    Spirit Halloween recalls Nickelodeon PAW PATROL Marshall Hat with flashlight

    The flashlight's batteries can overheat, causing the flashlight to become hot

    Spirit Halloween of Egg Harbor Twp, N.J., is recalling about 21,500 Nickelodeon PAW PATROL Deluxe Marshall Hats with flashlight sold in the U.S. and Canada.

    The batteries in the flashlight can overheat, causing the flashlight to become hot, posing burn and fire hazards.

    The firm has received four reports of the flashlight overheating. No injuries or fires have been reported.

    This recall involves the Nickelodeon PAW PATROL Deluxe Marshall Hat sold with an accessory flashlight.

    The PAW PATROL Marshall hats are red with a yellow ribbon, black and white spotted dog ears and a black flash light attached to the side of the hat.

    The flashlight is included with the hat and they share the SKU number. Only flashlights with SKU 01292093 and date codes 1703RY01, 1603RY01, and 1503RY01 are involved in this recall.

    The SKU number and date codes are on the sewn in label under the ear on the hat.

    The hats, manufactured in China, were sold at Spirit Halloween stores nationwide from September 2015, through November 2017, for about $13.

    What to do

    Consumers should immediately take the flashlight that was sold with the hat away from children, stop using it, remove the batteries and dispose of the flashlight and contact Spirit Halloween for a full refund. Customers will be asked to provide a photo of the tag located under the ear in the hat.

    REV recalls motorhomes with satellite dish issue

    The satellite dish could detach from the roof while the vehicle is being driven

    REV Recreation Group is recalling 2,055 model year 2018 Fleetwood Axon and Pulse, Holiday Rambler Reno and Prodigy, Monaco Marquis and American Coach American Dream motorhomes, model year 2017-2018 Fleetwood Bounder, Discovery, Discovery LXE, Flair, Jamboree, Southwind, Pace Arrow, Pace Arrow LXE and Storm motorhomes, Holiday Rambler Admiral XE, Endeavor, Endeavor XE, Navigator XE, Navigator, Vacationer, Vacationer XE and Vesta and American Coach American Eagle motorhomes and model year 2017 Holiday Rambler Scepter, Monaco Diplomat, and American Coach American Revolution motorhomes.

    The mounting screws for the satellite dish may not have been fastened through the sheet steel reinforcement, possibly allowing the satellite dish to detach from the roof while the vehicle is being driven, increasing the risk of a crash or injury.

    What to do

    REV will notify owners, and dealers will resecure the satellite dish to the roof with a metal reinforcement, free of charge.

    The recall is expected to begin May 25, 2018.

    The products may be contaminated with hard plastic

    Tony Downs Food Company of Madelia, Minn., is recalling approximately 96,384 pounds of chicken breasts.

    The products may be contaminated with extraneous materials – specifically hard plastic.

    There have been no confirmed reports of adverse reactions due to consumption of these products.

    The following thermally processed, commercially sterile items, produced on November 28 – 29, 2017, is being recalled:

    • 12.5-oz. cans in a case containing 6 cans of “Member’s Mark PREMIUM Natural Chunk CHICKEN BREAST IN WATER” with lot code 17333, case code 9816 and a use-by date of Nov. 29, 2020.
    • 50-oz. cans in a case containing 6 cans of “Member’s Mark FOOD SERVICE PREMIUM Natural Chunk CHICKEN BREAST IN WATER” with lot code 17332, case code 9817 and a use-by date of Nov. 28, 2019.

    The recalled products, bearing establishment number “P-65” inside the USDA mark of inspection, were shipped to retail stores nationwide.

    What to do

    Customers who purchased the recalled products should not consume them, but discard them or return them to the place of purchase.

      Newmar recalls Ventana, Dutch Star and Mountain Aire vehicles

      The vehicles could suffer electrical arcing and a power steering fluid leak

      Newmar Corporation is recalling 2,139 model year 2014-2017 Newmar Ventana, model year 2011-2017 Newmar Dutch Star, and model year 2011-2014 Newmar Mountain Aire vehicles, built on Freightliner XBR chassis and equipped with Cummins ISL engines.

      The power steering hose on the recalled vehicles may be routed incorrectly, and, as a result, the hose may rub against the power stud on the starter motor, possibly causing electrical arcing and a power steering fluid leak.

      If the power steering hose contacts the power stud causing a power steering fluid leak, additional steering effort may be needed increasing the risk of a crash or an electrical arc may occur, increasing the risk of a fire.

