    Hill Sportswear Recalls 'Kid' Pullovers, Sweats after Strangulation Death

    Hill Sportswear is recalling 300,000 Kid pullovers and sweatshirts following the strangulation death of a three-year-old boy in Fresno, Calif., when the drawstring on his hooded sweatshirt became entangled in a playground set.

    The sweatshirts — Kid Pullover Hood Sweatshirt and Kid Zipper Sweatshirt with Hood — have drawstrings through the hood. The drawstrings should be removed immediately since they can become caught on objects and strangle a child.

    More than 10 years ago — in February 1996 — federal safety regulators guidelines to help prevent children from strangling on the neck and waist drawstrings in upper garments, such as jackets or sweatshirts.

    The recall includes the Kid Pullover Hood and Kid Zipper with Hood. The Kid Pullover Hood has a flat style drawstring at the neck, one middle front pocket, and fleece inside lining. Top sewn in label reads 'HILL/ Made in USA.' The hooded Zipper children's sweatshirt has a rounded style drawstring at the neck, two front pockets, and fleece inside lining. Top sewn in label reads 'HILL/ Made in USA'. The recalled sweatshirts were sold at various small retailers in California and Texas from August 1999 through December 2008 for approximately $8.

    Due to the serious nature of this hazard, parents are urged to immediately remove the drawstrings from the sweatshirts or return the garment to either the place of purchase or to Hill Sportswear for a full refund.

    Extension Cord Recall

    United General Supply Co. is recalling about 8,200 extension cords. They have undersized conductors and no over- current protection which causes overheating, presenting a shock and fire hazard.

    United General has not received any reports of incidents involving these extension cords. This recall is being conducted to prevent the possibility of injuries.

    Description: The recalled extension cords were sold in yellow and brown and are approximately 9-, 25-, 50-, 100-feet long. The 9-foot extension cord was packaged in a green sleeve card with the words "Household Extension Cord" printed across the front. All remaining sizes were packaged in black sleeve cards with the words "Air Conditioner Extension Cord" printed across the front.

    The extension cords were sold nationwide from October 1999 through March 2001 for between $1 and $16.

    Consumers should unplug and stop using the extension cords immediately. Consumers should mail the cords to United General Supply Co., Lee-Tools Extension Cords, 9320 Harwin Drive, Houston, Texas 77036 for a refund, including the cost of postage.

    Extension Cord Recall...

    Lenovo Recalls Soundbar Power Adaptor

    Lenovo is recalling the power adaptor used with ThinkVision soundbars. The plastic shell of the AC Adaptor can crack, posing a risk of electric shock to consumers.

    Lenovo sold the AC adaptors as part of the ThinkVision Soundbar option. The FRU part number 89P8571 with date code 200810 is located on the back of the adaptor. The adaptors, made in China, were sold by Lenovo's Web sites, telephone and direct sales, and Lenovo authorized distributors nationwide from October 2008 through January 2009, as an accessory for about $30.

    Consumers should stop using the recalled AC Adaptor and contact Lenovo to receive a free replacement part.

      IBM Extends Monitor Recall

      August 29, 2003

      IBM and LiteOn Technology, of Taipei, Taiwan, are voluntarily expanding their recall to repair program for G51 and G51t computer monitors manufactured by LiteOn and sold by IBM.

      The original recall, which was announced on March 4, 2003, involved 56,000 computer monitors. The expansion includes 63,000 additional G51 and G51t monitors that were manufactured through September 1998. The monitor's circuit board can overheat and smoke, posing a fire hazard to consumers.

      IBM and LiteOn have received seven reports of monitors overheating and smoking, including one report of minor property damage and one report of minor smoke inhalation.

      The recalled IBM monitors include the G51 CRT (Cathode Ray Tube) and G51t Touch Screen CRT models. The G51 and G51t monitors have the following model numbers on a label on the back of the unit: 6541-02N, 6541-02E, 6541-02S, 6541-Q0N, 6541-Q0E, and 6541-Q0S. The label on the back of the recalled G51 models also has a date of manufacture between June 1997 and September 1998. The "IBM" logo can be found on the front of the units, which were manufactured in China, Malaysia, and the United Kingdom.

