    Hellenic Shag Leather Rugs Recalled

    Hellenic Rug Imports is recalling about 1,800 shag leather rugs. The large rugs fail to meet the federal mandatory standard for flammability under the Flammable Fabrics Act and could ignite, presenting a risk of burn injuries. The smaller size rugs are missing labels identifying them as flammable.

    The recall includes only beige and champagne leather shag rugs with Hellenics name on the label. Large sizes include 5-feet by 8-feet and 8-feet by 11-feet and small-size rugs include 3-feet 5-inches by 5-feet 5-inches, 2-feet by 3-feet and 2-feet by 8-feet. A label on the rugs reads, Hellenic Rug Imports, Inc.

    The runs were sold through Internet and catalog retailers nationwide from September 2002 through October 2005 for between $50 and $600, depending on the size.

    Remedy:

    • Large Rugs Consumers should stop using the rugs and contact the manufacturer to arrange for free replacement.

    • Small Rugs Consumers should keep these rugs away from sources of ignition and contact Hellenic Rug Imports to receive a label that complies with labeling requirements.

    Mongoose 20-Inch Mountain Bicycles Recalled

    April 2, 2004
    Pacific Cycle is recalling about 14,000 Mongoose aluminum 20-inch-wheel D-XR AL mountain bicycles. The rear shock absorber allows the aluminum, dual-suspension frame to flex severely, causing the frame to become unstable and break, posing the risk of injury to riders.

    Pacific Cycle Inc. has received two reports of the frames on these bicycles breaking, resulting in abrasions and bruises to the rider.

    These are Mongoose aluminum 20-inch-wheel, D-XR AL, dual-suspension mountain bicycles. They have model number R1590WMET written on the service sticker, located on the bottom bracket tube near the pedals. The bicycles are silver and red colored. Mongoose and DXR AL are on the frame of the bicycles. Only Mongoose 20-inch D-XR bicycles with aluminum frames are included this recall. Bicycles with steel frames are not included.

    The bikes were sold at bicycle stores and department stores nationwide from September 2003 through March 1, 2004 for about $99.

    Consumers should stop using these bicycles immediately and call Pacific Cycle Inc. to receive a free replacement rear shock and instructions.

    Lithonia High Intensity Light Fixtures Recalled

    The Lithonia Lighting Co. is recalling indoor high-intensity discharge light fixtures with acrylic lenses or reflectors. About 52,000 fixtures are affected.

    A component in the light fixture can leak fluid, which can degrade the acrylic lenses and reflectors, causing them to crack and fall. Falling pieces of acrylic can injure someone below.

    Lithonia is aware of 42 incidents where pieces of acrylic fell from fixtures. One person suffered a laceration on his forehead when a piece of an acrylic lens fell.

    These are Indoor HID light fixtures with acrylic lenses and/or reflectors. They are generally used in industrial and commercial locations such as retail spaces, warehouses, and gymnasiums. Only certain models of specific wattage lights are included in the recall. Check the Lithonia Web site for a list of the specific model and wattage combinations included.

    All recalled fixtures were manufactured in Crawfordsville, Indiana, and have a date of manufacture from November 2002 through October 2003. The models, wattages, city and date of manufacture, and Lithonia can be found on a label attached to the ballast housing.

    The fixtures were sold by lighting and electrical supply distributors nationwide from November 2002 through February 2004.

    Building owners and managers with recalled fixtures should contact Lithonia to verify that the fixtures are included in the recall and arrange for a replacement of the fixture or faulty component. Lithonia and their distributors are directly notifying customers who purchased the recalled fixtures.

      JVC Rear-Projection Televisions

      March 9, 2005
      JVC is recalling 26,500 rear-projection televisions. An internal electrical connection can cause electrical arcing, charring or smoking inside the television, which pose a fire risk to consumers.

      JVC has received two reports of incidents, including one case of melted television parts and one case of a minor television fire. No injuries have been reported.

