    First Samco Recalls Fobus Gun Holsters

    First Samco Inc. is recalling about 3,200 Fobus GLT gun holsters. A plastic or leather strap on the gun holster can catch the trigger of the gun when inserted into the holster causing the gun to unintentionally discharge, posing an injury hazard to the user.

    There have been eight reports of the Glock handgun unintentionally discharging when being inserted into the gun holster. One user injured his finger when his gun discharged.

    The gun holsters are designed to hold a Series 17 and Series 19 Glock handgun fitted with a laser-sight light. GL 2*EMZ is engraved in the top of the gun holster and FOBUS and MADE IN ISRAEL is printed on the back of the gun holster.

    This recall involves two early versions of the Fobus GLT gun holsters, one with a plastic retention strap less than one inch wide and one with a leather retention strap less than one inch wide. The new design includes a strap that is more than one inch wide with a plastic tip too wide to be caught inside the trigger guard.

    The holsters were sold at gun accessory retailers and distributors nationwide, as well as on-line at www.fobusholster.com between March 2002 and March 2003 for about $40 to $45.

    Consumers should stop using the gun holsters immediately and bring the recalled holsters to an authorized Fobus USA distributor for a free replacement gun holster or contact First Samco Inc. for instructions on how to return the product for a replacement item. Consumers also may send their recalled gun holsters to Fobus USA, 1300 B-3 Industrial Highway, Southampton, PA 18966. First Samco, Inc. will reimburse consumers for return shipping.

    People at risk for listeriosis and their family members or individuals preparing food for them should:

    Reheat until steaming hot the following types of ready-to-eat foods: hot dogs, luncheon meats, cold cuts, fermented and dry sausage, and other deli-style meat and poultry products. Thoroughly reheating food can help kill any bacteria that might be present. If you cannot reheat these foods, do not eat them.

    Wash hands with hot, soapy water after handling these types of ready-to-eat foods. (Wash for at least 20 seconds.) Also wash cutting boards, dishes, and utensils. Thorough washing helps eliminate any bacteria that might get on your hands or other surfaces from food before it is reheated.

    Do not eat soft cheeses such as feta, Brie, Camembert, blue-veined or Mexican-style cheese. You can eat hard cheeses, processed cheeses, cream cheese, cottage cheese, and yogurt.

    Do not drink raw, unpasteurized milk or eat foods made from it, such as unpasteurized cheese.

    Observe all expiration dates for perishable items that are precooked or ready-to-eat.

    Mainstay Garden Folding Chairs

    Rio Brands is recalling about 20,000 Mainstay Garden folding lawn chairs. The chair arms can break, posing a fall hazard to consumers.

    Wal-Mart has received 26 incident reports involving broken plastic arms. In 17 of these incidents, consumers have reported injuries such as a fractured wrist, torn ligament, minor back injuries, bruises and abrasions.

    The recalled folding lawn chairs have green plastic arms with green steel folding frames or blue plastic arms with blue folding steel frames. They are constructed of matching vinyl webbing. The chairs measure 31-inches high and 22.5-inches wide. The left hand arm of the chairs has the word RIO on it. Only the chairs with green or blue plastic arms are included in this recall.

    The chairs were sold at Wal-Mart stores nationwide from January 2004 through March 2004 for about $8.

    Consumers should immediately stop using the product and return the chairs to Wal-Mart for a refund.

      Mazda Miata Recall

      Mazda Motor Corporation is recalling the 2001 and 2002 Miata models to lower vehicle emission levels.

      Many of these cars could exceed federal emission standards because of inadequate emission control when the vehicle is started cold. EPA discovered the potential emission compliance problem in the 2001 and 2002 models after testing at its National Vehicle and Fuel Emissions Laboratory in Ann Arbor, Mich.

      Mazda will replace the vehicle's central processing unit to reduce emissions that come from cold starts. The calibrations will not have an effect on vehicle performance or fuel economy, the company said.

      The repairs will be made at no cost to vehicle owners.

      Hanna Andersson Childrens Corduroy Overalls

      November 24, 2004
      Hanna Andersson is recalling about 1,900 pairs of childrens corduroy overalls. The buttons on the front and side panels of the overalls can detach from the garment, posing a choking hazard to young children.

      The firm has received two reports of the buttons detaching from the garment. No injuries have been reported.

      The recalled corduroy overalls, style number 27267, are made of 100% cotton corduroy and sold in colors red and blue. They are finished with two buttons in the front that attaches to the shoulder straps, two buttons on each side, slash pockets, and a large bib pocket in the front.

      The overalls were sold exclusively through Hanna Anderssons catalog and Web site from July 12, 2004 through October 15, 2004 for about $38.

      Hanna Andersson will notify all known purchasers by mail regarding a refund.

