    Fourstar Childrens Folding Chairs

    Fourstar Group is recalling about 522,000 children's folding chairs. The chairs safety lock can fail, allowing the chair to collapse or fold unexpectedly. Childrens fingers can become caught or entrapped in the hinge and slot areas of the chair, posing a pinch or cut hazard. This can cause severe lacerations and finger tip amputations to childrens fingers.

    CPSC and Fourstar Group are aware of three incidents involving children. One resulted in a laceration to the finger and three finger fractures, the second incident resulted in a fracture and laceration to the fingers, and the third incident resulted in a pinched finger.

    The recalled kids folding chairs are made of metal tubing with a vinyl padded seat and seat back. They were sold in red, blue, yellow, green, pink, and lavender solid colors. They also come in pink with Princess printed in red on the seat and seat back along with stars, crowns and hearts, or in blue with pictures of basketballs, soccer balls, footballs, football helmets, and rollerblades. The recalled chairs were sold individually or as part of a set consisting of a table and two chairs.

    Each chair is about 22-inches high, 13.5-inches wide, and about 11.5-inches deep. MANUFACTURED BY FOURSTAR GROUP INC. OR IMPORTED BY FOURSTAR INTERNATIONAL TRADING COMPANY is written on a label located underneath the seat of the chairs.

    The chairs were sold at discount and grocery stores nationwide from July 2003 through July 2005 for about $5 individually or about $25 for a set.

    Consumers should stop using the chairs immediately, and contact Fourstar for instructions on how to receive a free repair kit to replace the locking pin.

    Oberle Meats Recalls Meat Products

    Oberle Meats, a St. Genevieve, Mo., establishment, is voluntarily recalling approximately 1,077 pounds of ready-to-eat meat products that may be contaminated with Listeria monocytogenes, the U.S. Department of Agriculture's Food Safety and Inspection Service (FSIS) announced today.

    Products subject to recall include:

    1- pound packages of "OBERLE, SMOKED PORK SAUSAGE, HICKORY SMOKED, READY TO EAT."
    1- pound packages of "OBERLE SAUSAGE, HICKORY SMOKED, READY TO EAT."
    .60- pound packages of "OBERLE, LIVER SAUSAGE, READY TO EAT."
    .15- pound packages of "OBERLE, SNACK STICKS, HICKORY SMOKED, READY TO EAT."
    .75- pound packages of "OBERLE, WIENERS, HICKORY SMOKED, READY TO EAT."

    Each package bears the establishment code "EST. 2334" inside the USDA mark of inspection.

    The products were produced on April 11 and were distributed to retail establishments in Illinois and Missouri. The problem was discovered through routine FSIS regulatory sampling. FSIS has received no reports of illnesses associated with consumption of these products.

    Consumption of food contaminated with Listeria monocytogenes can cause listeriosis, an uncommon but potentially fatal disease. Healthy people rarely contract listeriosis. However, listeriosis can cause high fever, severe headache, neck stiffness and nausea. Listeriosis can also cause miscarriages and stillbirths, as well as serious and sometimes fatal infections in those with weakened immune systems including infants, the elderly and persons with chronic disease, such as HIV infection or undergoing chemotherapy.

    Yamaha Recalls Synthesizers

    November 30, 2005
    Yamaha is recalling about 1,100 S90 ES Synthesizers. The unit could emit loud "white noise" when turned off and turned on again under high temperature conditions, which could possibly damage a consumers hearing.

    The S90 ES Synthesizer is black with a black and white keyboard. "S90 ES" and "Yamaha" are written on the front of the synthesizer.

    The units were sold at musical instrument retailers and dealers from August 2005 through October 2005 for a Manufacturers Suggested Retail Price of $2,599.

    Yamaha will contact each customer to arrange for a repair or replacement at no cost to the customer.

      Stork Craft Toy Boxes

      Stork Craft Manufacturing is recalling about 3,300 toy boxes. The lid support can fail, allowing the lid to collapse suddenly. This poses a strangulation hazard to young children, and possible impact injuries to a childs head, neck, fingers or hands.

      CPSC and Stork Craft have received two reports of the lid collapsing. No injuries have been reported. Stork Craft has received 11 consumer requests for replacement lid supports.

