    Target Recalls Mini Learning Cubes

    Maxim Enterprise Inc. is recalling about 12,000 "Little Tree Mini Learning Cube" toys sold at Target. Small wooden pegs in the top corners of the toy can come loose, posing a choking hazard to young children.

    Target has received three reports of incidents including two reports of children mouthing pieces of the learning cube. In one incident, an 18-month old girl swallowed a piece of the recalled learning cube after one side came loose. The piece became stuck and she had to have it removed in a hospital emergency room.

    In a second incident, a child started choking when she put two of the toys wooden pegs in her mouth. In the third incident, a consumer reported the wooden pegs popped out of the learning cube. No injuries were reported in the third incident

    The recalled learning cube is an 8-inch square wooden box with a different activity on each side, including an abacus, blocks with apples and bananas, shapes that can be moved in a zigzag cutout, and a blackboard. The wooden cover has spiral bead maze with wooden beads that slide up, down and around.

    The cubes were sold at Target stores nationwide from June 2005 through November 2005 for about $15.

    Consumers should immediately take the recalled learning cube toy away from young children and return it to Target for a $15 giftcard plus applicable state tax.

    Trojan Water Disinfection System Recalled

    April 28, 2004
    The "TrojanUVMax" water disinfection system is being recalled. Some of the lamp pins used in these systems are improperly soldered, and can cause the units to overheat, posing a fire hazard.

    Trojan Technologies has received 40 reports of the systems overheating worldwide, resulting in some damaged units. No injuries or other property damage have been reported.

    The recalled TrojanUVMax system, designed for residential and light commercial application, uses ultraviolet light to disinfect water. TROJANUVMAX is written on the systems steel chamber.

    Models included in the recall are D, D Plus, E, E Plus, F, F Plus, Pro 7 and 15. Model numbers are written on the Underwriters Laboratories label on the power supply. No other Trojan Technologies model or product is included in this recall. The lamps were manufactured in the U.S. and the system is assembled in Canada.

    The units were sold by water treatment dealers, plumbing stores and through direct sales from plumbers from August 2000 through April 2004 for between $500 and $1,500.

    Step 2 Toddler Swings

    January 4, 2004
    The Step 2 Co. is recalling about 9,300 toddler swings. The straps on the swings could break, throwing the child to the ground.

    The recall includes toddler swings with model numbers 7336KR and 733604. The model numbers are located on a label on the back of the swings. The swings are made from green plastic and the red Step 2 logo is located on the front of the swing. The straps on the recalled swings are grey and blue colored.

    The swings were sold at toy and discount department stores nationwide from May 2003 through December 2004 for between $17 and $20.

    Consumers should stop using these swings and contact the firm to determine if their swing is involved in the recall. If it is, the firm will provide a free repair.

      DK Publishing Recalls Baby Books with Sound Makers

      DK Publishing is recalling children's board books that include a sound maker. The sound maker mounted inside a plastic covering on the last page of the books poses a choking hazard to young children if removed.

      DK Publishing has received one report of a 22-month-old child detaching the sound maker of one of the books and putting it in his mouth. The child was not injured.

      The heavy cardboard books were sold under eight different titles that feature photos or illustrations of the title theme: Dinosaurs, Emergency! (with a fire truck on the cover), Kittys Adventure, On The Road (with a yellow Volkswagon Beetle on the cover), Puppys Busy Day, Tractors, Trains and Trucks. All the books have a raised sound button in the lower right corner, which features a sound relevant to the title. The DK logo is in the lower left corner of the books.

      The books were sold by bookstores, gift stores, news stands, discount department stores and warehouse clubs nationwide, and directly to consumers by phone order from March 2001 through April 2004 for about $8.

      Consumers should take these books away from young children immediately and contact DK Publishing to receive a replacement book of equal value or a refund.

      DK Publishing Recalls Baby Books with Sound Makers...

      Bratz Stylin Scooter

      MGA Entertainment is recalling 297,000 Bratz Stylin' scooters manufactured by Jurong Dumar Bicycle, Inc. of China. This recall does not include Bratz Stylin Scooters manufactured for MGA by any manufacturer other than Jurong Dumar Bicycle, Inc.

