    Baby Necessities Pacifiers Recalled

    OKK Trading is recalling nearly 3,000 Baby Necessities pacifiers. The pacifiers are banned under federal law. They failed federal safety tests when the nipples separated from the base. This poses a choking hazard to young children.

    This recall involves Baby Necessities pacifiers with a blue heart-shaped mouth guard. The pacifiers were sold four to a pack. The words Baby Necessities is printed on the cardboard packaging.

    The pacifiers, made in China, were sold at Dollar and discount stores nationwide from August 2007 through January 2009 for about $1.

    Consumers should immediately take the recalled pacifier away from children and contact OKK Trading for a refund or exchange.

    Samara Brothers Recalls Boys' Jackets

    Samara Brothers is recalling about 8,000 boys' heavyweight outerwear jackets. The snap closures on the outerwear jackets contain excessive amounts of lead, which poses a lead poisoning hazard.

    This recall involves boy's outerwear jackets with a Carter's-brand logo on the front. The jackets were sold in toddler and boy's sizes. Toddler sizes range from 2T to 4T. Boy's sizes include S, M and L. The outerwear jackets were sold in charcoal, navy and red.

    The jackets were sold at major department stores and discount department stores nationwide from October 2006 through November 2006 for up to $50.

    Consumers should stop using the products immediately and contact Samara Brothers to obtain a full refund.

    Recalled LaJobi Cribs May Be in Thrift Stores

    LaJobi Inc. is reissuing a 2001 recall of its 'Molly' and 'Betsy' cribs, The cribs have cut-outs in the end panels that can allow young children to get their heads entrapped, which can pose a strangulation hazard to infants and toddlers.

    Safety regulators say they are aware of the death of a one-year old girl whose head and neck became entrapped in the headboard cut-out of the 'Betsy' style crib in 2003 in Mississippi, following the 2001 recall. No additional incidents or injuries have been reported.

    This recall involves both 'Molly' and 'Betsy' style wooden cribs. The end panels on the "Molly" style cribs are made of solid wood with openings on both sides. The end panels on the "Betsy" style cribs are constructed with wood slats. The model numbers are printed inside the headboard at the bottom.

    The cribs were sold at juvenile specialty stores nationwide from May 2000 through September 2001 for about $700 for the Molly model and $650 for the Betsy model. The cribs have not been available at retail since 2001.

    The follow-up recall is intended to alert consumers who may still have these cribs in their possession and to alert thrift stores and other second-hand stores that these cribs should not be given away, sold or continued to be used.

    Consumers should stop using these cribs immediately and contact LaJobi to receive replacement end panels which eliminates the hazard.

      Child Dies; Wooden Storage Racks Recalled

      Wooden storage racks sold at Wal-Mart are being recalled after the death of an eight-month-old boy.

      The U.S. Consumer Product Safety Commission (CPSC) said the child was asphyxiated after he pulled on the storage rack and it fell over on him.

      Jetmax International Ltd., of Irving, Texas is voluntarily recalling about 36,000 of the storage racks and canvas totes.

      The storage rack is wooden with three levels and nine removable canvas totes. There are wooden handles on each side of the rack. The boys storage rack has natural color wood with red, yellow, green, and navy canvas totes. The girls storage rack has white colored wood with pink, yellow, lime, and purple canvas totes.

      Wal-Mart sold the storage rack under the brand Home Trend Kids 9 Canvas Bin Boys and Girls Organizers. They were made in China.

      The organizers were sold at Wal-Mart stores nationwide from August 2004 through July 2005 and Ollies stores nationwide from July 2006 through June 2007 for about $40. Consumers should immediately stop children from using the recalled storage racks and contact Jetmax International to receive a free repair kit that adds stability to the base.

      Polaris Assault Snowmobiles Recalled

      Polaris is recalling about 800 of its 2009 Assault model snowmobiles. Bolts on the rear suspension can break causing the rail tip to become dislodged and interfere with the snowmobiles track. This can cause the track to lock up and the rider to lose control of the snowmobile, posing a risk of injury.

      The firm has received reports of eight incidents in which the rail tip bolts broke, two of which resulted in the rider being ejected from the snowmobile. No injuries have been reported.

      This recall involves 2009 Polaris 800 Assault RMK 146 snowmobiles model numbers S09PL8EE and S09PL8ES. Assault 800 is printed on the snowmobiles hood. Polaris is written on the seat. The model number is located on the right side of the tunnel below the seat. The snowmobiles are red and black.