      The recall includes toy boxes with manufacture date codes between November 2002 and July 2004 and the following model numbers and colors: 01520-331 white, 01520-334 cherry, 01520-33C cognac, 01520-33L oak, and 01520-33N - natural. The manufacture date codes, model numbers, and colors are located on a label underneath the toy boxes. The toy boxes measure 27-inches long by 18-inches wide and 17-inches high. The boxes are constructed of pine.

      The boxes were sold at Wal-Mart and J.C. Penney Web sites, and at furniture, baby and juvenile stores nationwide from December 2002 through April 2005 for about $90.

      Consumers should immediately stop young children from using these toy boxes and contact Stork Craft for information to determine if their toy box lid support needs to be replaced. If so, the firm will provide a free replacement lid support.

      Dell, Duracell Power Adapters Recalled

      October 28, 2008    
      Duracell power adapters used with laptop computers are being recalled because they can overheat.

      The manufacturer, Battery-Biz, has received five reports of power adapters overheating. No injuries have been reported.

      This recall involves the Duracell 130W Combo AC/DC adapters with model numbers EA10900, AC-6501, and DRUM130 and date codes 0804 and 0805. The black power adapter is used with notebook computers. 'DURACELL UNIVERSAL POWER ADAPTER' is printed on the front of the power adapter. The model and date codes can be found on the rating label on the back of the power adapter.

      The adapters, made in China, were sold at www.dell.com and www.duracelldirect.com from February 2008 through March 2008 for between $80 and $120.

      Consumers should immediately stop using the power adapter and contact Battery-Biz to return it and obtain a free replacement. Battery-Biz has sent a direct notice to consumers with recalled power adapters.

      Didj Gaming System Batteries Recalled

      October 23, 2008
      Leapfrog Enterprises is recalling about 35,000 batteries and charging stations for the Didj custom gaming system. The batteries can overheat if the gaming system is placed into the recharging base upside down, posing a burn hazard to consumers.

      LeapFrog has received 12 reports of the rechargeable batteries overheating, including one report of a minor hand burn.

      The Recharging Station comes with two rechargeable batteries, an AC adapter, and a recharging base. The recharging base is white with a green indicator light on the bottom. The Recharging Station for Didj was sold as item number 30676, which is printed on the box and on the bottom of the recharging base.

      The devices were sold by department stores and toy stores nationwide and www.leapfrog.com, and other online retailers from July 2008 through October 2008 for about $35. They were made in China.

      Consumers should immediately stop using the recharging base and the two rechargeable batteries and contact LeapFrog for a full refund. Consumers can continue using the AC adapter connected directly to the Didj handheld.

      Cat and Dog Menorahs Recalled

      Continental Creations, Inc. is recalling about 480 Cat and Dog Menorahs. The cups holding the candles on these menorahs are not constructed of the correct material and could ignite, posing a fire hazard.

      Cat and Dog Menorahs RecalledThe recalled menorahs come in two styles. There is a cat menorah with cats sitting near the candle cup holders. There is a dog menorah with dogs sitting near the candle cup holders. The menorahs have a sticker on the base stating either "P1157" or "P1166".

      The menorahs were sold at specialty stores nationwide sold the menorahs from July 2005 through August 2005 for about $20.

      Consumers should immediately stop using the menorah and return it to the retailer for a full refund.

      Cat and Dog Menorahs Recalled...

      Children's Metal Charms

      March 3, 2005
      Hirschberg Schutz & Co. Inc. is recalling about 2.8 million metal charms because they contain high levels of lead, posing a serious risk of lead poisoning to young children.

      The Consumer Product Safety Commission received a report of a six-year-old girl who mouthed these charms worn on a homemade necklace. She developed elevated lead levels in her blood that may be related to the charms. Lead poisoning in children is associated with behavioral problems, learning disabilities, hearing problems and growth retardation.

      The recalled metal charms were sold under the name Charming Thoughts. Most of the charms are silver-colored with small silver loops. They were sold in packages of two to 12 pieces. The packages are marked The Card Connection, Charming Thoughts and Hirschberg Schutz & Co. Inc.

      The metal charms are various shapes including small hearts, crowns, birds, picture frames, perfume bottles and a cross. Some of the metal charms have small blue, pink or yellow stones and are printed with words including princess, congratulations, city girl, world traveler and lifes blessings. Insert photo here, cherish, love, and honor are printed on the picture frame charms.

