    High-Intensity Headlights Recall Ordered

    A Texas company has been ordered to stop selling high-intensity headlight kits.

    The National Highway Traffic Safety Administration (NHTSA) said the high-intensity headlights sold by Astex USA can cause excessive glare and are hazardous to other drivers. It ordered the company to stop selling the kits and recall 150 kits that already have been sold.

    High-intensity lights have a blue glow and wider beams than traditional halogen lights. They are standard equipment on some new vehicles, but the new-car equipment meets federal standards.

    Astex, of Dayton, Texas, is the latestof 24 companies ordered to stop selling the high-intensity conversion kits to vehicle owners.

    Feds Will Probe Hummer H2

    Federal safety regulators are opening an investigation into General Motors' Hummer H2. The probe follows consumer reports that a wheel fell off the hulking SUV after a steering part cracked.

    The National Highway Traffic Safety Administration (NHTSA) said it has opened a preliminary evaluation -- a step that often precedes a recall. Being studied are the 2003 and 2004 Hummer H2, which has scored poorly in quality studies conducted by J.D. Power and Associates.

    Four consumers complained that the steering knuckle fractured at the lower ball joint attachment, causing them to lose control of the H2, including three who said a wheel separated from the SUV, NHTSA said.

    One consumer said he crashed into a parked vehicle, and a second veered into oncoming traffic before running into a ditch on the opposite side of the road, the agency said.

    The consumers said their vehicles had not been driven off-road or under any abusive conditions.

    DaimlerChrysler Faces Dodge Ram Recall

    Chrysler may have to recall Dodge Ram pickups from the past two model years. Federal safety investigators say the truck may roll backwards when the driver tries to park it.

    An investigation by the National Highway Traffic Safety Administration covers 183,000 vehicles from the 2003 and 2004 model years, following reports of 42 crashes and three injuries in 50 incidents, agency said.

    NHTSA said the pickups rolled away while the driver had the gear shift in the "park" position with the engine running and the parking brake disengaged.

    Chrysler denies there is a problem. A spokesman said consumes may not be putting the gear shift level completely into "park." The spokesman said drivers should never leave the engine running when they get out a vehicle.

      Fitness Quest Ab Lounge

      Fitness Quest is recalling the Ab Lounge, Ab Lounge 2 and Ab Lounge Ultimate Exercisers following 15 reports of lacerations, crushing and amputation of finger tips.

      When opening or folding up these exercise machines, consumers can catch their fingers in the hinges. This can result in injuries to the finger. This does not occur while users are exercising.

      The machines have the name Ab Lounge or Ab Lounge 2 printed on the upper front fabric of the seat. The recall involves machines with the following serial numbers:

    • Ab Lounge machines: ALX-000001 through ALX-037999, and all machines that start with serial numbers 03 44 through 04 36.
    • Ab Lounge 2 machines: AL2X-000001 through AL2X-059060, and all machines that start with serial numbers 04 03 through 04 36.
    • Ab Lounge Ultimate machines: all machines that start with serial numbers 04 28 through 04 34.

      The serial number is located on a label on the bottom of the front frame. Ab Lounge exercise products with straight brackets that do not fold are not part of this recall.

      The machines were sold through infomercials, the Internet, catalogs and at discount department and sporting goods stores, including Wal-Mart and Dicks Sporting Goods, nationwide since October 2003 for between $100 and $210.

      Consumers should stop using these exercisers and contact the firm for a free repair kit. The firm is contacting all consumers who purchased the Ab Lounge directly from Fitness Quest via infomercial or website. Due to the large number of repair kits required to meet demand, these consumers may experience a delay of a few weeks before they receive their kits. Any consumer who owns an Ab Lounge purchased at a retail store should stop using the equipment and contact Fitness Quest immediately to receive the repair kit.

      "Poof" Chairs Recalled

      Palliser Furniture is recalling 155 "Poof" chair and table sets. A metal rod can protrude through the fabric and its sharp edges could cause scratches or cuts.

      Palliser has received one report of a consumer who was scratched.

      This set includes two semi-circular chairs and one circular table in blue, red, and yellow fabric. The law label on the bottom of the chair has Hong Feng Furniture Cushion Co. and Palliser Furniture Ltd.

      Furniture stores nationwide and in Canada sold the sets from June 2003 through January 2004 for between $150 and $200.

      Consumers should take the product away from children immediately and return the chairs and table to the retailer for a full refund.

      Poof Chairs Recalled...

      "DU" Brand Nasal Decongestant

      FDA has alerted consumers not to purchase or use a recalled lot of DU brand nasal decongestant spray (distributed by Drugs Unlimited, of Puerto Rico) because it may be contaminated with Burkholderia cepacia a bacterium that could cause serious, potentially life-threatening infections in some patients. Individuals with compromised immune systems, especially those with cystic fibrosis, could be at risk.

