    The product contains an undissolved ingredient -- citrus flavonoid

    Kingston Pharma is recalling all lots of DG Baby Gripe Water herbal supplement with organic ginger and fennel extracts.

    The product contains an undissolved ingredient -- citrus flavonoid.

    While use of the product should not be considered hazardous, it could result in difficulty for sensitive individuals when swallowing the product.

    The firm has received one report of a one-week old infant having difficulty when swallowing the product, and three complaints attributed to the undissolved citrus flavonoid.

    The recalled product, which is administered orally to infants and adults, is packaged in 4-oz., amber bottles, white plastic caps with safety seals and provided with an oral syringe and the UPC Code 8 5495400246 3.

    It was sold throughout the U.S. by Dollar General Corporation.

    What to do

    Customers who purchased the recalled product should stop using and discard it.

    The products pose a fire or burn risk.

    The following items, sold under the Armor All brand name and operated by plugging the cushion or blanket into a vehicle's 12-volt cigarette lighter socket, are being recalled:

    Tesla recalls model year 2014-2016 Model S vehicles

    The passenger front airbag inflator may explode

    Tesla is recalling 68,673 model year 2014-2016 Model S vehicles equipped with certain air bag inflators assembled as part of the passenger front airbag modules used as original equipment or replacement equipment.

    In the event of a crash necessitating deployment of the passenger front airbag, the inflators may explode due to propellant degradation occurring after long-term exposure to absolute humidity and temperature cycling.

    An inflator explosion may result in sharp metal fragments vehicle occupants resulting in serious injury or death.

    What to do

    Tesla will notify owners, and Tesla Service Centers will replace the passenger frontal air bag modules, free of charge.

    The recall is expected to begin in March 2019.

    Owners may contact Tesla customer service at 1-877-798-3752. Tesla's number for this recall is SB-19-20-001.

      The product may contain pieces of glass

      Sid Wainer and Son of New Bedford, Mass., is recalling 5.2-oz., containers of Jansal Valley brand Dukkah, an Egyptian condiment consisting of a mixture of herbs, nuts and spices.

      The product may contain pieces of glass.

      No injuries have been reported to date in connection with this problem.

      The recalled product, packaged in a circular tin can with a clear lid in cases containing six 2.5 oz., units each, have a best by date of 08/28/2020.

      The product lot codes are L2033968 for a case and L2034198 for individual tins. It was sold nationwide.

      CVB Recalls LUCID folding mattress-sofas

      The mattresses fail to meet the mandatory federal flammability standard

      CVB Inc., of Logan, Utah, is recalling about 86,000 LUCID folding mattress-sofas.

      The mattresses fail to meet the mandatory federal flammability standard for mattresses, posing a fire hazard.

      No incidents or injuries are reported.

      This recall involves LUCID folding mattress-sofas sold in twin, full, queen, and king sizes. The mattress-sofas fold into a sofa configuration and fold out into a mattress.

      The recalled mattress-sofas are two-tone gray colored and have a white federal tag with “Made For: CVB INC, 1525 W 2960 S, LOGAN, UT 84321” printed on it. The federal tag is located on the underside of the head panel of the mattress.

      The folding mattress-sofas, manufactured in China, were sold at Amazon.com, Overstock.com, Wayfair.com and Walmart.com from September 2015, through June 2018, for about $130.

      What to do

      Consumers should immediately stop using the recalled mattress-sofas and contact LUCID to receive a free fitted liner to cover the mattress and bring it into compliance with the federal standard.

      PurpleAir recalls power supply units for air sensors

      The power supplies can overheat, posing fire and burn hazards.

      PurpleAir Power is recalling about 4,400 PowerStream power supply units for air sensors sold in the U.S. and Canada.

      The power supplies can overheat, posing fire and burn hazards.

      The company has received 27 reports of the recalled power supplies emitting sparks and/or smoke of which 18 are reports of property damage, including one report of a small apartment fire. No injuries have been reported.

      This recall involves power supply units sold with PurpleAir PA-II and PA-II- SD air quality sensors with model number RKPO-UL052000C. The model number is located on the bottom side of the power adapter.

      The power adapters are black with a white label, and measure 4 inches long (including the mounting tabs) by 1-7/8 inches wide by 1-3/8 inches high.

      The air quality sensors are used outdoors by consumers to monitor air quality.

      The power supply units, manufactured in China, were sold online at www.purpleair.com and as giveaways in communities from October 2017, to August 2018, for about $230 to $260.

      What to do

      Consumers should immediately stop using the recalled power supplies and contact PurpleAir for instructions on how to obtain a replacement power supply.

      The firm has received five reports of the LED lights overheating while plugged in and charging, including one report of minor burns.

      The recall includes the Shop LC Rechargeable Party Disco LED Light, measuring 36 by 7 inches with Solar Panel and 2-Pin Plug Power Cord. The lights were sold in golden, blue, gray, and black.

      McDaniel Life-Line is recalling all lots of Indian Herb to the consumer level.

      The product is marketed without an approved New Drug Application/Abbreviated New Drug Application (NDA/ANDA).

      The therapeutic claims made in the labeling for this product establish that it is a drug because it is an article (other than food) intended to affect the structure or any function of the body.

      Additionally, it contains ingredients that have been determined to be caustic in nature that can cause serious injury. Use of the product could result in temporary or permanent damage or loss of body function or structure.