      The snowmobiles, made in the U.S., were sold by Polaris dealers nationwide from September 2008 through June 2009 for about $10,600.

      Consumers should immediately stop using the recalled snowmobiles and contact any Polaris snowmobile dealer to schedule a free repair. Polaris has notified registered consumers directly about this recall.

      Polaris Assault Snowmobiles Recalled...

      Amprobe Digital Clamp Meters

      Amprobe is recalling about 70,000 digital clamp meters used for electrical testing. The meters can fail to give an appropriate voltage reading, resulting in the operator believing the electrical power is off, which can pose a risk of shock, electrocution, or thermal burn hazard.

      Amprobe is aware of one report of a clamp meter displaying an incorrect voltage reading. No injuries have been reported.

      Only Amprobe brand digital clamp meter models ACD-10PRO, ACD-10 TRMS-PRO, ACD-14 and ACD-14TRMS, except those model numbers followed by "FX" or "PLUS," are included in the recall. "Amprobe" and the model numbers are printed on the front of the units. These clamp meters measure 0 to 600 volts alternating current (VAC), and 0 to 600 volts direct current (VDC). In addition, they measure 0 to 400 amps alternating current. The tester body is red and grey and measures 7-1/2 inches in length by 2-1/2 inches in width by 1-1/4 inches thick.

      The meters were sold by industrial distributors, electrical wholesalers and hardware stores nationwide from January 2002 through December 2006 for about $100.

      Consumers should stop using these recalled clamp meters immediately and contact Amprobe for a free replacement clamp meter.

      TOBY N.Y.C. Expands Childrens Metal Jewelry Recall

      TOBY N.Y.C. is expanding an earlier recall of TOBY & MEchildrens metal jewelry contaminated by excessive lead.

      The latest recall is for about 23,500 units. Another 14,000 jewelry sets were recalled on August 22, 2007.

      The recalled metal jewelry sets contain high levels of lead. Lead is toxic if ingested by young children and can cause adverse health effects.

      This recall involves jewelry sets sold in different styles: a pink and clear crystal bead necklace and bracelet set with a painted metallic crown pendant; a pink and white pearl necklace and bracelet set with a painted metallic poodle pendant; and a pink pearl necklace, earrings and ring set. This recall is expanded to include two additional styles: a pink crystal and wood bead necklace and bracelet set with a monkey pendant; and a pink and clear crystal bead necklace and bracelet set with a heart pendant. All sets are sold in a pink gift box with TOBY & ME printed on the front and TOBY & ME hangtags attached to the packaging.

      The sets were sold at T.J. Maxx, Marshalls, A.J. Wright, Cracker Barrel, and Shopko stores nationwide from August 2006 through August 2007 for between $8 and $15. They were made in China.

      Consumers should immediately take the recalled jewelry away from children and contact TOBY N.Y.C. for information on receiving a full refund or replacement item.

      Respironics Recalls SmartMonitor2 Infant Apnea Monitor

      Respironics Inc. is recalling the SmartMonitor 2 infant panea monitor, models 4002 and 4003 with serial numbers 3000033364 through 3000038740, because it may fail to sound an alarm.

      This product was manufactured from January 16, 2008 through November 13, 2008 and distributed from January 17, 2008 through December 31, 2008.

      The device is used for the continuous monitoring of respiration and heart rate of infant patients in the home or in the hospital. The monitor detects and alarms for periods of temporary interruption of breathing (central apnea) or low heart rates.

      On April 23, 2009, the firm sent a letter to their medical supply companies informing them of the potential problem. The firm requested the companies take the following actions:

      • return all units in their inventory

      • retrieve and return all units already shipped to patients

      Class 1 recalls are the most serious type of recall and involve situations in which there is a reasonable probability that use of these products will cause serious injury or death.

      Hunter's View Safety Harnesses

      Hunter's View is expanding an earlier recall of safety harnesses sold with tree stands. It is also recalling the harnesses which were provided as replacements under an earlier recall.

      The harnesses could fail during use, resulting in a hunter falling from the tree stand and suffering serious injuries or death.

      The recall involves all 2004 and 2005 model year Fall Arrest Systems (FAS), or harnesses with 'Model Year 2004' or 'Model Year 2005' printed on a white Hunter's View label attached to the harness or the tree strap. The recall includes all model number harnesses sold during these years. The recall also includes replacement harnesses provided to consumers as a result of the previous recall.