      The wheels of the scooter can break or become damaged, and users of the scooters can fall and suffer injuries.

      MGA has received six reports of cracked wheels on the recalled scooters. All six incidents resulted in cuts, scrapes, and bruises. In one case, a 9 year old suffered a broken arm.

      The product is a non-motorized two-wheeled scooter with a folding hinge and an adjustable handlebar. The scooter platform is purple with a bright pink Bratz logo on the top surface. Scooters containing information identifying the product as Item No. 266563, with a date of manufacture prior to July 2004, and manufactured by Jurong Dumar Bicycle, Inc. are included in this recall. This identification information can be located either in the area directly beneath the scooter platform or at the bottom of the scooter near the rear wheel.

      The scooters were sold at toy and discount chain stores nationwide from September 2003 through November 2004 for about $30.

      Consumers should stop using the scooter immediately and contact MGA for a refund or a replacement scooter.

      Bratz Stylin Scooter...

      First Years Car-Based Baby Bottle Warmers

      March 1, 2005
      RC2 Corporation is recalling 96,831 car-based baby bottle warmers because the units could overheat.

      Involved in the recall are the First Years-brand Quick Time Travel Warmer and the 2 in 1 Feeding Tote and Bottle Warmer with product numbers 1179 and 1320. Both products heat baby bottles using a vehicle's power adapter.

      The National Highway Traffic Safety Administration said the warmers may operate at too high a voltage, which could overheat the bottle's contents.

      A spokeswoman for RC2 says the company discovered the problem because of consumer complaints and alerted NHTSA. One minor injury due to the defect has been reported to the company, Busman said.

      RC2 is investigating why the units are overheating. The company reported the defect to NHTSA because it is a vehicle-based product, and NHTSA oversees all safety-related vehicle recalls, including recalls of child seats. RC2 plans to buy back affected units beginning in March.



      First Years Car-Based Baby Bottle Warmers...

      Perrigo Infants Oral Drops Recalled

      August 1, 2005
      The U.S. Food and Drug Administration (FDA) is announcing that the Perrigo Company has initiated a voluntary nationwide recall of all lots of concentrated infants' oral drops that are packaged with a dosing syringe bearing only a "1.6 mL" mark containing acetaminophen; acetaminophen, dextromethorphan HBr, and pseudoephedrine HCl; or dextromethorphan HBr, and pseudoephedrine HCl.

      The dosing syringe may be confusing in determining the proper dose for infants under 2 years of age as directed by a doctor and could lead to improper dosing, including overdosing. The following products are being recalled to the retail level:

      • Cherry Flavor Infant Pain Reliever 160 mg Acetaminophen (0.5oz. and 1.0oz)
      • Grape Flavor Infant Pain Reliever 160 mg Acetaminophen (0.5oz. and 1.0oz)
      • Cherry Flavor Cough and Cold Infant Drops (0.5oz)
      • Cherry Flavor Decongestant and Cough Infant Drops (0.5oz)

      The directions on the bottle and carton labeling for infants ages 2-3 years and weighing 24-35 pounds allow safe and effective dosing for this age and weight group. However, these products are also intended for use by children younger than 2 years and weighing less than 24 pounds. The labeling directs consumers to ask a doctor for dosing directions for this age and weight group.

      The oral dosing syringe enclosed with these products is not marked so as to accurately measure doses less than 1.6 mL when prescribed by physicians for infants younger than 2 years and weighing less than 24 pounds. Until recently these products were provided with a dropper, not the oral dosing syringe, and the dropper had two markings on it ("0.4 mL" and "0.8 mL").

      The single mark on the current syringe along with the changeover from the dropper to this syringe has caused some confusion among consumers and health-care professionals and may lead to improper dosing. Taking more than the recommended dose (overdose) of acetaminophen may cause liver damage. Consumers who have questions should discuss this with their doctor to accurately determine proper dosage.

      In using an alternative over-the-counter product, parents and doctors should thoroughly discuss the specifics about the product and the dosing device, particularly the labeling and marking, so that the proper dose can be measured and administered correctly.

