    Lithonia Lighting recalls commercial luminaires

    The plastic lens on the luminaire can detach and fall unexpectedly

    Lithonia Lighting of Conyers, Ga., is recalling about 398,000 BLT series commercial luminaires.

    The plastic lens on the luminaire can detach and fall unexpectedly, posing a risk of injury from impact.

    The firm has received 107 reports of plastic lenses falling from the luminaire. No injuries have been reported.

    This recall involves Lithonia Lighting BLT series luminaires intended for use in indoor commercial applications, such as office buildings, schools and stores.

    The low-profile, recessed LED luminaire is a metal light fixture in a white finish with a rounded or square plastic lens. The fixtures measure about 1, 2 or 4 feet long and 1, 1.5 or 2 feet wide.

    Only non-sensor luminaires with a manufacture date from October 2015 through August 2016 are included in this recall.

    Lithonia Lighting, the model and date of manufacture can be found on a label attached to the fixture’s housing and on the product packaging. The date code on the fixture’s housing’s label is in a MM/DD/YY format. The date code on the packaging is in a YYYY/MM/DD format.

    The luminaires, manufactured in the U.S and Mexico, were sold at lighting and electrical supply distributors nationwide and online at 1000Bulbs.com, Amazon.com, ATGStores.com, Build.com, HD.com, ShineRetrofits.com, and Wayfair.com from October 2015, through August 2016, for between $150 and $250.

    What to do

    Consumers should remove the lens and contact Lithonia Lighting to receive a free repair kit to secure the original lens. Consumers should prevent people from going into the immediate area under the fixtures until the lenses are removed or the luminaires are repaired.     Forest River recalls model year 2017 Cedar Creek recreational trailers

    The weight capacity on the vehicle's axles is listed in correctly

    Forest River is recalling 8 model year 2017 Cedar Creek recreational trailers manufactured September 8, 2016, to September 19, 2016.

    The axles on the recalled vehicles were incorrectly identified as having a 7,000 pound capacity when they were only rated and designed to carry up to 6,000 pounds.

    The owner of a vehicle with the incorrect axles could unknowingly overload the vehicle, possibly resulting in a loss of control or reduced braking distance, potentially increasing the risk of a crash.

    What to do

    All affected vehicles have been captured at the dealer level and are being repaired prior to sale. Therefore, an owner notification letter will not be issued for this campaign. Dealers will replace the axles that are rated for a 7,000lb capacity.

      Mid-Atlantic region Whole Foods Market Stores recall chocolate croissants

      The product contains eggs, an allergen not declared on the label

      Fifty-one Whole Foods Market stores in Pennsylvania, Maryland, Ohio, Kentucky, New Jersey, Virginia and Washington D.C., are recalling batches of chocolate croissants.

      The product contains eggs, an allergen not declared on the label.

      No adverse reactions have been reported to date.

      The recalled products were made and packaged in-house in clear plastic containers in counts of six. A white label on top of the package that included the product name “Chocolate Croissants 6pk,” UPC0248422008994 and sell-by dates on or before Nov. 5, 2016.

      What to do

      Customers who purchased the recalled product may bring the item or receipt into the store for a full refund.

      Model year 2016-2017 Cornerstone and Anthem motorhomes recalled

      The vehicle's Tempurpedic Sealy Mattress may burn quickly when on fire

      Entegra Coach is recalling 671 model year 2016-2017 Cornerstone and Anthem motorhomes manufactured April 7, 2015, to August 18, 2016.

      The affected vehicles are equipped with a Tempurpedic Sealy Mattress that may burn quickly when on fire. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 302, "Flammability of Interior Materials."

      If the mattress burns quickly, a vehicle occupant may not have enough time to exit the vehicle in the event of a fire, increasing the risk of injury.

      What to do

      Entegra will notify owners, and dealers will replace the Tempurpedic Sealy Mattress with a Sleep Number Mattress, free of charge. The recall is expected to begin November 18, 2016.

      Samsung initiates massive washing machine recall

      Machines have been a source of grievances for years

      Samsung is not just wrestling with a defective smartphone. The Korean electronics firm is now working with the U.S.Consumer Product Safety Commission (CPSC) to recall  some 2.8 million washing machines sold in the U.S.

      The CPSC warns the tops of the machines can come loose during use and cause injuries. It has issued an extensive list of the affected models.

      One of the models, WA400PJHDWR/AA, is in the home of Linda, a ConsumerAffairs reader from Saratoga Springs, N.Y. She contacted us last week to say she called Samsung after her machine suddenly stopped working.

      “They told me that my model number was one of the machines that they are trying to decide what to do with,” she wrote in a ConsumerAffairs post. “They told me if I was to get it to work to only use it on delicate cycle only.”

      D.C., of Roswell, Ga., wrote us to say she has one of the “exploding” Samsung washers.