      The harnesses were sold by hunting and sporting good stores nationwide beginning in January 2004 for between $30 and $300.

      Consumers should stop using these harnesses immediately and contact Hunter's View to receive a free replacement harness. Consumers who previously participated in this recall are being directly contacted about this recall. As an alternative to the free replacement harness, the firm also is offering an upgraded harness at a reduced cost.

      Home Decorators Collection is recalling about 500 leather suitcase trunks because of an entrapment and suffocation hazard.

      The trunk has an exterior clasp that can lock unexpectedly when the trunk lid closes. If a child climbs inside the trunk, he or she may not be able to open the trunk from the inside. This poses an entrapment and suffocation hazard to children.

      The recalled trunks are leather bound and have a handle on each side, and a clasp on the front. The trunks are burgundy or brown and are sold in three sizes: small, medium and large. The following catalog numbers were used to identify the product:

      Pirelli Recalls Pzero Corsa Tires

      Pirelli is recalling certain Pirelli Pzero Corsa tires, size 285/35 ZR19 (99Y) K1, IP 1778200 (left) and IP 1778300 (right) manufactured between June 2007 and July 2009.

      Partial tread and/or belt edge separation can occur at high speeds at and over 270 KMH or 168 MPH. Tread or belt edge separation could result in a vehicle crash. The manufacturer has not yet provided a remedy and notification schedule.

      Pirelli Recalls Pzero Corsa Tires...

      "March or Die" Fireworks

      Jakes Fireworks Inc. is recalling about 4,000 of its "March or Die" fireworks.

      The tubes on these fireworks devices could become loose, making the devices unstable during use. If the device tips over during use, it could pose burn and injury hazards to bystanders.

      The recalled fireworks are a 500-gram mine/shell device that consists of nine, 3-inch tubes with a single fuse for ignition. The device measures 16x16x13 and its packaging is a dark blue cardboard label with the words March or Die printed in red.

      The items were sold by World Class Distributors nationwide from April 2006 through May 2006 for $40.

      Consumers should immediately stop using the product and contact Jakes Fireworks for a replacement product.

      Fluorescent Ceiling Light Fixtures Recalled

      Lithonia fluorescent ceiling light fixtures sold exclusively at Home Depot are being recalled. A wire inside the light fixture could be loose, posing an electrical shock hazard to consumers.

      The recall involves the Lithonia Lighting Nickel End Wrap fluorescent ceiling light fixtures model number New 2 32 120 RE BN and date code N050807. The light fixtures measure four feet long and consist of two brushed nickel end caps and a clear plastic cover for the fluorescent tubes. Lithonia Lighting, the model number, and the date code can be found on the shipping box or on a label located inside the fixture, next to the fluorescent tube.

      To read the label on the fixture, consumers should turn the power off at the homes circuit breaker and remove the plastic cover of the light fixture. Light fixtures sold in a box marked with a round pink sticker are not included in this recall.

      The fixtures were sold exclusively at Home Depot stores from June 2007 through August 2007 for about $40. They were made in China.

      Consumers should stop using the light fixture immediately and return the product to Home Depot to receive a full refund or free replacement. Before removing the fixture, consumers should turn off the power at the homes circuit breaker and consult the installation instructions provided with the fixture.       Old Williamsburgh Mason Jar Candles

      Old Williamsburgh Candle Corp. is recalling about 3.7 million Mason Jar candles. The wick can move from the center of the jar to the side causing the glass to overheat and possibly crack or shatter. Exposure to broken glass and molten wax poses laceration and burn hazards to consumers.

      Old Williamsburgh has received 11 reports of the candles cracking or shattering, including one laceration injury.

      This recall involves all scents and colors of Mason Jar Candles in 3.5, 4.0, 4.5, and 5.0 oz. sizes. The size of the candle is specified on the label at the bottom of the jar, on the last line of the label. Replacement Mason Jar Candles, which are not included in the recall, have a "Q" on either side of the label.

      The candles were sold through dollar stores, discount stores, and grocery stores nationwide from May 2006 through December 2006 for between $1 and $1.50.

      Consumers should immediately stop using the candles and return them to the place of purchase for an exchange or full refund.