      The metal charms were sold as decorations for place cards, greeting cards, collages, memory boxes, gift cards, scrapbooks, invitations and gift bags. The charms also can be attached to necklaces and bracelets.

      The recalled metal charms were sold at Michaels Stores from July 2002 through February 2005, at Recollections stores from October 2004 through February 2005, and at Hancock Fabrics stores from January 2004 through January 2005 for between $3 to $4. All of the charms were manufactured in China.

      Candlsense Electric Candle Warmers Recalled

      May 29, 2008    
      'Candlsense' electric candle warmers are being recalled by Provo Craft & Novelty. The internal heating element of the candle warmer can detach and melt the bottom of its plastic casing, which can in turn ignite or scorch the surface on which the candle warmer is placed, posing a fire hazard to consumers.

      Provo Craft has received 11 reports of incidents in which the heating elements detached, including two fires and nine incidents of property scorching. One consumer reported a blistered finger.

      This recall involves the Candlsense warmers with product codes ranging from YD0629 through YD0652 and YD0701 through YD0708. The candle warmers measure about 4 inches in diameter. The heating plate is black and the surrounding area is white plastic. 'Provo Craft & Novelty Candle Warmer Air Freshener Model: WCY-8888' and the product code are embossed on the underside of the warmer.

      The items, made in China, were sold at Wal-Mart and other retailers nationwide from August 2006 through October 2007 for between $5 and $10.

      Consumers should contact Provo Craft to receive a voucher worth 120 percent of the purchase price of the candle warmer. The voucher can be redeemed at www.creativexpress.com.

      Candlsense Electric Candle Warmers Recalled...

      Radio Shack Video Head Cleaner

      May 11, 2005
      Radio Shack is recalling about 1 million units of its Wet System Video Head Cleaner. The cleaning solution bottle, which is labeled as containing isopropanol, actually contains methanol. Methanol can be fatal or cause blindness if swallowed.

      Methanol is much more toxic than isopropanol. Also, the cleaning solution bottle does not have special cautionary labeling and child-resistant packaging, as required by Federal regulation for methanol.

      RadioShack has received 39 reports from their poison center of children accessing this product. No injuries or illnesses have been reported.

      The recalled non-abrasive Wet System Video Head Cleaner was sold in a box labeled Video Head Cleaner, RadioShack and 44-1230 or 44-1213. The box contains a cleaning video cassette and a bottle of cleaning solution. The 4-inch-high white plastic cleaning solution bottle is labeled Video Head Cleaning Fluid or Cleaning Liquid for HEAD CLEANER. Other writing on the bottle includes Contents: Isopropanol, and the item number 44-1230 or 44-1213.

      RadioShack stores nationwide and www.radioshack.com from December 1995 through January 2005 for about $13.

      Consumers should immediately place this product out of reach of children and return the product to the nearest RadioShack store for a refund, or dispose of the bottle of cleaning fluid in accordance with all state and local requirements.

      Radio Shack Video Head Cleaner...

      Childrens Reebok Jacket and Pant Sets

      March 24, 2005
      Reebok Childrens Windwear and Fleece Jacket and Pant Sets are being recalled. The zipper slider and pull on the jackets can detach, if pulled when the jacket is open. The detached zipper slider and pull can pose a choking hazard to young children.

      Reebok has received three reports of zipper sliders/pulls that detached. No injuries have been reported.

      The recalled jacket/pant sets were sold in royal blue, pink, pink/blue, purple/green, pink, purple, red/blue, and gray/dark gray color combinations in sizes up to childrens size 7. Reebok is printed across the front of the hooded jackets. The style numbers were printed on the store tag only and end in: 451, 449, 448, 447, 446, 444, 435, 433, 429, 428, 424, 412, 537, 531, 530 and 526.

      The recalled jacket/pant sets were sold exclusively at Gordmans, Fred Meyer, Kids R Us, Ross, Gottschalks and Reebok Corporate Headquarters retail store in Canton, Massachusetts between August 2004 and February 2005 for between $15 and $20. All other jacket/pant sets purchased at any other retailer are not included in this recall.

      ESI-R Wooden Screening Blocks Recalled

      January 24, 2008
      RR Donnelley is recalling about 18,000 sets of ESI-R screening materials. Surface paint on the green wooden blocks contains excessive levels of lead, violating the federal lead paint standard.

