    "Dora the Explorer" Children's Book Recalled

    September 17, 2003

    Simon & Schuster is recalling about 26,000 copies of its "Dora the Explorer" Children's Board Book. A plastic replica of a balloon attached to the book can detach, posing a choking hazard to young children.

    No injuries have been reported.

    The heavy-cardboard book has "Whose Birthday Is It?" printed on the cover in yellow letters. Dora the Explorer cartoon characters are pictured on each page. A cutout in the top left corner of book has a small plastic replica of a balloon attached. The yellow and orange balloon is used as a slide to reveal answers to questions in the book.

    The books were sold at discount department stores and bookstores nationwide during August 2003 for about $10.

    Consumers should take these books away from young children immediately and contact Simon & Schuster at the Consumer Contact number indicated below to receive a replacement book.

    Cervlo Cycles Recall Bicycle Forks

    July 31, 2003

    Bicycle forks made by Cervlo Cycles Inc. are being recalled. The forks on the bicycles can develop cracks near the crown area resulting in failure of the fork and posing a risk of injury to the rider.

    The recall involves "Wolf" all-carbon road bike forks with a 1-inch diameter threadless steer tube. The bicycle forks were sold with 2003 model Soloist Team and Super Prodigy bicycles and framesets. A 17cm "Cervlo" logo is printed in white outlined letters on both fork legs.

    The forks were sold at bicycle specialty stores nationwide from April 2003 through July 2003 for about $3,000.

    Consumers should return the bicycles with the recalled forks to the dealer where purchased or contact Cervlo's recall hotline to locate the nearest service dealer to receive a free replacement.

    Erwin Beanie, Poseidon Beanie Knitted Hats Recalled

    Ambler Mountain Works is recalling Erwin Beanie and Poseidon Beanie children's knitted hats. The eyeballs on the Erwin Beanie and the octopus legs on the Poseidon Beanie can come loose, posing a potential choking hazard.

    The firm is aware of one incident involving the detachment of the legs of the octopus. No injuries have been reported.

    The Erwin Beanie is primarily green and white yarn knitted to configure a frog with white eyeballs and a red smile. The Poseidon Beanie comes in several colors of yarn and has hand sewn turtles, whales and octopus attached to the beanie. The beanies have the firm's logos 'Ambler' or 'A' printed on a fabric tag.

    The beanies, made in Nepal, were sold at specialty sports and ski shops nationwide from September 2008 through November 2008 for about $30.

    Consumers should immediately stop using the beanies and return them to Ambler Mountain for a free product replacement or cut off the frog's eyes and the octopus legs and permanently dispose of them.

      Fisher-Price Scooter/Mini Bike Recall

      November 13, 2003

      Fisher-Price is voluntarily recalling about 30,000 electric scooters and about 55,000 electric mini bikes. The motor control circuits can malfunction causing the scooters and mini bikes to continue to run after the power or throttle button is released, posing a risk of injury to children.

      Fisher-Price has received 56 reports of incidents with the scooters, including one report of a chipped tooth and one report of a broken arm. Fisher-Price has received 24 reports of incidents involving the mini bike, including one report of a leg laceration.

      The recalled Lightning PAC Scooters and MX3 Mini Bikes are battery powered ride-on toys designed for children ages six years and older. The product names are located on the side of the toy. The recalled scooters and mini bikes have model numbers of 73530 (Lightning PAC) or 73535 and B2222 (MX3 Mini Bike), which can be found inside the battery compartment. The recalled toys were manufactured in China.

      Mass merchants and toy stores nationwide sold scooters between November 2001 and October 2003 for about $250 and sold mini bikes between May 2003 and September 2003 for about $200.

      Consumers should take the toy vehicle(s) away from children immediately and contact Fisher-Price at (800) 528-7153 anytime or log on to www.service.mattel.com to receive information on how to have their toys serviced for free at a certified Power Wheels Service Center. The Service Centers will make modifications to the toy vehicles' circuitry.

      Novelty Cigarette Lighters Recalled

      July 17, 2003

      Novelty cigarette lighters distributed by De Bon Sales of Houston are being recalled. The novelty lighters are not equipped with child-resistant mechanisms. They pose fire and burn hazards to young children.

      There are two styles of lighters included in this recall. The first is a chrome and silver lighter, in the shape of a rearing horse. It is about 2.3 inches long and is labeled "DE-L-36." The second lighter is silver-colored, in the shape of a bird head holding a snake. It is about 2.5 inches long and is labeled "DE-L-23."

      The lighters were sold at Dollar stores in Texas from November 2002 through December 2002 for about $1.

