UnitedHealthcare plan types

UnitedHealthcare has four categories of plans: health insurance, Medicare, Medicaid and dental, vision and supplemental plans. Under its health insurance umbrella are individual and family insurance plans for those under age 65, including short-term, self-employed, ACA marketplace, student and work-based plans.

Participants can add supplemental coverage like dental, vision, fixed benefit and accident coverage, which can cover things UHC's standard health insurance plans don't, or provide added benefits beyond standard coverage.

ACA marketplace

Rather than buying health insurance through an employer, you can buy UHC plans through the ACA Marketplace. These plans cover preexisting conditions and include preventative care like annual checkups, immunizations and screenings. These usually cover virtual and primary care, too.

Employer-sponsored

UHC offers health plans through employers, including coverage for small businesses. Your employer selects the available plan options, so your medical benefits, provider networks, premiums, prescription coverage and optional dental or vision benefits will differ by workplace.

Medicare, Medicare Advantage and Medicare supplemental insurance

UHC offers Medicare Advantage plans, stand-alone Medicare prescription drug plans and Medicare Supplement insurance plans. The best choice for you depends largely on whether you want Medicare benefits through a private Medicare Advantage plan or want to keep Original Medicare and add coverage for its out-of-pocket costs.

Medicaid

In some states, UHC offers Medicaid coverage. Eligibility, benefits, provider networks and enrollment procedures vary by each state’s Medicaid program. Some plans include dental, hearing or vision care, too.

This coverage may be available for eligible children, pregnant people, families and adults who meet state income and other requirements. UHC offers a Community Plan site to determine benefits where you live.

UnitedHealthcare coverage

UnitedHealthcare offers individual and family, employer, Medicare, Medicaid and supplemental plans. These medical plans may cover preventative care, office visits, hospital care and prescription drugs. ACA Marketplace plans cover preexisting conditions and include $0 covered preventive care. Dental, vision, virtual care and wellness benefits might be included or available separately.

Benefits, costs and provider access vary by plan, state and ZIP code. You might need to use network providers for the highest coverage, and some plans require a PCP or specialist referrals. Deductibles, copays, prescription drug coverage, prior authorization requirements and excluded services also vary.

UnitedHealthcare cost

UnitedHealthcare doesn’t publish a price list as premiums and costs vary by location, plan type, coverage tier and household details. You may pay:

  • A monthly premium
  • A deductible before some benefits begin

    Absolutely awful service. I was promised coverage for mental health services but instead was signed up for dental, vision, and two other plans that I still am confused about. I did not ask for dental or vision and neither of the other plans cover mental health. Every time I tried calling to get help, I would be transferred a minimum of 10 times, all indirectly with no way of knowing if the next person would be able to help me. None of them could and I was informed that none of the plans they offer cover mental health services. Exceptionally frustrating and misleading. I will never ever work with United again and will tell everyone I know to avoid services with this and affiliated companies. Truly awful experience.

    We received a “JOIN US for a UHC Medicare Advantage Plan Event” at Black Bear Diner at 4PM. Space is limited so reserve your spot. We reserved because wife needs a plan for 2027. We drove 30 miles R/T. Arrived at 3:50 only to find others eating. Apparently, this was an earlier UHC group who WERE served food. Same presenter as ours, BUT WE DID NOT GET FOOD. What a UHC gimmick! Feed half of those invited and sucker punch the rest. Presenter literally read from an information sheet that was handed out. No new info. Same info could be found on UHC website. Insult to injury, they collected our personal contact info.

      United Healthcare is the worst insurance for providers to deal with. This is an American company and humans need to be answering the phone and that speak clear english. The providers are the ones that help this company grow and it is absolutely horrible to get a human and one that speaks english. Kudos to anyone who wants to come to this country and live, however they need to be able to speak clear english as again we are in America!!!!

      September 2026 of this year. They took my insurance away, then they called me back in October. The fifth to say I can have my insurance back. I asked them, could I get my September benefits back? They told me no because of insurance lap, but I called the other insurance. And had proven that I had insurance with the other insurance. So they still wouldn't give me my benefits back. This company sucks.. The representative got very smart with me. Why? She was trying to give me my insurance back.

      UHC Medicare Advantage Essentials offers a $10 reward for completing 10 days of walking activity each month. My wife and I completed the activity and logged it online on September 30, receiving acknowledgement and a statement that our rewards would be applied to our balances. However, we never received the rewards. When we telephoned UHC member services and reported the error, noting that it was due to their faulty website and not to any failure on our part, we were told that since it already was October, there was nothing they could do. In other words, they simply refused to make good on their offer when it is obvious that they easily could have issued compensation to us.

