    Polaris Accessory Skis Recalled

    About 1,700 accessory skis for Polaris snowmobiles are being recalled because they could break, resulting in injury or death.

    The recalled skis are the red composite (nonmetallic) skis and were sold as an accessory item for use on Polaris snowmobiles. The part number is inscribed on the tail of the snowmobile ski. The part numbers of the skis are:

    The rear portion of the ski will have the part number stamped on it as indicated in the following diagram:

    The skis were sold by Polaris dealerships worldwide from November 2001 through December 15, 2003 for between $78 and $125 per ski.

    Consumers should stop using their snowmobiles immediately if they have installed red accessory skis and bring the skis or snowmobile to their Polaris dealer for replacement.

    Toy Jewelry Sold in Vending Machines Recalled

    Risk of Lead Poisoning if Children Mouth or Swallow Jewelry

    In one of the biggest product recalls ever, four toy jewelry importers are recalling 150 million pieces of toy jewelry sold in vending machines across America. The Consumer Product Safety Commission (CPSC) has determined that some of this toy jewelry contains dangerous levels of lead, posing a risk of lead poisoning to children.

    With millions of pieces of jewelry involved in this recall, I urge parents to search their childrens toys for this jewelry, said CPSC Chairman Hal Stratton. Throw away this recalled toy jewelry.

    Only about half of the 150 million pieces of toy jewelry actually contain lead, but, because it is difficult to distinguish the lead jewelry from the non-lead jewelry, the industry decided to recall all of it.

    Young children sometimes mouth or swallow items like these, and lead can leach from the jewelry into the childs body. Lead poisoning in children is associated with behavioral problems, learning disabilities, hearing problems and growth retardation.

    The four firms have advised the Commission that they have stopped importing toy jewelry with lead and are committed to working with the CPSC staff on eliminating hazardous levels of lead in future importations of toy jewelry.

    CPSC has issued guidance urging firms to eliminate lead in consumer products, or, if lead is used, to assure it is not accessible. CPSC collected and analyzed some samples of toy metal jewelry sold in vending machines and found that 10 of those products had lead that could be accessible to children. The industry volunteered, out of an abundance of caution, to recall additional products.

    This toy jewelry recall involves various styles of rings, necklaces and bracelets. The rings are gold- or silver-colored with different designs and paint finishes with various shaped center stones. The necklaces have black cord or rope or gold- or silver-colored chains.

    The necklaces have pendants, crosses or various geometrical designs or shapes, and can include gemstones. The various styles of bracelets include charm bracelets, bracelets with medallion links, and bracelets with faux stones. All the jewelry was manufactured in India.

    The toy jewelry was sold in vending machines located in malls, discount, department and grocery stores nationwide from January 2002 through June 2004 for between $0.25 and $0.75. The industry estimates that this toy jewelry is kept in homes for a short period of time, but parents should look for these items in their homes.

    Bright Starts Doorway Baby Jumpers

    Kids II Inc. is recalling about 30,000 Bright Starts doorway baby jumpers. The plastic clamp that attaches the jumper seat to a door frame can break, which can cause the unit to fall to the floor. This poses an injury hazard to young children.

    Kids II has received 49 reports of the clamp breaking with 12 reported injuries, including a possible mild concussion, skinned knee, cut lips, and bumps and bruises.

    The recalled baby jumpers have a blue canvas seat, yellow straps, and a blue plastic clamp that attaches to an interior door frame. The recalled unit is the Bright Starts Jammin Jumper with model number 6607, which can be found on a white warning label on the canvas seat. The words Bright Starts are printed on the plastic housing that contains the clamp. The jumper was sold with a play mat that makes sounds when the child jumps on it.

    The jumpers were sold at mass merchandise and juvenile specialty stores nationwide from December 2003 through May 2005 for about $30.

    Consumers should stop using the product immediately and disable the jumper by cutting a one-inch portion of the yellow strap just below the clamp. To receive a free replacement product, consumers should cut out the white warning label from the canvas seat and mail the label and the one-inch portion of the yellow strap to Kids II at 555 North Point Center East, Suite 600, Alpharetta, GA 30022.

      Arlotta Mattress Pads Recalled

      Chris Arlotta Enterprises is recalling about 2,900 mattress paids which fail to meet the federal mandatory standard for flammability under the Flammable Fabrics Act. They pose a serious risk of burn injury or death to consumers if exposed to smoldering or burning cigarettes.