      The recalled product is sold over the counter in 15 milliliter (or 1/2 ounce) and 30 milliliter bottles labeled DU 12-Hour Nasal Spray with the lot number J4492 imprinted at the bottom of the carton and the back of the bottle label. The recalled product also bears an expiration date of 9/06.

      The product appears to have been distributed throughout Puerto Rico and it is likely linked to an earlier recall of other contaminated product lots that were sold to other distributors.

      Consumers who have used the recalled product may want to consult their physicians. Consumers who may still have the recalled product should not use it, but instead return it immediately to the stores where it was purchased. Wholesalers and retailers who purchased the product should return it to the appropriate distribution center. Anyone needing more information can call Rey Farinas at 305-592- 9216.

      Steel Toe Boots Recalled

      The Georgia Boot is recalling 10,000 pairs of steel toe logger boots. The boots may have been mislabeled to indicate that they are resistant to electrical current, which is incorrect. This may result in a serious shock or electrocution to the wearer.

      The recalled steel toe logger boots are brown and black and have laces that tie up to the calf. The recalled boots have stock numbers of G8320, G8322, and G9360, which can be found on a label under the tongue of the boot. The same label has stitching of the Georgia Boot name.

      Retail shoe stores nationwide and independent retail stores sold these boots from October 2002 through April 2004 for between $80 and $150.

      Consumers should stop using the boots immediately and return them to the company so they can be re-labeled or provided with a new pair of boots, free of charge.

      Medicare Drug Card Enrollment Nears 4 Million

      The Bush administration is nearly halfway toward its goal of enrolling 7.4 million people in the Medicare prescription drug discount card program. CMS Administrator Mark McClellan says that so far, nearly 3.7 million people have enrolled in the program, which Bush administration officials say has helped reduce prescription drug costs by 10% to 25% for beneficiaries since April.

      It's estimated that about 66% were enrolled automatically -- either through a managed care plan or a state prescription drug assistance plan. Health care analysts say as many as 700,000 low-income beneficiaries could be enrolled in the program if the federal government expanded automatic enrollment.

      The Bush Administration has been resisting calls to expand automatic enrollment, saying it does not want to limit individuals' choice of cards. Legislation introduced in the House and the Senate that would require automatic enrollment of low-income Medicare beneficiaries has gone nowhere.

      To date the White House has limited its efforts to enroll low-income beneficiaries by providing about $5 million to the Access to Benefits Coalition, a group of civic organizations dedicated to persuading low-income Medicare beneficiaries to enroll on their own.

      McClellan said he expects enrollment to picked up in the next few weeks, the Detroit News reported. He said that 25,000 people are signing up each day (San Francisco Chronicle, 7/6).

      Meanwhile, a number of industry analysts are predicting that as the cost of the new Medicare law increases in the next decade, insurers will pressure the federal government to institute price controls on prescription drugs, something the current law prohibits.

      Nike Get-Go, Little Get-Go Children's Shoes

      Nike is recalling its Nike Get-Go and Little Get-Go Childrens Athletic Shoes. The 1.5-inch to 1.75-inch rubber tab at the top of the heel can detach from the shoe, posing a choking hazard to young children.

      Nike has received three reports of heel tabs separating from shoes. No injuries have been reported.

      The GET-GO and LITTLE GET-GO childrens athletic shoes come in various color combinations and were sold in toddler and pre-school sizes 2C through 3Y. The Nike Swoosh symbol is on the outside and sole of the shoe. All shoes close with a strap. The style name GET-GO or LITTLE GET-GO is written on the shoe box.

      The recalled shoes have one of the following six-digit numbers on a label inside the shoe just above the UPC code: 308638, 308639, 308642 and 308647. Only these style numbers are included in this recall.

      The shoes were sold at Nike stores, major athletic shoe stores, independent shoe stores and childrens stores nationwide from June 2004 through August 2004 for between $35 and $45.

      Consumers should immediately take the recalled shoes away from young children and contact Nike to receive a refund voucher.

      Solis Hair Dryers Recalled

      Solis hair dryers are being recalled. They do not have an immersion protection device or ground fault circuit interrupter (GFCI) on the power cord, which poses a serious electrocution hazard if dropped in water.

      In 1991, a voluntary standard was implemented that called for all hair dryers used by consumers to protect against electrocution in both the "on" and "off" position.

      The recalled Solis model hair dryers include the following model numbers: 311, 340, 401, 405, 407, 407T, 411, and 415. The model name and number can be found either on the handle, the underside of the barrel or under the filter. The recalled units, which come in black and blue, do not have an immersion protection device at the end of the plug.