      Paula Deen Cast Iron Cookware Recalled

      QVC, Inc. is recalling about 51,000 sets of Paula Deen cast iron cookware. The recalled cookware can crack or shatter, posing burn and laceration hazards to consumers.

      QVC and Meyer Trading have received 79 reports of the cookware cracking or shattering while heated. One consumer reported a minor burn injury to an arm.

      This recall involves the Paula Deen 11-inch cast iron grill pans and griddles. They were sold as QVC item numbers K14984, K11970 and K135024. The cookware is black and has the Paula Deen logo and the product size engraved on the bottom. No other Paula Deen cookware is involved in this recall.

      The cookware was sold through QVC's televised shopping programs, its Web site www.qvc.com, and QVC retail and employee stores from October 2007 through July 2009 for between $35 and $55. It was made in China.

      Consumers should immediately stop using the recalled cookware. Known purchasers were mailed instructions for obtaining a full refund. Consumers who purchased the cookware at a QVC store should return it to any QVC store for a full refund.

      Paula Deen Cast Iron Cookware Recalled...

      Childrens Costume Bracelets

      January 11, 2005
      Riviera Trading Co. is recalling about 7,100 metallic costume bracelets sold at Belk, Bloomingdales, Carson Pirie Scott, Kohls, Parisians and Proffitts Department stores nationwide from October 2003 through November 2004 for about $6.

      The bracelets contain high levels of lead and pose a lead poisoning hazard to children.

      The recalled bracelets have heart, oval, and rectangular shaped charms that have the phrases; I like sports, I like movies, I like shopping and I like music printed on them. The bracelet also contains various colored plastic trinkets.

      Consumers should immediately take these bracelets away from young children and contact the company to receive a refund.

      Evenflo Recalls Cake Toys on Childrens Activity Centers

      December 8, 2009
      Evenflo is recalling about 66,000 cake toys sold as part of the Evenflo ExerSaucer 1-2-3 Tea for Me activity centers. The candle flame attached to the top of the cake toy can detach, posing a choking hazard to young children.

      Evenflo has received 11 reports of the toy flames detaching. Five of the incidents occurred in the United States and six in Canada. No injuries have been reported.

      This recall involves Evenflo ExerSaucer 1-2-3 Tea for Me activity learning centers. The tea party themed toys have a twotier cake that is light blue, dark pink and yellow. Models included in the recall are 6161834 and 6161920. The model numbers are printed on the packaging and on a label located on the underside of the base of the product. The recommended age for use of this product is 4 months to walking.

      The activity centers were sold at Toys R Us and juvenile product stores nationwide from December 2007 through March 2009 for about $70. They were made in China.

      Consumers should immediately remove the cake toy from the product and contact Evenflo to receive a free replacement toy. The ExerSaucer may continue to be used without the cake toy.

      Fluke Test Leads Recalled

      May 3, 2004
      Fluke Corporation is recalling about 110,000 modular test leads used for electrical testing multimeters.

      The leads, which are used to connect probes to handheld digital multimeters when testing for the presence and amount of voltage present in electrical circuitry, can result in incorrect multimeter readings. This poses a serious shock or electrocution hazard if the consumer touches live wires that the meter has read as having no electrical current.

      Fluke has received 29 reports of the leads for Fluke multimeters operating improperly, though no injuries have been reported.

      The recalled test leads are red and black with no permanent probes attached. They have the Fluke logo on the connector ends. They were sold individually, as well as with a variety of Fluke multimeters and accessory kits. The recall includes only Fluke Model TL221, TL222 and TL224 test leads.

      The model numbers are not written on the test leads, but are written on the packaging or on product documents. The recall does not include Fluke test leads with permanently attached test probes, and does not include test leads with a SureGrip symbol, shown below, either on a tag around the silicon wire or molded onto the connector end.

      Fluke test leads with a SureGrip symbol are not included in the recall.

      The leads were sold at home and hardware stores and electrical distributors nationwide from December 2002 through March 2004. The leads sold individually for about $18. Fluke multimeters and accessory kits containing these leads sold for between $45 and $450.

      Consumers should stop using the recalled modular test leads immediately and contact Fluke for information on getting free replacement components.

      Fluke Test Leads Recalled...

      GE, Midwest Electric Socket Load Centers

      November 19, 2004
      Midwest Electric and GE are recalling about 2,700 meter socket load centers manufactured from May through August 6, 2004.

      Some of these products were sold with loose internal bolt connections, which could result in excessive heat or fire. Six units have been returned with overheated connections, and one resulted in a fire that was contained to the product. There have been no injuries reported.

