    The helmet may not protect the wearer from impact

    Ultimate Leather Apparel is recalling 6,848 Zone HS1100 helmets, models HS1100-D1, HS1100-D2, HS1100-D3, HS1100-D5, HS1100-FLAT, HS1100-REBEL and HS1100-SHINY, in sizes XS, S, M, L, XL, and 2XL.

    The helmets may not adequately protect the wearer in the event of a head impact during a crash, increasing the risk of injury

    What to do

    Ultimate Leather will notify owners and provide a refund or a replacement helmet free of charge.

    The recall is expected to begin September 28, 2020.

    Acella Pharmaceuticals recall NP Thyroid Tablets

    Testing has found the product to be sub potent

    Acella Pharmaceuticals is recalling one lot of 15-mg and one lot of 120-mg NP Thyroid, Thyroid Tablets [levothyroxine (T4) and liothyronine (T3)].

    Testing has found the product to be sub potent. The product may have as low as 87% of the labeled amount of levothyroxine (T4).

    Patients being treated for hypothyroidism (underactive thyroid) who receive sub potent NP Thyroid may experience signs and symptoms which may include, fatigue, increased sensitivity to cold, constipation, dry skin, puffy face, hair loss, slow heart rate, depression, swelling of the thyroid gland and/or unexplained weight gain or difficulty losing weight.

    The firm has received four reports of adverse events to date.

    Patients who are currently taking the recalled medication should not discontinue use without contacting their healthcare provider for further guidance and/or a replacement prescription.

    The electronic control center circuit board may be damaged from voltages over 17 volts, causing the components on the board to become damaged, increasing the risk of a fire.

    What to do

    Jayco will notify owners, and dealers will install an additional circuit module to the existing component board to provide over-current protection free of charge.

      The recalled product, which comes in a 6-oz package with UPC 8-19898-01015-8 and a “best by" date of April 7, 2021, on the top of the box, was sold in retail stores nationwide.

      What to do

      Customers with a milk allergy who purchased the recalled products may return them to the place of purchase for a full refund.

      The hydraulic cylinder attachment bolts may fail

      PJ Trailers is recalling 694 model year 2020-2021 DE trailers.

      The attachment bolts of the hydraulic cylinder may fail if the hydraulic system is used to lower the dump box once the dump box is already in the fully lowered position.

      Use of the dump box after the attachment bolts fail may result in a sudden drop of the box as it is being raised, increasing the risk of injury.

      What to do

      PJ Trailers will notify owners, and dealers will replace the hydraulic power unit and the attachment bolts free of charge.

      RH recalls Industrial Three-Panel Mirrors

      The mirror panels can become loose and detach from the backing

      RH US of Corte Madera, Calif., is recalling about 270 Industrial Three-Panel Mirrors.

      The mirror panels can become loose, detach from the backing and fall, posing an injury hazard.

      The firm has received six reports of mirror panels detaching or becoming loose. No injuries have been reported.

      This recall involves RH’s mirrors from their Industrial Three-Panel Mirror collection sold in two sizes: 24 inches wide by 72 inches high by 1 inch deep (SKU 41100011SILV) and 32 inches wide by 96 inches high by1 inch deep (SKU 41100015SILV).

      The recalled mirrors have three square mirror panels adhered to a wooden black backing, surrounded by an antique-black painted metal frame. The mirror panels have a distressed appearance.

      The SKU is printed on a sticker on the back of the mirror and on the purchase receipt.

      The mirrors, manufactured in India, were sold at RH galleries and outlets nationwide and online at www.rh.com from October 2018, through June 2020, for between $190 and $1,500.

      What to do

      Consumers should immediately stop using the recalled mirrors and contact RH to receive a full refund of the purchase price, or a credit if the consumer’s purchase price cannot be determined.

      Medique recalls 31 different over-the-counter drugs

      The products packaging fails to meet child-resistant requirement

      Medique of Fort Myers, Fla., is recalling 31 Medique over-the-counter drugs from the Medi-First, Medi-First Plus, Medique, Dover, Otis Clapp and Ecolab product lines.

      The products contain regulated substances which must be in child resistant packaging when being used in the household as required federal law.

      The packaging is not child resistant, posing a risk of poisoning if the contents are swallowed by young children.

      No incidents or injuries are reported.

      The recall involves 31 different over-the-counter drugs purchased on or after June 1, 2018, that are unexpired from the following brands: Medi-First, Medi-First Plus, Medique, Dover, Otis Clapp, and Ecolab.

      They include aspirin-containing products, acetaminophen-containing products, ibuprofen-containing products, lidocaine-containing products, diphenhydramine, loperamide, and naproxen products.

      The expiration date for tablets and creams can be found on either the top or side panels of the container carton in the format [YEAR/MO].