      IBM, MicroTouch Systems, and major retail stores nationwide, including Best Buy, CompUSA, Office Max, and Radio Shack, sold the monitors from June 1997 through December 1998 for about $370.

      Consumers should stop using these monitors immediately and contact the IBM Repair Center toll-free at (866) 644-3155 between 9 a.m. and 7 p.m. ET Monday through Friday to confirm whether their monitor is covered by the recall.       Melting Pot Gift Sets Recalled

      September 25, 2003

      About 9,000 melting pot gift sets manufactured by Lang Candles Ltd. are being recalled. The melting pots can produce excessive flame and/or ignite presenting a fire and burn hazard.

      Lang has received nine reports of the melting pots producing excessive flame and/or igniting. Six of these involved burn injuries to hands and five involved minor property damage.

      Each melting pot gift set is packaged in a heart-shaped paperboard gift box with a ceramic melting pot (simmer pot), four individually wrapped scented wax melters of various scents, and six tea light candles that have "Lang" printed on the top. The gift sets diffuse aroma by liquefying the scented wax melter on top of the melting pot through the use of a tea light candle placed in the bottom of the melter.

      The gift sets were sold in nine different styles printed on the exterior of the gift box lid and melting pot. The name of the theme and its respective "Part Number" are printed on the label affixed to the underside of the gift box: Warm Your Heart 0614001, Above the Fruited Plains 0614002, Noel Rose 0614003, Holiday Bounty 0614004, All Decked Out 0614005, Stars Stars Stars 0614006, Herb Garden 0614007, Home and Heart 0614008, Frances' Garden 0614009.

      Small craft and candle stores sold the Melting Pot Gift Sets from May 2003 through August 2003 for about $20.

      Shakespeare Casting Game and Fishing Kits Recalled

      March 12, 2009
      Pure Fishing Inc. is recalling about 2,600 Shakespeare Casting Game and Fishing Kits. The label on the fishing rod contains a surface coating containing high levels of lead in violation of the ban on lead in paint.

      The recalled Shakespeare Casting Game and Fishing Kit consists of a red and blue rod and reel with red foam targets and hook. The label is found on the front of the reel and reads 'Casting Fun'.

      The kits were sold by specialty fishing retailers nationwide from November 2008 through January 2009 for about $15. They were made in China.

      Consumers should immediately take the recalled game away from children and call Pure Fishing for a free replacement product or full refund.

      Home Line Bunk Beds Recalled

      September 25, 2003

      About 3,600 bunk beds manufactured by Home Line Industries are being recalled. The beds have openings between the guardrails and between guardrails and the end structures that are too large.

      A child's body could slide between the opening and become trapped by the child's head. This poses a strangulation hazard to children.

      Description: The recalled Home Line bunk beds include the following models and styles: S130 (black), S131 (white), S132 (red), and S133 (blue) have a single top mattress and a double width bottom mattress; S135 (black) and S136 (white) that have a single top and a single bottom mattress; and S116 (black) and S117 (white) that have a single top mattress and a bottom mattress that also converts into a couch.

      Some units are labeled with the "Home Line" name and the model numbers on the top front railing. Contact the firm to help determine if your bunk bed is part of this recall.

      The beds were sold in furniture stores nationwide from September 2002 through May 2003 for about $190.

      Catlike Kompact Bike Helmets Recall

      May 22, 2003 -- About 2,250 Catlike Kompact bike helmets are being recalled because they failed federal impact tests.

      The recall involves Catlike Kompact adult bicycle helmets. The helmets were sold in two sizes (small/medium and large/extra large) and various colors. The sizing label inside the helmets reads "Kompact" and "SM/MD" or "LG/XL."

      The helmets were sold at bicycle shops nationwide from March 2002 through February 2003 for about $130.

