilation opening runs the entire length of the front of the toy chest just below the lid. The toy chests have one adjustable lid support.

    The Thornwood Furniture name and address are located on the product sticker located on the rear panel of the toy chest. Furniture stores nationwide, including Rooms To Go, Bedroom Superstores and MN Goldstein, sold the toy chests from January 1999 through November 1999 for about $170.

    Consumers should immediately stop children from using the toy chests and contact the store where purchase for information on getting a free replacement.       Barbie Sunglasses Recalled

      WASHINGTON, Feb. 21, 2001 -- IMT Accessories is recalling about 70,000 BarbieTM Sunglasses. The frames of the sunglasses can break, allowing the petroleum distillate and floating glitter to leak out. Petroleum distillates could be harmful to children's eyes and skin and could be fatal if ingested.

      IMT Accessories has received one report of a six-year-old child who received chemical burns in her right eye as a result of petroleum distillates leaking from the sunglasses when she was playing. The recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC).

      The recalled sunglasses have a pink tint to the eyeglasses, have floating glitter in the temple of the sunglasses, say "BarbieTM" and "Mattel®" on the left side of the earpiece, and say "China" on the right side.

      The sunglasses were sold nationwide in Target, Walgreen's and Bradlee's, from June 1999 through August 2000 for about $6.

      Consumers should stop children from wearing the sunglasses immediately. Return the sunglasses to the store where purchased for a full refund. Consumers can call IMT Accessories toll-free at (800) 868-7870 between 9 a.m. and 5 p.m. EST Monday through Friday.

      Stride Rite Recalls Munchkin T-Strap Girls Shoes

      WASHINGTON, Nov. 28, 2001 -- Stride Rite Children's Group Inc. is voluntarily recalling about 109,000 pairs of Munchkin T-Strap girls' shoes. The buckle on these shoes can break, posing a choking hazard for young children.

      Stride Rite Children's Group has received four reports of the buckle breaking, and one report of a child putting a broken piece of the buckle in her mouth and starting to choke.

      The Munchkin T-Strap girls' shoes were sold in children's sizes 4 through 12. They have a strap that is fastened by a metal buckle on the side of the shoe. The word "MUNCHKIN" is molded into the bottom of the shoe. The shoes have the following style numbers written inside the shoe below the words "Made in China.":

      Building Fire & Smoke Dampers Recalled

      WASHINGTON, Oct. 2, 2002 -- Invensys Building Systems, of Love Park, Ill., is recalling up to 560,000 Siebe actuators for testing and replacement, if necessary, which could be found in fire and smoke dampers.

      Actuators hold open fire and smoke dampers in a building's heating, ventilation and air conditioning (HVAC) system. A spring mechanism, integrated with the damper, is designed to close the damper during a fire to limit the spread of smoke, fire and fumes. The recalled actuators can jam, and prevent the dampers from closing. If dampers fail to close during a fire, serious injury or death can be suffered by building occupants because smoke, fire and fumes can spread through a building's ventilation system.

      Building owners, damper manufacturers, and fire and safety investigators have reported dampers not closing during testing.

      These MA-200 and MA-200-1 series actuators were used in fire and smoke dampers that were manufactured by various companies from December 1993 through October 1999, which were installed by private contractors in commercial and residential buildings. The actuators have manufacture date codes of 9348 through 9947. See the associated recall alert for more details on the dampers and companies involved.

      Building owners and managers whose buildings have dampers containing Invensys/Siebe actuators should immediately contact Invensys' Program Coordination Office at (877) 481-2372 between 7 a.m. and 7 p.m. CT Monday through Friday.       The recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC).

      To be eligible for a free actuator replacement, the building owner must test the actuator and report the results to Invensys. Only actuators that fail testing will be replaced.

      Building Fire & Smoke Dampers Recalled...

      Deep Fryer Basket Handles Recalled

      WASHINGTON, Oct. 17, 2001 -- National Presto Industries Inc. is voluntarily recalling to replace up to 50,000 Presto CoolDaddy electric deep fryer basket handles. The handles on the fryer baskets can come off, resulting in hot oil spattering the user and bystanders.

      Presto has received 63 reports of basket handles coming off, including 10 reports of consumers being spattered with hot oil. The injuries include minor burns to the arms and face.

      The recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC).