      The following models are included in this recall: model numbers R256EB1A, R256EB1NA, and R256EB1018A (Midwest) and TSLR1220CSAL (GE). Affected date code range is SAL421 through 432.

      The units were distributed by electrical product distributors.

      Midwest and GE have begun contacting all consumers, distributors and the utility companies to locate the affected products. They will repair or replace the product at no charge to the consumers

      Ethan Allen Recalls Bunk Beds

      April 13, 2004
      Ethan Allen is recalling about 2,000 bunk beds. A metal j hook on the guardrails can become dislodged, allowing the guardrail to slide or move out of position. This can allow the guardrail to detach from the bunk bed or allow the occupant to roll off the top bunk.

      Ethan Allen is aware of 22 incidents where a j hook became dislodged. Ethan Allen is not aware of any incidents or injuries caused by a dislodged j hook.

      The top bunk guardrails on Ethan Allen Ryan and P.J. Bunk Beds have item numbers 35-5659-4, 36-5659-3 or 36-5659-4. Item numbers can be found on the inside of the bottom bunk headboard or footboard. The j hooks were incorporated into the guardrails of bunk beds as a locking/unlocking device. When unlocked and out of position, the guardrail can be removed for easy access to change bed linen.

      The beds were sold at Ethan Allen stores nationwide from June 2003 through December 2003 for between $1,150 and $1,350.

      Consumers should stop using the top bunk if a j hook has become dislodged. Ethan Allen is directly notifying consumers who purchased these bunk beds about the recall and is providing free repair kits. Guardrails will be shipped to consumers with these recalled bunk beds on or about May 20, 2004.

      NHTSA Issues School Bus Recall

      June 9, 2005
      The National Traffic Safety Administration has issued two school bus recalls -- one for a window defect that the agency warns could result in serious injury or death, the other for improper welds on some seats.

      NHTSA says 200 Sturdibus units manufactured by the U.S. Bus Corporation are affected by the window-defect recall which is scheduled to begin this month.

      The agency reports on its Web site that "in the event of a vehicle crash, its is possible that passenger contact with the window could cause the window to dislodge from its gasket or seal and permit passage of a passenger's head through the resulting opening."

      This NHTSA concludes, "could result in serious injury or even death."

      The agency says that the Sturtibus school buses, manufactured in 2005 fail to comply with Federal Motor Vehicle Safety Standard No. 217, "Window Retention."

      U.S. Bus Corporation has not provided the Agency with a remedy or notification schedule. Bus owners who do not receive a free remedy for affected vehicles should contact U.S. Bus Corporation at 845-357-2510.

      In a second recall, NHTSA reports that and undetermined number of Sturdibus unites and Universe School Buses were manufactured with improper or missing welds on some school bus passenger seats and crash protection.

      In the event of a frontal crash, according to the agency, the barrier may fail to restrain a front seat passenger.

      U.S. Bus Corporation will repair the defect free of charge.

      BBK Recalls Tree Climbers

      BBK is recalling the BBK 10010 Tree Climbers. The steel back brace on the tree climber can weaken and fail, posing a serious injury hazard to the user.

      The recalled BBK Tree Climbers have a model number of BBK 10010, which can be found on the outside of the box. The climber, which is primarily used by hunters, has a 2-piece climbing stand with a green powder coat finish, a camouflage padded seat and an expanded metal platform. A yellow warning label on the climber has BBKs name, address, and phone number printed on it.

      Hunting gear retailers and distributors nationwide, including Mattoon Rural King Supply, Johnston Seed, Everharts Sporting Goods and Sportsradein, sold the tree climbers between August 2003 and October 2003 for about $90. The recalled units were also sold on the BBK Enterprises Web site: www.bbkhuntingsystems.com.

      Consumers should stop using these climbers immediately and call BBK or the retail store to return the units and receive reimbursement.

      Faulty Turn Signal Prompts GM SUV Recall

      General Motors is recalling 305,729 SUVs, including the Chevrolet TrailBlazer, to fix a faulty turn signal that can flash like a hazard warning light.

      The recall, mostly covering vehicles sold in the United States, includes the 2003-2004 model year GMC Envoy, Oldsmobile Bravada and Isuzu Ascender, which is built at a GM plant.

      GM, which had the most recalls in the United States last year, will notify owners and ask them to take their SUVs to a dealership in August, after replacement parts become available.

      Last month, GM recalled more than 2 million vehicles to fix a variety of potential safety defects, the latest setback for the automaker following its $1.1 billion loss in the first quarter.



      Faulty Turn Signal Prompts GM SUV Recall...

      Baby Bjrn Infant Carriers

      September 14, 2004
      About 49,000 Baby Bjrn Baby Carriers are being recalled. The back support buckle can detach from the shoulder straps, posing a fall hazard to the baby.