      The green wooden blocks are part of the ESI-R Screening Materials, educational materials used by schools to assess young children. The items include 10 green wooden blocks, a plastic yellow car, an 8-ft. white cloth ribbon, 10 black and white printed cards with shapes, a red rubber ball, and a blue plastic button.

      The blocks were distributed by Pearson Learning Group to schools, examiners, and educators nationwide via sales representatives, catalogs, and the firm's Web sites from January 2003 through November 2007 for about $23. The materials are part of a larger ESI-R kit that sold for about $120.

      The blocks were made in China.

      Purchasers should immediately stop using the blocks in the ESI-R Screening Materials. RR Donnelley will provide free replacement materials and instructions for returning the recalled product. RR Donnelley is directly contacting purchasers and posting information on its Web site.

      Intermatic Recalls Digital Timers

      January 9, 2008
      Intermatic is recalling about 8,500 DT27 Digital Self-Adjusting Timers. The recalled timers could have been wired incorrectly, which poses an electrical shock hazard to consumers.

      This recall involves the Intermatic DT27 digital self-adjusting timer with a date code of 04B. This product is a lamp and appliance timer and is packaged as either the DT27C Heavy Duty Digital Timer or the DT27CL Time All Digital Lamp and Appliance Self-Adjusting Timer.

      The timer is white with a rounded top, and measures 3 7/8 tall by 2 3/4 wide by 1 5/8 deep. The brand name Intermatic is molded on the front of the timer, and the model number (DT27) and date code (04B) are printed on the back of the timer.

      The times were sold at retailers nationwide, including Home Depot and Lowes, from February 2007 through December 2007 for between $15 and $25. They were made in China.

      Consumers should stop using the recalled timer immediately and unplug before checking the model number and date code. Consumers should contact Intermatic to return the timer and obtain a free replacement.

      Briggs & Stratton is recalling about 2,100 Murray riding lawn mowers. The two-piece fuel tanks on the mowers can separate at the seam, causing fuel to leak. This can pose fire and burn hazards to consumers.

      Briggs & Stratton has received two reports of fuel leaks. No injuries or incidents have been reported.

      This recall involves Murray front engine riding lawnmowers with the model and serial numbers listed below. The model and serial numbers are located on an identification tag on the rear frame of the unit.

      Pendants and Candle Charms

      Pendants and candle charms imported by Benjamin International are being recalled. They contain high levels of lead. Lead is toxic if ingested by young children and can cause adverse health effects.

      The recalled pewter pendants were sold in eight collections of 12 pendants each. The pendant collections included Signs of the Zodiac, Runes, Ritual, Goddess, Chinese Zodiac, and Celtic Harmonies. The pendants were sold separately and on candles of various colors and scents.

      The items, made in Korea, were sold at gift shops nationwide from January 2002 through November 2007 for between $6 and $8 for the pendants and between $12 and $15 for the charms with candles.

      Consumers should immediately stop using these pendants/charms and contact Benjamin International to receive a free replacement pendant/charm.

      Pendants and Candle Charms...

      Smoked Salmon Alert

      The U.S. Food and Drug Administration (FDA) is issuing a Safety Alert to consumers about the recall of Smoked Salmon, Skinless Sliced Sides packaged in various 2 to 4 lb. weight packages because they may be contaminated with Listeria monocytogenes.

      The products are sold under the brand names: Imperial Salmon House, Superior brand Norwegian Cure and Golden Eagle Smoked Salmon.

      The alert extends to packages produced on June 13th, 2005 and have a shelf life of 3-4 months if maintained in an un-opened frozen state, four days if kept refrigerated.

      They are sold in individual 2 to 4 lb. packages labeled as: "Processed by Hickory House, Hialeah, FL 33016", "21555, Product of the USA", "keep frozen until ready to use." The product was sold in Florida, Georgia, New York and Virginia.

      Listeria monocytogenes is an organism that can be serious and sometimes cause fatal infections in young children, frail or elderly people, and others with weakened immune systems.

      Although healthy individuals may suffer only short-term symptoms such as: high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, Listeria infection can cause miscarriages and stillbirths among pregnant women.

      If you experience the symptoms listed above, you should contact your health care provider. No illnesses have been reported to date.