      The Presto CoolDaddy electric deep fryers have model numbers 0544404 (product number 21-419) and 0544504 (product number 21-439). The deep fryers are white with the words, "Presto CoolDaddy or "Presto Pride CoolDaddy" written on the front. The model numbers are located on the bottom of the deep fryer and the product number is located on the bottom of the carton. Only the deep fryer baskets with silver screws attaching the basket handle are involved in this recall.

      Department stores sold the deep fryers nationwide from July 2000 through June 2001 for between $50 and $70.

      Outdoor Research Recalls Warming Gloves

      January 28, 2009
      Outdoor Research Inc. is recalling about 900 pairs of "Primovolta" and "Primavolta" warming golves. The glove's electric heating pad can short circuit and overheat, posing a burn hazard to consumers.

      The firm has received five reports of the gloves overheating, including one consumer who suffered burns to her hand.

      The recalled winter gloves are black-colored and have a battery-powered heating element. The gloves included in the recall are style number 77000 (men's) in sizes S, M, L, XL, and style number 77010 (women's) in sizes S, M, L. The product name is printed on the inside cuff of the left glove.

      The gloves were sold at major outdoor specialty retailers from September 2008 through January 2009 for about $260. They were made in China and Sri Lanka.

      Consumers should stop using these gloves immediately and contact Outdoor Research for a full refund or credit.

      Ameri-China Sued for Toy Safety Violations

      WASHINGTON, June 29, 2001 -- The U.S. Consumer Product Safety Commission (CPSC) has sued Ameri-China International Inc. of Los Angeles, alleging that it has imported more than 755,000 illegal children's toys between 1997 and 2000.

      The suit charges that Ameri-China imported toy cars, helicopters, phones, and bubble-blowing plastic hammers, all of which violated the federal safety standard for small parts. Toys with small parts are a choking hazard for children under 3. The company also imported or purchased for re-sale bouncing balls, mini-pool table sets, art sets and balloon helicopters, that contained labeling violations and are considered to be misbranded hazardous substances. The company's crazy ribbon party spray was found to be flammable.

      "We will aggressively pursue companies that import and endanger children with unsafe toys," said CPSC Chairman Ann Brown. "CPSC will take strong action to deter this kind of conduct, and stop companies from profiting at the expense of children's safety."

      The complaint charges that Ameri-China imported or purchased for re-sale eight separate shipments of toys that violate U.S. safety standards. Every time CPSC found a violative shipment, it notified the company; yet, Ameri-China continued to violate the law by importing more unsafe toys. In cooperation with the U.S. Customs Service, CPSC prevented the unsafe toys from reaching U.S. store shelves.

      CPSC is seeking a civil penalty and a court order preventing the company from committing any further illegal actions.

      The suit, filed in the U.S. District Court for the Central District of California, also names the company's president, Austin Wu.

      • Select toys to suit the age, abilities, skills, and interest level of the intended child. Toys too advanced may pose safety hazards to younger children.
      • For infants, toddlers, and all children who still mouth objects, avoid toys with small parts, which could pose a fatal choking hazard.
      • Look for sturdy construction on plush toys, such as tightly secured eyes, noses, and other potential small parts.
      • Avoid toys that have sharp edges and points, especially for children under age 8.
      • Do not purchase electric toys with heating elements for children under age 8.
      • Be a label reader. Look for labels that give age and safety recommendations and use that information as a guide.
      • Check instructions for clarity. They should be clear to you, and when appropriate, to the child.
      • Immediately discard plastic wrappings on toys before they become deadly playthings. Plastic wrappings can cause suffocation.

      BikeE Recumbent Bicycles Recalled

      WASHINGTON, Aug. 14, 2001 -- BikeE Corp. is voluntarily recalling 1,400 suspension forks on FX and RX model recumbent bicycles. The link pins in the forks can fall out, causing the rider to lose control of the bicycle, fall and possibly suffer serious injuries. BikeE Corp. has received two reports of pins falling out of forks, resulting in two minor injuries to riders.

      The recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC).

      The recalled forks were used on the front suspension of the BikeE FX models and on the full suspension of the RX models. The 2000 and 2001 model year bicycles can be identified by a serial number located below the crank bottom bracket on the frame.

      Samsung Over-the-Range Microwaves Recalled

      About 43,000 Samsung over-the-range microwaves ovens are being recalled. If an installation bolt comes in contact with an electrical component inside the unit and the microwave is plugged into an ungrounded outlet, it could create a shock hazard.