      The firm has received 93 reports of the back support buckle detaching from the shoulder straps on the infant carrier. No injuries have been reported.

      The recalled infant carriers were sold under the brand name Baby Bjrn which is printed on the front of the carriers and on the black molded plastic back support buckle. Model number 1-260 is printed on the care label on the strap of the carrier.

      The carriers are made of a cotton polyester blend fabric in black with red piping and blue with white piping. Baby Carrier Active is written on the packaging and above the warning label on the carrier.

      The carriers were sold at specialty retail stores, catalogs and internet sites from September 2003 through August 15, 2004 for about $120.

      Consumers should stop using the carrier immediately and contact Regal Lager for instructions on returning the carriers for repair.

      Aqua Scout Water Scooters Recalled

      September 2, 2005
      About 475 Aqua Scout Water Scooters are being recalled. Hydrogen gas can build up in the battery compartment and cause the battery cover and the battery package to forcefully expel from the product, posing a risk of injury to the user or bystanders.

      Aqua Scout has received five reports of the battery cover and/or the battery package being expelled from the water scooter, including two reports of minor injuries such as lacerations and bruising.

      The Aqua Scout Water Scooter is a hand-held, battery-powered product used to propel swimmers through the water. The product has a yellow or red plastic enclosure, a black plastic handle and propeller. There is no model numbers or brand name written on the scooter.

      The units were sold online at the Aqua Scout Web site and through eBay from May 2005 through July 2005 for about $120.

      An earlier recall affected similar units sold by CSK Auto.

      Consumers should stop using the recalled water scooter immediately and return it to Aqua Scout for a free replacement.

      Feds Look at VW Touareg Door Handles

      August 16, 2005
      Federal regulators are investigating the door handles on the Volkswagen Touareg after an owner complained that his daughter's hand was injured when the door opened, catching her hand in the frame.

      The National Highway Traffic Safety Administration (NHTSA) reports that the defect investigation involves the 2004-05 model years of the SUV.

      NHTSA says that the complaint indicates that the exterior door handle does not allow the door to latch properly.

      The investigation involves more than 53,000 vehicles.

      Volkswagen issued a technical service bulletin to dealers in June to correct a problem with the outside door handle sticking in the open position.

      Wal-Mart Recalls Zefal Bicycle Tire Inflators

      December 17, 2009
      Todson Inc. is recalling about 24,000 Zefal bicycle tire inflators sold at Walmart. The pressurized cartridge containing carbon dioxide (CO2) can forcefully separate from the pump head, posing a risk of injury to the consumer.

      This recall involves Zefal CO2 bicycle tire inflators with a small pressurized carbon dioxide cartridge. The metal cartridge is threaded into the inflator head, which allows for the controlled release of carbon dioxide into the bicycle inner tube. The recalled inflators have Zefal EZ+ CO2 inflator printed on the front of the package. Model number 5602 and UPC number 798661556020 is printed on the back.

      The inflators, made in Taiwan, were sold exclusively at Walmart stores nationwide from August 2009 through November 2009 for about $15.

      Consumers should immediately stop using the inflators and return them to Walmart for a full refund.

      Kia Recalls Spectra to Fix Air Bag Problem

      Kia Motors America Inc. is recalling more than 70,000 LD Spectra sedans because the computer operating the air bag system could confuse a child seat for an adult in the front seat.

      The recall will begin in late October and covers 2004-05 model years.

      The recall involves the same problems that prompted the recall of the 2004-05 Hyundai Elantra, according to Kia.

      The air bag system could miss-identify a child seat in the front passenger seat as an adult passenger if a child seat is installed after an adult has been in the passenger seat and then the ignition was turned off.

      When the vehicle was turned back on, the system's memory would recall the child seat as an adult passenger. The computer systems would then deploy the front air bag and side-impact air bag in a crash.

      The air bags normally would not deploy if the system properly identifies a child restraint seat because the impact of the bags could injure a child.

      Brakes Force Second Mitsubishi Eclipse Recall

      Mitsubishi Motors North America Inc. has recalled its redesigned Eclipse for the second time within a month. Both recalls have involved potential brake failure.

      The most recent recall involves 3,760 of the coupes built between March 30 and June 10.

      A seal inside the master brake cylinder may have been installed improperly and could cause the driver to use more force to brake, which could lead to increased braking distance.

      Mitsubishi says the brakes are still within federal standards.

      Mitsubishi recalled approximately 1,200 of the coupes May 24 because of a problem involving the vacuum brake booster, which could cause brake failure.

      The 2006 Eclipse went on sale May 19, 10 days ahead of its original schedule. The company sold 2,140 of the coupes in June vs. 890 in June 2004. Mitsubishi had projected 800 sales for the new model in June.
