    Healthtex Zoo Pacifiers

    Healthtex is recalling about 40,000 Zoo pacifiers because they fail to meet federal safety standards. The nipples can separate from the base easily, posing a choking hazard to young children.

    This recall involves Zoo pacifiers with animal cartoon figures as handles. The pacifier has a round-shaped mouth guard with three ventilation holes. Made in Spain is printed on the handle side of the mouth guard and Mi Tosoro is printed on the cardboard packaging.

    The pacifiers were sold at supermarkets and pharmacy stores nationwide from December 2002 through March 2009 for about $1.40.

    Consumers should immediately take the recalled pacifiers away from children and contact Healthtex for a refund or exchange.

    2002-2003 Jeep Liberty Recall

    Chrysler is recalling about 4,300 2009 Jeep models including the Commander, Grand Cherokee and the Wrangler 4-door 4x4. Wiring for the driver's airbag may be reversed, which could cause the airbag to fail.

    Dealers will inspect the airbag wires and replace the control module if necessary. The recall is expected to begin in May 2009. Owners may contact Chrysler at 1-800-853-1403 regaring Recall No. J13.

    2008 Tesla Roadster Recalled

    Tesla Motors is recalling 345 of its 2008 roadsters manufactured between March 2008 and April 22, 2009.

    The company said the rear hub flange bolts may be under-torqued, causing it to become loose, resulting in degraded vehicle handling.

    Tesla will notify owners and repair the vehicles free of charge. Owners may contact Tesla at 1-877-888-3752.

      Telephone Line-Sharing Devices Recalled

      September 10, 2003
      OnQ telephone line sharing devices manufactured by OnQ Technologies are being recalled. A security system connected to this device could be prevented from notifying emergency personnel.

      The recalled units are labeled "OnQ" and "1x8 ENHANCED TELECOM w/SURGE." The devices are professionally installed as part of a telephone line-sharing system in homes. One of the slots on the device is labeled, "SECURITY." The security line should get priority in an emergency.

      Part number 364551-01, written on the product label, and a date code (DC) of "2303" (meaning the 23rd week of 2003) or earlier are included in the recall. The date code is located on the edge of the device, above the mounting tabs.

      The units were sold by distributors of electrical devices and sold by professional installers nationwide and in Canada between November 2002 and June 2003 for about $80.

      Consumers should contact OnQ to receive free replacement of the telephone line-sharing device. Call OnQ at (800) 321-2343 between 8 a.m. and 7 p.m. ET Monday through Friday.

      NESCO Deep Fryer Cover Recall

      November 6, 2003

      The Metal Ware Corp. is recalling 47,000 deep fryers. The mesh filter in the cover of the deep fryer can allow moisture to build in the oil reservoir, causing the oil reservoir to boil over during or shortly after use. This poses a burn hazard to consumers.

      There have been eight reports of incidents, including one report of minor burns requiring medical attention.

      The recall involves NESCO brand, stainless steel deep fryers with a black plastic top and handles on either side. The model number is printed on the back of the product. Model numbers included in the recall are DF-250T, DF-1250T and DF-1250TL.

      The fryers were sold at discount department stores and retail Web sites from May 2001 through October 2003 for about $55.

      Consumers should stop using the deep fryers immediately and contact The Metal Ware Corp. to request a free replacement cover kit and complimentary gift package.

      Franzus Co. International Adapter Plugs

      July 2, 2003

      Franzus Company is recalling about 29,000 international adapter plugs. The adapter plug can separate, exposing live electrical conductors, posing an electrocution and shock hazard to consumers.

      The Franzus Co. has received two reports of the plugs separating. No injuries have been reported.

      The black or white plastic adapter plugs have names of specific countries or regions where the plugs are intended for use written on the side. The writing includes "NORTHERN EUROPE," "GREAT BRITAIN," "NO. AMERICA," "AUSTRALIA," and "SO. EUROPE." Except for the names of these countries and regions, the recalled plugs have no other writing on them. Plugs with "MADE IN USA" printed between the prongs, or that have a screw between the prongs, are not included in the recall.