      The recall involves Samsung 1000 watt over-the-range microwave ovens. The following model and serial numbers are included in this recall:

      Besea Diving Wings Recalled

      June 23, 2009
      Poseidon is recalling about 300 Besea W50 diving wings with Poseidon inner bladders. The inner bladder can break, causing the wing to fail to operate as a floating/buoyancy device. This poses a drowning hazard to divers.

      Poseidon has received 15 reports of inner bladders breaking. No injuries have been reported.

      The recall involves the Poseidon inner bladders with batch number 5445 sold with the Poseidon Besea W50 wings. The inner bladder is located inside the outer cover of the wing. The batch number and Poseidon are molded on a tab located between the legs of the inner bladder.

      The winds, made in Sweden, were sold at Poseidon dealers nationwide from September 2007 through June 2008 for about $366.

      Consumers who have wings that contain the recalled inner bladders should stop using the wing immediately and contact Poseidon for a free replacement.

      Tanaka Augers and Trimmers Recalled Because of Fire Hazard

      WASHINGTON, June 27, 2000 -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Tanaka America, of Auburn, Wash., is voluntarily recalling about 7,500 hedge trimmers and 1,400 augers. The fuel tank vents on these products can leak, posing a fire hazard.

      Tanaka and CPSC have received one report of a hedge trimmer catching fire and causing minor damage. No injuries have been reported.

      The products are orange with black handles and have "Tanaka" and the model number printed on the recoil starter cover. The serial number is located on a label on the side of the engine. The model and serial number ranges are:

      Lawn and garden stores and outdoor power equipment dealers sold the hedge trimmers and augers nationwide from September 1998 through September 1999. The hedge trimmers sold for about $370 and the augers sold for about $500.

      Consumers with a recalled hedge trimmer or auger should stop using it immediately and return it to any authorized Tanaka service dealer for a free repair. The equipment will be fitted with a redesigned fuel tank.       Cannondale Recalls Bikes With Defective Stems

      WASHINGTON, Aug. 15, 2002 -- Cannondale Corp. is voluntarily recalling about 1,300 bicycles with defective stems. The stem can break away from the bicycle, causing a loss of control, falls and serious injury to riders.

      Cannondale has received four reports of stems breaking, including one minor injury.

      The recalled stems are black and have the model name - 3T ZEPP XL printed in white on the sides of the body of the stem. The faceplate also has "3T" printed in white at the front of the stem. The stems were installed on 2002 and 2003 model year Cannondale bicycles. The recalled stems were manufactured in Italy by 3T (Tecno Tubo Torino) and distributed by parent company, Gruppo, SPA.

      Authorized Cannondale Bicycle dealers nationwide sold the stems from March 2001 through July 2002 for between $3,400 and $5,000 (includes the cost of the bicycle). The recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC).

      Cannondale Recalls Bikes With Defective Stems...

      Ziotek Extension Cords

      WASHINGTON, May 16, 2002 -- E-filliate Inc. is voluntarily recalling about 12,000 extension cords. The cords have reversed polarity, which can present electric shock and electrocution hazards to consumers.

      E-filliate has received one report of reverse polarity, though no one has been injured. The recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC).

      The recalled Ziotek brand extension cords are black and about one to two feet long. The letters "MSL" are imprinted on the cord's three- pronged plug. The single-receptacle cords have the website, www.ziotek.com, printed on the cord; while the double-receptacle cords have the name "ZIO TEK" printed on the splitter. These extension cords were manufactured in China.

      "Cyberguys" catalogs and the "Cyberguys" website (www.cyberguys.com) sold the extension cords nationwide from January 2002 through March 2002 for between $2 and $3.

      Consumers should unplug the cords immediately and call E-filliate at (800) 327-6703 between 7 a.m. and 5 p.m. PT Monday through Friday to receive a prepaid shipping label to return the cord and to select a free replacement or a full refund.       Cords that have the letters "MSL" imprinted on the plug, but which also have a yellow "QC" label wrapped around the cord, are not included in this recall.

      Ziotek Extension Cords...

      Black & Decker Table Saw Recall

      WASHINGTON, Aug. 20, 2002 -- Black & Decker is voluntarily recalling about 6,100 table saws. The motor housing on the saw may crack, posing a risk of electric shock to consumers.

      Black & Decker has not received any reports of injuries. This recall to repair is being conducted to prevent the occurrence of injuries.