      The confusion of the dosing syringe was noted after a physician filed a complaint with the American Academy of Pediatrics.

      The recalled products were sold nationally at retail chains under the following store-brand labels: American Fare, Best Choice, Brooks, Berkley & Jensen, CVS, Dollar General, Eckerd, Equaline, Equate, Family Dollar, Food Lion, Good Neighbor, GoodSense, Healthy Generations, Health Pride, Hy-Vee, Kroger, Leader, Longs, Major, Medicine Shoppe, Meijer, Parklane, Publix, Rite Aid, Safeway, Shop Rite, Sunmark, Target, Todays Health, Top Care, Walgreen, Western Family, and Winn Dixie.

      Perrigo is cooperating with the FDA in this recall and in the effort to alert consumers and retailers about this issue.       Perrigo Infants Oral Drops Recalled...

      Lakewood "Sun-Sational" Electric Heaters

      March 17, 2004
      Lakewood Engineering is recalling the Sun-Sational electric heater. Electrical connections inside of the heater can become loose. This could cause the metal portion of the heater to become energized, posing a serious shock hazard to consumers.

      The recalled Sun-Sational Deluxe Radiant heaters have a metallic gray heating element, white base, and red control knobs. The heater has a label at the base that reads, SUN-sational and Warning: Risk of Fire. The oval-shaped heater has an 800 Watt and 1200 Watt setting and has a sensor to alert users of overheating.

      The heaters wre sold at retail stores nationwide, including Sams Club, between August 1996 and February 2004 for between $30 and $40.

      Consumers should immediately stop using these heaters, unplug them, and contact the company for a free repair or replacement. Lakewood also will provide consumers with instructions as to how to return the heater to the company free of charge.

      Lakewood Sun-Sational Electric Heaters...

      Michelin Recalls Pilot Sport Extra Load Tires

      December 31, 2005
      Michelin North America has notified the National Highway Traffic Safety Administration (NHTSA) and Transport Canada that it is recalling approximately 6,500 Michelin Pilot Sport 255/35ZR20 97Y Extra Load tires in the United States and Canada.

      To date, no accidents or injuries have been reported in North America. The decision came after Michelin's internal quality control system indicated that the tires do not meet its quality standards.

      The recall involves these specific tires and has no impact on any other Michelin tires.

      In an affected tire, the condition is ordinarily noticeable as a blister on the sidewall of the tire. The blister may produce a noticeable tire vibration, which a driver might detect.

      The condition may result in a rapid loss of air pressure, which could potentially lead to a loss of control and potential vehicle crash.

      To date, there have been no accidents or injuries reported in North America. The majority of the approximately 6,500 tires affected by the recall were distributed in the United States with fewer than 400 distributed in Canada.

      Related actions are under way in other countries.

      The recalled tires were fitted as original equipment on some trim levels of Jaguar XJ8 vehicles and also were available in the replacement market.

      The affected tires' DOT markings begin with FHPNEFMX and end with 00, 01, 02, 03 or 05.

      The company will be replacing all potentially affected tires in a comprehensive commitment to retrieve from the market any tire that does not meet Michelin quality standards, the company said.

      Replacement tires are available at no cost (including mounting and balancing) to consumers through Michelin tire dealers.

      Any consumers in the United States or Canada who believe they are affected by the recall should not wait to receive notification but should call Michelin Consumer Relations at 1-800-847-3435 (1-888-871-4444 in Canada) or return to their servicing tire dealer for a free inspection and replacement of affected tires.

      Spencer Gifts Skull & Crossbone Necklaces Recalled

      Spencer Gifts is recalling about 8,400 skull-and-crossbone necklaces because they contain high levels of lead.

      This recall involves a necklace with a skull-and-crossbones symbol and a metal clasp. The skull has eleven pieces of rhinestones on its forehead and one rhinestone on both ends of the crossbones.

      The necklaces were sold at Spirit Halloween and Spencer Gift stores nationwide from November 2006 to December 2008 for between $8 and $10. They were made in China.