      The contamination was noted after routine testing by the Florida Department of Agriculture and Consumer Services revealed the presence of Listeria monocytogenes.

      Smoked Salmon Alert...

      Karl Kani Recalls Boys' Hoody Sweatshirts

      Karl Kani hooded sweatshirts with drawstring are being recalled because they present a strangulation hazard.

      Karl Kani boys full-zip fleece hoody sweatshirts with model numbers MB7808, MB7809, MB7810, MB7812. The model numbers were printed on the original hang tag. The words 'Kani Polo' are printed on the sweatshirt neck tag. The sweatshirts were sold in various colors including, blue, gray, black, red and brown in boy's sizes small (8-10) and medium (12-14).

      The sweatshirts were sold at A&E Store, DD Discount, Forman Mills, The Marmaxx Groups, and Suit Mart stores nationwide from July 2007 through October 2007 for about $50. They were made in China.

      Consumers should immediately remove the drawstring from the sweatshirts to eliminate the hazard and or contact Siegfried & Parzifal for a refund.

      International Playthings Inc. is recalling about 1,900 Viking Chubbies Toy Cars. The heads on the figures sold with the cars can detach, posing a choking hazard to young children.

      No injuries or incidents have been reported.

      This recall involves Viking Chubbies 10-inch toy sedans sold with two figurines. The multi-colored toys are made of plastic. The toy figurines measure about 3-inches tall. Viking Toys, Sweden and Made in Thailand are printed on the bottom of the car.

      The toys were sold at specialty toy stores nationwide from June 2005 through August 2005 for about $14.

      Consumers should contact International Playthings for information on returning the product for a free replacement item of similar value.

      'It's a Girl Thing' Jewelry Recalled

      Bead Bazaar USA is recalling about 13,000 sets of Its a Girl Thing Bracelets, Necklaces, and Phone Charms.

      The childrens jewelry could contain high levels of lead. Lead is toxic if ingested by young children and can cause adverse health effects.

      This recall involves Its a Girl Thing metal necklaces, bracelets, and phone charms. The jewelry features a variety of charms including flowers, shoes, letters and butterflies. Its a Girl Thing is printed on the packaging along with the UPC numbers 633870018419 (bracelet), 633870018426 (necklace), and 633870018433 (phone charm).

      The jewelry, made in China, was sold at retailers nationwide from February 2006 through June 2008 for about $6.

      Consumers should immediately take the recalled jewelry away from children and return it to the place of purchase for a full refund.

      Automakers recall older model trucks, SUVs Ford to fix nearly 4 million; Toyota calls back 978,000 CLICK HERE DETROIT (Reuters) -- Ford Motor Co. on Wednesday said it was recalling nearly 4 million pickup trucks and sport utility vehicles in the United States because of a faulty speed control deactivation switch that may cause underhood fires. Separately, Toyota Motor Corp. said it was recalling 978,000 pickup trucks and sport utility vehicles sold on the U.S. market because a steering relay rod on the vehicles may fracture, causing a loss of control. The estimated 3.8 million Ford vehicles affected by the recall are from the 1994-2002 model years and include the Ford F-150 pickup as well as the Ford Expedition, Lincoln Navigator and Ford Bronco SUVs, the second-largest U.S. automaker said. Ford spokeswoman Kristen Kinley said the recall is the fourth largest in Ford's history. Ford said the brake fluid in the recalled vehicles may leak through the speed control deactivation switch into the speed control system electrical components, potentially corroding them and leading to fires. The automaker said it will install a fused wiring harness between the switch and the speed control mechanism of the affected vehicles to act as a circuit breaker, eliminating the electrical current to the switch. The second-largest U.S. automaker in January recalled about 792,000 pickup trucks and SUVs for a similar problem involving the speed control switch. The recall involved some of Ford's biggest money-makers from 2000 model-year F-150 pickup trucks, Expedition and Navigator SUVs and 2001 model-year F-Series Supercrew pickup trucks equipped with cruise control. The National Highway Traffic Safety Administration has been investigating the recalled Ford vehicles since early this year after the safety agency received about 68 complaints of fires from customers. Toyota vehicles affected by its recall are from the 1989-1996 model years and include power-steering equipped 4Runner sport utility vehicles and compact pickups and T-100 pickups, Toyota said.
