    GSK Consumer Healthcare recalls children's cough medicines

    The dosing cups may be missing some graduation markings

    GSK Consumer Healthcare is recalling two lots of Children's Robitussin Honey Cough and Chest Congestion DM, and one lot of Children's Dimetapp Cold and Cough.

    The dosing cups may be missing some graduation markings and contain only the 20 mL graduation, posing a risk of accidental overdose.

    There are no reports of adverse events.

    The following products, sold nationwide between February 5, 2020, and June 3, 2020, are being recalled:

    Thor Motor Coach recalls various motorhomes with window issue

    The frameless window glass panels may detach

    Thor Motor Coach (TMC) is recalling 71 model year 2020-2021 ACE, 2021 Four Winds, and model year 2020 Gemini, Axis, Compass, Delano, Windsport, Hurricane, Outlaw, and Vegas motorhomes.

    The adhesive bond between the glass and the metal hinge frame of the frameless windows may fail which would allow the glass to detach and fall out.

    If the window glass detaches while the vehicle is moving it could become a projectile, increasing the risk of a crash.

    What to do

    TMC will notify owners, and dealers will inspect the frameless windows for proper adhesive bond strength, replacing the windows -- as necessary -- free of charge.

    This recall is expected to begin July 5, 2020.

    Rexair recalls Rainbow SRX vacuums

    The vacuum 's circuit board can spark, posing fire and burn hazards

    Rexair of Troy, Mich., is recalling nearly 43,000 Rainbow SRX vacuums sold in the U.S. and Canada.

    The vacuum 's circuit board can spark, posing fire and burn hazards.

    There have been two reports of the SRX vacuum catching fire. No injuries have been reported.

    This recall covers only the Rainbow SRX water-based filtration vacuum cleaner with model number RHCS19 Type 120 and a Serial Number that falls within the range 22003399 through 22077889.

    The vacuum is predominantly black with blue trim and rubber trim around the base. The “SRX” logo is located on the right and left front panels of the vacuum. There is a clear water basin at the base of the vacuum.

    The vacuums, manufactured in the U.S., were sold at independent authorized Rainbow distributors nationwide to consumers from June 2019, through June 2020, for about $3,200.

    What to do

    Consumers should immediately unplug and stop using their affected SRX vacuum and contact an authorized Rainbow distributor for a free repair.

      Daimler Vans USA recalls model year 2019 Metris vans

      Driveshaft vibrations may cause the front airbags to deploy

      Daimler Vans USA (DVUSA) is recalling 12 model year 2019 Metris vans [platform 447 (VS20)] with gasoline engines and automatic transmissions.

      The driveshaft may not be attached to the intermediate bearing correctly, causing excessive wear to the driveshaft joint and excessive vibrations.

      Damage to the driveshaft joint could result in a loss of drive, increasing the risk of a crash. Excessive vibrations may cause the front airbags to deploy unexpectedly, increasing the risk of injury.

      What to do

      DVUSA will notify owners and dealers will inspect the drive shaft, replacing it -- as necessary -- free of charge.

      This recall is expected to begin July 24, 2020.

      Jayco is recalling 449 model year 2020 Motorized Alante, Greyhawk, Melbourne, Precept, Redhawk, Seneca, Accolade, Emblem, Esteem, Odyssey, Qwest and Vision recreational vehicles.

      The balls can suddenly burst, posing a fall injury to the user.

      There are no reports of incidents or injuries.

      This recall involves Domyos Swiss Ball Stable and Swiss Ball Basic pilates balls, with or without handles and elastic straps, in various colors.

      The Domyos logo is inscribed on the ball.

      The trailer's safety chains may have incorrect hardware

      Forest River is recalling 73 model year 2021 Prime Time Avenger travel trailers.

      The safety chains that help prevent a total trailer separation may have been incorrectly attached to the trailer.

      If the vehicle trailer becomes detached from the tow-vehicle, the safety chains may break, leading to complete detachment and potential property damage, personal injury or a crash.

      What to do

      Forest River will notify owners, and a dealer or service center will install the missing safety chain attaching hardware free of charge.

      The recall is expected to begin July 15, 2020.

      Rusty’s Seafood Market of Lorida, Fla., is recalling all five of its Captain Rusty’s Seafood Dips.

      The products contain fish, shellfish, wheat, eggs, soy and milk, allergens not declared on the label.

      No confirmed illnesses have been reported to date.

