    Westfield Outdoor recalls Outfitter and Big Horn tents

    The tent’s stove jack ring can deteriorate at high temperatures

    Westfield Outdoor of Indianapolis, Ind., is recalling about 5,900 Outfitter and Big Horn tents sold in the U.S and Canada.

    The tents' stove jack rings can deteriorate when exposed to high temperatures from contact with a stove pipe and create smoke inside the tent. This poses a risk of injury from smoke inhalation.

    The firm has received three reports of the stove jack ring deteriorating due to heat and creating smoke and one report of smoke inhalation and frostbite related to discontinuing the use of a wood stove while camping.

    This recall involves only the Cabela’s Ultimate Alaknak Outfitter, the Instinct Outfitter, and the Big Horn 6P tents supplied by Westfield Outdoor.

    The tents’ sizes range from 10 feet by 10 feet to 13 feet by 27 feet and are sold in green. The name, model number, SKU, and date code (HF MMDDYY) providing the place and date of manufacture are printed on the inside of the tent on the sewn-in warning label.

    The tents are designed for the optional use of a wood stove inside. The stove pipe exits the tent through the stove jack ring which provides a seal between the stove pipe and the tent. A list of the recalled model numbers may be found here.

    The tents, manufactured in China, were sold exclusively at Cabela’s and Bass Pro Shops nationwide and online at www.cabelas.com and www.basspro.com from August 2019, through November 2020, for between $900 and $1,500.

    What to do

    Consumers should immediately stop using a wood stove inside the tents and contact Westfield Outdoor for a free replacement stove jack ring.

    Consumers may remove the old stove jack ring from the tent and install the replacement ring by following the instructions provided with the replacement ring. Westfield Outdoor is contacting all known purchasers directly.

    What to do

      The recalled product, which comes in a 16-oz. container with lot number 042122-2K and a “Use By” date of 6/30/22, was sold in refrigerated cases in the deli department of select grocery stores in California, Iowa, Illinois, Massachusetts, Maine, Michigan, New Hampshire, New York, Ohio, Texas, Virginia, and Washington.

      Sakar International recalls Tony Hawk Silver Helmets

      The helmets can fail to protect consumers in a crash

      Sakar International of Edison, N.J., is recalling about 12,655 Tony Hawk Silver Helmets.

      The helmets do not comply with the positional stability and retention system requirements of the federal safety standard for bike helmets. That means they can fail to protect wearers in the event of a crash.

      No incidents or injuries have been reported.

      This recall involves Tony Hawk Silver Metallic multi-purpose helmets. They are silver and include black straps, a black buckle, and Tony Hawk’s signature printed on the outside.

      The white warning label on the inside of the helmet contains Item No. AGE2515STH-SIL on the top right corner.

      The helmets were also provided as replacement helmets for Sakar Dimensions Bluetooth Speaker Helmets in March 2022, which are now also being recalled. Sakar will contact consumers who received the replacement helmet as a remedy.

      The helmets, manufactured in China, were sold exclusively at Walmart stores nationwide and online at Walmart from March 2022, through June 2022, for about $30.

      What to do

      Consumers should immediately stop using the recalled helmets and contact Sakar for instructions on receiving a refund in the form of a $40 Walmart gift card.

      Consumers should not return the helmet to Walmart. Instead, they should contact Sakar for the gift card and to facilitate returns using prepaid postage packaging. Walmart will contact all known purchasers.

      The product contains Gluconacetobacter liquefaciens

      Vi-Jon is recalling one lot of CVS Magnesium Citrate Saline Laxative Oral Solution Lemon Flavor.

      The product contains Gluconacetobacter liquefaciens, a bacterium that could put immunocompromised people at an increased risk for invasive infections that could lead to serious, life-threatening, and adverse health consequences.

      No consumer complaints have been received to date.

      The product, which is used to relieve occasional constipation, comes in a 10-oz. clear round plastic bottle under batch number 0556808 and expiration date 12/2023. It was sold at CVS drug stores nationwide.

      What to do

      Customers who purchased the recalled product should stop using and return it to the place of purchase.

      Samson International recalls Cayden 9-drawer chests

      The chests are unstable if they are not anchored to the wall

      Samson International is recalling about 5,400 Universal Broadmoore Cayden Gentleman’s 9-drawer chests.

      The chests are unstable if they are not anchored to the wall, posing tip-over and entrapment hazards that can result in death or serious injuries.

      The firm has received one report of a tip-over incident involving a 10-year-old child.

      This recall involves Samson International’s Universal Broadmoore Cayden Gentleman’s 9-drawer chests, model/item number M71C3180. The chests are brown and measure about 49 inches tall, 55 inches wide, and 18 inches deep. A label located on the back of each unit contains “Samson International,” the model/item number, the month and year of manufacture, and the Costco customer item number 1335751. The chests are made of wood.