      The recalled 52-inch and 61-inch JVC rear projection televisions were manufactured between May 2004 and November 2004. The televisions have the following model and serial numbers:

      HD-52Z575: 10980014 through 16980772
      HD-52Z575: 16986471 through 16989999
      HD-52Z585: 10980031 through 16981502
      HD-61Z575: 10980031 through 16982712
      HD-61Z585: 10980031 through 16980942

      The model and serial numbers can be found on the back of the television in the lower center portion of the unit. The JVC logo appears on the front of the television.

      The sets were sold at consumer electronic stores nationwide from July 2004 through January 2005 for between $3,000 and $5,500.

      Forte Flyte and Weyless Carbon Handlebars Recalled

      Forte Flyte OS and Weyless CF200 carbon handlebars are being recalled.

      The handlebars can develop cracks that may not be visible, which can cause the handlebar to break without warning, resulting in serious injury or death.

      Only the Performance Forte Flyte OS Carbon Road handlebars and the Supergo Weyless OS Carbon Road handlebars are included in the recall. Both handlebars are natural carbon gray.

      The Performance Forte Flyte has printing along the top of the handlebar. On the left front is Forte in red, and on the right front is Flyte. On the right top is ACT in large white letters and Advanced Carbon Technology in smaller letters.

      The Supergo Weyless handlebar says Weyless in white and has a red oval with Full Carbon Composite printed in white. On the left side is CF in red, 200 in white, and Hi-Pressure Solid Compaction in white. The Performance handlebars were sold separately, not as part of a bicycle. The Supergo handlebars were sold separately and as part of the 2004 Scattante SC-R bicycle.

      The recalled handlebars were sold by mail order, Web sites, and Performance and Supergo retail stores for about $169. The Scattante bike equipped with the defective handlebars sold for $3,295.

      Consumers should stop using the bicycles with these handlebars immediately and return the handlebars to the nearest Performance or Supergo stores for a replacement or refund.

      Hamilton Beach Recalls Espresso/Cappuccino Makers

      Hamilton Beach is recalling 20,000 cappuccino and espresso/cappuccino makers. The steam tube inside the espresso/cappuccino maker can burst under pressure, presenting a risk of injury to consumers. This can occur if the frothing nozzle becomes clogged and the espresso button is pushed while attempting to froth milk.

      Hamilton Beach has received 10 reports of burst steam tubes, one of which included a minor burn.

      The recalled units are Hamilton Beach Cappuccino Plus Espresso and Cappuccino Maker, model 40714. They are black plastic and stainless steel. The Hamilton Beach brand name is printed on the front of the unit, and the model number is printed on the bottom.

      The units were solt at internet retailers and discount department stores nationwide from March 2003 through March 2004 for approximately $70.

      Hamilton Beach will ship free replacement espresso/cappuccino makers to consumers.

      Hamilton Beach Recalls Espresso/Cappuccino Makers...

      Telemark Ski Boots

      Black Diamond Equipment is recalling about 2,300 pairs of Scarpa T2X Telemark Ski Boots. The plastic boot cuff can crack when flexed open in an unbuckled, non-skiing position causing discomfort on the ankle or shin.

      The firm has received two reports of cuffs cracking during boot fittings. No injuries have been reported.

      The two recalled models of T2X Telemark ski boot can be identified by a dual color ankle cuff and the marking T2X on the outside of each boot (boxes are marked with model numbers: 142045 Mens and 142046). Repaired or replaced product can be identified by a single color cuff: Mens cuff are red, Womens cuff are blue.

      The boots were sold by Diamond Equipment, Salt Lake City via retailers, mail order and the Black Diamond Web site from September 2005 through October 10, 2005 for about $560

      Consumers should stop using the ski boots and return them to the nearest dealer for a free replacement.

      Telemark Ski Boots...