      McDaniel Life-Line has received one report of an adverse effect to date.

      The product, which can be taken internally or applied externally to abnormal skin growths, is packaged in 6-dram vials.

      It was distributed online to consumers in the U.S., Italy, Canada, Australia and Poland.

      Heartland Recreational Vehicles is recalling 78 Milestone recreational trailers, model 377MB.

      The liquid propane (LP) gas hose for the water heater may have been improperly secured with a zip tie which could fail, allowing the hose to contact the tire.

      If the LP hose contacts the tire, it can cause a propane leak, increasing the risk of a fire.

      Heartland will notify owners, and dealers will secure the hose with a P-clamp and replace the LP hose if damage is found, free of charge.

      Mazda North American Operations is recalling approximately 14,370 model year 2016-2019 Mazda MX-5 Miatas.

      Electrical noise in the automatic transmission's range signal may be detected while driving in “D” range, causing the vehicle to unexpectedly downshift, resulting in an abrupt deceleration of the drive wheels.

      A sudden decrease in the speed of the drive wheels may negatively affect the directional stability of the vehicle, increasing the risk of a loss of vehicle control and potentially resulting in a crash.

      There have been no reports of accidents or injuries.

      VICS Acquisition of Harlingen, Texas, is recalling nearly 100,000 pounds of chicken products containing eggs, an allergen not declared on the product.

      There are no confirmed reports of adverse reactions due to consumption of these products.

      The following items, bearing establishment number “P-34622” on the package and produced from August 7 through October 15, 2018, are being recalled:

      • 1.5-lb. packages containing one frozen steam bag meal of “happi foodi BLOODY MARY INSPIRED CHICKEN” with lot codes 14528 and 15092 and use by dates of March 2, 2020 and April 15, 2020.
      • 1.5-lb. packages containing one frozen steam bag meal of “SE Grocers MARSALA CHICKEN” with lot codes 14177 and 14532 and use by dates of Feb. 8, 2020 and March 3, 2020.
      • 1.5-lb. packages containing one frozen steam bag meal of “SE Grocers MEDITERRANEAN STYLE HERB CHICKEN” with lot codes 14145 and 14531 and use by dates of Feb. 7, 2020 and March 4, 2020.

      Delta Electronics recalls solar inverters

      The capacitors can fail and allow heat to build up

      Delta Electronics USA of Plano, Texas, is recalling about 2,600 Eltek solar inverters.

      The capacitors can fail and allow heat to build up and the cover can eject with force from the unit, posing fire and impact hazards.

      The company has received 11 reports of the capacitors failing and allowing heat buildup, including two reports of the cover forcibly ejecting off the inverter units. No injuries have been reported.

      This recall involves the Eltek THEIA HE-t string inverters that convert DC power from residential solar panels to AC power supplied to the electrical grid system.

      The recalled inverters measure about 28 inches high by 14 inches wide by 6.5 inches deep, and were sold in four different kilowatt ratings: 2.0 kW, 2.9 kW, 3.8, kW, and 4.4 kW.

      “Eltek,” and the model designation “THEIA” and “HE-t” are printed on the front display panel of the inverter. The inverters could be installed in different places in consumers’ homes.

      The inverters, manufactured in China, were sold through various solar installers in Arizona, New York, Tennessee and Washington from March 2012, through May 2017, for between $2,000 and $4,000.

      What to do

      Consumers should immediately stop using the recalled inverters, turn off the switch on the inverter’s front cover, switch off the circuit breaker for the inverter and contact Delta Electronics USA to arrange for a free replacement inverter.

      Delta Electronics will remove the recalled inverter and install a replacement.

      Dell Inc., of Round Rock, Texas, is recalling about 9,400 hybrid power adapters sold with power banks sold in the U.S. and Canada.

      The hybrid power adapter casing can break and detach, exposing the internal metal components, posing an electric shock hazard.

      The firm has received 11 reports of the hybrid power adapter breaking and exposing the internal components. No injuries have been reported.

      This recall involves Dell hybrid power adapters sold with power banks. The adapter is designed to be disconnected from the power bank via release tabs.

      The recalled adapters were manufactured from January 2017 through March 2017 and can be used separately from the power bank while connected to a power outlet.

      The adapter is black and darker in color than the power bank with a Dell logo on the back label.

      The adapters have the following manufacture code –“CN-05G53P - LOC00 – XXX – XXXX – AXX” printed on the back of the unit, where “X” represents a numerical value.

      A full listing of the recalled adapter manufacture codes included in the recall are listed on www.dellproduct.com.

      The adapters, manufactured in China, were sold at Dell.com, Amazon.com, Microcenter.com, and other electronic online retailers nationwide from February 2017, through August 2018, for between $125 and $200.

      Consumers should immediately stop using the recalled hybrid power adapters and contact Dell for instructions on receiving a free replacement adapter.

      Crate and Barrel Canada is recalling about 393 Holiday Bear Acrylic Milk Bottles.

      The plastic straw may crack, creating a laceration hazard.

      There are no reports of incidents or injuries to date.

      This recall involves the Crate and Barrel Holiday Bear Acrylic Milk Bottle (SKU 684364) which is a clear acrylic bottle measuring seven centimetres wide at the base and 14 centimetres high (20 centimetres with removable plastic straw).

      The bottles, manufactured in China, were sold from August to November 2018

      What to do