      Consumers should return the lighters to the store where purchased for a refund.

      "Candle and Soap Making For Dummies" Recalled

      October 15, 2003

      John Wiley & Sons has recalled its "Candle and Soap Making For Dummies" book. The instructions in the book for making lye combine sodium hydroxide and water in an incorrect order. This could cause the mixture to bubble over, posing a burn hazard.

      The paperback book is titled "Candle & Soap Making For Dummies." The book's cover is black and yellow and has a photograph of candlesticks and slices of soap.

      The books were sold at bookstores and discount department stores nationwide from August 2003 through September 2003 for about $20.

      Consumers should return these books to the store where purchased for a full refund.

      Trudeau Fondue Set Recall

      March 13, 2003

      Trudeau Corp. is voluntarily recalling about 45,200 fondue pots sold as a part of a 17-piece set at Bed, Bath and Beyond stores. The plastic handles that are attached to the fondue pots can melt and crack during heating on a range top, posing burn and fire hazards to consumers.

      Trudeau has received 13 reports of the plastic handles breaking or melting, which involve 6 reports of burn injuries to hands, feet, and other body parts.

      The recalled fondue pots are labeled on the box as model number 82-384. The pots are brushed stainless steel, with a brushed stainless steel fork guide on the rim of the pot. The sets included six black stoneware sauce/condiment bowls and six chrome-plated forks with color-coded tips. There is a black triangular-shaped handle with a center hole on either side of the fondue pot. The fondue pot sits on a black, four-legged stand, which houses the burner. The fondue sets were sold under the brand name "Trudeau."

      Bed, Bath and Beyond stores nationwide sold the fondue sets from August 2001 through July 2002 for about $20.

      Consumers should immediately stop using the fondue set and contact Trudeau at (877) 659-4844 anytime to receive a replacement fondue pot with handles that will not melt or crack, and an incentive gift set of six stoneware fork rests.       Little Tikes Recalls Clubhouse Swing Sets

      July 30, 2009
      Little Tikes is recalling about 2,000 clubhouse swing sets. The recalled swing sets did not come with assembly directions for the swing seat harness. The swing seat harness assembly needs to be completed by the consumer. The swing seats can detach if the harness is not assembled properly, which could result in fall and injury during use.

      The Little Tikes Clubhouse Swing Set is a plastic molded product that features a climbing wall, rope ladder, slide and two swings with blue and yellow seats. Only the blue seat model is being recalled. The model numbers are 400V and 612398.

      The sets, made in the U.S., were sold at various retailers including Toys R Us and Walmart from October 2008 through March 2009 for about $450.

      Consumers should stop using recalled swing sets immediately until they have obtained and carefully read the assembly instructions for the swing seat harness. Instructions can be obtained by accessing the Little Tikes Web site or by calling the firm's hotline.

      MarineTech Recalls Tabletop Rope Cutters

      July 3, 2009
      MartineTech is recalling about 450 tabletop rope cutters. They can overheat, posing fire and burn hazards. The firm has received four reports of the rope cutters overheating. No injuries or property damage have been reported.

      The recalled tabletop rope cutters are used for cutting and cauterizing rope. This recall includes MarineTech part number 75-7070 and West Marine part number 559419. The Marine Tech part number is located on the side of the rope cutter. Panther is printed on the rope cutter.

      The cutters, made in Taiwan, were sold at West Marine stores nationwide from December 2006 through April 2009 for about $100.

      Consumers should immediately stop using the rope cutters and contact MarineTech to receive a free replacement rope cutter. MarineTech is directly contacting purchasers of this product.

      Academy Sports and Outdoors Recalls Propane Heaters

      August 21, 2003

      About 40,000 Timber Creek propane heaters made by Academy Sports and Outdoors are being recalled. The heaters can emit high levels of carbon monoxide (CO), posing a risk of CO poisoning if used indoors.

      The heaters have a 6 inch heat reflector and operate with a disposable 16.4-ounce or 14.1-ounce propane tank, sold separately. The product has a chrome neck, a black "on/off" switch, and a black plastic base that houses the propane cylinder.

      The units were sold at Academy Sports and Outdoors in Alabama, Florida, Louisiana, Mississippi, Oklahoma, Tennessee and Texas from Sept. 2001 through May 2003 for about $20.

      Wincraft Recalls Sports-Themed Lamps

      October 21, 2009
      Wincraft Inc. is recalling sports-themed lamps sold by sports memorabilia stores. An electrical problem with the lamps can pose fire, burn and shock hazards to consumers.