      Read & believe reviews on UHC. - they’re extensive & not good. INCOMPETENT or DISHONEST - Maybe both. The most simple inquiries turns into mass confusion for all involved. Deny, Delay, Confuse, Frustrate until you literally give up trying. That’s what we did - we gave up; you literally can’t get a consistent answer - they win. A different benefit answer each time you call🤪. It truly makes you wonder if it’s a company strategy by default, by purposely hiring incompetent customer service reps that don’t know what they’re doing or how to interpret benefits - so much lying, deception, confusion, run-a-round you just give up; then they don’t have to pay. We’ve spent DAYS trying to get consistent answers with these idiots. I feel sorry for people who are really sick, elderly, alone trying to navigate medical issues/needs with UHC. All while the fat cats at the top get rich. Disgusting. How do they get away with this?

      This is the WORST Customer Service ever. You need a number to get past the computer. After a long time of fighting with the computer I was finally given a person, who could not help me. I was then again transferred to another person who all of a sudden could not understand me. If this is the customer service they offer, DO NOT SIGN UP WITH THEM! They suck!!! My sister died 1 1/2 years ago, I just wanted her name removed from the m;ailing list. Never happened.

      I rated United Healthcare a 1 star for their customer service or lack thereof. First of all they are located in Asia (probably India) which I don't like to begin with but on top of that they are inept. All I needed was a letter of active coverage so I could cancel my previous medical insurance and discontinue paying the premiums. They were going to mail me the document..... Three weeks later no document.

      I called and asked them to email me the document and they said they couldn't. I find it very hard to believe they don't have the means to email me a document. RIDICULOUS! So I requested to speak with someone in the USA. They don't have the means to do that either. So I asked to speak to a supervisor. 40 minutes later the SAME girl got back on the line and said there were no supervisors available. I still don't have the documentation I need! Thank God I didn't have a claim problem! If you can help it DON'T DO BUSINESS WITH UNITED HEALTHCARE!!

      Since signing up with United Healthcare, I have attempted three Primary Care Physicians and they either no longer accept UHC, are not accepting new patients, or are only open partial times per day. It has gotten ridiculous to try and get medical care. I am now behind in yearly appointments all because I switched to UHC and they require a referral from a PCP that does not exist. Worst experience with health insurance I have ever had. Cannot get a UHC person on the phone.

      Sorry to submit a negative review for UHC. I have been with them for MANY years and always thought they were the best. But recently they have become stinkers. Making changes without adequate warning, taking away perks and leaving many of us stranded. They especially DO NOT seem to care about seniors. I can no longer be covered to see my primary Doctor that I have been going to for many years and really like. I DO NOT want to change Doctors. Also I needed to see my ENT on an emergency situation and NOW, even though I was just in to him a few months ago, need a referral. I will be changing my insurance plan.. Goodbye UHC. :(

      Their rewards program where you get paid to stay active, get a physical, get vaccines and get sufficient sleep is a bait and switch. I went to transfer the money I had earned over the year (almost $1000). Their system didn't recognize United's own HSA!? Which is Optum Bank so I had to manually enter the routing and account number. Plaid was supposed to verify the account with a deposit which never happened. After I started this process to transfer the money I had earned, it disappeared from United's system and was "unavailable". I never got the money that I had spent a year keeping track doing what United asked.

      This has been a horrible experience in South Florida. I’m not all that surprised considering it is United and their reputation precedes. I have been waiting for a month only for multiple doctors to fall through either the day of or a day before because of malpractice or sickness. These people who work for the company need to look themselves in the mirror every day, and may God have mercy on their souls.

      Waiting for EOB for nebulizer prescriptions over 3 months. I have Medicare and Medicaid and for 3 months since 7/15/26. I have communicated with them over a dozen times and each time I get a different answer as to why I do not have one. I also do not know how much I should be paying as this comes under Part B. I need EOB for something. Now that the annual enrollment starts soon. I would think twice.

I am extremely dissatisfied with how they've handled my case, especially their arbitration process, which I feel is heavily biased.       UHC engages in "step therapy" or "fail-first" protocols that force people off medications that work in favor of ones that don't. I have been successfully managing symptoms of 2 illnesses with branded drugs without side effects, but UnitedHealthcare is requiring me to first fail up to five other medications before it will authorize continued access to the treatment I know works. This is not a hypothetical burden — I am currently experiencing physical and mental side effects from medications forced on me by this policy, medications my own treatment history did not warrant. They do this EVERY YEAR.