      The recalled mattress pads are natural colored, made of 100 percent cotton Chenille, and were sold in twin, full, queen, king and California king. The Web and catalog retailers and the mattress pads model numbers, listed below, are identified on the packaging:

      Dollar General Heart-Shaped Pendants

      Dollar General is recalling about 80,000 heart-shaped pendants. They contain high levels of lead, posing a serious risk of lead poisoning to young children. Lead poisoning in children is associated with behavioral problems, learning disabilities, hearing problems and growth retardation.

      The heart-shaped pendant is silver-colored, ribbed on the front, hollow on the back, and hangs on a pink suede cord with a silver-colored clasp.

      The pendants were sold at Dollar General stores nationwide from May 2003 through April 2005 for about $1.

      Consumers should take these necklaces away from children immediately and return them to Dollar General stores for a refund.

      Wii Battery Recharge Stations Recalled

      Griffin International is recalling about 220,000 Psyclone Essentials and React Wii 4-Dock Battery Recharge Stations. The battery pack can overheat, posing a burn or fire hazard to the consumer.

      Six incidents of overheating have been reported to the firm. Two consumers reported minor burns to the hand.

      The Wii 4-Dock Recharge Station includes a white docking station with four recharge stations and a four rechargeable battery pack. Brand names are on the front of the packaging and the model numbers, Psyclone (PSE6501) and React (RT530), can be found on the bottom side of the product.

      The Psyclone Essentials brand was sold at Target, Toys R Us and Amazon.com nationwide; React was sold at Best Buy stores nationwide. Both were sold from January 2008 through July 2009 for about $50. They were made in China.

      Consumers should immediately stop using these recharge stations and contact Griffin International to obtain information on how to return the product and receive a free replacement.

      Avanti Water Dispensers Recalled

      October 19, 2004

      About 124,000 Avanti Water Dispensers are being recalled. The hot water faucet on these units has a child-resistant safety feature to prevent young children from accessing the hot water, but the device can malfunction so that the faucet will not turn off, causing burn injuries to children.

      Avanti has received 10 reports of the hot water not shutting off, nine of which resulted in burn injuries to children, including a 10-month-old baby who suffered burns on his arm and chest.

      There are six models of water dispensers with hot water faucets included in the recall. The water dispensers have Avanti written on the front of the base. They have model number WD32, WD 50, WD 50.1, WDT51, WDR 52 or WHC 59. The model number is written on a silver sticker on the back of the unit. Model number WDR 52 is a combination refrigerator and water dispenser. All six models have removable drip trays and can hold either 3- or 5-gallon bottles of water.

      The units were sold at Office Depot, Staples, and other appliance and electronic stores nationwide from July 2000 through August 2004 for between $120 and $250.

      Consumers with small children should immediately disable the hot water by turning the red switch on the back of the dispenser to off and contact Avanti to get a free repair.

      Reedy Quasar Battery Chargers Recalled

      June 15, 2004
      Associated Electrics Inc. is recalling about 4,900 Reedy Quasar Pro Battery Chargers used to power remote control race cars.

      A defective fuse and program error in the charger can cause the battery packs being charged to overheat and explode, posing a risk of serious injury to consumers.

      Associated Electrics has received three reports of incidents involving exploding battery cells. One consumer sustained a bruised hand, while another received an eye injury from flying debris.

      The recalled #611 Quasar Pro Chargers were manufactured from February 2002 through February 2004. The grey charger box has words Quasar Pro printed in yellow on the front and shows a display of "QUASAR PRO VERSION 1.0" when first turned on. The recalled chargers either have no barcode label on the underside or a barcode label with the numbers "611" or "611A".

      The units were sold at hobby dealers nationwide from February 2002 through February 2004 for about $200.

      Consumers should stop using the charger immediately and contact Associated Electrics to receive a free repair.

      Reedy Quasar Battery Chargers Recalled...

      Vail Enclosed Beds Recalled

      Vail Products, Inc. is recalling about 5,000 "enclosed" bed systems. The beds have been found to cause patient entrapments, resulting in severe neurological damage or death due to asphyxiation.

      Under the terms of the recall, the company has sent new instruction manuals and warning labels to every customer informing them of FDAs advice to stop using the bed system, move patients to alternative beds systems if possible and consult with their physician.

