    Loyal Bedding Recalls Mattress

    Loyal Bedding Inc. is recalling about 2,700 mattresses that do not meet federal flammability standards.

    This recall involves all Loyal Bedding mattress sets (Mattresses and Mattresses with Foundations). They were sold in twin, full, queen, and king sizes. "Loyal Bedding" and "Classic Rest", "Designer Collection", or "Smart Loyal" are printed on a label at the foot of the top of the mattress.

    The mattresses were sold by retailers throughout Puerto Rico from July 2007 through February 2009 for between $165 and $300. They were made in the U.S.

    Consumers should immediately contact Guaynabo Industrial for a store credit or a refund.

    Sony Recalls Computer AC Adapters

    Sony is recalling about 69,000 AC adapters supplied with its VAIO computers. Insulation inside the AC adapter can fail over time, posing an electrical shock hazard to consumers.

    Sony has received four reports of adapters short circuiting. No incidents occurred in the United States. No injuries have been reported.

    The items were sold by SonyStyle stores and Web site, authorized electronics retailers and authorized business-to-business dealers nationwide from September 2005 through October 2009 for between $900 and $3300 for desktop computers and $250 and $300 for docking stations. They were made in China.

    Consumers should turn off their computer, unplug it and stop using the recalled AC adapters immediately and contact Sony to arrange for a free replacement of the affected AC adapter.

    The recalled AC adapter model is the Sony VGP-AC19V17, which was supplied for use with the following Sony products:

    Kolcraft Iinfant Seats


    WASHINGTON, Dec. 20, 1999 -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), and the National Highway Traffic Safety Administration (NHTSA), Kolcraft Enterprises of Chicago, Ill., is recalling about 754,000 infant car seat/carriers for repair.



    When used as an infant carrier, the handle can unexpectedly move from the intended carrying position, causing the seat to suddenly rotate. When this happens, the infant can fall to the ground and suffer serious injuries.

    "Kolcraft is offering a free repair kit to address the problem," said CPSC Chairman Ann Brown. "It's an easy repair to make, and it is designed to protect your baby from a very serious injury."

    There have been more than 3,000 reports of handle-related problems, the majority of which report that the handle of the infant car seat/carrier unexpectedly moved from the intended use position, resulting in 42 injuries to infants. Some children were restrained in the carrier and others were not. These injuries range from a skull fracture and concussions to cuts, scrapes and bruises.

    "Riding in a child safety seat on the car's rear seat for every trip is the safest way to transport young children in a motor vehicle," said NHTSA Acting Administrator Rosalyn Millman. "The recall announced today is not related to how the seat performs while in a car. Parents should continue using these carriers as car seats."

    The recall involves Kolcraft infant car seat/carriers with the model names: Infant Rider, Secura, Travel-About, Plus 4, Plus 5, Kolcraft Infant Restraint, Kolcraft Infant Car Seat and Playskool Infant Car Seat manufactured from January 1, 1993, through June 30, 1999. The five-digit model/style number and manufacture date, written as month/day/year and possibly the model name are written on a label on the side of the seat.

    Some car seat/carriers also were sold with strollers. If the first digits of the model/style numbers on your car seat/carrier match the numbers in the following list, then it is subject to this recall.     *There are a total of 50 Model/Style numbers affected by this recall. Juvenile product, mass merchandise and major discount stores nationwide sold the car seat/carriers beginning in January 1993 for about $30 to $60 when sold alone, or $100 to $150 when sold with strollers. Kolcraft will provide a free repair kit designed to make the handle more secure. The repair can be done easily in the home.

    Consumers should not carry the seat by the handle until it has been repaired. Consumers can continue to use the product as a car seat or in combination with the stroller. CPSC, NHTSA and Kolcraft remind parents and caregivers to always restrain infants when in the car seat/carrier according to instructions.

    To receive a free repair kit or for more information, call Kolcraft toll free at (877) 776-2609, 24 hours a day, seven days a week. Seats manufactured from July 1, 1999, have newly designed handle mechanisms and are not part of this recall.

    The Kolcraft Rock N Ride is not part of this recall.

      Vtech Toy Phones Recalled

      WASHINGTON, June 22, 2000 -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Vtech Industries LLC is recalling about 34,000 toy baby phones. The toy phones have a ball-shaped antenna which can detach, presenting a choking hazard to young children.

      CPSC and Vtech Industries have received nine reports of the plastic ball detaching from these phones. One child reportedly started to choke on one of these plastic balls.