      What to do

      Newmar will notify owners, and Freightliner dealers will reroute the power steering hose, free of charge.

      The recall is expected to begin May 25, 2018.

      Octane Fitness recalls cross trainers

      The foot pedals may drop unexpectedly, resulting in a fall hazard

      Octane Fitness of Brooklyn Park, Minn., is recalling about 570 Octane Fitness XT- One elliptical cross trainers.

      The incline lead screw can shear off, causing the foot pedals to drop unexpectedly, resulting in a fall hazard to the user.

      The firm has received seven reports of the foot pedal dropping unexpectedly, including one report of a scrape to the knee or leg area.

      This recall involves the Octane Fitness XT- One elliptical cross trainer. “Octane Fitness” is printed on the rear of the keypad and the side of the cross trainer.

      These standing elliptical cross trainers measure 33 inches by 99 inches and were sold in various shades of gray, green and orange.

      The machines have steel tubing with plastic shrouds and a 10 inch glass capacitive touch screen or a large LED display with integrated capacitive buttons.

      Only cross trainers with model numbers XT5700 are included in the recall. The model name XTOne is printed on the incline support column.

      The cross trainers, manufactured in Taiwan, were sold at specialty fitness stores, distributors and direct sales to commercial clubs and gyms nationwide from August 2015, through November 2017, for between about $8,000 and $10,500.

      What to do

      Gym and fitness center operators should immediately stop using these recalled cross trainers and contact Octane Fitness LLC for repair instructions. The firm is contacting all known purchasers of the recalled cross trainers directly.

      Tesla recalls model year 2012-2016 Tesla Model S vehicles

      The vehicle could suffer a reduction or complete loss of power steering assist

      Tesla is recalling 70,421 model year 2012-2016 Tesla Model S vehicles equipped with Bosch steering racks.

      The aluminum bolts that attach the power steering gear assist motor to the gear housing may corrode and fracture causing a reduction or complete loss of power steering assist.

      Loss of power steering assist would require a higher steering effort, especially at lower speeds, which may increase the risk of a crash.

      What to do

      Tesla will notify owners, and Tesla Service Centers will replace the steering gear mounting bolts and add a corrosion-preventative sealer, free of charge.

      The recall was expected to begin March 29, 2018.

      The top mounting screws for the exterior rearview mirrors may not be installed correctly, possibly resulting in the mirrors falling off while the vehicle is being driven.

      Mandatory recall ordered for kratom products

      The products may be contaminated with Salmonella

      The Food and Drug Administration (FDA) has issued a mandatory recall order for all food products containing powdered kratom manufactured, processed, packed, or held by Triangle Pharmanaturals.

      Several of the products were found to contain Salmonella.

      The action, which marks the first time the FDA has issued a mandatory recall order to protect consumers from contaminated food products, comes after the company failed to cooperate with the agency's request for a voluntary recall.

      “This action is based on the imminent health risk posed by the contamination of this product with salmonella, and the refusal of this company to voluntarily act to protect its customers and issue a recall, despite our repeated requests and actions,” said FDA Commissioner Scott Gottlieb, M.D. “We continue to have serious concerns about the safety of any kratom-containing product and we are pursuing these concerns separately.”

      Failure to act

      The FDA advised Triangle Pharmanaturals on March 30 that the agency could order the firm to cease distribution and notify applicable parties within 24 hours if the company did not conduct a voluntary recall.

      Triangle did not comply with the request and, on March 31, the FDA ordered the firm to cease distribution of the products and the provided it with an opportunity to request an informal hearing.

      Again, the company did not respond within the timeframe specified, therefore waiving its opportunity for an informal hearing, and the agency ultimately issued the mandatory recall order in the interest of public safety.

      This marks the third time the FDA has invoked its mandatory recall authority, but the first time the agency ordered a mandatory recall because a company has opted not to act.

      What to do

      The FDA is advising consumers to discard the products that are part of the mandatory recall, which include, but are not limited to:

      • Raw Form Organics Maeng Da Kratom Emerald Green,
      • Raw Form Organics Maeng Da Kratom Ivory White, and
      • Raw Form Organics Maeng Da Kratom Ruby Red.

      Consumers who have one or more of these products in their homes, should discard them immediately. As a precaution, kratom no longer stored in its original packaging should be discarded and the containers used to store it should be thoroughly washed and sanitized.