      This is not clinical decision-making. It is a cost-containment tactic that substitutes an insurance algorithm for the judgment of the patient and the physician who actually knows their case. The human cost is real: prolonged suffering, unnecessary side effects, lost work and quality of life, and in many cases, irreversible harm before a patient is finally permitted to return to the treatment that was working from the start. If you have any other option besides UHC, take it.

      My Dr. requested/ordered MRI of neck & head due to enormous pain in my neck. Insurance denied MRI for my neck: "... determined not to be medically necessary: 4/7/2026". MRI for head was approved - insane! I started my sessions w my Chiropractor, and the insurance immediately sent a warning: more than 20 visits will not be covered (!). For years and years, I was healthy, continuously paying insurance & not getting any services, but when I need them to cover expenses for needed procedures, they immediately deny and/or limit necessary procedures. With every year passed, insurance cost is rising, however, the coverage gets limited. Shame, shame & scam!

      I am shocked that United HealthCare denied my outpatient surgery for breast revision from breast cancer. I have had it done before due to my breast being constructed differently after having a double mastectomy. I need some revision done again. THEY DENIED IT!! They decided it is not medically necessary! How dare a insurance company denied a breast cancer patient revision. I was told breast cancer surgeries were covered for life! My Blue Cross insurance before this one always approved any surgery regarding my breast cancer. Shame on United for denying a breast cancer patient to feel comfortable in her body.

      I am covered under a UnitedHealthcare Choice Plus HRA plan through my employer. I am filing this complaint due to UnitedHealthcare's negligent handling of my international medical claim and its failure to treat a serious cardiac condition with appropriate urgency. I underwent a medically necessary heart catheterization procedure abroad. The procedure was recommended by two U.S. cardiologists and one German cardiologist. Procedure documentation can be provided upon request.

      I have been continuously covered by UnitedHealthcare since 2014 and, other than annual preventive exams, have made very limited use of my medical benefits. When I finally faced a serious heart condition and needed care, I was unable to obtain the procedure in a timely manner in the United States and ultimately underwent the catheterization abroad.

      For more than four months, UnitedHealthcare has repeatedly failed to process my international claim properly. I have submitted the required documentation multiple times, filed several appeals through the UHC portal, and spent countless hours following up. Despite these efforts, I continue to receive notifications that documents were not received, requests to resubmit the same paperwork, and messages indicating that documents were sent to the wrong fax number.

      The repeated delays, lost documentation, poor communication, and lack of accountability have created an unnecessary burden on both me and my employer. Most concerning is the apparent lack of urgency and compassion shown toward a patient dealing with a serious cardiac condition. No patient should have to spend months fighting to have a medically necessary claim reviewed while repeatedly resubmitting the same information. This experience reflects poor claim administration, negligence in handling submitted documentation, and failure to take a patient's serious medical needs seriously.

      Fortunately I am pretty healthy for my 57 years. I recently had an injury to my shoulder that required medical intervention. I did the 8 weeks of PT as required by United. My Dr and I agreed that an xray was not necessary and wouldn't show anything since my mobility was good. We suspected tendonitis but wanted to rule out a tear. United denied an MRI because an xray hadn't been done so I had the xray and sure enough it didn't show anything. United still denying mri. They say I can appeal which could take another 30 days. Coming up on 4 months since the injury and my pain level is exactly where it was the day of the injury. Zero common sense by United. Terrible company.

      I cost UHC zero for Drugs-see a Dr every year or so for a checkup, no special tests, occasional bloodwork. I do suffer from debilitating conditions, COVID Rebound (not designated as autoimmune) that just must be tolerated (missing disc in spine and needed joint replacement) but only 5 conditions are considered serious enough for continued support with food and utility. UHC will however allow me hundreds of dollars for OTC-generally selected brands. From my perspective, maintenance and quality of life should be the priority. Unfortunately, someone else is benefiting from UHC generosity, but it is not those enrolled.

      Investigate all the fine print for your medicare advantage plans. Dentist won’t accept because they don’t want to pay them anything. Vision contact allowance is total scam. Glad open enrollment is coming soon. Don’t find out the hard way with this customer. Don’t rely on your Medicare advantage advisor either. They don’t look into all the specifics. Taking advantage of seniors is disgusting.