      The toy is called Little Smart Soft Songs Baby Phone. The cordless baby phone has a mirror, four colorful talking musical buttons, a ringing light-up phone button and a red on/off button. The phone is yellow with a blue padded border and a yellow ball-shaped antenna. A label on these toys reads "Soft Songs Baby Phone 39100." Writing on the phone includes "VTECH" and "Little Smart Soft Songs Baby Phone."

      Wal-mart stores nationwide sold the toy phones from January 2000 through May 2000 for about $10.

      Wiggle Waggle Caterpillar toys recalled

      WASHINGTON, July 21, 2000 -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Child Guidance, of New York, N.Y., a division of JAKKS Pacific Inc., is voluntarily recalling almost 1 million Wiggle Waggle Caterpillar toys. Young children can choke on the small balls attached to these toys.

      CPSC has received one report of a 5-month-old girl in El Paso, Texas, choking to death after one of the small balls attached to these toys lodged in her throat. CPSC also received reports of two children starting to choke on a ball from this toy.

      The Wiggle Waggle Caterpillar is a pull toy that plays four songs and various sounds. It has five ball shapes raised from its body on the end of tightly wound coil wire springs. The two smaller ball shapes on the caterpillar's head resemble antennas. The toy is about 10 inches long, and is green with yellow oval wheels that cause it to wobble when it moves. The segments of the caterpillar's body have lights that flash to the music and sounds. "Child Guidance" and "MADE IN CHINA" are imprinted on the bottom of the toy. "Ages 12 months & up" is written on the packaging.

      Toy and discount department stores nationwide sold the caterpillar toys from May 1998 through June 2000 for about $13.

      Parents and caregivers should take these toys away from young children immediately. Consumers should send the toy back to the firm to receive another toy of similar value.       Kinderkids Playrings Recalled

      WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), KinderkidsTM, of Lawrence, Kan., is voluntarily recalling about 18,000 inflatable playrings. Babies sit inside the playring, which is used as an activity center and looks like a pool float. When a baby leans his face against the side of the playring, the soft surface covers the baby's nose and mouth, restricting airflow, and posing the risk of suffocation.

      CPSC has received one report of a 6-month old baby found gasping for air after falling asleep face down in the playring. The baby was removed from the playring without injury. CPSC is aware of infant suffocation deaths when their airflow is restricted from contact with soft surfaces.

      The recalled playring, which measures 40 inches in diameter, has a yellow, plastic, inflatable, inner tube with a multicolor cloth covering. The cloth covering features eight activities for babies, including a boat rattle, a mirror flower, a soft teether on a puppy's collar, a butterfly, and a squeak ball and fish. A tag on the bottom of the cloth covering reads "Warning!" and "Recommended for children 6 to 18 months." The packaging reads, "Discovery Playring," "MADE IN CHINA" and "Not to be used in water." They are sold with plastic foot pumps for inflating the playrings.

      Walmart stores, and One Step Ahead, a mail-order company, sold the playrings from July 1998 though April 2000 for between $20 and $35.

      Consumers should stop using the playrings immediately and contact Kinderkids for information on how to receive a refund. Consumers should call Kinderkids toll-free at (888) 433-6251 from 9 a.m. to 4 p.m. PT Monday through Friday.

      Race Car Collectibles

      WASHINGTON, Feb. 8, 2000 -- Action Performance Companies Inc. is recalling about 1,600 race car collectibles, displayed as illuminated race car panels.

      The Consumer Product Safety Commission said the fluorescent light base of the unit becomes extremely hot and can melt, presenting a fire hazard. No incidents have been reported.

      The race car is etched into a glass panel, measuring about 11 inches long, 6 inches wide, and .25 inch thick. The glass panel sits in a black plastic base, which is equipped with a fluorescent light and plug.

      The available glass panels include the Tony Stewart Home Depot Car, Jeff Gordon Dupont Car, Dale Earnhardt GM Goodwrench Car and Dale Earnhardt Jr. Budweiser Car. The recalled collectibles are model numbers W9927077, W9927308, W9927019, and W9927215.

      The model number is found on a sticker affixed to the "Action Racing Collectibles, Inc." box. Specialty stores nationwide sold the product during December 1999 for about $75.

      Consumers should stop using the fluorescent light bases immediately and contact the company to receive a free replacement base.       HP Computer Speakers Power Adapter Recalled

      WASHINGTON, May 16, 2002 -- Philips Electronics is voluntarily recalling about 93,000 speaker power adapters sold with Hewlett-Packard Co. (HP) business desktop computers. The speaker power adapter's power cord connector is not grounded, posing a potential shock hazard to consumers.