      In order to prevent cross-contamination, consumers should wash their hands, work surfaces and utensils thoroughly after contact with these products, and not prepare any food in the area at the same time.

      REV recalls motorhomes with Aurora HVAC systems

      The HVAC potentiometer wires may get disconnected, posing a fire risk

      REV Recreation Group (REV) is recalling 600 model year 2018 American Coach American Dream and American Revolution motorhomes, Fleetwood Discovery, Discovery LXE, Pace Arrow and Pace Arrow LXE motorhomes, Holiday Rambler Endeavor, Endeavor XE, Navigator and Navigator XE motorhomes and Monaco Marquis and Signature motorhomes equipped with an Aurora heating, ventilation and air conditioning (HVAC) system.

      The wires for the HVAC system potentiometer may become disconnected from the quick connect terminals.

      If the potentiometer wires detach and are reconnected incorrectly, there is an increased risk of a fire.

      What to do

      REV will notify owners, and dealers will inspect the potentiometer and solder the wires or replace the control panel, if necessary, free of charge.

      The recall is expected to begin May 14, 2018.

      The elevator cab can rise unexpectedly to the top of the elevator shaft and stop abruptly

      Residential Elevators of Crawfordville, Fla., is recalling about 3,700 elevators.

      The elevator cab can rise unexpectedly to the top of the elevator shaft and stop abruptly, posing an impact hazard to those in the elevator cab.

      The firm has received four reports of the elevator cab rising unexpectedly to the top of the shaft. No injuries have been reported.

      This recall involves traction residential elevators with models LLT-952, LLT-953, LLT-954 and LLT-955 shipped and installed between January 2015 and April 2017.

      The recalled elevators are used in consumers’ homes.

      “Residential Elevators” is printed on a label on the operating panel inside the elevator cab.

      The elevators, manufactured in the U.S.. were sold at Residential Elevators’ dealers and home builders nationwide from January 2015, through April 2017, for between $18,000 and $50,000.

      What to do

      Consumers should immediately stop using the recalled elevators and contact Residential Elevators to schedule a free inspection and, where a replacement part is needed, a free repair. Elevators that have been inspected by the company since May 2017 do not require a new inspection.

      Residential Elevators is able to determine whether a residential elevator is included in the recall by the consumer’s street address.

      The firm is contacting all known purchasers directly.

      Propane may leak out of the refrigerator when it is used in 'gas' mode

      Forest River is recalling 448 model year 2017-2018 Forest River Cherokee recreational trailers, models CCKT16BF, CCKT16BHS, CCKT16FQ, CCKT16GR, CCKT16GRH, CCKT17RP and CCKT18TO.

      The vehicles are equipped with Norcold N306 mini refrigerators that may have a loose or missing propane gas valve test port screw, allowing propane to leak out when the refrigerator is used in 'gas' mode, increasing the risk of a fire.

      Model year 2018 Passport recreational trailers recalled

      The spare tire and carrier may detach from the trailer

      Keystone RV Company is recalling 977 model year 2018 Passport recreational trailers, models 153ML, 171EXP, 175BH, 195RB, 199ML, 217EXP and 239ML.

      The spare tire carrier may not have been mounted properly to the back wall of the trailer, and, as a result, the spare tire and carrier may detach from the trailer, increasing the risk of a crash.

      What to do

      Keystone will notify owners, and dealers will inspect the spare tire carrier mounting and install reinforcing backers if necessary, free of charge.

      The recall is expected to begin May 22, 2018.

      Primo recalls beverage dispensers

      Hot water can drip from the machine

      Primo Water Operations of Winston-Salem, N.C., is recalling about 11,000 Primo hTrio Multi-purpose beverage dispensers.

      Hot water can drip from the machine when it is used in high altitude areas over 6,000 feet in elevation, posing a burn hazard to users.

      The company has received three reports of hot water leaking from the product being used in high altitude areas, including one burn injury to a customer’s hand.

      This recall involves the Primo hTrio Multi-Purpose beverage dispensers purchased on or before December 31, 2017 which provides both hot and cold water and brews 6-, 8- and 10-oz. hot beverages.

      Model numbers 601225, 601229, and 601240 are printed on the bottom of the product and Primo is printed on the front.

      The dispensers, manufactured in China, were sold at Sam’s Club and online at Amazon.com, and Wayfair.com from January 2017, through December 2017, for between $200 and $250.

      What to do

      Consumers in high altitude areas over 6,000 feet in elevation should immediately stop using the recalled dispensers and contact Primo for a free replacement.