      The following products, which come in a 7-ounce, clear plastic container, 16-oz. clear plastic container and 6-lb. white plastic buckets with the lot code on the bottom and the expiration date on the top label, are being recalled:

      The operator's manual may contain incorrect automatic parking information

      Jayco is recalling 666 model year 2019-2020 Entegra Qwest and model year 2019-2021 Jayco Melbourne & Melbourne Prestige Class C motorhomes built on Mercedes-Benz Sprinter chassis.

      The operator's manual may not contain the correct information about how the automatic parking function ("Auto-P") operates.

      If the driver relies on information in the operator's omnual, vehicle rollaway or movement may occur, increasing the risk of a crash.

      What to do

      Jayco will notify owners, and Mercedes-Benz or Freightliner Sprinter dealers will provide the correct description of the Auto-P function for the operator's manual free of charge.

      The booklet with correct information is also available online free of charge.

      This recall is expected to begin June 12, 2020.

      Maxon recalls GPTLR and GPTWR Lift Gates

      The license plate light may be aimed toward other drivers

      Maxon Lift Corp. is recalling 2,280 GPTLR and GPTWR Lift Gates that contain License Plate Kit part number 298872-01 that includes Light part number 908120-01.

      If installed, the license plate light may aim rearward and not toward the license plate.

      A license plate light aimed toward other drivers may increase the risk of a crash.

      What to do

      Maxon will notify the vehicle manufacturers who may, or may not, have used the Maxon provided License Plate Kit with Light and will provide a corrective kit as necessary free of charge.

      Rong Shing Trading recalls Xi Zhi Liang Fruit Jelly Cup

      The product may pose a choking hazard

      Rong Shing Trading of Brooklyn, N.Y., is recalling its 21.1-oz (600g) Xi Zhi Liang Fruit Jelly Cups (喜之郎什果果凍杯).

      The small jelly cups containing konjac powder have previously been implicated in choking deaths of children. The cups are not permitted for entry into the U.S.

      No incidents or injuries are reported.

      The recalled product comes in pink, yellow and blue plastic backpacks, with numerous individual plastic 21.1oz (600g) jelly cups inside each back pack, and a toy car in a pouch on the front of the plastic backpack. UPC code is, “6902934982138,” and the “BEST BEFORE” date is December 30, 2020

      Davina Wellness recalls Wintergreen Essential Oils

      The packaging is not child resistant as required by law

      Davina Wellness of Mapleton, Utah, is recalling about 300 bottles of Davina Wintergreen Essential Oil and Davina Kneads Relief Sore Muscle Blend.

      The packaging of the products, which contain methyl salicylate, is not child resistant, posing a risk of poisoning if the contents are swallowed by young children.

      No incidents or injuries are reported.

      This recall involves Davina Wintergreen Essential Oil and Davina Kneads Relief Sore Muscle Blend bottles with black continuous thread closures.

      The wintergreen essential oil includes the 10 mL size and the blend includes 2 mL, 5 mL and 10 mL sizes. The blend was also sold in kits.

      The black label on the front of each bottle displays the Davina logo, product name, and the volume amount of the bottle.

      The oils, manufactured in the U.S., were sold online at BrickyardBuffalo.com, DavinaWellness.com, Groopdealz.com, Jane.com, PickYourPlum.com and Zulily.com from April 2017, through June 2019, for between $5 and $11.

      What to do

      Consumers should immediately store the product in a safe location out of reach of children. Contact Davina Wellness for a free replacement child-resistant cap. All known purchasers are being notified directly about the recall by Davina Wellness or by the online retailer where the product was purchased.

      The trailer's underrated safety chains may break

      Forest River is recalling 122 model year 2020-2021 Haulin, US Cargo, and US Cargo Trail N Sport cargo trailers.

      The safety chains are insufficient for the trailer's Gross Vehicle Weight Rating (GVWR) and may break if the trailer comes unhitched.

      If the trailer becomes unhitched from the tow-vehicle, the safety chains may break, leading to complete detachment and potential property damage, personal injury or a crash.

      What to do

      Forest River will notify owners, and a dealer or service center will replace the safety chains free of charge.

      Miniware recalls teething spoons

      The spoon can break in a child’s mouth when being used as a teether

      Miniware of Sausalito, Calif., is recalling about 32,200 Miniware teething spoons.

      The spoon can break in a child’s mouth when being used as a teether, posing a choking hazard.