      I had to call the toll free number this morning. They shipped the wrong face mask for my cpap. The first person dropped the call. When I called back the call was declined. I waited and called back. The last person understood my problem gave me the Flowood MS number to change the order. Dana at the Flowood MS office is exceptional. Dana immediately changed the mask and listen to my concerns about the 6ft tubing that has a chip in it. I have dealt with Dana many times and she has been so good at her job. The employees in that office should be trained buy her. I am the patient. If the company makes a mistake they are liable. My health conditions are complex. The toll call number these people barely speak English.

      United Healthcare is the worst company I have dealt with as a patient AND as a physician. They are blood sucking vampires that prey on the weak and vulnerable.. They deny every single claim, and create endless piles of paperwork. How is it possible that they are still in business and able to get away with selling policies that have no max out of pocket no deductible to consumers? How do they have a MONOPOLY in our current society? When do we get to actually get HEALTH back and say NO we won't actually let insurance companies dictate the health of our patients or our country anymore! It is absolutely DISGUSTING what has happened to our health system and in turn the practice of medicine. Demand CHANGE! This company is one of the worst offenders.

      This insurance denies anything and everything, even though we had the so-called Cadillac plan paying $1500 monthly fees during 2025. Customer service people are insensitive and not helpful at all. (Spoke to many) Been calling now for over 10 months, fighting for them to pay my cancer bill of $12,000 that they DON’T want to accept. Now my bill is in arrears.

      For the past 4 months, they kept telling me to send medical records that have been sent numerous times via certified mail, (another $120), faxes and phone calls with a Claim number. And to call back in 10 days after review. (Even had a 3-way call with the hospital.) Well, now 60 days went by (after calling each week) and now the claim itself has changed on their end. They said they see only a bill with a different surgery date than I had, for $5000 and a check will be sent to me. What??? So, I am able to get a check for a surgery date I didn't even have for half the amount??? You should get it in the mail in 5-10 days. I had to provide the address. So, will I get this check???

      I am writing because of billing issues. I had 2 visits with a new Tier 1 Primary care provider. I was charged a $40 copay for each visit (totaling $80), even though my plan states there is no copayment for a PCP visit. While the clinic owner waived this $80 fee after I send a letter, the claim details incorrectly listed "your share" as $40 per visit.

      I visited a specialist at Tier 1 and was charged $55 instead of a $35 copay because UnitedHealthcare incorrectly listed "your share "as $55.I ended up paying this $55 fee. Optometrist visit: I was charged an incorrect $55 copay for an in-network annual vision visit. These billing errors are misleading and incorrect. I request that UnitedHealthcare review these claims, refund the overcharges, and correct my billing profile to prevent these improper copayments on future visits.

      Do not use United Healthcare. They will cover up doctor fraud and not investigate over a date. In a claim there is a tab to click on to appeal or file a grievance. When you click on that tab it starts the complaint and it puts the claim number right there on form. They ask for a start and end date which should be unnecessary because it's right in the claim. For 1 particular appeal I submitted, date of service was 5/4/26, the claim was filed 5/6/26 and it was approved on 5/15/26. I used 5/4/26 as start date and 5/15/26 as end date. All of those dates are clearly indicated on the claim.

      I received a letter 6 weeks later saying there was nothing they could do because they found no claim on 5/15/26. So, they completely disregarded everything I wrote in the explanation, $ amount, what the service was, what the Doctor did and the claim number automatically included, because I put end date as 5/15/26, and that to them was the date they expected as the claim start date. This is just a despicable attempt to cover up and or not do anything because they know if they look at the claim per the claim number, the correct dates they need are right there. You cannot file an appeal or grievance unless you are in the completed claim. I imagine they will do the same for another grievance I filed against the same Doctor, whom I have now dropped. I would rate 0 if that was an option.

      We will be changing back to Anthem HMO! Hard to get a pre approval on certain injectable meds even when I have been using it previously for 3 years using Anthem Blue Cross. Also, UnitedHealthCare makes you use only their specialized mail order Pharmacy for specialized injectables that need refrigeration so I am paying for an additional fee of $40 per month for them to ship it to me on ice. My local pharmacy has the injectable med I need (** SureClick) but no, have to still go through their mail order pharmacy. Ridicuous. Can't wait to switch back to Anthem that will allow me to go to my local pharmacy to get the injectable plus prior authorization was quick. United Health Care just fights it not to get it approved.