      The recalled lamps feature logos from a professional or college sports team and rotate 360 so that the logo can be easily seen. Lamps feature team logos from the NFL, MLB, NBA, NHL, NCAA and NASCAR. The lamps have a black base with a light. The lamps measure about 6 inches wide and 13 inches high.

      The lamps, made in China, were sold during August 2009 for about $40.

      Consumers should immediately stop using the recalled lamps and contact Wincraft for instructions on how to receive a free replacement lamp. Wincraft is directly contacting consumers who purchased the lamps.

      Blast Balls Recalled

      WASHINGTON, Nov. 14, 2001 -- JA-RU Inc. is voluntarily recalling about 43,000 "Blast Balls" toys. When struck together, the toys create a cracking sound similar to that of a cap gun.

      The directions instruct consumers to strike one ball against the other in the palm of hand to create the cracking sound. When consumers strike the "Blast Balls" together, sparks can ignite, posing a burn hazard.

      The recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC).

      CPSC and JA-RU have received six reports of injuries, including minor burns to hands and fingers.

      The recalled toys are packaged as "Super Bang...Blast Balls". The balls are sold two in a pack, in a variety of colors.

      Convenience and novelty stores nationwide sold these toys from June 2001 through October 2001 for about $2.

      2009 Dodge Journeys Recalled

      March 24, 2009
      Chrysler is recalling about 17,000 2009 Dodge Journeys without the optional 115-volt auxilary power outlet. An unused connector for the auxilary power could become corroded and cause a short circuit.

      If the fuse for the affected circuit fails to blow, the connector could overheat and catch fire.

      Dealers will reroute the wiring and pull the fuse to remove power from the unused circuit.

      2009 Dodge Journeys Recalled...

      Game Tracker Climbing Sticks and Tree Steps Recalled

      WASHINGTON, Dec. 19, 2001 -- Game Tracker is voluntarily recalling 25,000 climbing sticks and tree steps. The buckles on the climbing sticks and tree steps can fail, posing a risk of serious injury to hunters.

      Game Tracker and the U.S. Consumer Product Safety Commission (CPSC) have received 16 reports of buckles failing, including one hunter who suffered a partial finger amputation and five others who suffered cuts, sprains, and bruises.

      The recalled Gorilla Ultralite Climbing Sticks, model number 3163, have the serial numbers 040501, 050501, and 060801, printed on the main brace of the stick. The climbing sticks come in three to a package, are gray in color, have black swivel fold down foot pegs, and a black strap and buckle system. The recalled Strap-on Tree Steps, model numbers 3152 and 3153, have serial number 2899 stamped on the side of the metal step. The tree steps have a black strap, with a metal J hook, black buckle, and a metal step.

      Hunting supply stores and catalogs nationwide sold these climbing sticks and tree steps from February 1999 through September 2001 for between $7 and $100

      Consumers should stop using these climbing sticks and tree steps immediately.       Pella Recalls Casement Windows Due to Impact Hazard

      August 25, 2009
      Pella is recalling about 6,500 casement windows from its Architect and 850 series. The hinge can break, causing the window sash to fall out, posing a risk of injury to persons beneath the window.

      The firm is aware of one report of the hinge breaking, causing the sash to fall. No injuries have been reported.

      This recall involves the Architect Series and 850 Series casement windows. The windows were sold in a variety of sizes and typically contain a heavier window glaze such as the HurricaneShield Impact-Resistant glass.

      The windows were sold by authorized distributors nationwide and Home Improvement Stores in Florida from September 2007 through April 2009 for between $500 and $2000. They were made in the U.S.

      Consumers should immediately stop using the window and contact Pella to schedule a free repair. Owners have received direct mail notification of this recall.

      Garage Doors Can Be Dangerous

      The most dangerous thing in your home? It may well be your garage door and its electric opener.

      Overhead garage doors can weigh as much as 600 pounds -- more than enough to crush and kill a child or small adult. The electric openers that move them up and down have safety interlocks that are supposed to protect anyone who gets in their way but Underwriters Laboratories (UL) says the devices don't always work as planned.

      It's especially dangerous to leave a garage door partially open -- perhaps to let a pet go in and out freely -- because when the open/close button is pushed, the door may unexpectedly go down rather than up.

      Panicky consumers may make matters worse by repeatedly pushing the open/close button, which can increase pressure on the trapped person. UL says it has received reports recently of four children being trapped under doors

      Some safety tips:

      • Never leave garage doors partially open. They should either be fully closed or fully open.
      • Always have the garage door in full view when pressing the down button.
      • Don't let children play with the remote control.
      • Test the door monthly. It should automatically reverse upon contact with any object on its way down.
      • NEVER pull the emergency release cord when the door is open. It may fall suddenly and uncontrollably. possibly injuring or killing anyone underneath it.
      • Keep the door balanced. The springs on the door should keep it from closing suddenly when the electric controller is not attached. If not, call trained service personnel.