      HP received one report of a Philips speaker power adapter plug that was not grounded which was discovered by a consumer during an electrical inspection. No injuries have been reported.


      The speaker power adapters were included in HP's P1534A External Amplified Speaker sets. There is a label on the bottom of the speakers with the model number "P1534-60001." The package includes a black rectangular power adapter, which is about 3-inches high, 2-inches wide and 2-inches long. The adapter has two permanently attached power cords: one that connects to the light or dark gray speakers and a "T-junction" plug that connects to the computer. The speaker power adapter has "Model D12-1-A-950" and "Made in China" printed on a white label on the power adapter.

      HP and business computer retailers sold personal computers with the speaker power adapters nationwide mostly to businesses. Some of these businesses then sold the computers to employees for personal use. HP's web site also sold the powered speakers and adapters with HP business desktop computer bundles, as well as packaged individually. The adapters were sold between October 2000 and April 2002 for about $34.

      Consumers should unplug the T-junction plug immediately and stop using the speakers. Consumers should contact Philips Electronics at (800) 870-7193 between 8 a.m. and 5 p.m. CT Monday through Friday for a free replacement speaker power adapter.

      Paramount Fitness Recalls Leg Curl Machines

      April 28, 2009
      Paramount Fitness Corp. is recalling about 150 leg curl machines. A consumer's hand can become caught between the cylindrical counter weight and the frame of the fitness machine, posing a crushing hazard that can result in lacerations and finger amputation.

      Paramount has received three reports of incidents, including a finger amputation and two finger-crushing injuries.

      This recall involves the Paramount Fitness PL2100 leg curl machine. The single station weight machine is designed to exercise the hamstring muscles. Serial numbers involved in this recall include: PL21-9301-000 through PL21-9510-099. The serial number is printed on the back of the weight stack frame on the lower right. Model number PL2100 is printed on the exercise procedure label on the front of the machine. The Paramount Fitness logo is printed vertically on the frame.

      The machines, made in the United States, were sold by Paramount Fitness and fitness specialty dealers nationwide from January 1993 through October 1995 for about $2,400.

      Consumers should immediately stop using the fitness machines and contact Paramount Fitness to receive a free repair kit.

      Empire Riding Toys Recalled

      WASHINGTON, Aug. 30, 2000 --In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Empire Industries, of Tarboro, N.C., is recalling about 113,000 battery-powered children's riding vehicles, marketed as "Power Drivers" or "Buddy L," for repair. The vehicles' battery chargers can overheat, presenting fire and injury hazards to children.

      CPSC and Empire have received about 100 reports of chargers overheating, melting or burning. Five house fires have been reported resulting in at least $100,000 in property damage. One child and one adult received burns to hands on overheated charger jacks.

      The Power Driver vehicles being recalled are powered by two 6-volt batteries, and were manufactured from July 1995 to December 1996. The vehicles are made of plastic and were sold under the following model names: Go-Kart, Beach Splash, Sun Dream, Trail Tracker, Rescue Patrol, and Chevy Z-71 Pick-Up Truck. The model name and Power Drivers appear on each vehicle. The company will help consumers identify if their model is part of the recall. Power Driver vehicles are intended for children 3 to 8 years old, and the vehicles' speed ranges from 2.5 to 5 mph, depending on the model.

      Toy stores and retail catalogs sold the vehicles nationwide from July 1995 through December 1997 for about $130 to $250.

      Consumers should stop using the vehicles immediately and contact Empire at (800) 872-1869 between 8:30 a.m. and 5 p.m. ET Monday through Friday to receive a free, new charger adapter with an overcurrent fuse. Consumers should not charge the vehicles or let children use the vehicles until they have received the new charger adapter with fuse.

      John Lennon Crib Mobiles Recalled

      WASHINGTON, Oct. 27, 2000 --The Betesh Group is voluntarily recalling about 47,000 "John Lennon" crib mobiles. The screws that connect the mobile's arm assembly and crib clamp can become loose if overtightened and tThe arms can detach and fall into the crib, injuring the baby inside.

      The recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC).

      CPSC and The Betesh Group have received seven reports of bumps and scratches to babies after the mobiles attached to their cribs fell on them.

      John Lennon Musical Mobiles hang from white wooden dowels that attach to cribs with white clamps. "John LennonTM" and "MADE IN CHINA" is written on the wind-up music box that turns the mobile.

      The music box plays the song, "Imagine." The mobile has colorful wooden cut-outs of a rhinoceros, two elephants, a giraffe and a bird hanging from yellow ribbons. Below each animal is a round disk with a drawing of the animal it hangs from on the bottom.