      The units were sold by professional beauty distributors between November 2002 and December 2003 for about $90.

      Consumers should stop using the hair dryers immediately and return them to Fromm International-Solis USA for a new hair dryer with a GFCI plug.

      GM Recalls 2009 Saturn Vue

      Potential steering problem

      December 2, 2008
      General Motors is recalling about 2,100 Saturn Vues from the 2009 model year because of a problem with the steering linkage.

      The company said that some of the vehicles may have been built with an incorrect steering knuckle, which could cause the tie rod ball to pull out of the tie rod housing. This could cause the affected wheel to turn out and skid.

      If both assemblies were defective, the driver would lose steering control of the vehicle.

      Under the recall, dealers will inspect the steering knuckles and, if necessary, replace the knuckles and rods. The recall is expected to begin around Dec. 8.

      Plan Toys Recalls Wooden Penguins

      Plan Toys Inc. is recalling about 3,000 wooden toy penguins. The head of the penguin toy can detach, exposing connectors with sharp points, presenting a laceration hazard to consumers.

      Plan Toys has received one report of the head of a penguin toy detaching. No injuries have been reported.

      The wooden penguin-shaped toy has a black head with rubber fins, a yellow nose and a white body with red on the base. The toy is round-shaped and creates a soft bell jingle when spun. The toy measures about 3 inches in height and about 3 inches in diameter. PLAN TOYS and UPC number 084543521109 or EAN number 8854740052117 are printed on the outside of the packaging of the toy.

      The toys were sold at specialty toy stores nationwide and on-line from May 2007 through February 2008 for between $15 and $20. They were made in Thailand.

      Consumers should immediately take the penguin toys away from children and return them to the store where purchased to receive a refund. Consumers also can contact Plan Toys to receive instructions on returning the penguin toys via mail for a refund.

      JCPenney Recalls 'Arizona' Newborn, Infant Pants

      JCPenney is recalling about 24,000 pairs of "Arizona" newborn and infant denim pants. The metal snap at the waist can detach posing a choking hazard to infants.

      The recall involves Arizona brand newborn and infant girls' and boys' denim pants. They were sold in sizes 0-3 months, 3-6 months and 6-9 months. Only those pants made in Bangladesh are affected; pants made in China are not included in the recall. The country of origin is located on a label sewn into the waistband.

      The pants were sold at JCPenney stores nationwide and at www.jcp.com from July 2008 through October 2008 for about $15.

      Consumers should immediately stop using the recalled pants and return them to any JCPenney store for a full refund. Pants purchased through the firms Web site may be returned to the Catalog Desk at any JCPenney store.

      Waxcessories Recalls Electric Simmer Pots

      Waxcessories is recalling about 830,000 electric simmer pots. The simmer pots have wire connections that can become loose, posing a risk of fire and electric shock to consumers.

      There have been 161 reports of discolored cords, loose wires, and wires separating from the pot. No injuries have been reported.

      The electric simmer pots were sold in a variety of styles and were designed to melt scented wax in a ceramic cup. The cup sits on a ceramic base that contains a 40-watt bulb, socket and electric cord. The recall includes all designs of the electric simmer pots and all item numbers. The words ELECTRIC SIMMER POT and the design name are printed on the bar code label, which is located on the bottom of the base.       The pots were sold at gift and novelty shops nationwide from April 2002 through April 2008 for between $17 and $25. They were made in China.

      Brunswick Recalls 'Colonial' Folding Game Chairs

      Brunswick Bowling & Billiards Corp. is recalling aboug 1,700 folding game chairs. The retaining washers on the legs can loosen, causing the chair to become unstable. This poses a fall hazard to consumers.

      Brunswick has received four reports of loose chair leg hardware. No injuries have been reported.

      This recall involves the 'Colonial' model, folding wooden chairs for use with game tables. They were sold in sets of two. The chairs have a black seat cushion and either a cherry or chestnut finish.

      Authorized Brunswick dealers nationwide sold the chairs from March 2007 through March 2008 for about $400 per set. They were made in China.

      Consumers should immediately stop using the folding game chairs and contact the dealer where the chairs were purchased to schedule a free repair.

      Daiso Recalls Children's Rain Ponchos

      Daiso is recalling children's rain ponchos with drawstrings because they represent a stangulation hazard.

      The recalled plastic rain ponchos were sold in yellow or blue and have a drawstring in the hood. Japanese lettering is printed on the packaging.

      The ponchos were sold by Daiso retail stores located in California and Washington states from February 2007 through November 2007 for $1.50. They were made in China.

