    Smoke Alarms Recalled

    WASHINGTON, May 23, 2000 --In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Universal Security Instruments Inc., of Owings Mills, Md., is voluntarily recalling about 34,000 smoke alarms. The smoke alarms can fail to go off when smoke or fire is present. Also, a capacitor in the alarm can burn out, releasing smoke and melting the cover.

    Universal has received three reports of alarms failing, releasing smoke. There has been one report of minor smoke damage to the wall of a mobile home. No injuries have been reported.

    Consumers can identify the recalled models by removing the alarm cover from its base and looking for the manufacture date code and model number. The smoke alarms are made of white plastic and have the manufacture date code and model number imprinted on the back of the alarm.

    Some alarms do not have the brand name printed on the unit. Consumers should look at the model number to determine if it is a recalled alarm.

    Electrical distributors, housing suppliers, and hardware stores sold these smoke alarms nationwide from April 1998 through June 1999 for between $10 and $15. Most of these smoke alarms were pre-installed in homes during construction.

    K2 In-Line Skates Recalled


    WASHINGTON, Jan. 24, 2000 -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), K2 Corp., of Vashon, Wash., is recalling about 12,000 "FLIGHT ALX" brand in-line skates. The skate's plastic brake mount could crack and fail, causing the skater to fall and suffer serious injury.

    There have been no incidents or injuries reported. This recall is being conducted to prevent injury.

    The recalled skates are the "FLIGHT ALX" brand with model number S00161 or S00162, manufactured before November 20, 1999. The brand name and model number are on the skate's tongue.

    The manufacturing date is stamped in the heel of the skate, on the footbed. It is written as the year, month and day (for example, 19991002 is October 2, 1999). Women's skates are maroon and silver in sizes 4 through 11. Men's skates are gray and orange in sizes 5 through 14.

    Sporting goods stores nationwide sold the skates from August 1999 through November 1999 for about $200.

    Consumers should stop using these skates immediately and return them to the store where purchased for free replacement skates.     Krell Amplifiers Recalled

    About 2,300 Krell amplifiers are being recalled. A component input device can fail and cause the amplifiers to overheat, posing burn and fire hazards to consumers.

    Krell has received 50 reports of the amplifiers overheating including reports of smoke and electrical fire. No injuries have been reported.

    This recall involves Krell power amplifiers with model numbers KAV-250a, KAV-250a/3, KAV-500i and KAV-1500. The amplifiers are used to provide power to a loudspeaker in a home audio playback system. The Krell logo is printed on the front of the amplifier.

    The amplifiers, made in the United States, were sold by authorized dealers and distributors of audio equipment nationwide from January 1997 through February 2001 for between $3,000 and $8,000.

    Consumers should immediately stop using the amplifiers, and contact Krell to schedule the free installation of replacement fuses.

      Ace Shingling Hatchets Recalled

      WASHINGTON, June 22, 2000 -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Ace Hardware Corp. is recalling about 22,000 Ace-brand shingling hatchets. The heads on these tools can detach when in use, striking the user or a bystander and causing serious injury.

      Ace Hardware Corp. has received two reports of hatchet heads detaching, one of which struck a bystander in the head, resulting in a cut requiring 32 stitches.

      The hatchet, used to remove and hammer on roof shingles, is about 13-inches long. It has a hickory handle with a leather strap on the end and a forged steel head with a hatchet on one side and a hammering face on the other. The word "ACE" is etched on one side of the tool head and "TAIWAN" is on the other side. These hatchets have item number 2014710, which is printed on a label on the hatchet's handle.

      Ace Hardware Stores nationwide sold the hatchets from March 1994 through April 2000 for about $18.

      Glade Extra Outlet Scented Oil Air Fresheners Recaled

      WASHINGTON, April 19, 2002 -- SC Johnson is voluntarily recalling about 2.5 million Glade Extra Outlet Scented Oil Air Fresheners. The units may have been misassembled during manufacture, which could pose a risk of fire.

      SC Johnson has received five reports of misassembly and no reports of injury or property damage.

      The recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC).

      The air fresheners come in two fragrances, Sky Breeze and Mystical Garden. They have a unique rotating plug-thru outlet, which allows other electrical devices to be used in the same outlet as the plug-in. The rotating outlet says "15 Amps Max" on the front and has a yellow disk on the back. The Extra Outlet Scented Oil units have a model number "SCJ079" on the back.

      No other products sold under the Glade PlugIns brand names are part of the recall.

      Grocery and retail stores nationwide sold the air fresheners between January 2002 and April 2002 for between $4 and $5.