      If, after consulting with their physician, it is determined that no alternative bed systems are available for a particular patient, users are advised to follow the safety precautions contained in the new instruction manuals and warning labels to help minimize risk of injury (also available on FDAs website, see below.)

      Vail enclosed bed systems are canopy-like padded beds covered with nylon netting that is zipped into place to enclose the patient. They are used for at-risk patients with cognitive impairment, unpredictable behavior, spasms, seizures and other disorders. The beds are an alternative to physical or drug restraint to reduce falls or other injury to patients.

      The bed systems pose a hazard in that patients can become entrapped between the side-rail and the mattress or between the canopy and mattress. Due to the presence of the canopy, if their head is entrapped, the patient may experience asphyxiation, which can result in permanent neurological injury or death. FDA is aware of approximately 30 entrapments, of which at least 8 resulted in death.

      "FDA is making every effort to make sure that patients and healthcare providers are aware of this problem and are given the information needed to help minimize risk," said Dr. Daniel Schultz, Director of FDAs Center for Devices and Radiological Health.

      FDA first issued a Public Health Notification on March 25, 2005 about the potential risk posed by these bed systems. FDA has today further updated its Public Health Notification (available at www.fda.gov/cdrh/safety/032505-vail.html) to reflect the latest information on this problem.

      On June 23 and 24, 2005, Vail Products mailed corrected instruction manuals and labeling, including warning labels to all users of its Vail 500, Vail 1000, and Vail 2000 enclosed bed systems.

      Vail Products publicly stated on June 16, 2005, that it is permanently ceasing the manufacture, sale and distribution of all Vail enclosed bed systems. Vail Products will no longer be available to provide accessories, replacement parts, or retrofit kits. Users who have not received a copy of the corrected instruction manual may attempt to contact Vail Products at 1-800-235-8245.

      Vail Enclosed Beds Recalled...

      Lilly Bean Bag Chair, Ottoman

      Design Ideas Ltd. is recalling about 1,100 Lily Chair and Lily Ottoman Bean Bag Sets. The chair and ottoman do not have locking zippers or warning labels. CPSC is aware of children who have died from suffocation when they unzipped, inhaled and ingested small pellets in similar bean bag furniture.

      The Lily chair and Lily ottoman were sold together as a bean bag furniture set. The chair and ottoman sets were sold in ivory/white (model 170001), ivory/blue (model 170003), ivory/black (model 170004) and ivory/orange (model 170009). Model numbers can be found on a label affixed to the bottom of the furniture.

      Specialty gift and furniture stores nationwide sold the chair and ottoman sets from January 2004 through July 2005 for about $170.

      Consumers should stop using the chair and ottoman set immediately, keep them out of reach of young children and contact Design Ideas to arrange to return the product for a full refund.

      Kawasaki Recalls Hedge Trimmers

      November 9, 2005
      Kawasaki is recalling about 1,500 hedge trimmers. On some units, the engine mount springs between the gear case and handle/frame could separate from their receptacle in the frame during heavy operation. Continued use could cause the engine and blades to swing free of the frame and hit the user, possibly causing lacerations.

      Kawasaki Motors has received two reports of engine mount springs separating from the frame. No injuries have been reported.

      The recalled hedge trimmers include model numbers KHT600D and KHT750D. The KHT600D has a 24-inch, double-sided blade, and the KHT750D has a 30-inch, double-sided blade. Both models have a gray handle/frame and a red and gray engine, and they can be identified by the model designation located on a label on the recoil starter.

      The units were sold by a nationwide network of independent Kawasaki Power Product dealers between April and July of 2005. The KHT600D model retails for about $360, and the KHT750D model retails for about $400.

      Consumers can contact the nearest Kawasaki Power Product dealer to receive the necessary repairs, free of charge.

      Dynamic Cooking Systems Wall Ovens Recalled

      Dynamic Cooking Systems is recalling about 1,400 wall ovens.

      When the oven is in the self-cleaning mode, the ovens exterior door temperature exceeds the allowable surface temperature limits under the industry voluntary standard for household electric ranges. Sustained contact with the door during self-cleaning mode could cause burns.

      These wall ovens come in single or double oven models in three color options: stainless steel, black, or white.

