    Nestlé Waters North America recalls AccuPure water dispensers

    The water dispensers can overheat, causing internal parts to melt and smoke

    Nestlé Waters North America of Stamford, Conn., is recalling about 10,900 AccuPure countertop filtered water dispensers.

    The water dispensers can overheat, causing internal parts to melt and smoke. This poses burn and fire hazards.

    The company has received seven reports of parts melting, smoking or burning. No injuries have been reported.

    This recall involves Nestlé Waters North America AccuPure countertop water filtration dispensers. The water dispensers are black and measure about 17.5 inches tall and 12.8 inches wide.

    The units connect to the plumbing system, draw water from the system, filter the water and dispense hot and cold water through the unit. “AccuPure” is printed on the front and side panels. The model number is HCB215n and is printed on the back of the water dispensers.

    The filtration dispensers, manufactured in China, were were sold and rented to businesses nationwide for consumer use from January 2013, to May 2016, as part of individual service contracts that Nestlé Waters has with these customers. A limited number of units were purchased by these customers for a list price of $599.99.The units are not sold or provided at retail.

    What to do

    Consumers should immediately unplug the water dispensers and stop using them. Nestlé Waters is contacting affected customers directly to schedule installation of a free replacement unit. Customers with questions should contact Nestlé Waters.

    No illnesses have been reported to date in connection with this problem.

    IKEA recalls safety gates and safety gate extensions

    The locking mechanism can open unexpectedly, posing a fall hazard

    IKEA North America Services of Conshohocken, Pa., is recalling about 108,000 safety gates and safety gate extensions in the U.S. and Canada.

    The locking mechanism can open unexpectedly, posing a fall hazard to children and other consumers.

    The company has received 55 reports of incidents worldwide, including 10 injuries that resulted in two concussions, and cuts and bruising. Four of the incidents were reported in the U.S. -- two resulting in cuts and bruising to children.

    This recall involves IKEA PATRULL, PATRULL FAST and PATRULL KLÄMMA safety gates and safety gate extensions (The PATRULL KLÄMMA safety gates were previously recalled in May 2015).

    The gates and gate extensions are white, made of beech wood or steel and plastic. The pressure-mounted gates open both inward and outward. They measure about 28 ¾ inches high with an adjustable width from about 28 ¾ inches to 34 ¼ inches.

    The gate extensions measure about 28 ¾ inches high with an adjustable width from about 2 ½ inches to 5 inches.

    A permanent label is attached to the metal bar at the bottom of the safety gate containing “IKEA” and the article number.

      U.S. Cado Holdings expands recall of imported Swai filets

      The products were distributed into U.S. commerce without meeting federal importation requirements

      U.S. Cado Holdings of Santa Ana, Calif., is adding approximately 1,650 pounds of imported, frozen Swai fillets products to an earlier recall -- for a total of 29,645 pounds.

      The products were distributed into U.S. commerce without meeting federal importation requirements.

      There have been no confirmed reports of adverse reactions due to consumption of these products.

      The following Deep Water Sea Food brand Frozen Swai fillets, produced in Vietnam on April 5, 2016, are being added to the recall:

      • 15-lb. boxes containing bulk pieces of “Frozen Swai Fillets” with the label DEEP WATER SEA FOOD, “Production Date: 04/05/2016,” and “Best Before Date: 10/05/2017.”

      The recalled products bear the Factory Number UCH001 on the box and bear the lot number 1683475. They were shipped to institutional locations in Puerto Rico. The boxes are marked as “9/11 oz.”

      Customers who purchased the recalled products should not consume them, but throw them away or return them to the place of purchase.

      The throttle pedal position sensor on the affected vehicles may not return to zero

      Winnebago Industries is recalling 705 model year 2014-2016 Winnebago Tour motorhomes manufactured May 25, 2013, to November 25, 2015, and Itasca Ellipse motorhomes manufactured May 25, 2013, to November 18, 2015.

      The throttle pedal position sensor on the affected vehicles may not return to zero, causing the engine brake to suddenly, not activate as expected, reducing the vehicle's deceleration ability.

      A sudden loss of an engine brake without warning on a motorhome may lead to a reduced brake ability, potentially increasing the risk of a crash.

      What to do

      Recall of IKEA chocolate products expanded

      The products contain milk, hazelnuts and/or almonds, allergens not declared on the label

      IKEA is expanding an earlier recall of chocolate products to include Chokland Mörk, Chokland Mörk 70%, Choklad Lingon & Blabar, Choklad Nöt, Godis Chokladkrokant, Godis Chokladran and Choklad LJUS sold from April 30, 2015, to July 1, 2016.