      The recall involves 10-inch table saws, made in Taiwan, with the model number BT2500 and dates codes 200128-CT through 200148-CT. The model number and date codes are located on a name plate on the front panel above the power switch. The saw is grey with an orange front panel. The name "Black & Decker" is located on the lower right hand portion of the front panel.

      Home centers and hardware stores nationwide sold the table saws from August 2001 through April 2002 for about $400.

      The recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC).

      Consumers should stop using these table saws immediately, unplug them, and call Black & Decker at (866) 357-0324 between 8 a.m. and 4:30 p.m. ET Monday through Friday to arrange for a free repair. For more information, consumers can log on to the company's website at www.blackanddecker.com.

      Wall-Mounted Radiators Recalled by Rettig Belgium

      Wall-Mounted Radiators are being recalled by Rettig Belgium after a child's death. the radiators can come loose from the wall and fall on persons nearby.

      Only 36 of the units were sold in the United States but there are about 113,000 worldwide.

      No incidents have been reported in the U.S. There have been 52 incidents reported outside the U.S., including one that involved the death of a child in Belgium.

      The recalled radiators are used to heat a home and are mounted vertically to the wall. The recall involves the Radson Vertical 20 and 21, Radson Kos V21, and Radson Faro V21.

      The radiators, made in Belgium, were sold by plumbing and heating supply companies from October 2007 through January 2008 for between $300 and $700.

      Consumers should avoid standing or sitting near the recalled radiators and contact their plumbing or heating supplier or Rettig for a free replacement suspension system.

      Brunswick Mountain Bike Recall

      WASHINGTON, May 9, 2000 -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), By Us International Co. of Taiwan is recalling about 13,500 Ballistic front suspension forks installed on certain Brunswick mountain bicycles. The forks on these bicycles can break apart, causing riders to lose control and fall.

      By Us and Brunswick have received 18 reports of forks breaking, resulting in nine riders, including teenagers, suffering serious head and bodily injuries, abrasions, bruises and chipped teeth.

      The recalled forks were installed on Mongoose S-20 and MGX S-20 bicycles, which are orange, and the Roadmaster Ridge Rider bicycles, which is red. The forks are black with decals that read "BALLISTIC" and "105" on the sides of the suspension fork legs. A serial number is located on the inside of the suspension fork legs. The recall only includes forks with the following serial numbers:

      Discount department and toy stores nationwide sold these bicycles from September 1998 through May 2000 for between $125 and $150.

      Consumers should immediately stop using these bicycles and call Brunswick Bicycles for information on receiving a free replacement fork, including installation. Call Brunswick toll-free at (877) 211-3525 between 8 a.m. and 5 p.m. CT Monday through Friday.

      STK Toys Recalled

      WASHINGTON, Aug. 20, 2001 -- STK International is recalling about 110,000 children's toys. The toys can break causing small balls to be released, posing a choking hazard to young children.

      The recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC). STK International and CPSC have not received any reports of incidents. This recall is being conducted to prevent the possibility of injuries.

      The following information will help consumers identify the recalled children's toys:

      • "2 PC Tambourine Set": Two tambourines consisting of a colorful plastic ring with three balls attached. Inside of the balls are beads that create a rattle sound when shaken. On each side of the tambourine is a sticker with a female cartoon character and the number 985.
      • "Bathtime Water Wheel": A multi-colored plastic toy with a suction cup base and six small cups attached to a water wheel. "Made in China NO:368" is printed on the back of the water wheel.
      • "Funny Loco Wind-Up": A multi-colored train with a pig character on a sticker placed on both sides of the toy. "Made in China NO:883" is printed on the back of the train.
      • "Pull Back Duck in Boat": A plastic blue and white boat with colored stickers on the side, a transparent red windshield, and white duck mounted on top of the boat. The toy also comes with a miniature fishing pole and a plastic lobster. A yellow sticker on boat reads, "My Baby Products" and "PULLBACK DUCKS IN BOAT, MADE IN CHINA."

      Dollar stores nationwide sold these toys from September 1999 through May 2001 for about $1.

      Consumer should take the toys away from children immediately, and return them to STK International for a full refund. Consumers should call STK International toll-free at (800) 536-7855 between 8 a.m. and 5 p.m. PT Monday through Friday for information about where to send the toys.

      CPSC discovered the violations during an inspection of the company's warehouse.