      Consumers should immediately remove the drawstrings from the rain ponchos to eliminate the hazard.

      Delta Recalls 600,000 drop-side Cribs after Infant Death

      Spring peg failure can create entrapment hazard

      The U.S. Consumer Product Safety Commission (CPSC) and Delta Enterprise Corp. are recalling about 600,000 drop-side cribs. The cribs drop-side can detach when the spring peg is not engaged, which can cause an entrapment and suffocation risk to infants and toddlers.

      Delta is also recalling about 985,000 drop-side cribs to replace missing safety pegs.

      If the spring pegs in the base of each leg of the crib are pushed back into the lower track, the pegs can become nonfunctional. This can cause the drop-side to detach from the crib and create a gap that can entrap an infant or toddler.

      CPSC staff is aware of a death of an 8-month-old child who suffocated when the drop-side of his crib detached. CPSC is also aware of one child entrapment and one incident in which the drop-side detached due to a disengaged spring peg.

      The recall involves all Delta drop-side cribs manufactured prior to 2006 that use Crib Trigger Lock with Spring Peg drop-side hardware design. The recalled model numbers and country of origin include: 4340, 4343, 4520, 4600, 4620, 4624, 4625, 4629, 4660, 4665, 4720, 4750, 4751, 4850, 4855, 4857, 4880, 4920, 4925-2, 4925-2B, 4925-6, 4980, and 8605. The model number, country of origin and manufacture dates are located on top of the mattress support board. The recalled cribs have Deltas name and address on the mattress support boards and Deltas logo on the teether rail.

      The cribs, which were made in China, were sold at major retailers nationwide from January 2000 through January 2007 for between $33 and $200. Cribs currently sold at retail are not involved.

      Parents and caregivers are encouraged to find a safe, alternative sleep environment for their child until the repair kit, with new safety pegs, is safely installed are the recalled cribs.

      CPSC would like to remind parents not to use any crib with missing, broken, or loose parts. Make sure to tighten hardware from time to time to keep the crib sturdy. When using a drop-side crib parents should check to make sure the drop-side or any other moving part operates smoothly. Always check all sides and corners of the crib for disengagement. Any disengagement can create a gap and entrap a child. In addition, do not try to repair any side of the crib, especially with tape, wire or rope.

      Horseshoe Magnets Recalled

      Dowling Magnets is recalling about 91,000 horseshoe magnets. The plastic coating on the magnets contains excessive levels of lead, which violates the federal lead paint standard.

      This recall involves the Classic Horseshoe Magnets. The 6-inch tall horseshoe magnet has a red plastic coating on the 'handle' end. Each 'foot' of the magnet has an engraved 'S' or and 'N.'

      The mangets were imported from China and sold at specialty retail stores and educational supply distributors nationwide from March 2003 through April 2008 for about $10.

      Consumers should immediately take the classic horseshoe magnets away from children and contact Dowling Magnets to receive a free replacement horseshoe magnet.

      Cabot Recalls Composite Deck Cleaner

      Cabot is recalling about 24,000 cans of its Composite Deck Cleaner. One of the components of the composite cleaner can react with metal foil residue on the packaging, releasing heat and chlorine gas. This poses a fire and inhalation hazard to consumers.

      Cabot has received one report of the composite cleaner's plastic container partially melting and emitting a chlorine odor. No injuries were reported.

      This recall involves the Cabot Composite Deck Cleaner #3502. The deck cleaner is a granular concentrate that, when mixed with water, is used to clean composite surfaces such as decks, railings, steps, fences and siding. It was sold in a 2-pound plastic container. The label on the container reads: 'Cabot Composite Cleaner' and has number 3502 at the bottom of the front of the label.

      It was made in the United States and sold at home improvement centers and hardware stores nationwide from February 2007 through February 2008 for about $20.

      Consumers should immediately stop using the product and return it to the store of purchase for a full refund. If the container looks deformed or if it feels hot to the touch, immediately call Cabot anytime to arrange for the safe handling of the product.

      Igloo Recalls Marine Elite Coolers

      September 24, 2008
      Igloo Products is recalling about 7,200 Marine Elite coolers. Sharp edges on the stainless steel latch attached to the cooler can pose a laceration hazard to consumers.

      Igloo Products has received eight reports of lacerations requiring stitches on the consumer's legs or arms.

      This recall involves Marine Elite coolers sold in white with stainless steel latches. The coolers were sold in four sizes: 72 quart, 94 quart, 128 quart and 162 quart.

      The coolers were sold at West Marine stores nationwide from January 2008 to July 2008 for between $80 and $375.

      Consumers should stop using the cooler immediately and contact Igloo Products Corp. to receive a free latch replacement kit.