      The firm has received one report of an infant who bit through the silicone teething spoon, resulting in a piece separating inside the mouth.

      The recalled teething spoons are made from food-grade silicone designed for infants to use during the teething phase -- approximately 4-24 months of age.

      They were sold in a set of two and came in gray, aqua, peach, key lime (green), cotton candy (pink), and lavender, with “Miniware” stamped in raised letters on the handle of the spoons.

      The spoons were also sold in Meal Kit Configurations. The kits were “First Bites” that included spoons, bowl, suction foot, and lid. The spoons colors came in key lime, aqua, cotton candy, lavender, gray and peach.

      The “First Bites Travel Kit” included a spoon, bowl, suction foot, and travel bag. The spoons came in lavender and aqua color.

      The “Sip and Snack” kit included a spoon, bowl, suction foot and drinking cup. The spoon came in peach and gray color.

      The spoons, manufactured in Taiwan, were sold online on Miniware.com, Amazon.com, and in stores including Dillard’s, The Tot, Turquoise, The Mellowland, Mom Loves Me, and Bitte nationwide from December 2017, through March 2020, for between $14 and $42.

      What to do

      Consumers should stop using the recalled teething spoons immediately and contact the firm for a free replacement product.

      Keystone RV Company is recalling 41 model year 2020 Springdale trailers, model 335BHWE.

      The trailers incorrectly have 4400 pound axles and ST255/75R15D tires and rims, instead of the correct 5200 pound axles and ST255/75R15E tires and rims as specified on the federal identification tag.

      The incorrect axles and tires may cause the trailer to be overloaded, increasing the risk of a crash.

      What to do

      Keystone will notify owners, and dealers will replace the 4400 pound axles, ST225/75R15D tires & rims with 5200 pound axles, ST225/75R15E tires & rims.

      Lupin Pharmaceuticals recalls Metformin Hydrochloride Extended-Release Tablets

      The medication may be contaminated with a probable cancer-causer

      Lupin Pharmaceuticals is recalling one lot of Metformin Hydrochloride Extended-Release Tablets USP (generic equivalent of Fortamet).

      Analysis shows the prescription oral medication, used as an adjunct to diet and exercise to improve blood glucose control in adults with type 2 diabetes mellitus, exceeded the Acceptable Daily Intake Limit for the impurity N-Nitrosodimethylamine (NDMA).

      NDMA is classified as a probable human carcinogen (a substance that could cause cancer).

      The firm has not received any reports of adverse events to date.

      Ushio America recalls Indiglow LED T8 lamps

      The lamps can overheat causing the glass tube to fall

      Ushio America of Cypress, Calif., is recalling about 3,000 Indiglow LED T8 lamps.

      The lamps can overheat causing the glass tube to fall and strike those standing nearby, posing an injury hazard.

      The firm has received reports of five incidents involving the lamp tubes overheating and falling to the ground. No injuries have been reported.

      This recall involves Ushio America’s Indiglow LED T8 backlight lamps, with a 4ft. tube.

      The lamps are direct drop-in replacements for 30W and 32W T8 fluorescent tubes. The firm name USHIO and brand name Indiglow are on the lamp.

      The manufacturing date code is imprinted on the silver aluminum end cap of the lamp indicated by 16xx or 17xx.

      The lamps, manufactured in China, were sold at Candela Corporation, Bulb America, Atlanta Light Bulbs, Dial Electric, Television Production Services, and 1000 bulbs.com and distributors nationwide and online at Ushio.com from March 2017, through March 2018, for about $25.

      What to do

      Consumers should immediately stop using the recalled lamps and contact the firm for a full refund or a free replacement lamp. Ushio America is contacting all known purchasers directly.

      A wire harness in the rear of the vehicle may contact the vehicle frame, resulting in damage to the circuits that support the fuel pump and the anti-lock braking system (ABS).

      Damage to the fuel pump circuit in the wiring harness could result in an unexpected engine stall. Damage to the ABS wiring can lead to reduction of ABS functionality and a loss of roll stability control (RSC) functionality. These scenarios increase the risk of a crash.

      What to do

      TMC will notify owners, and Ford dealers will inspect the wire harness for damage.

      If no damage is found, dealers will apply anti-abrasion tape over the area and ensure clearance to surrounding components. If damage is found, dealers will splice in new wire and apply anti-abrasion tape over the area and ensure clearance to surrounding components. All services will be performed free of charge.

      The recall is expected to begin July 27, 2020.
