    Lakewood Recalls Fan-Forced Mini-Personal Heaters

    Lakewood Engineering is recalling its Lakewood MPH-25 fan-forced mini-personal electric heater. Electrical failure can lead to overheating and melting of plastic parts, creating a fire hazard.

    Lakewood is aware of three incidents where the heater failed, causing plastic parts to melt. No injuries have been reported.

    The electric heater is a small white plastic heater with these dimensions: 6 x 7 x 3 inches. It has a gray grill and a gray knob. The Lakewood logo appears in gray letters on top of the heater. The model number MPH-25 is embossed or appears on a sticker label on the bottom of the heater.

    The heaters were sold by retailers nationwide from August 2001 through November 2004 for about $13 to $15.

    Consumers should immediately stop using these heaters, unplug them, and contact Lakewood to get instructions on how to receive a free replacement heater.

    Lakewood Announce Recall of Fan-Forced Mini-Personal Heaters...

    Mossimo Suede Sandals

    Target is recalling about 1.2 million Mossimo beaded sandals. The sole of the sandal can become slippery, causing the consumer to fall.

    Target has received 15 reports of incidents involving falls. There have been nine injuries reported including injuries to the back, head and wrist, and reports of broken bones and bruising.

    The Mossimo thong sandals have beads and shells sewn onto the straps. The sandals were sold in beige or brown suede in sizes ranging from 5 to 10, whole sizes only. Mossimo is engraved on the inner sole, and the size of the sandal is printed on a clear label affixed to the side.

    The sandals were sold at Target Stores nationwide from January 2004 through August 2004 for about $10.00.

    Consumers should return their sandals to their nearest Target store for a gift card from Target in the amount of a full refund of the purchase price plus applicable state taxes.

    Mossimo Suede Sandals...

    Simplicity White Lancaster Cribs

    Simplicity is recalling about 575 White Lancaster Cribs. The white paint on the cribs can chip, posing a choking hazard to young children.

    Simplicity Inc. has received four reports of consumers who noticed paint peeling off their crib. No injuries have been reported.

    The recalled cribs are made of wood and painted white. Model numbers 8554W-PT and 8554WW are printed inside the headboard and on the envelope attached to the mattress support. The Simplicity name, address, manufacturing date and model number are written on a label found on the inside bottom of the headboard.

    The cribs were sold at department stores, childrens product stores and Target.com from June 2004 through April 2006 for about $200.

    Consumers should contact Simplicity Inc. to receive a refund.

      NETGEAR Wall Plug Ethernet Bridges Recalled

      NETGEAR Inc. is recalling about 53,000 wall plug Ethernet bridge units. The plastic housing on these units can detach, posing a shock hazard.

      NETGEAR has received one report from a consumer of a detached casing. No injuries were reported.

      The Wall Plug Ethernet Bridge is used to extend internet availability throughout a home or business. It is 2.75 inches high, 3.5 inches wide, and 2 inches thick, with a grayish silver plastic housing. NETGEAR is written on the top of the housing. The plastic housing contains a label with the model number and the serial number on it. The model number is XE102 and the serial numbers are XE12A32LB000001 through XE12A32LB005000 and XE12134LB005021 through XE12143LB053586.

      The units were sold at various electronics stores, Web sites, and through the NETGEAR Web site from February 2003 through May 2004 for between $50 and $80.

      Consumers should stop using the product immediately. Remove electrical power source to the receptacle and unplug the unit. Contact the company for a free repair kit. The free repair kit when affixed will ensure that the plastic housing will not become loose or separated.

      Children's Animal Masks, Pendants

      September 9, 2009
      About 1,400 children's animal masks and pendants are being recalled by Team Work Trading of Los Angeles. They contain high levels of lead. Lead is toxic if ingested by young children and can cause adverse health effects.

      This recall involves a children's animal mask and four styles of metal pendants. The mask resembles the face of a cat. The pendants have various animated symbols including a dog tag with fire symbol (Bleach), knife and lion symbol (Final Fantasy), Mickey Mouse symbol (Kingdom of Hearts), and a burning sun symbol (Naruto). The pendants were sold with silver-colored chains.

      The items, made in China, were sold at gift shop and modeling stores nationwide, and the Team Work Trading store from November 2008 through March 2009 for between $4 and $8.

      Consumers should immediately stop using the recalled items and contact Team Work Trading for a refund or exchange.

      DeWALT is recalling about 3,800 of its Model D55143 Three-Gallon Hand-Carry Oil Free Air Compressor. Due to a wiring insulation defect, the unit may pose a possible shock hazard.