      J.C. Penney Okie Dokie Striped Bodysuits

      April 28, 2005
      J.C. Penney is recalling nearly 400,000 Okie Dokie Striped Bodysuits with Puffy Appliqus, in both newborn and infant sizes. The appliqu can detach from the garment, posing a choking hazard to young children.

      J.C. Penney has received three reports of the appliqus detaching from the bodysuit. No injuries have been reported.

      The multicolored bodysuits are 100 percent cotton and have a heat-sealed appliqu on the front. An appliqu is also attached to the side. The bodysuits were sold in newborn sizes (0 to 9 months) and infant sizes (12 to 24 months). Okie Dokie is sewn in the neckline and state Made in China.

      The bodysuits were sold at Penney stores and Web site from October 2004 through February 2005 for about $6.

      Consumers should stop using the recalled bodysuits immediately and return them to any J.C. Penney store for a full refund.

      FAO Schwarz Recalls Fire Engine Pull-Along Toys

      FAO Schwarz is recalling fire engine toys sold in its stores from September 2002 through January 2004 for about $20. Small parts can detach from the toy, posing a choking hazard to small children.

      These fire engine pull-along toys are red with a yellow ladder, and are made of wood. They are about 18-inches long and come with six toy firemen. Fire Dept. is written on the sides of the toy.

      Consumers should take the product away from small children immediately and mail the product to FAO Schwarz Inc., 2520 Renaissance Blvd., King of Prussia, PA 19406 for a full refund, including postage.

      PLEASE NOTE: Returns to this address must be received before March 31, 2004. The seller of this product has declared bankruptcy and will be able to accept returns only until March 31, 2004.

      Payless Recalls Baby Athletic Shoes

      Payless ShoeSource is recalling about 441,000 Smart Fit and Teeny Toes athletic shoes. The metal eyelet lace holder at the top of the shoes can detach, posing a choking hazard to young children.

      Payless ShoeSource has received one report of a child starting to choke on a detached eyelet from one of these shoes.

      These are childrens athletic shoes in prewalk and toddler sizes. The shoes are white with different color trim and come in sizes 0 to 10. The brand name Smart Fit or Teeny Toes is written on the sole and insole of the shoes. The recalled shoes have one of the following five-digit numbers, found on a label on the bottom of the shoe tongue: 31056, 31057, 31219, 32264, 32265, 33060 and 33061. No other styles or models are included in this recall.

      The shoes were sold at Payless ShoeSource stores and Web site nationwide from December 2003 through April 2004 for about $10.

      Consumers should take the recalled shoes away from young children immediately and should return the shoes to a Payless ShoeSource store for a cash refund or exchange.

      FDA Issues Smoked Salmon Warning

      The Food and Drug Administration is issuing a nationwide alert to consumers about Mama's Smoked Nova Salmon packaged in 8-oz vacuum-packed bags because they may be contaminated with Listeria monocytogenes, an organism which can be serious and sometimes cause fatal infections in young children, frail or elderly people, and others with weakened immune systems.

      Although healthy individuals may suffer only short-term symptoms such a high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, Listeria infection can cause miscarriages and stillbirths among pregnant women and can cause more serious or life-threatening conditions in other patients.

      The products are sold in individual 8-oz. packages marked: "SELL BY SEP 17 2005 27183", "SELL BY SEP 18 2005 27183". They may also be in shipping cartons labeled: "09/17/05 SELL BY 27183", "09/18/05 SELL BY 27183.

      The contamination was noted after routine testing by the Florida Department of Agriculture and Consumer Services revealed the presence of Listeria monocytogenes.

      FDA Issues Smoked Salmon Warning...

      Shimano Bicycle Brake Cables

      February 10, 2005
      Shimano is recalling about 13,000 Shimano Road Racing Bicycle Brake Inner Cables. The cable can detach during braking, causing the rider to lose control and fall.

      Shimano has received one report of the brake inner cable detaching. No injuries have been reported.