      Department and specialty stores sold these mobiles from June 1999 through August 2000 for about $39.

      Cottage Bunk Beds Recalled

      February 5, 2009
      The Land of Nod is recalling about 750 Cottage bunk beds. When screws are missing from the upper bunk's guardrails, the vertical slats on the guardrails can detach and pose the risk of children falling when getting into or out of the upper bunk.

      The firm received two reports of vertical slats detaching from the upper bunk guardrails. No injuries have been reported.

      The Cottage Bunk Beds can be configured as bunk beds or as twin beds. The assembled bunk bed dimensions are 81.5 inches (length) by 43.5 inches (width) by 74.5 inches (height). The beds were sold in both white (model #s 236-519 and 188-174) and pine/honey finishes (model #s 459-410 and 172-596) and included a long and a short upper bunk guardrail, each of which should be secured to the inside of the upper bunk bed's side rail with screws. Stickers bearing the model numbers are located on the inside of the lower bunk headboard.

      The beds were sold through the Land of Nod catalog, Web site www.landofnod.com and at The Land of Nod stores in Illinois and Washington state from January 2006 through January 2009 for about $1,100. They were made in Brazil.

      Consumers should check that all screws that attach the guardrails to the upper bunk have been installed. If the screws have not been installed, stop using the beds as a bunk bed immediately and contact The Land of Nod to receive new screws free of charge. All known purchasers have been notified of the recall.

      Cottage Bunk Beds Recalled...

      Ti-Dee Helper Toy Brooms Recalled

      WASHINGTON, March 6, 2001 -- EMSCO is voluntarily recalling about 2,200 children's toy brooms that are contaminated with lead.

      The recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC).

      The Ti-dee Helper brooms are about 37 inches long with a wood handle painted yellow and natural straw bristles. A silver label on the handle reads, "MADE IN MEXICO." The plastic packaging that covered the broom bristles reads, "Ti-dee HelperTM child's broom," and contains the model number "6089." Toy, discount and grocery stores sold these brooms from April 2000 through February 2001 for about $9.

      The paint on the handles of these brooms contain high lead levels. CPSC standards ban toys and other children's products containing high levels of lead. Young children could ingest lead from the broom handles, presenting a lead poisoning hazard.

      The Connecticut Department of Consumer Protection alerted CPSC to this product hazard.

      Ti-Dee Helper Toy Brooms Recalled...

      Crosley, Frigidaire, Kelvinator, Kenmore, Wascomat, and White-Westinghouse Clothes Washers Recalled

      Frigidaire is recalling about 35,000 clothes washers sold under the Crosley, Frigidaire, Kelvinator, Kenmore, Wascomat, and White-Westinghouse brand names. An internal defect in the washer's drain pump can cause heat to build up, posing a fire hazard to the consumer.

      This recall involves the Crosley, Frigidaire, Kelvinator, Kenmore, Wascomat, and White-Westinghouse top load washers, 3.1 cubic foot front load washers and laundry centers. A complete listing of model names and serial numbers appears below.

      The washers, made in the U.S., were sold by Sears and other retailers nationwide from February 2009 through May 2009 for between $300 and $1100.

      Consumers should immediately unplug and stop using the machine. Consumers should contact Frigidaire or Sears (if unit was purchased there) to schedule a free repair.

      Rand Bicycle Helmets Recalled

      WASHINGTON, July 5, 2000 -- Rand International is voluntarily recalling about 70,000 bicycle helmets. The helmets fail impact testing and labeling, violating the Consumer Product Safety Act. Riders wearing the helmets are not properly protected from falls, and could suffer severe head injuries.

      The recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC). CPSC and Rand International said they are not aware of any injuries or incidents involving the helmets

      Kmart and Rose's stores nationwide sold these bicycle helmets from April 1999 through March 2000 for about $8.

      The recalled helmets are various models of the L.A. Cruisin' bicycle helmets. The helmets were sold in various child, youth and adult sizes. The youth and adult-sized helmets are white or black and the child sizes are either blue or pink. A label inside the helmet reads, "Manufactured by Rand International," and MADE IN CHINA." Writing on the packaging reads, "RAND," "L.A. Cruisin'," and "Meets CPSC & ASTM Standards78."

      Consumers should stop wearing the helmets immediately, and return them to Rand International for a free replacement helmet.       Cumberland Outfitters Girls' Hooded Sweaters

      Ely and Walker is recalling about 580 Cumberland Outfitters girls' hooded sweatshirts. The sweatshirts have a drawstring through the hood which can pose a strangulation hazard to children.