      Sears Mini Beach Chairs Recalled

      Sears "Time Out" Set

      WASHINGTON, April 2, 2002 -- Intercon Merchandising Source Inc. is voluntarily recalling about 100,000 Time Out folding mini beach chairs which can be used by both children and adults. The chairs can collapse, posing crush and amputation hazards to consumer's fingers and toes.

      CPSC and Intercon are aware of one incident in which the tip of a 3-year old girl's finger was amputated when the chair collapsed.

      The recalled mini beach chair was a free gift with the purchase of a Time Out cosmetic product at Sears stores. The mini beach chairs are white aluminum with a blue canvas seat and back. The name "Time Out" is printed on the canvas backrest.

      Sears stores nationwide distributed the mini beach chairs with the purchase of $20 worth Time Out cosmetics from June 2000 through August 2000.

      The recall is being conducted in cooperation with the U.S. Consumer Producer Safety Commission (CPSC).

      SunKids Convertible Cribs Recalled

      April 2, 2009
      About 1,900 SunKids convertible cribs are being recalled because of entrapment and suffocation hazards.

      The sides of the crib are made of mesh that expands, creating a gap between the side and the cribs mattress if its not zipped into place, or a gap between the side and an added mattress. A young child can slip into this gap and become entrapped or suffocate. Also, the cribs drop side can fail to fully latch posing a fall hazard to young children.

      No incidents have been reported with this crib. However, federal safety regulators say they have learned of the death of a 5-month-old child in August 2008 involving another companys nearly identical crib that was recalled. The child became entrapped between the mattress and the mesh side and suffocated.

      The recall involves the SunKids convertible crib/playpen/bassinet/bed with model number PY256. SunKids is embroidered on the bottom left of the cribs drop side. The convertible cribs have a drop side rail, stationary side rail, canopy assembly, and bassinet. The sides of the convertible crib are fabric and mesh. The mattress support, bassinet, canopy, and bed skirt are covered in fabric. The fabric and the mesh were sold in navy blue, light blue, pink, beige, white, beige checker, and pink heart.

      The cribs, made in China, were sold at small juvenile product retailers in N.Y., N. J., and Calif. from January 2007 through October 2008 for about $100.

      Consumers should immediately stop using the recalled convertible cribs and return them to the store where purchased for a full refund.

      SunKids Convertible Cribs Recalled...

      "Busy School Bus" Toys Recall

      WASHINGTON, Jan. 23, 2001 -- Playskool is voluntarily recalling to replace about 12,500 Busy School Bus toys. On certain units, the yellow "awning" piece above the bus' door can break loose, posing a choking hazard to young children.

      The recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC).

      Playskool has received five reports of the yellow awning piece breaking loose. No injuries were reported.

      Only item number 5527 toys with date codes 91671 through 91883 are being recalled to replace. The item number and date code are printed on the bottom of the toy. A yellow Playskool logo is imprinted on one side and "MADE IN CHINA" imprinted on the bottom.

      Playskool's Busy School Bus toy is an 8-1/2 inch-long yellow plastic school bus-shaped toy with a carry handle and blue wheels. The front of the Busy School Bus toy has a smiling face and a male figure that moves up and down when a child rolls the toy. A bell rings when the toy is rolled and when the sliding red stop sign is moved back and forth. One side of the toy features a green door with a red Playskool logo and a yellow awning piece. When the door is opened, a decal of a child with animals is revealed.

      Mass merchandise and toy stores sold these toys nationwide for about $10. The toys were sold from June 1999 through January 2001.

      Consumers should take these toys away from young children immediately, and contact Playskool at (888) 510-1561 or visit their web site to receive a free toy of equal value.

      Busy School Bus toys, with item number 5527 but with date codes lower than 91671 or higher than 91883 are not part of this recall to replace program.

      Little Tikes Recalls Childrens Toy Workshop Sets, Trucks

      Little Tikes is recalling about 1.6 million workshop sets and toy trucks. The recalled toys have oversized, plastic toy nails that can pose a choking hazard to young children.

      The firm has received one report of an 11-month old boy from Goose Creek, S.C. who choked when the toy nail become forcefully lodged in his throat. The child was hospitalized and made a full recovery.

      The oversized, plastic toy nail is about 3 inches long by 1 inch in diameter and comes in red or blue colors. It has a large round head; about 1 inches below the head is a plastic ridge, slightly smaller than the nail head and about 1 inch in diameter. They were sold with a variety of Little Tikes Childrens products. The names, model numbers and UPC code of the products included in this recall are listed below:

      2009 Dodge Ram Recall

      Chrysler is recalling about 37,000 2009 Dodge Ram picks equipped with manual temperature controls.

      The company said the software in the heating and air conditioning module may cause the windshield defrosting/defogging functions to become inoperative.

      Dealers will reprogram the HVAC module free of charge when the recall begins during June 2009. Owners may contact Chrysler at 1-800-853-1403 about Recall No. J12.

