    Two deaths prompt the recall of Triangle Tube Prestige and Aerco gas boilers

    The boilers are suspected of producing carbon monoxide

    Triangle Tube has issued a recall of about 60,000 Triangle Prestige and Aerco gas boilers due to the risk of carbon monoxide poisoning. Two deaths have been linked to the boilers.

    The U.S. Consumer Product Safety Commission said the boilers can release carbon monoxide (CO) due to delayed ignitions or component damage, posing a risk of serious injury or death.

    Triangle Tube has received 25 reports of CO leaks from the recalled boilers

    Since 2011, approximately 60,000 boilers were sold in the United States and about 7,500 of the boilers were sold in Canada. 

    The recalled Prestige Solo, Prestige Excellence, and Aerco Esteem boilers are wall-hung condensing gas boilers housed in a white metal box. “Prestige” is printed on the white front panel of the recalled Prestige Solo and Prestige Excellence boilers. 

    “Aerco” is printed on the front of the recalled Esteem boilers. Please visit the Triangle Tube recall website for directions on identifying if you have a recalled boiler by serial number, which can be located either on the Boiler Rating Label located on the white front panel, on a panel on the right hand side of the boiler, or behind the boiler front door. 

    The boilers were manufactured by Burnsen SA, of Belgium, from December 2011 through April 2019 and were sold by wholesale distributors and installed by independent contractors nationwide for between $3,400 and $9,700.

    The boilers were imported by Triangle Tube/Phase III Co. Inc., of West Deptford, New Jersey, and manufactured in Belgium.

    What to do

    Consumers with recalled boilers should immediately contact Triangle Tube to schedule a free boiler inspection and, if applicable, free installation of a new boiler ignition system. Consumers who continue using the recalled boilers while awaiting repair should have working carbon monoxide alarms on each level of their homes and outside sleeping areas. 

    Consumers who do not have a carbon monoxide alarm can contact Triangle Tube to have one shipped to them free of charge.

    Forest River recalls 2025 Coachmen Catalina trailers for incorrect weight label

    Incorrect weight label may lead to overloading risk in trailers

    Forest River, Inc. has issued a voluntary recall for certain 2025 Coachmen Catalina travel trailers. The recall number is 203-1889.

    It involves an incorrect Federal Placard label displaying the wrong vehicle dry weight. This issue was identified during an internal review and may lead to unintentional overloading.

    The recall applies to approximately 144 units sold through Coachmen dealerships in various regions of the United States. Forest River, Inc. is replacing the labels free of charge and working with relevant agencies to address the situation.

    Dietary supplement with iron and vitamins recalled for undisclosed ingredient

    The product has been found to contain tadalafil, an ED drug

    Natural Dior, is recalling the affected lots of Vitafer-L Gold Liquid, a dietary supplement with iron and vitamins, to the consumer level. The product has been found to contain undeclared tadalafil, an ingredient in FDA approved products for treatment of male erectile dysfunction in the family of drugs known as phosphodiesterase (PDE-5) inhibitors. 

    Products containing tadalafil cannot be marketed as dietary supplements. Vitafer-L Gold Liquid is an unapproved new drug for which safety and efficacy have not been established and, therefore, subject to recall.

    Consumption of products with undeclared tadalafil may interact with nitrates found in some prescription drugs (such as nitroglycerin) and may cause a significant drop in blood pressure that may be life threatening. People with diabetes, high blood pressure, high cholesterol, or heart disease often take nitrates. 

    Among the adult male population, who are most likely to use this product, adult males who use nitrates for cardiac conditions are most at risk. To date, Natural Dior LLC has not received any reports of adverse events related to this recall.

    The product is used as a state indication(s) and is packaged in 16.9 fl oz (1.06 pt) 500 mL bottles in a folding cardboard box.

      Nova Bus recalls 2023-2024 LFS and LFS Artic buses over faulty coolant sensors

      Faulty sensors may cause unexpected engine shutdowns in Nova buses

      Nova Bus (US) Inc. has announced a safety recall of certain 2023-2024 LFS and 2023 LFS Artic buses. These vehicles are equipped with Casco engine coolant level sensors (part number N108036) that may send a false signal indicating critically low coolant levels. This recall has been reported to the National Highway Traffic Safety Administration (NHTSA) and covers approximately 673 vehicles manufactured between April 20, 2023, and August 8, 2024.

      The problem was first identified on September 29, 2023, following a customer complaint. If these sensors send a false signal, the engine may shut down unexpectedly, increasing crash risk. The affected buses were sold to commercial and municipal transit operators throughout the United States. Nova Bus is coordinating with NHTSA to address the issue by replacing the faulty sensors.

      This recall involves OdorStop Boot and Shoe Dryers and Deodorizers with model numbers OSOBSDD2 and OSOBSDD.  The model number and OdorStop name are on a label at the bottom of the product. The products are used to dry and deodorize two or four boots with high-output fans and a 3-hour timer.