      This recall involves Cumberland Outfitters girls' hooded sweatshirts with drawstrings. The sweatshirts were sold in four colors (melon, white, purple, and turquoise) and in sizes XS to L. Graphic designs such as a horse, vine or flower are printed on the front and back of the sweatshirts. RN number 14119 and one of the following style numbers are printed on the care label: 561560EC, 561565CO, 561570RA and 561575TU.

      The sweaters, made in India, were sold at department and apparel retail stores nationwide from April 2007 through April 2009 for between $19 and $24.

      Consumers should immediately remove the drawstrings from the sweatshirts to eliminate the hazard. Consumers can also return the garment to either the place of purchase or to Ely and Walker for a full refund.

      Browning and Rocky Mountain Elk Foundation Knife Sheaths Recalled

      Browning and the Rocky Mountain Elk Foundation are recalling about 10,000 skinner knife sheaths.

      The knife can cut through the sheath because there is no blade guard in the sheath. This poses a laceration hazard to consumers.

      The recall involves the Browning and Rocky Mountain Elk Foundation co-branded skinner knife sheath (model 322087). The sheath is black with the "Rocky Mountain Elk Foundation" logo stitched on the front. The packaging is black and yellow with "Browning" printed on the side and the model number printed above the UPC. No other Browning or Rocky Mountain Elk Foundation products are included in this recall.

      The sheath and knife, made in China, were given away as a promotional item for new membership in the Rocky Mountain Elk Foundation from November 2008 through April 2009.

      Consumers should stop using the sheath immediately and discard it. Consumers with the recalled sheath will be notified directly and will be sent a new one free of charge.

      Custom Buoyancy Inc.

      WASHINGTON, Jan. 30, 2002 -- Custom Buoyancy Inc. (CBI) is voluntarily recalling about 7,700 overpressure valves (OPV) used with buoyancy control systems. The overpressure valve can stick in the open position, posing a drowning hazard to divers.

      The recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC). CPSC and CBI have received three reports of the valves sticking open. No injuries have been reported.

      The recalled overpressure valve is installed on buoyancy control systems with the following brand names: Diving Unlimited International (DUI), International Divers, Ocean Management Systems, Rip Tide and Steam Machines. Brand names and logos are printed on the epaulettes and pockets. These buoyancy control systems come in two versions, a jacket style and a wings style. The date code, printed on an instructional label sewn to the buoyancy control system, is the first four numbers after the brand name abbreviation. The first two numbers indicate the month and the second two numbers indicate the year. The recall includes date codes from October 2000 through June 2001. Valves with an asterisk below the serial number are not involved in this recall.

      Authorized CBI distributors sold the buoyancy control systems with the overpressure valves nationwide from October 2000 through November 2001 for about $590.

      Consumers should stop using the buoyancy control systems with these overpressure valves immediately and contact the CBI dealer where purchased for a free replacement valve.       General Motors is recalling about 1,200 Chevrolet Camaros equipped with V8 engines from the 2010 model year. The positive battery cable may contact the starter motor, causing wear on the cable and, eventually, a short.

      KB Toys Recalling Electronic Light N' Learn Activity Gyms

      WASHINGTON, June 14, 2001 -- KB Toys is recalling about 115,000 Electronic Light N' Learn activity gyms. Five detachable hanging rattle toys have small round pegs at the top of the toys that can break off, posing a choking hazard to young children. The recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC).

      CPSC and KB Toys have received six reports of the pegs breaking off, including two reports of a 6- month-old-boy and a 14-month-old-boy beginning to choke off of a broken piece of the rattle attachment.

      The Electronic Light N' Learn activity gym is a multi-colored toy that converts into three different toys, including a crib toy, a floor gym and a musical keyboard. The gyms' main console has five buttons with numbers that make music when pressed and automatically light up when the continuous song play option is selected. Five detachable rattles in the shape of a bear, snail, star, half-moon and horse hang from the bottom of the console. A label under the handle on the gyms' main console reads in part "ELECTRONIC LIGHT N' LEARN ACTIVITY GYM MODEL: 8735, MADE IN CHINA."

      KB Toy store, KB Toy Works, KB Toy Outlet, Big Lots, Odd Lots, Pic N Save and MacFrugals nationwide sold the toys from September 2000 through January 2001 for between $20 and $30. The toys also were sold online at KBKids.com.

      Consumers should take these toys away from young children immediately and return the toy to the store where purchased for a refund. Consumers who purchased the toy online, should return the toy to the closest KB Toy store or contact KB Toys for return information.