      This recall involves aftermarket Shimano 1.6 mm X 1,700 mm brake inner cables for ATB and road racing bicycles. Dual ended brake cables packaged in quantities of ten have a part number of Y80098400/Z80098400. Individual packages have a part number of Y80098110/Z80098110.

      All single ended brake cables were packaged individually with part number Y80098300/Z80098300. Brake cables installed on mountain, BMX and flat handlebar bicycles are not affected by this recall.

      The cables were solt at sporting goods and bicycle specialty stores nationwide from April 2003 through October 2004 for between $2 and $17.

      Consumers should contact their local bicycle dealer immediately to arrange for a free inspection. Consumers with the recalled brake cables will have a replacement cable installed free of charge.

      Shimano Bicycle Brake Cables...

      Bath & Body Works Votive Candles

      March 18, 2004
      Bath & Body Works is recalling more than 700,000 "Real Essence" candles. The candles can burn with a high flame or irregular flame posing a fire hazard to consumers.

      The company has received nine complaints of votive candles burning with high flame, two of which involved minor injuries.

      The votive candles were sold individually and in sets. A label on the bottom of the votive candle reads: Real Essence 1 x 2 . The specific recalled votive candles are: Rosewood Tea; Lilac Blossom; Vintage Leather; Brushed Suede; Warm Vanilla Sugar; Sweet Cinnamon Pumpkin; Spiced Cider; and Amber Myrrh. These votive candles were also sold in a six-pack set: Lilac Blossom, Spiced Cider, and Sweet Cinnamon Pumpkin.

      In addition, Sweet Cinnamon Pumpkin was sold in the scented home quartet set. This recall does not apply to any other Real Essence votive candles.

      Bath & Body Works and The White Barn Candle Company stores nationwide sold these votive candles and sets from March 2003 to February 2004 for $1.25 to $18.00.

      Consumers should stop using these recalled votive candles immediately and return them to any Bath & Body Works or The White Barn Candle Company store for a merchandise credit, merchandise exchange, or a full cash refund.

      Bath & Body Works Votive Candles...

      GM Recalls Full-Sized Vans to Fix Seat Belt

      General Motors Corp is recalling 425,593 full-size passenger and cargo vans because the seat belt will not latch properly, according to the National Highway Traffic Safety Administration.

      NHTSA reports that the recall involves Chevrolet Express and GMC Savana vans from the 2003-2006 model years.

      The Express and Savana passenger vans have seating for eight people. The cargo version seats two people.

      GM discovered the seat belt problem through an analysis of warranty claims. Consumers reported problems latching the buckle or unlocking it once it was in place.

      Dealers will inspect the buckles and replace the upper buckle cover. If the restraint isn't working properly, dealers will replace the entire buckle assembly.

      LeapFrog Recalls Electronic Plush Toys

      LeapFrog is recalling aboudt 3,700 "My Pal" electronic plush toy dogs. The decals on the paws of the plush toy can be removed and ingested by a child, posing a choking hazard.

      LeapFrog has received two reports of children removing the decals from this toy, including one report of a child ingesting part of the decal.

      This recall involves My Pal Scout, a green and beige electronic plush dog with plastic decals on its paws. When those decals are pushed, the dog plays songs, speaks and barks. Scout is printed on the dog tag at the dogs neck. The plush dog measures about 9 inches high by 9 inches wide. LeapFrog Ent. Item No. 19142 is printed on the white tag attached to the dog and on the toys packaging. My Pal Scout dogs with embroidered paws are not included in this recall.

      The toys, made in China, were sold at Toys R Us stores nationwide, and on the Web at www.toysrus.com, www.leapfrog.com and other retail Web sites from May 2009 through June 2009 for about $20.

      Consumers should immediately take the toy away from young children and contact LeapFrog to receive a replacement My Pal Scout with embroidered paws rather than decals.