      Consumers should contact the Monarch Velo Co. for information on receiving a free replacement helmet.

      Wahl Clipper Recalls Curling Irons

      October 23, 2003

      Wahl Clipper Corp. is recalling 6,000 Duet 2 in 1 Hair Styler with 1 inch barrel curling irons. The electrical insulation in the curling irons may break down, resulting in a possible electric shock and injury to the user.

      Wahl Clipper Corp. has not received any reports of electric shock or injury related to this product or defect.

      The recalled curling iron package reads Duet 2 in 1 Hair Styler with 1 inch barrel. Two model numbers are involved: 5266-500 (sold in a plastic package) and 5266-500B (sold in a box). The curling irons have a gold and silver handle with gold barrel and silver clamp. The words Wahl and Duet are on the handle of the curling iron. The product was made in China.

      Wahl Clipper Corp. sold the curling iron for about $20 to $22 in discount stores, drug stores, and online, from May 2003 until September 2003.

      Consumers should stop using these curling irons immediately return them to the place of purchase for an exchange or refund.       Rusty North America is recalling boys' hooded sweatshirts. The sweatshirts have a drawstring through the hood which can pose a strangulation hazard to children.

      The boys hooded fleece sweatshirts were sold with the following names and style numbers printed along with the RN# 123549 located on the country of origin and care label in the center back neck: Industrial R48210106, Continue R48210105, Warp R48210122, and Enuff R38203012. All styles were sold in sizes S-XL and came in a variety of prints and colors including stripes and plaids.

      The retailers listed below sold these products nationwide from August 2008 through December 2008 for prices ranging from between about $40 and $65: Adrenaline, Bunger Kids, Carlsbad Pipelines, Driftwood Beach Shoppe, Farias Surf & Sport, Heritage Surf & Sport, K Five Board Shop, Nomad Surf Shop, Noreaster Surf Shop, Offshore Surf & Skate, Pitti Bimi, Riders Outlet, Rusty, Shoppers Department Store, Spellbinders, Surf Hut, Third Ave Surf Shop Inc, TJX Companies/Marshalls, Wallerbears Surf & Sport and Wave Riding Vehicles.

      Starbucks Recalls Children's Tumbler Cups

      May 28, 2003

      Starbucks is voluntarily recalling approximately 38,000 plastic Bearista Bear tumbler cups. When the flexible straw on the cups is chewed, small pieces of plastic can detach, posing a choking hazard to young children.

      Starbucks has received two reports of pieces of the straws becoming detached in the children's mouths. No injuries have been reported; however, in one instance, a child reportedly began to choke on a detached piece of the straw and the Heimlich Maneuver was administered by the parent to remove the small piece.

      The tumbler cup is constructed of translucent plastic in the shape of a bear with the words "Bearista" Bear" printed on the front. The cup was sold in both blue and green. The cup has a screw-on lid in the shape of a bear's head, from which a soft, flexible straw protrudes. The product's lid can rotate such that the straw attachment can be bent (closed) to prevent liquid from leaking out.

      Starbucks stores in the United States, Canada and Taiwan sold the recalled cups from April 2003 through May 2003 for approximately $7 (U.S.).

      Parents and caregivers should stop young children from using these cups immediately, and return the cups to any Starbucks location for a refund and a free beverage of any size.       Wal-Mart Fabric Lanterns Recalled

      May 8, 2003

      About 64,000 fabric lanterns sold at Wal-Mart stores nationwide are being recalled. The fabric is not flame retardant and can be ignited by the lantern's votive candle, posing a fire hazard.

      The cylinder-shaped lanterns are 7 1/2-inches high and 6- inches wide. Beige fabric with red and green floral designs covers the lanterns. Wal-Mart sold the lanterns from December 2002 through March 2003 for about $7.

      Consumers should return the lanterns to any Wal-Mart store to receive a refund.