      If you have a wooden door, consider replacing it with an aluminum door. They are much lighter and, thus, safer.

      Lamplight Farms Recalls Clear-Vue Torch Fuel Guards

      October 1, 2009
      Lamplight Farms is recalling about 720,000 flame guards for Clear-Vu torch fuel containers. The flame guards can malfunction and cause the containers to melt while the torch is in use, posing a fire hazard.

      Lamplight has received 35 reports of the fuel containers melting while the torch was lit including one report of a minor burn injury.

      The recall involves Clear-Vu Torch Fuel Containers sold as replacements. RC is stamped on the underside of the flame guard. Clear-Vu torch fuel containers that came with torches are not included in this recall.

      Discount, hardware, and home improvement stores nationwide sold the guards from January 2008 through August 2009 for between $1 and $3. They were made in China.

      Consumers should immediately stop using the recalled flame guards on Clear-Vu torch fuel containers and contact Lamplight to receive a free repair kit.

      Lamplight Farms Recalls Clear-Vue Torch Fuel Guards...

      BikeE Recumbent Bicycles Recalled

      WASHINGTON, Feb. 28, 2002 -- BikeE Corp. is voluntarily recalling about 13,500 BikeE recumbent bicycles. The recumbent seats on these bicycles can crack and break, causing the seat to come off the frame and resulting in injury to the rider.

      BikeE Corp. has received two reports of seats on these bicycles breaking while riding, resulting in one injury that included a broken leg.

      The recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC).

      The recall includes all 1999 through 2001 model BikeE bicycles with 27- inch-tall seat backs, silver-colored seat frames with black mesh seat backs. "BikeE" is written on the frame of these bicycles.

      BikeE-authorized dealers nationwide sold the recalled recumbent bicycles from February 1999 through February 2002 for between $550 and $2,150.

      Consumers should stop using these bicycles immediately, and contact BikeE for information on getting a free repair kit. BikeE dealers also have repair kits available.       Recumbent BikeE bicycles with seat backs lower than 27 inches are not included in this recall. Seats with silver-colored rings around the black plastic clamps that attach the seat to the frame of the bicycle have been repaired or are not part of the recall.

      CPSC and BikeE announced the recall of 700 recumbent tandem bicycles on November 14, 2001, because the steerer tube on the front forks could separate.

      Haier Toaster Oven/Broilers Recalled

      Haier America is recalling about 106,000 toaster oven/broilers. Electrical connections can become loose, posing electrical shock and burn hazards.

      Haier America has received two reports of minor burns and one report of a minor electrical shock to consumers, and one report of minor property damage.

      This recall involves toaster oven/broilers with model number RTO1400SS. The units are stainless steel and black plastic. Haier is printed on the front and the model number is printed on a label on the back of the toaster oven/broilers.

      The toaster ovens, made in China, were sold at mass merchandisers and specialty retailers nationwide from September 2006 through January 2009 for approximately $20 to $40.

      Consumers should immediately stop using the recalled toaster oven/broilers and contact Haier America to receive a free replacement toaster oven/broiler.

      HDC Electrical Products - extension cord, portable fluorescent light, work light

      June 19, 2003

      Homier Distributing Co. is recalling HDC extension cords, portable lights and fluorescent work lights are being recalled because of undersized wiring, improper polarization and lack of overcurrent protection. The defects can cause overheating, posing a shock and fire hazard to consumers.

      The company has received one report of an individual who sustained an electric shock and burn injury when using the fluorescent work light near his car.

      Several hundred thousand units were sold at traveling sales shows nationwide, the company's two retail stores (in Huntington and Lafeyette, Ind.) and online at www.homier.com from January 2000 through May 2003 for between $3 and $7.

      Consumers should stop using the product immediately and mail to Homier Distributing Co., Customer Service Center, 100 Commerce Drive, Huntington, IN 46750 for a full refund, including the cost of shipping.

      Undersized wiring, are not properly polarized, have inadequate grounding, faulty electrical conections, and plastic handles that once ignited continue to burn and spread flames, posing a shock, electrocution and fire hazard to consumers. The lights have a silver metal shroud, an orange plastic handle with a three-prong receptacle for an extension cord, and a 12-ft. orange cord. The metal shroud has the following labeling stamped into it, CAUTION AVOID HAZARD PULL PLUG WHEN RELAMPING - USE ONLY 75 WATT OR SMALLER BULB.