      I am writing to express my deep frustration and disappointment with the home healthcare services provided through UnitedHealthcare. My husband was discharged from a rehabilitation center on June 30, 2026, following treatment for a stroke. Today, August 26, 2026, nearly two months later, he has still not received the four home healthcare disciplines that were prescribed as part of his recovery plan. We have repeatedly been informed that delays are related to UnitedHealthcare's business contracts and provider network issues.

      Unfortunately, the personal healthcare nurse service assigned to assist us has not been a reliable resource. When we call, our calls are rarely answered, and we are typically required to leave voicemail messages and hope for a return call. Responses are often delayed, creating additional stress and frustration during an already challenging time.

      The lack of timely coordination and communication has negatively impacted my husband's recovery and our confidence in the services provided through UnitedHealthcare. Access to necessary medical care should not be delayed because of administrative or contractual issues. I also intend to share this complaint with my employer. It is my hope that during this year's open enrollment period, employees will be offered better healthcare options. Based on our experience, UnitedHealthcare has failed to provide the level of service and support that patients and families deserve.

      Very unhappy that it was impossible for me to cancel my mother's dental plan when it was no longer needed. I had been paying for it with my credit card with automatic monthly payments. She lives in another state with my sister but the customer service rep refused to cancel the plan unless my mother called to cancel it. Mom is 98 years old! She no longer hears so can't talk on the phone! I gave the rep my sister's cell # but she only made one attempt to call her. My sister didn't answer so the rep was done helping me with the cancellation request. I had to dispute the automatic credit card payments with Chase to get this plan cancelled! On the plus side: Chase credited my account for the previous month's premium because UHC didn't dispute my complaint. So if you have trouble cancelling your policy that is the best way to do it if you are paying via credit card automatic payments.

      UnitedHealthCare Colorado Medicare Advantage service has seriously degraded in 2026. In 2026 the only Medicare Advantage Plan available for me was an HMO requiring PHP referrals to doctors affiliated with PHP Prime HMO. Getting referrals is now a tangled web of delays and misinformation. PHP Prime does not provide a list of member docs to patients. When I asked PHP Prime about a specific Dermatologist that I had seen in the past I was told he was in network. Yet when requesting a referral from my PCP I am told the referral was declined since the doc is not in the PHP network.

      To add to the confusion, If I go directly to the United Healthcare's site for my plan the Dermatologist is shown in network, but my PCP says that does not mean he is in the PHP network and available for a referral. This certainly is a case of bait-and-switch where doctors who were listed in network on the UHC site are not actually available for referrals. I have been trying to get a referral for over 7 months with this in network/out of network confusion. While the UHC plan has some good attributes such as access to fitness facilities, UHC fails completely with what counts and that is providing the medical care that is the primary purpose of plan.

      My monthly UHC premium for my family of three is a little over $800 a month. Three years ago, I had a colonoscopy which found multiple polyps and the doctor said a follow up colonoscopy would be needed three years later. Fast forward three years and I have the procedure scheduled. The week of the follow up I receive a call from the doctor's office advising UHC will not cover this procedure because UHC will only cover one colonoscopy every five years and it will cost me around $3k out of pocket if I wanted to keep the appointment. Looks like I'll be waiting two more years and hoping nothing bad develops in that time. We also have a cardiac CT scan scheduled for this week and just received notice from UHC that this will cost us over $800 out of pocket!!!! Crooks!!!!

      Horrible experience. I pay a lot of money each week. They don't have doctors available for new patients. You have to wait for the new doctors getting the job. I waited a month for an appointment and now I have to wait for another month. So mad and frustrated right now. Worst decision ever.

      I think this company is the biggest scam artist of anyone on the planet. They absolutely care NOTHING about patient care. They do not pay timely, if they do pay and that's a big if. United Healthcare uses every tactic in the book to deny a claim and only have people on the phone that don't speak English and can't even understand you. They think because they are so big they can do whatever. Everyone needs to drop them and all Doctors need to get together and hire attorneys that have experience fighting insurance companies and TAKE THEM DOWN A NOTCH OR TWO.

      AARP is the only decent thing about United Healthcare but they are outrageously expensive. Medicare needs to oversee these supplemental plans and help the seniors on Medicare be able to afford these plans. Please people, there are SO many other insurance companies out there that don't treat their members and Doctors trying to provide Medical Care to their patients with the disrespect and disregard for human life like they do.