      Only DeWALT D55143 three-gallon hand-carry oil free air compressors with the following date codes: 200448, 200453, 200502 through 200505, 200508, and 200509 are included in the recall. The date code is printed on the name plate on the rear air tank of the unit. Units marked with an R on the end cap of the unit are not included in this recall.

      The units were sold at home center and hardware stores nationwide from January 2005 through April 2005 for about $250.

      Consumers should stop using the compressors immediately and contact DeWALT for the location of the nearest service center to receive a free inspection or repair if necessary.

      Hunter's View Safety Harnesses

      April 5, 2005
      About 500,000 Hunter's View safety harnesses are being recalled. The harnesses could fail during use, resulting in consumers falling from tree stands and suffering serious injuries or death.

      The harnesses were sold by hunting and sporting good stores nationwide beginning in January 2004 for between $100 and $300.Consumers should stop using the harnesses immediately and contact Hunters View to receive a free replacement harness.

      The 2004 model year Fall Arrest Systems (harnesses) have model number 3333 and Model Year 2004 written on a white label attached to the harness or the tree strap. They were included with every Hunters View and every Comfort Zone brand tree stand sold under the following names and model numbers:

      Hunter's View Safety Harnesses...

      Children's Folding Chairs

      May 24, 2005
      Summit Marketing LLC is recalling about 2 million childrens folding chairs. The chairs safety lock can fail, allowing the chair to collapse or fold unexpectedly. Childrens fingers can become caught or entrapped in the hinge and slot areas of the chair, posing a pinch or cut hazard. This can cause severe lacerations and finger tip amputations to childrens fingers.

      The firm has received seven reports of the chairs collapsing, resulting in four finger tip amputations, one laceration, and bruises to childrens fingers.

      The recalled childrens folding chairs are made of metal tubing with a padded seat. They were sold in red, blue, yellow and green colors either individually or as part of a set consisting of a table and two, four, or six chairs.

      Each chair is about 22-inches high, 10-inches wide, and about 11-inches deep. MANUFACTURED BY ZHANGZHOU STEEL TUBE FURNITURE FACTORY is written on a label underneath the seat of the chairs.

      The chairs were sold at hardware, discount department, toy, grocery and drug stores nationwide from September 2002 through May 2005 for about $10 individually and for about $45 for a set.

      Consumers should stop using the chairs immediately, and contact Summit for instructions on how to receive a repair kit to replace the locking pin.

      Tippmann A-5 Paintball Markers Recalled

      March 12, 2009
      About 13,000 Tippmann A-5 Paintball Markers are being recalled. The end cap assembly on the rear of certain A-5 markers can break and eject during use, posing a risk of injury to the operator.

      Tippmann Sports has received three reports of the end cap assembly breaking, ejecting and hitting the operator. One minor injury has been reported.

      This recall involves Tippmann A-5 markers with serial numbers 0409301 to 0422200. The serial number appears on the packaging accompanying each marker and is engraved on the left side of the marker near the front grip. Serial numbers ending in an 'R' have already been repaired and are not included in the recall.

      The paintball markets, made in the U.S., were sold by major sporting goods retailers, paintball dealers and online retailers nationwide from May 2008 through February 2009 for between $180 and $350.

      Consumers should stop using the recalled paintball markers immediately and contact Tippmann Sports for a free repair kit.

      Tippmann A-5 Paintball Markers Recalled...

      Maui Ocean Center Toddler Cups

      February 18, 2005
      Charles Products is recalling about 700 Maui Ocean Center Toddler Drinking Cups. A container inside the cup holds petroleum distillates, which can leak onto the outside of the cup and could come into contact with the user posing a poisoning hazard to children.

      CPSC and Charles Products have received one report of the petroleum distillates leaking from the bottom of the cup. No injuries were reported.

      The toddler cups are made of clear plastic with two handles and have a blue lid with a spout for young children to drink from. The dome-shaped container inside the cup contains blue and clear liquid, along with two toy dolphins, which appear to be swimming in the liquid. MAUI OCEAN CENTER is written on the side of the cups.

      The cups were sold exclusively at the Maui Ocean Center in Wailuku Maui, Hawaii from June 2003 through September 2004 for about $10.

      Consumers should immediately take these cups away from young children. Consumers should return the cups to the store where purchased for a refund or contact firm.

      Maui Ocean Center Toddler Cups...

      Progress Lighting Recalls Fluorescent Light Fixtures

      February 10, 2004
      Fluorescent Light Bathroom Fixtures distributed by Profess Lighting are being recalled. The lamp holders on the fluorescent lights can overheat, causing the fixture to melt or burn, presenting a fire hazard.

      Progress Lighting has received four reports of fixtures overheating and burning, including one report of a fire causing smoke damage. No injuries have been reported.