      Consumers should immediately take the recalled necklaces away from children and contact Spencer Gifts for a refund or exchange.

      Spencer Gifts Skull & Crossbone Necklaces Recalled...

      Kawasaki Prairie ATVs

      March 15, 2005
      Kawasaki Motors is recalling the Prairie 700 ATV. The age recommendation warning label is missing. Children under 16 years old riding these adult-size ATVs can suffer serious injury or death.

      Kawasaki has not received any reports of incidents. No injuries have been reported.

      The recalled Kawasaki ATVs include the Prairie KVF700-D1 4x4 model, which were manufactured between June 2004 and February 2005. The ATVs come in green and red and have the words Kawasaki and Prairie printed on each side of the vehicles fuel tank.

      Consumers have been contacted directly and will receive a free sticker to adhere to their ATV immediately.

      Bugaboo Recalls Bee Strollers

      June 2, 2009
      Bugaboo is recalling about 22,500 Bee strollers because of a risk of brake failure.

      Bugaboo has received 121 reports of the stroller's brakes failing. No injuries have been reported.

      Bugaboo Bee strollers have item code 580210 on a label on the back of the seat and item code 50100 on a label on the plastic support under the seat. The strollers were sold in blue, dark khaki, pink, red, yellow, and black. "bugaboo bee" is printed on the left side of the seat.

      The strollers, made in Taiwan, were sold at juvenile product retailers nationwide and on various Web sites from August 2007 through April 2009 for about $530.

      Bugaboo Recalls Bee Strollers...

      Snowmen Holiday Lamps Recall

      May 19, 2004
      Great Gifts Inc. is recalling 1,900 snowman holiday lamps. The lamps have undersized wiring, no strain relief on the electric cords, and the molded plastic enclosure is flammable. The lamps pose fire and electrocution hazards.

      There are three styles of snowmen lamps included in the recall. The Turbo 1541 model is a train with a snowman and light with Joyful written on the top. The model 1536A consists of three snowmen with a light in a campfire. Model 1628 is three snowmen with a choir book and light. Each holiday light has the model number listed on its base with the label Made in China for Great Gifts McKeesport PA 15132.

      The lamps were sold at small gift stores nationwide from October 2003 through December 2003 for about $10.

      Consumers should stop using the lamps, and contact the firm to get information on returning them and receiving a refund.

      Snowmen Holiday Lamps Recall...

      GE Built-In Dishwashers

      February 18, 2005
      General Electric recalling about 74,000 built-in dishwashers, because of a connector that can short-circuit and overheat during normal use, posing a fire hazard to consumers.

      GE has received 29 reports of connectors overheating, including one report of a fire that spread outside the dishwasher and caused minor property damage. No injuries have been reported.

      The following models are included in this recall and were sold after January 20, 2004: GE dishwasher models GSD5500G, GSD5560G, GSD5800G, GSD5900G, GSD5960G, EDW3000G, and EDW3060G, with serial numbers with the first letter A through T and the second letter G, or the serial letters VF. The serial number is important as not all dishwashers with these model numbers are included in this recall. The model and serial number are located inside the door wall of the dishwasher.

      The units were sold at appliance retail outlets and builder distributors nationwide from January 2004 through February 2005 for between $300 and $400.

      Consumers should immediately stop using the dishwasher and contact GE to arrange for their dishwashers to be repaired free of charge.

      Backyard Products Recalls Swings

      May 27, 2004
      Backyard Products is recalling 47,000 swing sets because chains on the swing can detach during use, causing the child to fall to the ground and possibly suffer injuries.

      The company has received 31 reports of chains breaking, resulting in five reports of injuries including a sore back, cut lip, and bumps and bruises.

      This recall involves single swings and acrobatic swings sold separately or as part of the Backyard Products wood gym set. Additional options to the gym sets include overhead ladders, slides, swings and forts. A yellow, triangular metal gusset on the gym sets contains the brand name, Backyard Products followed by Collingwood, ON Canada.

      Model names and numbers are located on the assembly instructions. Contact Backyard Products or visit their Web site for model names and numbers included in the recall. Swings can be used on other gym sets.