      “I was told they would refund my money,” D.C. wrote in her post. “Then my refund was denied stating my receipt was 'blurry.' I honestly feel retailers should step in and help consumers. I now sit with a washing machine that is not usable as it sounds like a jet ready to take off when it is in use.”

      Four year old machines

      ConsumerAffairs has received a rather large number of reports about Samsung washers, many over the last month. One recurring theme appears to be that the source of the issue was purchased “four years ago,” which would put it within the time frame of the 2.8 million recalled machines.

      Here's what Virginia, of Louisburg, Kan., told us:

      “Top load washing machine: wa5471abpxaa01 - Purchased the washer from Nebraska Furniture Mart four years ago along with extended warranty. The first machine didn't work upon delivery and had to be replaced. Had several service calls including one in which the technician replaced the motherboard a year ago. At the time, the washer was rusting around the dispenser cup but warranty company said it was aesthetic and wouldn't fix it. Now it has stopped working again and rust has gotten so bad the washer is virtually unusable.”

      Smoke coming from the laundry room

      Leia of Nanaimo, British Columbia, contacted ConsumerAffairs in mid October to say her Samsung washer was one of those that caught fire.

      “I put a load in and continued to make my turkey soup,” Leia wrote in her ConsumerAffairs post. “I then heard a loud bang and went running downstairs. As I approached the stairs I saw smoke coming from my laundry room. I then got in there and saw the smoke pouring out of my washer.”

      The consumers who own a recalled washer have the option of in-home repairs, rebates, or refunds. In the meantime, consumers are being advised to operate the machines on lower speeds.

      “Our priority is to reduce any safety risks in the home and to provide our customers with easy and simple choices in response to the recall,” Samsung spokesman John Herrington told the Wall Street Journal. “We are moving quickly and in partnership with the CPSC to ensure consumers know the options available to them and that any disruption in the home is minimized.”

      Samsung, as you may recall, was forced to withdraw all Galaxy Note 7 smartphones from the market last month after it was unable to rectify overheating batteries in the phones that produced a fire hazard.

      KTM North America recalls closed-course/competition off-road motorcycles

      The front brake master cylinder can malfunction

      KTM North America of Amherst, Ohio, is recalling about 1,200 closed-course/competition off-road motorcycles.

      The front brake master cylinder can malfunction, posing a crash hazard.

      No incidents or injuries are reported.

      This recall involves model year 2017 KTM brand and Husqvarna motorcycles brand closed-course/competition only motorcycles.

      Five KTM models are being recalled: 150 XC-W, 250 XC-W, 300 XC-W, 350 SX-F and 450 XC-F. Three Husqvarna models are being recalled: TC 250, TX 300 and FC 350.

      Recalled KTM motorcycles are orange and black with the KTM logo and engine size on both sides of the shrouds covering the fuel tank. Recalled Husqvarna motorcycles are white with blue and yellow markings and the Husqvarna logo on both sides of the shrouds covering the fuel tank.

      The engine size is located on both sides of the rear fender below the rear of the seat. Model year 2017 motorcycles have a letter “H” in the 10th position of the vehicle identification number (VIN), located on the right side of the steering head.

      The motorcycles, manufactured in Austria, were sold at KTM and Husqvarna Motorcycles authorized dealers nationwide from June 2016, to August 2016, for between $8,000 and $11,000.

      What to do

      Consumers should immediately stop using the recalled motorcycles and contact an authorized KTM or Husqvarna Motorcycles dealer to schedule a free repair.

      Pier 1 Imports recalls glass knobs

      The knobs can break while in use, posing a risk of laceration

      Pier 1 Imports of Fort Worth, Texas, is recalling about 158,000 glass knobs sold in the U.S. and Canada.

      The knobs can break while in use, posing a risk of laceration to users from broken glass pieces.

      The company has received seven reports of injuries as a result of glass knobs breaking during installation or while in use.

      This recall involves Pier 1 Imports Basic, Kira and Facets glass knobs commonly used with dressers, cabinets and small drawers.

      The glass knobs have a metal stem affixed to one side and were sold in seven colors: clear, teal, pink, green, aquamarine, champagne and silver.

      No other decorative knobs are included in this recall.

      The knobs, manufactured in India, were sold exclusively at Pier 1 Imports stores nationwide and online at www.pier1.com from September 2011, through October 2016, for between $6 and $7.

      What to do

      Consumers should immediately stop using the recalled glass knobs and return them to any Pier 1 Imports store for a full refund or merchandise credit.

      Polaris Industries is recalling 17,572 model year 2015-2017 Slingshot motorcycles manufactured March 4, 2014, to September 7, 2016.

      The recalled vehicles may have been manufactured with a swing arm that has insufficient strength.

      Due to insufficient swing arm strength, the swing arm may fracture resulting in a loss of momentum or vehicle control, increasing the risk of a crash.