      Linn Recalls Sound System Power Amplifiers

      Linn Inc. is recalling about 2,100 power amplifiers and Melodik bass extension speaker systems. The amplifiers capacitors can overheat, blow a fuse, and damage the products to which they are connected, presenting a fire hazard.

      Among the 70 reported incidents, there were two reports of heat damage to the cover of the connected speakers. No injuries reported.

      The Linn AV5150 2-Channel Power Amplifier comes in black or silver and is 3 inches by 15 inches by 14 inches; all serial numbers of this model are recalled. The Linn LK240 Single-Channel Power Amplifier comes in black or silver and is 3 inches by 12 inches by 12 inches; all serial numbers of this model are recalled.

      The Linn AV5150 Melodik Bass Extension Loudspeaker System comes in black ash, American cherry, or maple and is 19 inches by 25 inches by 19 inches; only serial numbers 1001 to 714451 are recalled. The model numbers and serial numbers are on the front panel, printed on the bottom of the product, or on the control panel near the main power supply.

      The units were sold at specialty retail stores throughout the U.S. and Canada from May 1996 through December 2003 for about $1895 (AV5150); $1300 (LK240); and $4265-$4950 (AV5150 Melodik).

      Consumers should stop using the amplifiers immediately and contact Linn Inc. at (800) 595-6770 for information on how to obtain free repair of the recalled product.

      Brand Imports is recalling about 1 million children's rings sold in vending machines nationwide. The rings contain high levels of lead, posing a risk of lead poisoning to young children.

      The metal rings are silver in color with shapes including hearts and stars, with slashes of colored paint.

      The rings were sold at vending machines in malls, discount department and grocery stores nationwide from December 2002 through August 2003 for about 25 cents.

      Consumers should throw the ring away or contact the company for more information.

      Valvoline, NAPA Brake Cleaner

      Valvoline is recalling its Pyroil Brake Parts Cleaner and NAPA Brake Cleaner. The affected aerosol cans contain a defective spray valve that might stick when depressed, possibly emptying the cans flammable contents.

      Valvoline has received nine complaints about sticking spray valves. One consumer was injured when the product sprayed into his eye.

      The recall involves 13-ounce cans of Pyroil Non-Chlorinated Brake Parts Cleaner and NAPA Non-Chlorinated Brake Cleaner. This product is used by professional mechanics as well as do-it-yourself consumers who perform brake repairs on their own vehicles.

      The recalled cans contain one of the following four digits in their date codes, which are found on the bottom of the cans:

      Martha Stewart Matches Recalled

      Kmart is recalling 588 boxes of Martha Stewart "Everyday" safety matches. The matches may ignite upon impact, posing a fire hazard to consumers.

      The recalled matches come in a yellow checkered box, with the UPC code 045774700237 printed on the back. The matches are 11 inches long.

      The matches were sold at Kmart Stores from January 2004 through March 2004 for about $2.

      Consumers should stop using the matches immediately and return them to the store where purchased for a full refund.

      "Webster" Spider Toy Recalled

      The "Webster" spider toy distributed by Mary Meyer Corporation is being recalled. The round stuffed feet on the spider can detach, posing a choking hazard to young children.

      Mary Meyer Corporation has received five reports of the round stuffed feet of the plush spider toy detaching. No injuries have been reported.

      The toy is a plush bug-shaped activity toy, which also can be hung from a crib, carriage or other object. The plush spider toy has eight legs with round stuffed feet that crinkle or rattle and a mirror on the underside that makes a squeaky sound when bounced.

      Department and specialty stores nationwide sold the toy from January 2002 through September 2003 for about $10.

      Consumers should return the toy to Mary Meyer Corp. to receive a replacement product of equal value.

      Consumer Contact: Mary Meyer Corporation at (800) 451-4387 between 9 a.m. and 5 p.m. ET Monday through Friday for instructions on returning the spider toy and receiving the replacement product.