      The Wall Oven/Single (WOS) models have these serial numbers indicating the date of manufacture in 2003 and 2004 (the blank can be any of the letters A through L):

      • 04_00001A to 04_01093A

      The Wall Oven/Double (WOSD) models have these serial numbers (the blank can be any of the letters A through L):

      The serial numbers are located on the oven frames left trim post, approximately 12 inches from the bottom. Open the oven door to read the serial number.

      Appliance dealers nationwidesold the ovens from January 2003 through May 2004 for between $2,179 and $3,920.

      Consumers should not use the oven in self-cleaning mode until free replacement doors have been installed. Call the company to make arrangements for free replacement oven doors.

      Seventh Avenue Recalls Tealight Candleholders

      March 4, 2009
      About 430 Seventh Avenue tealight candleholders are being recalled because they pose a fire hazard. The faceted resin accents on the sides of the candleholder can ignite.

      Seventh Avenue has received one report of the candleholder igniting resulting in minor property damage. No injuries have been reported.

      This recall involves a wire formed tealight candleholder with ball feet. The sides have small and large round resin accents in amber and red. The holder holds up to five tealight candles (included). The model number is 609073 located on the original packaging.

      They were sold at Seventh Avenues spring 2008 catalogs and online at www.seventhavenue.com from February 2008 through October 2008 for about $10. The candleholders were made in China.

      Consumers should immediately stop using the candleholder and return it to Seventh Avenue for a full refund or store credit, and a $10 coupon. All known users have been contacted.

      Seventh Avenue Recalls Tealight Candleholders...

      Picasso Two-Bike Folding Racks Recalled

      October 25, 2005
      Delta Cycle Corp. is recalling about 3,500 Picasso Two-Bike Folding Racks. The bike racks do not have sufficient hardware to support their weight on the wall. This can cause the bike rack to unexpectedly fall, hitting those nearby.

      This recall involves Picasso bike racks with a collapsible shelf. The bike rack can be mounted on the wall and has slots to support two bicycles. The metal rack has the Delta logo and name printed on the front of the rack.

      The racks were sold at LL Bean, Bike Nashbar and independent bike shops nationwide from January 2002 through September 2005 for about $40.

      Consumers should contact Delta Cycle immediately to receive additional hardware and installation instructions.

      Picasso Two-Bike Folding Racks Recalled...

      DOLMAR Chain Saw Recall

      September 12, 2005
      DOLMAR is recalling about 1,300 chain saws. The flywheels on some of the chain saws may come apart during use, which could cause serious personal injury.

      DOLMAR has received three reports of the flywheel coming apart. There have been no reports of injury.

      The recall involves DOLMAR PS6400 and PS7900-model chain saws in which the last five digits of the serial numbers are within the following ranges:

      The chain saws housing is warm red with DOLMAR written on it. The chain saws serial number can be found on the right hand side of the silver nameplate which is located on the back of the fuel tank. No other DOLMAR chain saws are involved in the recall. Any PS6400 or PS7900 chain saws with the letter N preceding the serial number on the nameplate and a blue dot on the shipping carton have been repaired and are not involved in the recall.

      Outdoor power equipment distributors and industrial suppliers nationwide sold these chain saws from October 2004 through August 2005 for between $570 and $750.

      Consumers should stop using these chain saws immediately and contact DOLMAR to arrange for a free repair. Known purchasers were sent direct mail notification of this recall.

      DOLMAR Chain Saw Recall...

      Eyelash Curlers Recalled

      July 29, 2004
      More than 200,000 "Frosted Grip" eyelash curlers are being recalled. The plastic handles can detach and cause consumers to lose their grip, resulting in injuries to the eyes and face.

      The importer, W.E. Bassett Company, has received 11 reports of injuries from the grip handles breaking off, causing abrasions, contusions and bruising in and around the eye.

      The Frosted Grip eyelash curlers have pink-plastic handles and are sold with a rubber re-fill pad. The eyelash curlers were sold under various brand names including TRIM and Salon Plus. There are no identifying marks on the eyelash curler. However, item numbers are printed on the packaging. Item numbers involved in the recall include 5-87B (sold at Wal-Mart, CVS and Shopko), 5-87RA (sold at Rite Aid) and 5-87S (sold at Kroger).

      The curlers were sold at CVS, Meijer, Kroger, Rite Aid, SaveMart, Shopko, and Wal-Mart stores nationwide from September 2003 to June 2004 for between $3 and $4.