      This recall involves linear light fixtures intended for use in bathrooms. The fixtures were sold in 2-, 3- and 4-foot lengths with a ribbed, plastic lens covering the fluorescent tube. The Progress Lighting catalog numbers are 717430-EBO, 717330-EBO and 717230-EBO. Catalog numbers and manufacturing dates can be found on the UL label located under the plastic lens. Fixtures included in this recall were manufactured from January 1998 through February 2003.

      The units were sold at lighting and electrical supply distributors, and home improvement stores by special order from January 1998 through April 2003 for between $225 and $270.

      Progress Lighting Recalls Fluorescent Light Fixtures...

      Memory Trunks

      February 3, 2005
      Once Upon a Family is recalling about 1,600 memory trunks. The lid can fall on a child who is looking or reaching inside entrapping the child at the neck, which poses a risk of strangulation. Also, the lid can close and self-fasten, entrapping the child inside the trunk, which poses a suffocation hazard.

      CPSC and Once Upon A Family are aware of one incident where the lid of a trunk fell on a 2-year-old boy, entrapping him between the lid and the base. The child was not injured.

      The yellow and tan Memory Trunk is about 22-inches long by 14-inches wide by 13.5-inches high and made of fiber board. It is decorated with photos of family activities and various phrases, such as A Childhood Cherished, A Place in Time and Mementos and Memories. Once Upon A Family intended the trunk for use as a storage box for childhood mementos and does not intend it for use by children.

      The trunks were sold by "sales consultants" at home events nationwide from November 2002 through October 2004 for about $100.

      Consumers should keep the Memory Trunk out of reach of children and contact the firm to receive a free repair kit. The repair kit includes a warning sticker and a padlock so that the Memory Trunk can be locked. Consumers can also request a free replacement lid that is not attached by hinges, which eliminates the risks of entrapment and strangulation.

      Memory Trunks...

      Mitsubishi Issues Urgent Stop Sale Notice for '06 Eclipse

      May 25, 2005
      Mitsubishi has issued an "urgent notice of stop sale" on the redesigned 2006 Eclipse. The problem involves the vacuum brake booster, which could cause brake failure.

      The company informed the National Highway Traffic Safety Administration on Tuesday, May 24. A company spokesman says up to 1,185 vehicles could be affected. He says 44 have been sold to consumers, about 400 are at dealerships, and the rest are in transit.

      The problem was detected on two vehicles that were being produced at the plant in Normal, Ill. Company technicians have been in the field investigating the vehicles and have not discovered the problem in any other vehicles yet. Mitsubishi technicians will go to consumers' homes to check their vehicles.

      The plant has not stopped production of the sports coupe, and he said the problem should be resolved by the end of the week.

      Mitsubishi began shipping the sports coupe last Thursday, 10 days ahead of its original schedule. In Japan on Monday, the company said it is boosting production at the Normal, Ill., plant from 760 per week to 1,000 per week. The company cited high demand from U.S. dealers.

      Ruger Gun Locks Recalled

      WASHINGTON, May 9, 2001 -- Sturm, Ruger & Co. Inc. is voluntarily recalling 1.2 million padlocks and 800,000 cable gun locks that bear the "Ruger" name. The padlocks can be opened using keys other than those originally provided with the locks and the cable locks can be opened if struck with force potentially allowing unauthorized access to a firearm.

      Ruger is aware of three incidents where a consumer was able to open a padlock without the use of its designated key. No injuries have been reported.

      The recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC).

      Lockable boxes using these padlocks were shipped by Ruger with its pistols and revolvers from 1987 until 1995. The red cable locks were shipped with Ruger rifles and shotguns from 1998 until March 2001. These locks were distributed nationwide.

      Ruger began its voluntary lock replacement program in March 2001. Consumers with the recalled padlocks and cable locks should send them to Sturm, Ruger & Co. Inc., Dept.       Whirlpool Washer and Gas Dryer Units

      WASHINGTON, July 2, 2002 -- Whirlpool Corp. is voluntarily recalling about 17,000 combination washer and gas dryer units. The gas dryer can overheat, posing a fire hazard.

      Whirlpool has received two reports of the unit overheating. No injuries or property damage have been reported. The recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC).

      The recalled 24-inch washer and gas dryer units were sold under the Whirlpool, Kenmore and General Electric brand names, which is written on the control panel of the units. They are white or light tan. The model and serial number are located behind the dryer door at the top of the opening. Model and serial numbers of units covered by this recall are as follows:

      Snow Peak Gas Camping Stove

      July 15, 2004
      The Snow Peak Giga Power White Gas Camping Stove is being recalled by its manufacturer, Snow Peak USA, Inc., of Clackamas, Ore.