      Bunn-O-Matic Recalls Single Cup Tea/Coffeemakers

      Bunn-O-Matic is recalling about 35,600 single cup pod coffee and tea brewers. The pod drawer of the pod brewer can open unexpectedly during a brew cycle, posing a burn hazard to consumers.

      Bunn-O-Matic has received 10 reports of the drawer opening unexpectedly, including one report of minor burns.

      This recall involves Bunn Single Pod Brewers. Models included in the recall are MCP (My Caf' Pourover) and MCA (My Caf' Automatic). The pod brewers have a black body with stainless backsplash and measure 12 inches high by 8 inches wide. "Bunn" is printed on the front and lights up when the pod brewer is on.

      The devices, made in the U.S., were sold at Web retailers, department and hardware stores nationwide between August 2004 and March 2009 for about $300.

      Consumers should immediately stop using the recalled coffeemakers and contact the firm to obtain two replacement pod drawers.

      "Bear in the Box" Toys Recalled

      November 18, 2003

      Schylling Associates Inc. is recalling about 300 Bear "Jack-In-the-Box" toys. The red bead on the crank of the toy can detach, posing a choking hazard to young children.

      The multicolor tin Jack-In-the-Box-type toys have illustrations of bears in various activities on the sides. Each side features the enlarged single letters of the word "BEAR" and "BEAR" is written on the top of the toy.

      Other writing on the sides includes, "Bears in a Boat," "Bears Eating Eggs," "Bears in the Air," and Bears Racing Rabbits." When the crank is turned, the song, "Pop Goes the Weasel" plays and a toy teddy bears pops up out of the top of the box. "Schylling" is written on the bottom of the toys. Toys with date code stickers on the bottom are not included in the recall.

      Specialty stores, gift shops and book stores nationwide sold the toys from August 2003 through October 2003 for about $20.

      Consumers should take these toys away from children immediately and call Schylling Associates Inc. at (800) 767-8697 between 9 a.m. and 5:30 p.m. ET Monday through Friday for information on receiving a refund or replacement toy.

      Super-Rig Toy Trucks Recalled

      Super Rig toy trucks are being recalled because they fail to meet federal lead paint standards.

      This recall involves Super Rig Transport toy truck with trailer and vehicles. The truck and trailer are multi-colored, holding two vehicles and two action figures.

      Super Rigs Play Set is labeled on the outside of the packaging. The box back has a bar code square with Item No. 67007, Made in China and Bar Code 6-98567-67007-3. The bottom of the trailer has an engraved code 45TNGO9.

      The trucks wer sold at discount stores in the Southeast from September 2009 through November 2009 for about $20.

      Consumers should immediately take the recalled toy away from children and return it to the place of purchase for a full refund or replacement product.

      Super-Rig Toy Trucks Recalled...

      RCA Home Entertainment Amplifiers Recall

      April 17, 2003

      Thomson Inc. is voluntarily recalling about 50,000 home entertainment amplifiers. The amplifier can overheat due to a lack of ventilation, which can cause melting of the plastic front cover and pose a shock hazard to consumers.

      Thomson has received 12 reports of the amplifiers overheating, some of which resulted in melting of the plastic front cover or faceplate. No injuries have been reported.

      The recalled RCA 650-Watt Home Entertainment amplifiers include model numbers RT2600, RT2600DVD and RT2600DVD5 and serial numbers beginning with 220 through 230. The model and serial numbers can be found on a label on the back of the unit in the lower left corner. The recalled units may also have an "A" as the last character in the model number. The amplifier units are silver and are about 17 inches wide, 16 inches deep, and 5 inches high. The amplifiers were manufactured in China.

      Radio Shack, Best Buy, and other retailers sold the amplifiers from May 2002 through March 2003 for between $300 and $380.

      Halloween House Tea Light Holder

      Oct. 2, 2003

      Pottery Barn is recalling the Halloween House Tea Light Holder. Using a plastic tealight in the turret may cause the tealight to flare up, presenting a fire hazard. Only metal tealight holders should be used.