      Fazoli's Pasta Pals Recalled

      WASHINGTON, Aug. 29, 2000 -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Fazoli's Management Inc., is voluntarily recalling about 310,000 Pasta Pals toys included with Fazoli's Kids Meals.

      The toy is a plastic, barrel-shaped container with small, plastic tomato and ravioli figures inside. The bottom of the container can fit over a child's nose and mouth, which could pose a suffocation hazard to children under 3 years of age.

      Fazoli's has received one report of a child putting the container over his mouth. No injuries have been reported.

      The Pasta Pals toy's blue container measures about 2.5 inches in diameter and 2 inches deep. Inside are three red tomato figures and three yellow ravioli figures. Children can empty the figures out of the container and link them to form a chain.

      "FAZOLI'S ITALIAN FOOD...FAST" is imprinted onto the top of the barrel. "(c)2000Fazoli's Management, Inc." and "Made in China" are imprinted into the bottom of the barrel. Fazoli's restaurants nationwide included the Pasta Pals toys with Fazoli's Kids Meals from January 2000 through August 2000.

      Consumers should immediately take the barrel-shaped container away from young children. Discard the container or return it to any Fazoli's restaurant for a free Italian Lemon Ice.       Fender Recalls Bass Amplifiers

      Fender Musical Instruments Corp. is recalling about 90 SWR Blonde acoustic bass amplifiers. Wiring in the amplifiers can overheat, posing a fire hazard.

      The recall involves the SWR Natural Blonde 120V acoustic bass amplifier, model 44-60600-000. The amplifier measures about 26 inches x 16 inches x15 inches, and has a blonde-textured vinyl covering with a black control plate and black stamped steel vibration-free grill. The model number is located on the top rear panel of the amplifier. 'SWR' and 'Natural Blonde' are printed on the front of the amplifier.

      Consumers should stop using the recalled amplifiers immediately and contact FMIC for a free inspection and repair. Fender is directly contacting consumers who purchased the recalled bass amplifiers.

      Graco Announces Replacement Instructions for Activity Tray

      WASHINGTON, Feb. 21, 2002 -- Graco Children's Products is providing a new instruction sheet for installing the suction cups on about 8,900 Snack and Activity Trays. The Snack and Activity Trays are sold with detached suction cups that the consumer installs. If the suction cups are not properly attached, they can detach, posing a choking hazard to young children.

      The action is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC). Graco has not received any reports of injury from the suction cups detaching. This recall is being conducted to prevent the possibility of injuries.

      The Snack and Activity Tray can attach to high chairs, strollers, swings and other flat surfaces. There are four toys on each tray, including a toucan with a rainbow colored beak, a clear ball with colored beads inside, a yellow star with a mirror on one side and a green dog. "Mix 'N Move" is molded on the toucan, ball and dog. The toys are removable and interchangeable with other Mix 'N Move toys, which are purchased separately.

      Discount, department and juvenile product stores, including Sears, nationwide sold these activity trays from August 2001 through February 2002 for about $15.

      Children's Place Baseball Shirts and Rompers Recalled

      WASHINGTON, Aug. 28, 2000 -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), The Children's Place is voluntarily recalling about 120,000 boys baseball shirts and rompers. The buttons can come off, posing a small parts choking hazard to young children.

      The Children's Place has received three reports of buttons detaching. No injuries have been reported.

      The recalled baseball shirts are grey, blue, white or red with contrasting colors at the trim. They were available in boys sizes 3 to 36 months. The shirts have four baseball-shaped buttons down the front. A tag at the neck reads in part, "The Children's Place, Made in Turkey". On the left front of the shirt there is an embroidered baseball surrounded by the words, "Baseball Place USA". "Place" and the number "9" are embroidered on the back of the shirt.

      The recalled one-piece rompers are grey, blue or red with contrasting colors at the trim. They were available in boys sizes 3 to 36 months. The rompers have four baseball-shaped buttons down the front and snaps around the crotch. A tag at the neck reads in part, "The Children's Place, Made in Turkey". On the left front of the romper there is an embroidered baseball surrounded by the words, "Baseball Place USA". "Place" and the number "9" are embroidered on the back of the romper.

      The Children's Place stores nationwide sold the shirts and rompers from January 2000 through July 2000. The shirts cost about $12 and the rompers cost about $14.

      I-Tech Recalls Hockey Face Guards

      WASHINGTON, Oct. 10, 2000 -- I-Tech Sport Products is voluntarily recalling about 1,000 hockey face guards for replacement. These face guards, which attach to helmets, can become dislodged on impact causing them to fall off, leaving the user's face exposed to possible injury.

      The recall is being conducted in cooperation with the U.S. Consumer Products Safety Commission (CPSC).