      The collision mitigation radar may not perform properly

      Newmar Corporation is recalling 15 model year 2017 King Aire motorhomes manufactured January 22, 2016, to October 4, 2016.

      The recalled vehicles are equipped with WABCO OnGuard collision mitigation radar which may have been mounted upside down on the front lower bumper, resulting in decreased performance.

      If the collision mitigation radar has reduced performance, there may be an increased risk of a crash.

      The recalled products pose aspiration hazards

      Livos Canada of Proulxville, Quebec, is recalling about 1,500 containers of SVALOS 222 tool cleaner and thinner.

      The recalled products pose aspiration hazards, which can result in severe adverse health effects including pulmonary injury, chemical pneumonia and death.

      There are no reports of incidents or injuries related to the use of these products.

      The following products are being recalled:

      • SVALOS 222 tool cleaner and thinner – 500ml
      • SVALOS 222 tool cleaner and thinner – 1L
      • SVALOS 222 tool cleaner and thinner – 5L

      There may be insufficient clearance between the fuel line and the hood structure

      Polaris Industries is recalling 17,795 model year 2015-2017 Slingshot motorcycles manufactured March 4, 2014, to October 7, 2016.

      The affected vehicles may have insufficient clearance between the fuel line and the hood structure, which could allow the fuel line to rub against the hood until it leaks fuel. A fuel leak in the presence of an ignition source can increase the risk of a fire.

      What to do

      Polaris will notify owners, and dealers will replace the replace the engine fuel line, and inspect the retention clips to reduce the risk of interference, free of charge. The recall is expected to begin in November 2016.

      The bamboo votive stand can catch fire, posing a fire hazard

      Kikkerland Design of New York is recalling about 7,300 Kikkerland and Cost Plus World Market brand teapots with bamboo stands.

      The bamboo votive stand can catch fire, posing a fire hazard to consumers.

      The firm has received six reports of the votive stands burning. No injuries have been reported.

      This recall involves Kikkerland and Cost Plus World Market brand teapots with bamboo stands. The 30-ounce glass teapots have a removable glass tea infuser sold with a votive-candle holder inside the bamboo stand.

      The Kikkerland teapots have model number TP02 printed on the packaging next to the UPC code. The Cost Plus World Market teapots have a white sticker with number 25024784 printed on the packaging.

      No votive candles were included with the teapots.

      The teapots, manufactured in China, were sold at Amazon retailers, Cost Plus World Market, Uncommon Goods and Urban Outfitters and online at www.Amazon.com from March 2014, through September 2016, for between $40 and $70.

      What to do

      Consumers should immediately stop using the recalled teapots and stands and contact the firm for return instructions. Consumers who purchased online will be mailed instructions on how to receive a refund. Consumers who bought the teapots at stores can return the teapots and stands for a full refund.

      The products do not meet the flammability requirements for children’s sleepwear

      2 blue ducks is recalling about 450 footed sleepers featuring Dr. Seuss's Thing 1 and Thing 2 characters sold in Canada.

      The loose-fitting children's sleepwear can contact ignition sources such as stove elements, candles, and matches more readily than tight-fitting sleepwear. Thus, they do not meet the flammability requirements for children’s sleepwear under Canadian law.

      No incidents or injuries have been reported.

      The footed red sleeper with some red/white stripes and featuring Dr. Seuss's Thing 1 and Thing 2 characters is made from 100% cotton. The item code FTSLP-C22-9M and the UPC # 0 14292 98193 0 can be found on the sales tag.

      Brake fluid may leak, resulting in reduced brake performance

      Polaris Industries, Inc. is recalling 6,860 model year 2016-2017 Slingshot motorcycles manufactured December 28, 2015, to April 27, 2016.

      The brake pressure switch may have been over-tightened, damaging the seal, allowing brake fluid to leak and resulting in reduced brake performance, increasing the risk of a crash.

      What to do

      Polaris will notify owners, and dealers will install new brake pressure switch seals, free of charge. The recall is expected to begin in November 2016.

      ICAPP recalls frozen strawberries

      The Food and Drug Administration is investigating an outbreak of Hepatitis A.

      The International Company for Agricultural Production & Processing (ICAPP) is recalling certain lots of its frozen strawberries in response to a U.S. Food and Drug Administration investigation of an outbreak of Hepatitis A.

      ICAPP is conducting the recall after learning that frozen strawberries that it distributed may be linked to a recent Hepatitis A outbreak in the U.S. Hepatitis A virus was detected in four lots of frozen strawberries that were exported to the U.S. by ICAPP.

      The recalled products were distributed for sale to and use in food service establishments nationwide -- not for use in food products offered for retail sale to consumers.

      What to do

      Anyone who may have consumed the recalled product should consult a health care professional or local health department to determine if a vaccination is appropriate.