      The swings were sold at department, toy and discount stores nationwide from December 2003 through April 2004 for between $300 and $1,200 for the sets. The swings were sold separately from January 2004 through April 2004 for between $12 and $16.

      Consumers should not let children use the swings until they have been repaired. Backyard Products will provide consumers with a free, in-home repair kit.

      Pier 1 Imports Recalls Tea Light Candleholders

      May 27, 2004
      Pier 1 Imports is recalling about 48,000 beaded fish tea light candleholders. Hazard: The beads on these tea light candleholders can catch fire during use.

      Pier 1 has received 10 reports of these candleholders either melting or catching fire, causing some minor property damage. No injuries have been reported.

      These Beaded Fish tea light candleholders are red, pink or blue, and about 4-inches high and 8-inches long. The price sticker reads Pier 1, China, and SKU 1997504, SKU 1998132 or SKU 1998145.

      The items were sold at Pier 1 Import stores nationwide and on the firms Web site from February 2004 through May 14, 2004 for about $10.

      Consumers should stop using these tea light candleholders and return them to Pier 1 for a refund or store credit. Consumers also can remove and discard the tea light hanger inside the item, and use the beaded fish as a decorative light catcher only.

      Pier 1 Imports Recalls Tea Light Candleholders...

      Kawasaki Brute Force ATV

      February 28, 2005
      Kawasaki is recalling the Brute Force ATV. On some units, the nut which secures the front upper suspension arm pivot bolt could come loose during vehicle use.

      Continued use of the vehicle could allow the nut to come off, and the upper suspension A-arm pivot bolt to back partially out. This would result in a loss of steering control, and possibly cause the rider to crash and suffer injury or death.

      Kawasaki has received 14 reports of incidents with three minor injuries reported.

      The recalled Kawasaki ATVs include the Brute Force 750 4x4i model, which was manufactured between April 26, 2004 and September 10, 2004. The ATVs come in red, green, yellow or camouflage, and have the words Kawasaki and Brute Force printed on each side of the vehicles fuel tank. VTwin 750 is printed on each side of the rear fenders, and Kawasaki is embossed on the front grille and rear mud flaps above the rear wheels.

      Authorized Kawasaki dealers sold the ATVs from July 2004 through early September 2004 for about $7,599.

      Consumers should stop using the ATVs immediately and contact their Kawasaki dealer to schedule an appointment for a free inspection and repair.

      Cannondale Recalls Road Bicycles Due to Fall Hazard

      March 17, 2009

      Cannondale is recalling about 1,300 road bicycles because they fail to meet the federal safety standard for bicycles. Spoke protector discs, required on bicycles to prevent the bicycle chain from interfering or suddenly stopping the wheel, are missing from these bicycles. This poses a fall hazard to the rider.

      Named in the recall notice are 2009 Six 5, Six 6, Six Carbon 5 and Six Carbon 6 Bicycles. "Six" is printed on the bicycles top tube and "Cannondale" is printed on the down tube.

      The bicycles, made in Taiwan, were sold by authorized Cannondale dealers nationwide from October 2008 through February 2009 for between $1,500 and $2,100.

      Consumers should stop using the recalled bicycles immediately and contact any authorized Cannondale bicycle dealer for a free repair.

      HeatSeal Pouch Laminators

      General Binding Corp. is recalling its HeatSeal Pouch Laminators. Improper wiring at the heating coil could cause the pouch laminators to overheat and become a fire hazard.

      The recall includes the H300 and H200 model HeatSeal: Pouch Laminators. They are used to laminate documents and photos. The recalled H300 Pouch Laminators have serial numbers starting with PI, PJ, PK, PL, QA, QB, QC or QD. The recalled H200 Pouch Laminators have serial numbers starting with PI or PJ. The H300 and H200 model numbers are found on the top of the Pouch Laminators, and on the rating plate on the bottom of the Pouch Laminators. The serial numbers are found on the rating plate on the bottom of the Pouch Laminators.

      The units were sold at office supply stores nationwide from September 2003 through September 2004 for between $200 and $260.
