    BCI Burke recalls metal playground climbers

    A child’s body could become lodged in the two welded handgrip locations at the top of the climbers

    BCI Burke Company of Fond du lac, Wis., is recalling about 200 Merge playground climbers.

    The two welded handgrip locations at the top of the climbers can be too large creating a strangulation hazard. A child’s body could become lodged in these openings, posing a strangulation hazard.

    No incidents or injuries have been reported.

    This recall involves the Burke Merge Metal Climbers that are used on play equipment as an attachment that allows children to climb up and access the play structure.

    The metal climbers were sold in many color combinations with small geometric shaped openings with a platform and two welded handgrips at the top.

    “Burke” is printed on identification labels that are placed on the exterior of the entire play structure.

    The climbers, manufactured in the U.S., were sold at BCI Burke Company and its distributors to public and private organizations including schools, public parks and home owners associations nationwide from November 2015, through March 2018, for $1,400.

    What to do

    Consumers should immediately remove these climbers from use and contact the firm for a free repair kit. BCI Burke is contacting all owners directly by mail.

    Heartland recalls travel trailers that risk axle failure

    The axle bearings may burnout or cause the hub to overheat

    Heartland Recreational Vehicles is recalling 633 model year 2019 Heartland Mallard, Pioneer, Sundance XLT, Wilderness, North Trail, Prowler and Trail Runner travel trailers.

    The axles may be missing inner bearing races on the hubs, which can cause the bearing to burnout or cause the hub to overheat.

    The missing bearing race may cause premature hub bearing failure, possibly resulting in a separation of the hub from the axle, thereby increasing the risk of a crash.

    What to do

    Heartland will notify owners, and dealers will replace the defective hubs with new hubs or replace the axles, free of charge.

    The recall is expected to begin October 26, 2018.

    The products may contain milk (whey), an allergen not declared on the label

    Canteen/Convenco of Middletown, Pa., is recalling approximately 1,778 pounds of ready-to-eat breaded chicken tenders.

    The products may contain milk (whey), an allergen not declared on the label.

    There are no confirmed reports of adverse reactions due to consumption of these products.

    The following breaded chicken tenders with BBQ sauce and hot sauce items, produced and packaged from September 7, 2018, through October 4, 2018, are being recalled:

    • 6 oz. clear plastic pouches containing “FRESH TO YOU, Breaded Chicken Tenders w/ BBQ Sauce,” case code 1077 and “Fresh Thru” dates ranging from 09-14-18 to 10-05-18.
    • 6 oz. clear plastic pouches containing “FRESH TO YOU, Breaded Chicken Tenders w/ Hot Sauce,” case code 6141 and “Fresh Thru” dates ranging from 09-16-18 to 10-07-18.

    The recalled products, bearing establishment number “P-40088” inside the USDA mark of inspection, were shipped to retail stores -- specifically vending machines -- in Maryland, New Jersey, New York and Pennsylvania.

    Customers who purchased the recalled products should not consume them.

      Kia recalls model year 2017-2018 Niro hybrid electric vehicles

      Increased electrical resistance can overheat the rear seat

      Kia Motors America is recalling 27,030 model year 2017-2018 Kia Niro hybrid electric vehicles.

      The Main Relay within the Power Relay Assembly (PRA) may have inadequate connections between its contacts, causing increased electrical resistance.

      The increased electrical resistance can overheat the rear seat which is above the PRA, increasing the risk of a fire.

      What to do

      Kia will notify owners, and dealers will inspect the PRA for damage. If no damage is found, the main relay will be replaced. If damage is found, the PRA will be replaced. These repairs will be performed free of charge.

      The recall is expected to begin November 23, 2018.

      The catalytic converter may overheat, damaging engine components

      Chrysler (FCA US LLC) is recalling 37,228 model year 2017-2018 Alfa Romeo Giulias and model year 2018 Alfa Romeo Stelvios equipped with 2.0L engines.

      The engines may misfire causing the catalytic converter to overheat, possibly resulting in damage to the nearby wiring and engine components.

      Heat damage to the engine compartment wiring or components can increase the risk of a fire or it can cause an engine stall, increasing the risk of a crash.

      What to do

      Chrysler will notify owners, and dealers will inspect the vehicles for diagnostic codes, and update the engine control module software, free of charge.

      The recall is expected to begin November 3, 2018.

      The axe head can detach from the handle

      Walmart of Bentonville, Ark., is recalling about 246,000 Ozark Trail camp axes.

      The axe head can detach from the handle, posing an injury hazard.

      The company has received two reports of axe heads detaching from the handle, resulting in minor cuts and abrasions.