      Catalog and retail stores nationwide sold the adapter plugs in sets of five from April 2002 through June 2003 for between $10 and $40.

      Gorilla Inc. Recalls EXO-Tech Safety Harness

      Three Sisters Toys is recalling the Yayita baby hammock. The hammock can flip over, posing a serious fall hazard and strangulation hazard to infants who become entrapped in the seat's restraint straps while upside down.

      The recalled baby hammock is a cream-colored canvas and hangs from a wooden pole. The hammock measures about 29 x 43 inches. It has three restraint straps with red buckles and is attached to a cushioned pad for the infant. A mounting fixture, also called a fixing set, is sold with the hammock. 'La Siesta' is printed on a tag located below the restraint straps. Model number YABN-1 is printed on a white tag sewn into the hammock's side seam.

      The hammocks were sold by Three Sisters Toys online at www.threesisterstoys.com from December 2007 through July 2008 for about $100. They were made in Colombia.

      Consumers should immediately stop using the baby hammock and return it to Three Sisters Toys to receive a full refund. The firm has contacted all known purchasers of this product.

      "Spit Smatter" Spray Foam Recall

      July 2, 2003 -- About 1.3 million units of "Spit Smatter" Spray Foam are being recalled. The aerosol cans can forcefully break apart, posing a risk of serious injury to nearby consumers.

      The manufacturer has received four reports of the aerosol cans breaking apart, including one incident that resulted in a laceration injury to a consumer and several incidents involving property damage.

      The "Spit Smatter" spray comes in a pressurized can and emits a colored foam based on one of six brands: "Original Smatter," "Blueberry Smatter," "Banana Cream Smatter," "Lemon Meringue Smatter," and "Fatter Smatter." The brand name is printed on the can and the word "Nickelodeon" is printed on the orange trigger mechanism.

      The cans were sold at discount department and toy stores nationwide sold the "Spit Smatter" foam cans between February 2002 and June 2003 for about $10.

      Remedy: Consumers should immediately take the aerosol can away from children and contact the company for instructions on disposal and how to receive a full refund.       Wal-Mart Recalls 'Your Zone Loft Collection' Entertainment Stands

      Ameriwood Industries is recalling about 195 "You Zone Loft Collection" entertainment stands sold at Wal-Mart. Warning labels sold with the entertainment stands indicate incorrect size and weight limits for televisions used with the stands. A television that is too heavy or wide for the entertainment stand can make the unit unstable, posing a tip-over hazard to consumers.

      This recall involves Your Zone Loft Collection entertainment stands. The stands are intended to be used as gaming centers. The recall includes Autumn Beech YZ084-104-13-19 stands with model number 1156020WY. Model information is printed on the instruction manual.

      The standards, made in Canada, were sold exclusively at Walmart stores nationwide from May 2009 through July 2009 for about $100.

      Consumers should immediately check the size and weight of the television used with the entertainment stands. If the television exceeds 50 pounds or does not sit entirely on top of the stand, consumers should immediately remove it from the stand. CRT televisions larger than 20 inches and flat panel televisions larger than 27 inches should not be placed on the stands. Consumers should contact Ameriwood for a replacement warning label.

      18 Kick Body Boards Recalled

      "18 Kick" body boards are being recalled. The green surface coating of the screened logo "Big Lizard" body boards on the underside of the body board contains excessive levels of lead, violating the federal lead paint standard.

      This recall involves the 18 Kick Body Board. The boards have various designs on the front including peace signs, white hibiscus, or yellow smiley face. A green lizard and "Big Lizard body boards" are printed on the underside in green.

      The boards, made in China, were sold at Beachwear Outlet, Bargain Beachwear, Beach Village Inc., FJ Health & Beauty, Mor USA Inc., Shipwreck-Ocean Jewels, Walmart and Wilcor International Inc. from January 2009 through May 2009 for about $4.

      Consumers should take the recalled body boards away from children immediately and return them to place of purchase or contact JGR Copa for a full refund.

      Top Goods Flashing Pacifiers Recalled

      Top Goods Trading is recalling about 38,000 flashing pacifiers which do not comply with federal safety standards. The pieces of the pacifier can separate, posing a choking hazard. The necklaces pose a strangulation hazard.