      The products, which include all “Best Before Dates," contain milk, hazelnuts and/or almonds, allergens not declared on the label, although the products do carry an advisory statement, stating that they may contain milk and tree nuts..

      There have been no complaints or reported incidents in the U.S. to date. IKEA of Sweden has had one reported complaint in Japan.

      The recalled products were distributed to IKEA retail stores nationwide and sold individually.

      What to do

      Customers should return the recalled products to the nearest IKEA store for a full refund. Proof of purchase is not required.

      Home Source Recalls Floor Lamps

      The lamps can short circuit due to defective sockets

      Home Source Industries of East Rutherford, N. J., is recalling about 2,700 floor lamps.

      The lamps can short circuit due to defective sockets, posing fire and shock hazards.

      The company has received three reports of the lamp’s sockets burning and smoking. No injuries have been reported.

      This recall involves brown metal floor lamps with alabaster glass shades. They are illuminated with a single 100-watt light bulb and measure about 70 to 72 inches tall. The model numbers LMP4229, LMP4168 and LMP10771 can be found on the label at the bottom of the lamps.

      The lamps, manufactured in China, were sold at small retail stores located in the New York metropolitan area and online at Amazon.com, Houzz.com, Target.com and Wayfair.com from February 2016, to April 2016, for about $60.

      Aston Martin recalls DB9, DBS, V8 Vantage, Virage, Rapide, Vanquish, Vantage, and V12 Zagato vehicles

      Interior door unlocking mechanisms may not function properly

      Aston Martin Lagonda of North America is recalling 6,076 model year 2010-2015 DB9, 2010-2012 DBS, 2010-2016 V8 Vantage, 2012 Virage, 2010-2016 Rapide, 2014-2016 Vanquish, 2011-2016 V12 Vantage, and 2012-2013 V12 Zagato vehicles.

      The recalled vehicles are equipped with interior door unlocking mechanisms that may not function when the vehicle has been locked from the outside. As such, these vehicles fail to conform to Federal Motor Vehicle Safety Standard (FMVSS) number 206, "Door Locks and Door Retention Components."

      If the doors cannot be opened by an occupant from inside the vehicle, they may be trapped inside, increasing their risk of injury.

      What to do

      Aston Martin will notify owners, and dealers will disable the double locking feature by reprogramming the software, free of charge. The recall is expected to begin August 1, 2016.

      Recall of Flushmate III Pressure-Assist flushing systems expanded

      The system can burst at or near the vessel weld seam, releasing stored pressure

      Flushmate of New Hudson, Mich., is recalling about 465,000 Flushmate® III Pressure-Assist flushing systems in the U.S. and Canada. Recalls were previously issued January 2014 and June 2012.

      The system can burst at or near the vessel weld seam releasing stored pressure. This pressure can lift the tank lid and shatter the tank, posing impact and laceration hazards to consumers and property damage.

      The company has received two reports of the units included in this recall bursting, resulting in property damage, including one report of a minor injury.

      This recall is for Series 503 Flushmate III Pressure Assist flushing systems installed inside toilet tanks that were manufactured from July 2009 through April 2011. The units are rectangular, black, two-piece vessels made of injection molded plastic.

      Previously recalled systems were manufactured from October 1997 through June 2009.

      Recalled units have a date code/serial number that is 16 characters long and is located on the label on the top of the Flushmate III. The first six numerals of the serial number are the date code. The date code range for units included in this recall in MMDDYY format is 070109 (July 1, 2009) through 043011 (April 30, 2011). The date code range for previously recalled systems in MMDDYY format was 101497 (October 14, 1997) through 063009 (June 30, 2009).

      Units included in this recall were sold individually and installed in toilets manufactured by American Standard, Copperfit, Crane, Ecotech, Eljer, Gerber, Kohler, Lamosa, Mancesa, Mansfield, Orion, St. Thomas, Universal Rundle, Vitra, Vitromex, Vortens and Western Pottery.

      The flushing systems, manufactured in the U.S., were sold at Home Depot and Lowe’s stores, toilet manufacturers, distributors and plumbing contractors nationwide from 2009 through 2011 for about $108 for the units without toilets.

      What to do

      Consumers should immediately stop using the recalled Flushmate III systems, turn off the water supply to the units, flush the toilet to release the internal pressure and contact the firm to request a free repair kit.

      The recalled dressers are unstable if they are not properly anchored to the wall

      Bernhardt Furniture Company of Lenoir, N.C., is recalling about 1,700 Marquesa dressers and nightstands.

      The recalled dressers are unstable if they are not properly anchored to the wall, posing a serious tip-over and entrapment hazard that can result in death or injuries to children.

      The company has received two reports of the furniture tipping over when the drawers were opened, including one injury to an adult involving an arm bruise and neck stiffness.