      For products in spray bottles, the expiration date in the same format is located on the front of the bottle. The expiration date is found on the bottom for the spray cans.

      The recalled products, manufactured in the U.S., were sold exclusively at Amazon.com from June 2018, through June 2020, for between $2 and $59.

      What to do

      Consumers should immediately store the recalled products in a safe location out of reach of children and contact Medique for information on how to dispose of the product and receive a full refund. All known purchasers are being notified directly.

      The products may contain methanol (wood alcohol), which can result in nausea, vomiting, headache, blurred vision, permanent blindness, seizures, coma, permanent damage to the nervous system or death.

      There are no reports of adverse events to date.

      The product, used as a hand sanitizer to reduce bacteria when soap and water is not available, is packaged in 480-mL plastic bottles with UPC 7502272121085.

      The lot being recalled, 20DF8307 with the expiration date April 2022, was sold in Miami beginning in April 2020.

      The bedroom window does not have a proper crank handle

      Keystone RV Company is recalling 130 model year 2020-2021 Dutchmen Yukon 399ML, 400RL and 410RD recreational trailers.

      The bedroom egress window has a black round knob installed rather than a narrow crank handle.

      Without the narrow crank handle, the emergency exit may be difficult to open, impeding the ability to open the window and resulting in an increased risk of injury in an emergency.

      What to do

      Keystone will notify owners, and dealers will remove the existing black round knob and replace it with a narrow crank handle free of charge.

      The elevator cab can rise unexpectedly to the top of the elevator shaft and abruptly stop

      Residential Elevators of Crawfordville, Fla., is recalling about 5,250 traction elevators.

      The elevator cab can rise unexpectedly to the top of the elevator shaft and abruptly stop, posing an impact hazard to any occupants.

      The firm has confirmed six reports of a failure in the motor gearbox assembly. No injuries have been reported.

      This recall involves traction residential elevators used in consumers’ homes, with models LLT-952, LLT-953, LLT-954 and LLT-955 shipped and installed between January 2014 and April 2017.

      “Residential Elevators” is printed on the light in the operating panel inside the elevator cab.

      The elevators, manufactured in the U.S., were sold at Residential Elevators’ dealers and home builders nationwide from January 2014, through April 2017, for between $18,000 and $50,000.

      What to do

      Consumers should immediately stop using the recalled elevators and contact Residential Elevators to schedule a free repair.

      All affected elevators must be repaired, including elevators that have been inspected as part of the firm’s March 29, 2018 recall. Some elevators may have already received the new CPSC approved remedy for this recall.

      The exterior griddle hose is too long

      Forest River is recalling eight model year 2021 Coachmen Catalina CAT261BHS-8 and CAT263BHSCKLE recreational trailers.

      The exterior griddle was built with a six foot hose, allowing the griddle to be stowed in the vehicle with an open flame.

      Storing the griddle while it has an open flame can increase the risk of a vehicle fire.

      What to do

      None of the vehicles have been retailed yet.

      Forest River dealers will install a three foot hose instead of the six foot one free of charge.

      The product, which contains methyl salicylate, does not come in child resistant packaging as required by the Poison Prevention Packaging Act, posing a risk of poisoning.

      No incidents or injuries are reported.

      This recall involves Organic Aromas Wintergreen Essential Oil in 10-mL, 30-mL, and 50-mL amber glass bottles with white or black caps.

      The firm name “Organic Aromas,” the firm’s logo, and “Wintergreen Pure Essential Oil,” along with the volume amounts are printed on the white label. A 2-mL size that was sold as part of a set of sample sizes of assorted oils and called the “Discovery Collection,” is also subject to this recall.

      “WNTRGRN” is printed on a green label on the cap with the firm’s logo on the side of the package.

      The oils, manufactured in Taiwan, was sold exclusively online at www.organicaromas.com from November 2017, through January 2020, for between $12 and $84.

      The vehicle’s control pad can malfunction

      WHILL (USA) of San Mateo, Calif., is recalling about 1.160 WHILL Personal Electric Vehicles, Model Ci.

      The vehicle’s control pad can malfunction causing the power to turn on/off and the speed to increase/decrease, posing crash and injury hazards.

      Fifteen device malfunctions have been reported with no injuries.

      This recall involves WHILL Model Ci, a personal mobility device that is powered by a lithium ion battery, is occupant-controlled and motorized.

      The serial number of the recalled device ranges from C_1711188_US to C_1909011_US. The serial number label is located on the seat assembly. WHILL and Model Ci are printed on the battery compartment.

      The vehicles have one chair with a platform and four wheels. The chairs are black and the arm covers come in white, red, blue and black.

      The vehicles, manufactured in Taiwan, were sold online at spinlife.com, scootaround.com, accessnsm.com and topmobility.com and other online retailers from November 2017, through September 2019, for about $4,500.