      This recall involves Ozark Trail camp axes.

      The steel shaft tubular axes measure about 14 inches long from handle to axe head, and weigh about 1.25 lbs.

      The axes have a black, non-slip rubber grip and claw feature. “Ozark Trail” and model number 60111140 are printed on the product packaging.

      The camp axes, manufactured in India, were sold exclusively at Walmart stores nationwide and online at www.walmart.com from January 2017, through July, 2018 for about $8.

      What to do

      Consumers should immediately stop using the recalled axes and return them to Walmart for a full refund.

      The firm has received two reports of the wood surrounding the grill burning. No injuries have been reported.

      This recall involves a wooden cabinet, and comes with a separate cooler and grill compartment.

      The grills are used as display enhancers in retail, liquor, convenience and other stores and may have been received by consumers as promotional giveaways and at charity auctions.

      The cooler/grill units measures approximately 35 inches high 44 inches wide and 16 inches deep.

      The words “Tito’s Handmade Vodka” are inscribed on the front of the product.

      UberScientific recalls topical anesthetic

      The packaging is not child resistant as required by law

      UberScientific of Kissimmee, Fla., is recalling about 76,000 packages of Uber Numb topical anesthetic cream and spray.

      The packaging is not child resistant as required by the Poison Prevention Packaging Act. The products contain lidocaine, posing a risk of poisoning to young children if they put it on their skin or ingest it.

      No incidents or injuries have been reported.

      This recall involves UberScientific Uber Numb Topical Anesthetic Cream and Uber Numb Spray.

      The recalled cream is in a white jar with a smooth, rounded cap and a green label with “Uber Numb Topical Anesthetic Cream” printed on the front.

      It was sold in one, two and four ounce sizes with the lot numbers printed on the bottom of the jar and the UPC number printed on the label to the left of the product name. The brand name UberScientific is printed on the label below the net weight.

      The recalled spray is in a green bottle with a white cap and has a white label with “Uber Numb Spray” printed on the front.

      It was sold in four ounce size with the lot numbers printed on the bottom of the bottle and the UPC number printed on the label to the right of the product name. The brand name UberScientific is printed on the label to the left of the product name.

      The joints on the vinyl dolls can break, creating broken pieces

      Boy Story of Tampa, Fla., is recalling about 8,500 Mason & Billy action dolls and Mason & Billy HeForShe special edition action dolls sold in the U.S. and Canada.

      The joints on the vinyl dolls can break, creating broken pieces that pose a choking hazard to young children.

      The company has received about 32 reports of the vinyl breaking on the joints. No injuries have been reported.

      This recall involves four styles of Boy Story action dolls. They include the Mason and Billy Action Dolls, and the Mason and Billy HeForShe Special Edition Action Dolls.

      The recalled dolls are vinyl and measure 4 inches long by 7 inches wide by 18 inches high. They also came with outfits consisting of a button-up shirt, white t-shirt and jeans.

      Boy Story LLC is stamped on the left foot of each doll.

      The action dolls, manufactured in China, were sold online at www.biddleandbop.com and small retailers from December 2016, through May 2018, for about $100.

      What to do

      Endo Pharmaceuticals is recalling two lots of Robaxin (methocarbamol tablets, USP) tablets.

      The product label contains incorrect daily dosing information due to an error which misstates the daily dose as "two to four tablets four times daily" rather than the correct dosage of "two tablets three times daily."

      The company has not received any reports of adverse events related to this recall.

      The following product lots, shipped by wholesale distributors to retail pharmacies, are being recalled:

      • Robaxin 750mg, 100 Count Bottle pack, Lot 216702P1, Expiration Date: September 2020; and
      • Robaxin 750mg, 100 Count Bottle pack, Lot 220409P1, Expiration Date: January 2021.

      The glass shade can detach and fall

      IKEA Supply of Switzerland is recalling about 37,000 CALYPSO ceiling lamps.

      The lamp's glass shade can detach and fall, posing impact and laceration hazards to consumers.

      The company has received 19 reports of the glass shade detaching and falling, resulting in three minor injuries, including bumps and cuts.

      This recall involves IKEA CALYPSO ceiling lamp units with manufacturing date codes between 1625 and 1744. The manufacturing date code is printed on a sticker on the light bulb socket base plate.

      The lamps, manufactured in China, were sold exclusively at IKEA stores nationwide and online at www.ikea-usa.com from August 2016, to July 2018, for about $30.

      What to do

      Consumers should immediately stop using the recalled ceiling lamps and return them to any IKEA store for a full refund or replacement lamp.