      The Halloween House is all black in color with wire-web windows, mesh rooftops and a brick-front exterior. The house is made of metal and measures 14.5 inches in height. The base of the house is a thin, flat piece of metal surrounded by a black wire metal fence. The house holds four tealights on the bottom floor and one inside the turret. "Made In China" is written on a sticker on the bottom of the house.

      The units were sold at Pottery Barn retail stores nationwide from July 2003 through September 2003 for about $40.

      Consumers should go to their Pottery Barn store with the purchase receipt, if available, to receive a complimentary set of metal tealights and a warning sticker to place on the item for future users that states: "Warning - Do not use plastic tealights. Use only with metal tealights."

      Daiso Recalls Children's Toys, Purses, Pen Cases

      October 6, 2009
      Daiso California LLC is recalling children's toys, purses and pen cases. The surface paint on the balancing toys and zippers of the purses and pen cases contain excessive levels of lead, violating the federal lead paint ban. The inflatable baseball bat toys contain excessive levels of DEHP, violating the federal phthalate standard.

      This recall involves balancing toys in the shape of dragonflies, children's purses in the shape of the head of a lion, frog or bear, children's purses with rainbow stripes, children's pen cases shaped like a piece of candy with stitching 'cool ice cream,' 'sweet cake,' or 'fad hamburger' on the front, and inflatable baseball bats with 'Home Run' printed on the side. Daiso is printed on the product's packaging.

      The toys were sold at Daiso stores in California from March 2008 through May 2009 for between $1 and $4.

      Consumers should immediately take the recalled products away from children and contact Daiso for a free replacement product or a full refund.

      Random House Recalls Children's Board Book Sets

      Feb. 27, 2003

      Random House is voluntarily recalling about 360,000 children's board book sets. The book sets were sold in cardboard boxes with plastic snaps. The plastic snaps can detach, posing a choking hazard to young children.

      The U.S. Consumer Product Safety Commission (CPSC) and Random House have not received any reports of injuries involving these book sets. This recall is being conducted to prevent the possibility of injuries.

      The boxed sets included in the recall have children's characters on the front of the box, a colored plastic handle and plastic snaps. The book set titles are "MONSTERS TO GO!," "DISNEY PRINCESS - DISNEY THE PRINCESS COLLECTION 2," "DISNEY'S WINNIE THE POOH - A VERY MERRY CHRISTMAS," and "BARBIE - MY BARBIE FUN BOX." Each book set contains four board books inside. Only book sets with plastic snaps are included in the recall. Book sets with metal snaps are not part of the recall.

      Book, discount department stores and online retailers sold the recalled books nationwide from August 2002 through January 2003 for about $10.

      Consumers should immediately cut off the lid of the box with the plastic snaps and throw away the rest of the box.       Child's Strangulation Death Prompts Vertical Land Window Blinds Recall

      Vertical Land Inc. is recalling about 32,000 sets of blinds and cellular shades.

      The horizontal blinds do not have inner cord stop devices to prevent the accessible inner cords from being pulled out. If an inner cord is pulled out, a child can become entangled in the loop and strangle. The vertical blinds can cause strangulation if a childs neck becomes entangled on the free-standing loop formed by the bead chain or by the cord with a weighted device. The cellular shades have a free-standing looped cord which could also cause strangulation.

      The U.S. Consumer Product Safety Commission (CPSC) said taht in May 2006, it received a report of a 4-year-old girl from Pensacola, Fla., who strangled in the loop of a vertical blind cord that was not attached to the wall or floor.

      This recall involves all Vertical Land custom-made vertical and cellular window coverings that do not have a cord tensioning device and horizontal blinds that do not have inner cord stop devices. A sticker identification label can be found on the head rail of some of the window coverings which lists the companys name, address and telephone number.

      The blinds were sold at Vertical Land stores in Panama City and Pensacola, Fla. from January 1992 through December 2006 for between $60 and $200 (depending on the size of the blind).

      Consumers should immediately stop using the window coverings and contact Vertical Land to receive a free retrofit kit.