      The date codes are printed in MMDDYYYY (month/day/year) format, and recalled units have manufacturing date codes between September 2019, and November 2019.

      The dresser chests, manufactured in China, were sold exclusively at Costco Wholesale Warehouses nationwide and online at www.costco.com from December 2019, through April 2020, for about $700.

      What to do

      Consumers should immediately stop using recalled chests if they are not properly anchored to the wall, place them in an area that children cannot access, and contact Samson for a full refund or a free tip-over restraint kit.

      Samson will provide prepaid shipping labels so that consumers can remove the chest’s drawer slides and return them to Samson for a full refund. Samson will also provide free in-home installation of the tip-over restraint kit upon request.

      Consumers may also return the item to any Costco Warehouse for a full refund.

      Daily Harvest of New York, N Y., is recalling all French Lentil + Leek Crumbles.

      The recall was prompted by consumer reports of gastrointestinal illness and potential liver function issues.

      The company has received approximately 470 reports of illness or adverse reactions to date.

      The recall of the frozen product, packaged in a 12-oz white pouch with the words “Daily Harvest” at the top and “CRUMBLES” immediately below the top and the words “French Lentil + Leek” in bold, involves all lot codes.

      Approximately 28,000 pouches of the recalled product were sold in the continental U.S. online, through direct delivery, and through retail sales at the Daily Harvest store in Chicago, Ill., and a “pop-up” store in Los Angeles.

      Scholastic Inc., of New York is recalling about 187,000 Shake Look Touch books sold in the U.S. and Canada.

      The pom poms on the book can detach, posing a choking hazard to young children. The firm has received two reports of the pom poms detaching, but no injuries have been reported.

      This recall involves the Shake Look Touch book, a plush book with touch and feel components for young children.

      The books measure 5.9 x 1.7 x 8.4 inches and have pink and green pom poms attached to them with a string. Only books with the pom poms are included in this recall.

      The books, manufactured in China, were sold at bookstores, specialty, big box stores, and other stores nationwide. They were also available online at www.barnesandnoble.com, www.homegoods.com, www.booksamillion.com, and other websites from January 2020, through April 2022, for about $13.

      What to do

      Consumers should immediately take the books away from young children and cut the pom poms off the products.

      Owners may participate in the recall and receive a $10 gift card by registering online, submitting a picture of the book and removed pom poms, and affirming that the removed pom poms will be disposed of. Consumers may use the book after removing and disposing of the pom poms.

      Laars Heating Systems recalls residential boilers

      The blocked vent temperature switch can malfunction, causing the release of gasses

      Laars Heating Systems Company of Rochester, N.H., is recalling about 570 residential atmospheric gas-fired boilers sold in the U.S. and Canada.

      The blocked vent temperature switch (BVTS) can malfunction, which can cause the release of gasses, including carbon monoxide (CO), into consumers’ homes. This can increase the risk of CO poisoning.

      No incidents or injuries have been reported.

      This recall involves residential atmospheric gas-fired boilers manufactured from January 28, 2021, through January 12, 2022, and sold under the Laars Mini-Therm JX and Bradford White Brute Mini brand names.

      A list of the recalled models and serial numbers, which can be found on the boiler rating plate on the right side of the boiler, can be viewed here.

      The boilers, manufactured in the U.S., were sold at independent contractors, plumbers, and plumbing supply distributors nationwide from January 2021, through January 2022, for between $6,000 and $8,000, including installation.

      Consumers should immediately contact Laars Heating Systems to arrange to have a certified technician install repairs at no cost.

      The garments fail to meet the flammability standards for children’s sleepwear, posing a risk of burn injuries.

      This recall involves Kids Tales-branded children’s 95% cotton and 5% elastane pajamas sold in various print designs and colors. They were sold individually or as a set of two.

      The single-piece, short-sleeved, footless pajamas were sold in five sizes -- 66 (3-6 Months), 73 (6-12 Months), 80 (12-18 Months), 90 (18-24 Months), and 100 (2-3T) -- and in various prints. “Kids Tales,” “Made in China,” and the size are printed on the pajama’s neck label.

      The garments were sold online at www.amazon.com from November 2021, through April 2022, for between $13 and $33, depending on the style and if they were sold individually or as a set.

      What to do

      Consumers should immediately take the recalled sleepwear away from children and contact Kids Tales for a full refund. Those who purchased the pajamas will be asked to destroy them by cutting them in half and to send the recalling firm a photo of the destroyed garment.

      Upon receipt of the photo, consumers will be issued a full refund of the purchase price.

      Testing by the U.S. Food and Drug Administration found detectable levels of per- and polyfluoroalkyl (PFAS) chemicals, a diverse group of human-made chemicals that may cause increased cholesterol levels, increases in high blood pressure, and preeclampsia in pregnant women.

      There have been no reports of illnesses to date