      The recalled flashing pacifier has a yellow heart-shaped mouth guard and handle. There is a 15-inch cord attached to the handle. LED lights are activated by pushing the heart-shaped handle into the base of the pacifier.

      They were sold at retail stores in the Los Angeles area from November 2007 through November 2008 for about 50 cents and were made in China.

      Consumers should immediately take the recalled pacifier away from children and contact Top Goods Trading for a refund or exchange.

      Easter Oil Lamp Recall

      September 10, 2003

      DesignPac is recalled about 500 easter oil lamps sold at Target Stores. The lamp can tip over easily, posing a fire hazard.

      The lamps are oval glass bottles with purple or pink dots. They are filled with blue or pink liquid paraffin. The lamps were sold with bunny-shaped tags identifying them as "hoppy go lucky" oil lamps. Item number 054-03-1843 or 054-03-1844 is printed on the tag. Some of the lamps were sold with round metal disks meant to prevent the lamps from tipping.

      Consumers should return the oil lamp sets to any Target store for a refund.

      The Home Trends Kiddy Sling Chair sold at Wal-Mart is being recalled. The small plastic bolt covers pose a choking hazard to small children.

      There have been two reports of children removing the small covers and placing them in mouths. No injuries have been reported.

      The chairs are steel outdoor chairs with a white frame and a pink or blue sling fabric seat and back. The chairs measure about 9.5 inches in height. The seats have two small plastic covers located behind the back of the seat and two covers under the seat.

      Wal-Mart stores nationwide sold these chairs from Nov. 2002 through May 2003 for about $13.

      Sun Kids and Happy Baby Baby Walkers Recalled

      September 10, 2003

      SunTech Enterprises is recalling about 4,100 "Sun Kids" and "Happy Baby" baby walkers.

      The walkers will fit through a standard doorway and are not designed to stop at the edge of a step. Babies using these walkers can be seriously injured or killed if they fall down stairs.

      The recalled baby walkers are made with a plastic frame supported by six or eight wheels on the bottom. The walkers are blue, pink or bright green with a padded seat and an activity tray. "SUN KIDS" or "HAPPY BABY" labels appear on some of the walkers.

      The walkers were sold at small retailers and flea markets in Texas and California from November 2002 through April 2003 for between $15 and $25.

      Falls Creek Army Flight Jackets Recalled

      November 24, 2003

      Meijer Inc. is voluntarily recalling about 7,500 children's Falls Creek Army Flight Jackets with hooded drawstrings.

      Children can get entangled and strangle in the drawstrings that catch on objects, including playground equipment, fences and tree branches. There have been numerous deaths from similar neck and hood drawstrings.

      To help prevent children from strangling by their clothing drawstrings, in 1996 the U.S. Consumer Product Safety Commission issued guidelines for drawstrings on children's outerwear, which were subsequently incorporated into a voluntary standard.

      Meijer has not received any reports of incidents. This recall is being conducted to prevent the possibility of injuries.

      The recalled Falls Creek Army Flight Jackets are for boys size 4 through 7 and 8 through 18, as well as toddlers sizes 2T through 5T. These jackets are made of blue or olive nylon fabric and are decorated with assorted patriotic patches on the sleeves, chest and back. The hood is bordered with synthetic brown and tan fur. These jackets were manufactured in Korea.

      Meijer stores in Michigan, Ohio, Indiana, Illinois, and Kentucky sold the jackets from July 2003 through October 2003 for between $26 and $33.

      Consumers should stop their children from wearing the jacket immediately and contact Meijer toll-free at (866) 280-8419 anytime or log on to their Web site at www.meijer.com for information on how to return the jacket for a full refund. Parents also can remove the drawstring, which will make the jacket safe for children to wear.

      Crate and Barrel Recalls Hanukkah Menorah

      Crate and Barrel is recalling a Hanukkah menorah sold in its stores and Web site from October through December 18 for about $25. The acrylic base of the menorah can ignite if the candles are allowed to burn completely, creating a fire hazard.

      One incident has been reported.