      This recall involves Marquesa mirrored glass front dressers with model number 359-044 and Marquesa mirrored glass front nightstands with model number 359-234.

      Both models have a gray cashmere finish and mirrored glass drawer fronts with metal overlays. The series and style numbers are printed on a white label on the back of the product. The first three characters of the number will be 359 followed by the style number. Recalled products include:

      The furniture, manufactured in Vietnam, was sold at independent furniture stores nationwide from March 2015, through May 2016, for between $2,000 and $4,200.

      What to do

      Consumers should immediately stop using any recalled dressers and nightstands that are not properly anchored to the wall and place them into an area that children cannot access. Contact the store where purchased or Bernhardt for a free counterweight repair kit including a wall anchor.       Dream Body Weight Loss recalls various weight-loss products

      The products contain sibutramine, which was withdrawn from the U.S. market in 2010

      Dream Body Weight Loss is recalling all lots of Dream Body Extreme Gold 800-mg 30 gold capsules, Dream Body 450-mg 30 white capsules and Dream Body Advanced 400-mg 30 purple capsules.

      The products contain sibutramine, an appetite suppressant that was withdrawn from the U.S. market in 2010. Sibutramine is known to substantially increase blood pressure and/or pulse rate in some patients and may present a significant risk for patients with a history of coronary artery disease, congestive heart failure, arrhythmias or stroke.

      This undeclared ingredient makes these products unapproved new drugs for which safety and efficacy have not been established. The products may also interact in life-threatening ways with other medications a consumer may be taking.

      The company says it has not received any reports of adverse events related to this recall to date.

      The recalled products, used as a dietary supplement and packaged in clear packer jars and brown foil packets with UPC codes 6903023120128, were distributed Nationwide from January 2013, to June 16, 2016, via retail sales and internet transactions.

      What to do

      Dream Body Weight Loss is notifying its customers via e-mail and is arranging for return of all recalled products. Customers should stop using and discard the recalled products.

      The firm has received three reports of incidents, one resulting in a scraped knee.

      This recall involves all models of the URB-E electric scooter. The foldable electric scooters are aluminum and carbon fiber. They have two wheels and came in black, gray or white.

      The scooters, manufactured in the U.S., were sold at URBAN626 stores in Pasadena, Calif., and online at amazon.com and URB-E.com from August 2015, through June 2016, for between $1,500 and $2,000.

      What to do

      Consumers should immediately stop using the recalled units and contact URBAN626 for a free repair kit. URBAN626 will reimburse consumers if they choose to have a bike repair shop replace the bolt.

      Propane gas may leak into the regulator sight glass causing it to degrade and potentially crack

      Forest River is recalling 175 model year 2016-2017 Flagstaff recreational trailers manufactured August 11, 2015, through October 27, 2015, and Work N' Play recreational trailers manufactured from October 25, 2015, through May 6, 2016.

      The MB Sturgis liquid propane (LP) regulator on these vehicles may allow propane gas to leak into the regulator sight glass causing it to degrade and potentially crack.

      If the indicator sight glass cracks, propane gas may leak out and a fire may occur i In the presence of an ignition source.

      K-Z RV is recalling 61 model year 2016-2017 Spree travel trailers manufactured March 13, 2015, to May 3, 2016.

      The refrigerator vents on the affected trailers may be installed in an incorrect location, causing the refrigerator compartment to overheat, triggering the high temperature sensors to shut down the refrigerator.

      If the high temperature sensor fails to shut down the refrigerator, the refrigerator would overheat, increasing the risk of a fire.

      What to do

      K-Z RV will notify owners, and dealers will install a larger refrigerator vent, and extend the internal baffle, free of charge. The manufacturer has not yet provided a notification schedule.

      MBUSA will notify owners, and dealers will update the software for the electric power steering control unit, free of charge. The recall is expected to begin in July 2016.

      The inside range burners may be extinguished unintentionally

      Crossroads RV is recalling 114 model year 2016-2017 Hill Country and Sunset Trail recreational trailers manufactured May 13, 2015, to April 18, 2016 and equipped with an outside kitchen compartment.

      The inside range burners may be extinguished unintentionally when the outside kitchen compartment door is opened.

      With the burners extinguished, the propane may continue to flow, creating a build up of propane gas, which in the presence of an ignition source, can increase the risk of a fire and/or explosion.

      What to do

      Crossroads has notified owners, and dealers will install a piece of trim to cover the air gap behind the inside range to the outside kitchen, free of charge. The recall began on June 9, 2016.

      Owners may contact Crossroads customer service at 1-855-226-7496. Crossroad's number for this recall is 16-254.