      I-Tech has received one report of a hockey face guard becoming dislodged. No injuries have been reported.

      Only face guards with "OPTECH System," written on the black side clips, which lock on the plastic eye shield, are part of this recall. "ITECH" is written at the top center of the plastic eye shield, on the chin guard and on the chin guard strap. The model number of the face guards, "FX.50," is written on the packaging.

      Sporting good stores nationwide sold these face guards from July 2000 through August 2000 for about $100.

      Consumers should stop using the recalled hockey face guards immediately. Consumers should take the recalled face guards back to the store where purchased for a free replacement, or they can contact I-Tech Sport Products Inc. to get a free replacement face guard sent to them.       Edushape Recalls Snap Beads

      Edushape is recalling its snap bead toys because the tip of the snap bead peg may break off under repetitive pressure, posing a choking hazard to small children.

      The recalled product is multicolor and textured soft plastic beads that are designed to connect to one another. The beads are packaged in a 6 x 6 x 8 clear plastic jar with a photo of a child playing with the beads on the label. The word Edushape is written on the label in green and red.

      The beads, manufactured in China, were sold at various retail stores and online from July 2009 to September 2009 for about $20.

      Consumers should immediately stop using the snap beads and contact Edushape for a replacement set.

      Discovery Channel Store Oil Lamps Recalled

      WASHINGTON, Aug. 30, 2001 -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), The Discovery Channel Store, Inc. is voluntarily recalling 16,000 glass and metal oil-burning lamps. The glass on these lamps can shatter when lit, posing a fire and laceration hazard to consumers.

      The Discovery Channel Store, Inc. has received four reports of the lamps shattering, including two reports of minor property damage. There have been no reports of injuries.

      The recalled lamps come in two shapes: 16 by 7 inch-rectangular, and 7.5 inch-square. The top piece of the clear rectangular lamp has an opening for five wicks, and the top piece of the clear square lamp has an opening for one wick. The lamps have two stacking glass pieces supported by a black metal frame. The lower glass piece has a reservoir for oil.

      Discovery Channel Stores and The Nature Company stores nationwide sold the lamps from September 1999 through July 2001 for between $18 and $24.

      Consumers should stop using these lamps immediately and return them to any Discovery Channel Store or The Nature Company store nationwide for a refund.       Domestications Recalls Bed Steps

      July 30, 2009
      Domestications is recalling about 700 sets of bed steps. The bed steps can break during use, posing a fall hazard to consumers.

      Domestications has received 14 reports of the bed steps breaking or cracking during use, including four reports of injuries from falls.

      The recalled wooden bed steps with style numbers 17593A and 17878A have two steps and are 16 inches high by 24 inches wide and 18 inches deep. They were sold in white, cherry and oak.

      The bed steps, made in China, were sold by Domestications Web site and catalog nationwide. Style 17593A sold from June 2008 through July 2008 for about $50 and style 17878A sold from October 2008 through November 2008 for about $60.

      Consumers should immediately stop using the recalled bed steps and return it to Domestications to receive a refund. The company has directly contacted all known consumers who purchased the bed steps.

      Little Miss Matched Children's Pajamas

      Little Miss Matched Inc. is recalling about 7,300 sets of children's pajamas sold in the U.S. and Canada. The sleepwear fails to meet the federal childrens sleepwear flammability standard.

      This recall involves long sleeve toddler and girls pajama sets. The sets were sold in sizes XXS (2t-3t), XS (3t-4t), S (5-6), M (7-8) and L (10-12), and in three varieties:

      The pajamas, made in Honduras, were sold by various retailers nationwide from March 2008 through July 2009 for between $30 (U.S.) and in Canada from March 2008 through November 2009 for about $30 (CAN).

      Consumers should immediately stop using the recalled pajamas and contact the firm to receive a full refund.

      Wooden Stacking Toys Recalled

      WASHINGTON, May 10, 2000 -- Jack Rabbit Creations Inc. is recalling about 9,000 hand-painted, wooden stacking toys called Stacking King, in cooperation with the Consumer Product Safety Commission CPSC. One of the wooden pegs can come off, presenting a choking hazard to young children.

      Jack Rabbit Creations has received four reports of the pegs coming loose but no injuries have been reported.

      These stacking toys have seven interlocking multi- colored, doughnut-shaped pieces mounted on pegs. The pieces are green with blue pok-a-dots, orange with yellow stripes, and purple with green swirls. The bases of these toys have black and white checked squares. The top of the toy has a round, hand-painted face with a wooden yellow crown. A label on the toys packaging reads in part, "Stacking King," "Jack Rabbit Creations" and "Made in China." Specialty and Gift shops have sold these products from February 1999 through April 2000 for about $25.