      Plastic parts in the pump connected to the camping stove can crack or become dislodged. When exposed to extreme temperature change, the blue plastic shrinks around the aluminum parts and has been observed to crack the outer plastic housing. The air tube on some of the pumps can become unglued and dislodged. Both of these failures can permit gas to leak.

      Snow Peak has received 17 reports of cracked or dislodged pumps. No fires, property damage or injuries have been reported.

      This recall involves the Snow Peak Giga Power White Gas Stove GS-010. The stove is a white gas camping stove with an output of 10,000 BTU. The stoves main feature is that it will ignite without priming. Model numbers are located on the instruction manual. The brand name Snow Peak is on the front base of the stove, two of the four support legs, and the fuel bottle.

      The stoves were sold at camping and outdoor recreation stores nationwide from August 2001 through June 2004 for about $150. The pump was sold separately for about $40. See www.snowpeak.com for a list of retailers.

      Consumers should return the pump to the store where purchased or to the firm for a free replacement pump.       International Playthings Inc. is recaling about 6,000 Flexitoys monster-size vehicles. Small parts can detach, which pose a choking hazard to young children.

      The recalled Flexitoys Monster-Size vehicles are plastic toys that measure approximately 5 1/2-inches in length. Models included in the recall are: dump truck (model 9000-1), race car (model 9000-2), tractor (model 9000-3), SUV (model 9000-4), fire truck (model 9000-5) and digger truck (model 9000-6). The name and model numbers are stamped on the underside of the plastic vehicles.

      They were sold at specialty toy stores nationwide from May 2005 through September 2005 for about $4.

      Consumers should contact International Playthings for information on how to return the product to receive a free replacement item of similar value.

      Flexitoys Monster-Size Vehicles...

      Sea Urchin Shell Candles

      Twos Company Inc. is recalling about 18,000 Sea Urchin Shell Candles. The candles can burn at an excessive height or cause the outer shell to catch fire, posing a potential fire and burn hazard.

      There have been four reports of the candles catching fire, but no reports of property damage. No injuries have been reported.

      These recalled candles are wax-filled sea urchin shells, and were sold under item number 2659, which is found on the packaging tag. The candles are about 2.5-inches in diameter. They were sold at various gift and specialty stores nationwide from April 2005 through June 2005 for about $5.

      Consumers should stop using these candles and return them to the place of purchase for a full refund.

      Sea Urchin Shell Candles...

      Toyota Sienna Recall

      Toyota Motor Co. is recalling 345,000 Sienna minivans because the seat belt in some seating positions might become difficult to retract when unbuckled.

      The company said there was a possibility the shoulder portion of the seat belt in the middle row seats might get stuck in a plastic piece that the belt slides through, binding it up and making it difficult to put on or take off.

      The voluntary recall in the United States involves Siennas from the 2004 and 2005 model years. The company had received 14 complaints but there have been no accidents or injuries from the problem.

      In certain 2004 and 2005 model year Sienna vans, there is a possibility that the shoulder portion of the seatbelt in the middle row seats may bind in the bezel trim piece. The affected seatbelt bezel is located on the shoulder portion of the right side captain's chair of the seven passenger models and the center seating position on second row bench seat eight passenger models.

      In the event of a seatbelt binding in the bezel, the seatbelt may not properly fit a passenger. Until the repair is completed, vehicle occupants sitting in the affected seating positions should make sure the shoulder belt portion of the seatbelt is retracting smoothly. If the seatbelt binds, it can be unbound by pulling the seatbelt outward and then allowing it to retract.



      Toyota Sienna Recall...

      Nordstrom Recalls Girls' Shoes

      Nordstrom is recalling about 31,000 pairs of girls' shoes. Surface paint on the outer sole of these shoes contains excessive levels of lead, violating the federal lead paint standard.

      The recall includes six styles of Nordstrom-brand shoes for girls. Clarice-Fab flats are bone colored with a bow. Eva-Fab are bone colored T-strap sandals. Fern-Fab are ankle strap sandals sold in bone, pink, white, blue polka-dot or pink polka-dot. Lilly-Fab are fuchsia gingham ballerina flats with a bow. Rita-Fab are ankle strap dress shoes sold in ivory linen, pink linen, white linen, cafe satin, silver satin or white satin. Vivi-Fab are open-toe dress shoes in bone, white, green polka-dot, lavender polka-dot or yellow polka-dot.

      The shoes were sold in girls sizes 10 through big-kids 7. All shoes have an embossed NORDSTROM printed on the insole. Only the colors identified above are subject to the recall.

      The shoes, made in China, were sold at Nordstrom stores nationwide from September 2006 through February 2009 for between about $35 and $45.

      Consumers should take the recalled shoes away from children immediately and return them to any Nordstrom store for a full refund or exchange.