      Sports Authority Recalls Aluminum Water Bottles

      Sports Authority is recalling about 11,000 "Alpine Design" aluminum water bottles. The surface paint contains excessive levels of lead, violating the federal lead paint standard.

      This recall involves the Alpine Design aluminum water bottles sold at Sports Authority. The water bottles are sold in different colors and measure between six and 10 inches tall. The water bottles are silver, blue, red, blue with pink flowers, red with the depiction of a mouse, or blue with the depiction of a zebra. 'Alpine Design' is painted on the water bottle.

      The bottles were sold at Sports Authority stores nationwide from April 2006 through September 2007 for between $5 and $8. They were made in China.

      Consumers should stop using the bottle immediately and return it to a Sports Authority store for a full refund.

      Home Decorators Collection is recalling about 900 Oyster Bay wall sconces. A missing back plate exposes consumers to live wires, posing a risk of electrical shock to consumers changing the light bulb.

      The recall involves "Oyster Bay" model wall sconces. The Tiffany-style design includes multi-colored glass with a black outline.

      Surface paint on the recalled sunglasses can contain excessive levels of lead, violating the federal lead paint standard.

      The two-tone sunglasses come in Spiderweb Blue and Spiderweb Red with a spider web etched in between the eyes and Made in China engraved on the right-hand side of the frame.

      They were sold by party stores, discount dollar stores, and hardware stores nationwide from May 2007 through August 2008 for between $1 and $5.

      Consumers should immediately take the recalled sunglasses away from children and return them to the place of purchase for a full refund.

      H&M Recalls Boy's Jackets Due to Choking, Poisoning Hazards

      H&M is recalling boy's Jackets with an attached compass sold through its stores.

      The attached compass on these jackets can break, posing a choking hazard for young children. Additionally, the liquid mineral oil, inside the compass can be harmful if swallowed.

      The firm received one report of a child outside of the U.S. who began to choke on a broken piece of the attached compass. No injuries have been reported.

      The recalled product is a boy's cotton canvas jacket made exclusively for children in sizes 18M to size 6Y. It has style number 93196 written on the inside care label. There is an attached removable compass on the side. The compasses are copper or turquoise colored, about 2.75-inches long, and have a clip to attach to the jacket.

      The jackets were sold at H & M stores nationwide from February 2006 through October 2006 for $30.

      Consumers should immediately remove the compass from the jacket and throw it away, or mail the compass to H & M to receive a store discount coupon.

      Target Recalls Childrens Toy Gardening Tools and Chairs

      Target is recalling about 350,000 "Happy Giddy" gardening tool toys and children's "Sunny Patch" chairs because the paint contains excessive levels of lead.

      The toys were sold at Target stores nationwide from August 2006 through August 2007 for between $3 and $10.

      Detailed descriptions of the toys:

      Gardening tools: caterpillar trowel with an orange and multi-colored handle; kitty cat broom with pink flowers and green handle; pink and green cultivator with a solid green tool portion and pink flowers on the handle; orange and green wood handle hoe and rake with orange flowers; pink and green shovel and trowel with pink flowers; green, orange and light blue snail push broom; multi-colored trowel, hoe, rake with orange on the tool body; multi-colored shovel with solid red on the shovel and eyes on the top of the handle; red and blue owl shaped broom with bristles painted blue, with yellow and black large eyes on the face.

      Childrens Chairs: multi-colored childrens Sunny Patch chairs with a giraffe, monkey, and elephant on the back. The cloth seat and back are striped in orange, yellow, green and blue. The chameleon, butterfly and lady bug chair are not included in the recall.

      The toys were made in China.

      Consumers should immediately take the products away from children and return the item to the nearest Target store for a full refund.

      Tequila Rose Strawberry Cream candle sets are being recalled. The martini glass containing the gel candle can break while the candle is burning, posing fire and burn hazards to consumers.

      The firm has received three reports of glasses breaking while the candle was burning. No fires or injuries have been reported.

      The recalled gel candles came in the Tequila Rose Strawberry Cream set, a Valentine's Day gift set. The candle has pink gel and is contained in a martini glass. The words 'Tequila Rose' and 'Made in China' are printed on the glass. The gift set also includes a 750 milliliters bottle of Tequila Rose Strawberry Cream liqueur.

      The candles were sold at licensed alcohol beverage retail stores nationwide from January 2007 through March 2007 for about $20.

      Consumers should immediately stop using the gel candle and contact the firm for instructions on how to get a free replacement candle.