      The menorah has a clear acrylic base with brass candle cups. The menorah accommodates nine standard menorah candles. Its SKU number is 589-055.

      Consumers should stop using the menorah immediately and return to the store for a full refund. You may mail the menorah to Crate and Barrel and receive a refund check.

      Speedy's Chldren's Hooded Zipper Jackets

      About 80,000 Speedy children's hooded zipper jackets are being recalled. The jackets have a drawstring through the hood, which can pose a strangulation hazard to children.

      This recall involves Speedy children's hooded zipper jackets and hooded pullover jackets with drawstrings. The jackets were sold in various colors (black, white, charcoal gray, heather gray, burgundy, green, royal blue, navy blue, baby blue, red, pink, brown and in sizes S-XXL. 'Speedy' can be found on a label located on the inside of the collar.

      The jackets were sold at sportswear and clothing retail stores in the Los Angeles area from December 2004 through December 2008 for about $10. They were made in the U.S.

      Consumers should immediately remove the drawstrings from the sweatshirts to eliminate the hazard, or return the garment to either the place of purchase or to Speedy for a full refund.

      Rothschild & Co. Recalls Girls' Hooded Winter Jackets

      March 20, 2003

      S. Rothschild & Co. is voluntarily recalling for repair about 37,000 girls' iridescent, hooded winter jackets. The rubber petal and metal snap can break off of the jacket, posing a choking hazard to young children.

      S. Rothschild has received two reports of the rubber petal coming off the jacket, including one child who reportedly put the petal in her mouth. The snaps have come off in subsequent testing.

      The recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC).

      The recalled jackets are blue, lilac, pink, raspberry and magenta and include sizes XS/2T - L4T, 2/4 - M/5-6 - L/6X, 3/6 - 6/9 - 12M - 18M 24M, and S/2T - M/3T - L/4T - XL/5T. The style numbers, which can be found on a tag attached to the left sleeve, are 52913, 52813J, 12913K, B2913, 32813, 12816C, 3281J, 82819D, 82816D, and 32816C. The jacket labels include the words, "Little Impressions," "Rothschild," "Izzi's Kids," or "Clockwise." The recalled jackets were made in India.

      Major retail stores nationwide sold the jackets from May 2002 through December 2002 for about $25 to $45.

      Viking Range Recalls Toasters

      Viking Range Corp. is recalling about 10,000 four-slice toasters. Wiring inside the toaster can become loose and contact the toaster body, posing a shock hazard to consumers.

      Viking has received five reports from consumers who experienced a minor shock when touching the outside of the toaster.

      The recall involves Viking Professional Four-Slot Toasters, model numbers VT400WH, VT400BK, VT400GG, VT400SG, VT400BR and VT400CB. The toaster has a label on its underside that states in part, Viking, UL Listed 73D3, Viking Range Corporation, Model VT400 Series.

      The toasters were sold by culinary products retailers, online dealers, and Vikings web site from April 2005 through November 2008 for about $300. They were made in China.

      Consumers should stop using the toaster immediately and unplug it, and contact Viking to obtain a free replacement toaster.

      Matchstick-Shaped Lighters Recalled

      May 8, 2003

      Multi-purpose lighters shaped like matchsticks distributed by Amen Wardy Home are being recalled. The lighters lack child-resistant mechanisms that meet federal safety standards. Young children could operate these lighters, which poses a fire hazard.

      No injuries have been reported.

      The lighters are refillable, gas-fueled lighters shaped like a wooden match. They have a brushed, silver-colored metal finish with a black plastic slide lever that is moved to ignite the lighter. The end of the lighter is painted red to resemble the head of a match. The lighters were available in both 7-inch- and 14-inch-long sizes. They were sold in a clear plastic sleeve and do not have any labeling on either the lighter or package.

      The lighters were sold at the Amen Wardy Home store in Caesar's Palace, at Las Vegas, Nev., and the firm's Web site sold them from July 2002 through January 2003 for between $10 and $20.

      Contact Amen Wardy Home for instructions on how to obtain a refund for these lighters. Dispose of the lighters in a manner that is in compliance with all state and local requirements. The lighter should not be incinerated or punctured.