      Consumers should stop using these eyelash curlers immediately and contact The W.E. Bassett Company for a prepaid mailer to return the eyelash curler for a free replacement plus a $5 off coupon towards any TRIM product.

      Roubaix Bicycles Recalled

      Specialized Bicycle Components Inc. is recalling about 3,000 Roubaix bicycles. The cable stop on the bicycle can loosen, which can cause the rider to lose control and fall from the bike.

      Specialized received reports of two falls that resulted in fractures requiring medical attention and surgery.

      The recalled bicycles are as follows: 2004 Roubaix Comp 18, 2004 Roubaix Comp 27, 2004 Roubaix Pro 18, and 2004 Roubaix Pro frameset models. Only bicycles with a single rivet fastening the cable stop to the frame are subject to recall. The cable stop is attached through bonding materials and a single rivet mounted at the front of the bicycle. The cables pass through eyelets in the cable stop on the down tube. Bicycles with a double rivet fastening the cable stop to the frame are not subject to recall. The model names of the recalled bicycles can be found on the top tube of the bicycles.

      The bicycles, made in Taiwan, were sold by Specialized authorized dealers nationwide from September 2003 through August 2004 for between $1,600 and $4,000.

      Consumers should immediately stop using their bicycle and return their bicycle to a Specialized authorized dealer for an approved repair, at no cost to consumer.

      Roubaix Bicycles Recalled...

      Roark Titanium Bicycle Frames

      Roark titanium frames for road bicycles are being recalled. The frames on these bicycles can crack and fail during use, posing a risk of injury to riders.

      Roark has received nine reports of bicycle frames cracking, but no injuries have been reported.

      The recall involves custom titanium bicycle frames for road bicycles manufactured and distributed by Roark. The bicycle frames have a Titanium sticker on the top tube and a Roark sticker on the down tube. Every bicycle frame is built to the unique specification of each customer and has serial number and manufactured date located on the bottom bracket. Bicycle frames affected have serial numbers 001 through 422.

      The frames were sold through direct Web site transactions from January 1997 through December 2004 for between $1,975 and $2,175.

      Consumers should inspect their bicycle frame immediately for a crack in the chain stay bridge area or return the bicycle frame to Roark for inspection. Contact Roark for shipping or delivery instructions, at Roarks expense. The company will offer a $100 discount on their frame refreshment program, as an incentive for consumers who have their bicycle frame inspected by Roark. Bicycle frames that exhibit a crack in the chain stay bridge area will either be repaired or replaced at no cost to the consumer.

      Roark Titanium Bicycle Frames...

      Polaris Industries is recalling about 18,500 Predator 500 all-terrain vehicles. The front brake lines can crack and leak brake fluid, possibly resulting in loss of braking capability. This could result in severe injury or death.

      Polaris has received reports of cracked and leaking front brake lines on at least 127 units, with no reported injuries.

      The recall includes Predator 500 ATVs from model years 2003 and 2004 with these model numbers: A03GJ50AA, A03GJ50AB, A03GJ50AS, A03GJ50CA, A04GJ50AA, A04GJ50AB, A04GJ50AC, and A04GJ50AD. The model number is on the lower left frame tube ahead of the engine. Predator is prominently displayed on the right and left side of the front cab covering the fuel tank. The ATVs have red, blue, gray or black front and rear cabs.

      Polaris dealers sold these ATVs nationwide from November 2002 through February 2004 between approximately $6,000 and $6,500.

      Consumers should contact their Polaris dealer to schedule an appointment for free inspection and repair.

      IKEA Recalls Fargglad Childrens Chair

      IKEA is recalling the Fargglad multi-color childrens chair because of a choking hazard from red plastic feet that can detach from chair legs.

      Fargglad multi-color childrens chairs have the article number 400.548.40 and the supplier number (either 16592 or 17128) on a label on one of the chairs rear legs, or on the underside of the chair seat. The chair has a tubular steel frame with woven multi-color solid plastic piping used for the chairs seat and back. The plastic feet are red.

      The chairs were sold at IKEA stores sold the recalled childrens chair from May 2003 through September 18, 2004, for about $15.

      Consumers can obtain a full refund. Take the Fargglad multi-color childrens chair away from young children immediately and return it to the Returns and Exchanges department at any IKEA U.S. store for a full refund.
