    In March of 2012 I went to Quest Diagnostics in Foxborough MA. to have routine blood work done. The lab tech that did the draw went in thru the wrong vein. She was digging and probing in my arm. I told her twice to stop but she continued. The third time I told her to stop she told me to kick my legs! I told her the tourniquet had been on almost 3 minutes, my arm felt like electric shock going up and down it. A large Hematoma was already forming. I removed the tourniquet myself. She pulled a piece of tape off the sleeve of her lab coat. She did not even apply pressure! This blood draw has left me with Complex Regional Pain Syndrome, as she hit a nerve. I am in the final stage, it cannot be stopped or reversed. Please be very careful when having your blood drawn!

    This is a Complaint against Quest Diagnostics located in Hauppauge on Wheeler road. On January 17, I took an early lunch from my job got to Quest at 12:40. The office was closed lights out. There is a permanent sign on the door with the hours and it said they take lunch 12-1 so I was there way before that. I waited there like 10 minutes and went to another office in the building to ask if they were even open that day and yes there were. I was totally confused. Went back and waited some more. I was really sick and had to fast to have blood drawn. So now I have to find another place to go lost a few hours pay from my job couldn't eat until I found another place to go. I called the office that was closed because I was so upset that I lost hours from work and the women that answered the phone said she takes lunch at 12:30 and she was chewing in my ear the whole time. I Said to her how do you just go to lunch and not put a note on the door when the hours she took doesn't match what it says on the door? She didn't care at all. I asked to speak with her manager and the manager Mary still hasn't got in touch with me. Is this a business? Does anyone at Quest care at all? It's bad when you cant even get to speak with a manager.

    This is a formal complaint against Quest Diagnostics, for the poor care I received at the Service Center on Wednesday January 8th at 10:15 am in the office located at 115 E. 57th street New York, NY. The medical personnel servicing me for a blood draw was not paying attention at the time. She stabbed the needle very deep into my arm, to which I gave an audible response of pain. She continued to hold a conversation with another staff member through the open door to the hallway during the whole procedure. The results of this negligence in properly placing the draw needle, is now extreme pain in all areas of my arm throughout the day, over the past 6 days. In addition I have bruising around my elbow measuring 10 square inches.

    I had reported that we mailed 2 checks for payment and was told refund would be 6 to 8 weeks. Well they mailed my refund check this week. I was shocked as the billing department felt that I saw right.

    I had blood work done in Sept. 2013 and my wife mailed a check in for payment. As we were checking our checking account, she had realized she had sent another check for payment on 10-07-13. We paid them twice. So I called on 11-04-13 and was told I will have to wait 6 to 8 weeks to get my money. That is terrible why I have to wait 3 months to get the money. Maybe nobody should pay them for 3 months after getting blood work done and see how they feel.

    I had numerous stool tests ordered due to continuing severe diarrhea taking Quest ran kept it under control. The Quest facility gave me confusing directions. The lab tech didn't seem familiar with the tests. I looked up the directions on the Quest website and proceeded to collect samples. I had problems with one collection because I had to skip medication in order to have a movement and wouldn't be home when it occurred. I took a clear, never used Ziplock Baggie with me. Collected the specimen directly into the Baggie but could not unscrew the top of the collection vial. I took my medicine to stop further diarrhea and went to the lab to get assistance opening the vial. You would have thought I committed murder because I had not defecated into the plastic hat I was issued by the lab.

    The supervisor finally allowed me to use the specimen to prepare the vial but only after I explained how difficult it was to collect multiple specimens on different days and that the website allowed for different containers for collections. One plastic "hat" for different specimens is not near as clean as using a clean Baggie or other such container and who can travel around carrying such a large piece of equipment. The whole thing was silly. The first person I spoke with was rude told me I could go to another lab and have a nice day. The supervisor was at least willing to be reasonable. Quest appears to hire people off the street and train them to only follow a prescribed script because they are unable to use reasonable judgement from clinical knowledge.

    On Wednesday, 9/4/2013 @ 8:15 pm, we were driving west bound on Layton Avenue and was about to turn right into a Clark Gas station located on 13th Street when a White Quest Diagnostics van (# **) cut us off on the RIGHT SIDE! He also entered the station... If my husband wasn't going slow, we would have been hit. This van would have hit us on the passenger door where I was sitting. I could have gotten hurt, or worse my daughter who was sitting behind me!

    My husband politely notified the man to be a little more courteous and CAREFUL. The man came back with a very rude voice stating: "Had a long day, call it in, the number's right there". I tried locating a place to submit a written complaint online and couldn't. Their website does not have an email link for such things. I will be sending a copy of this complaint via snail mail to the 6th and Layton location address. He obviously hates his job and really has no regard for other drivers or cares whether he was complained against. He probably should not be working there especially as a DRIVER of such a prestigious company. Gives the company a real bad name.

    I had lab services done on 3.25.2013, and subsequently rec’d a bill for 99.89. I contacted the company, and as I had numerous medical bills arriving due to that illness, requested an installment plan. I was told to make the 1st payment and call to confirm, which I did. I then rec’d a letter from Invoice Audit Service, a billing company. I called and s/w David who told me no payment plan is in the system, and that bill not sent to credit agencies, and to continue the plan. I made payments the first of every month, and just sent the final payment. Now I receive a letter stating the final balance, which was paid, was being sent to collection.

    I called Quest on 9.3.2013, s/w Hazel, who acknowledged the payments were all received, but said the payment plan was never in the system, and although the payments were recorded as the plan was not implemented in the system, was "not OK with them." Apparently, our conversations weren't either. This company operated in bad faith has terrible and nasty customer service. Although, Hazel and David both stated this wasn't reported to the credit services, I will have to follow up as I don't trust them.

    I called them during their business hours and I got their "answering service" with a two-minute lecture of everything they do and how busy they are. The message said to "Leave your name and phone number and we will get back to you". I did and I NEVER RECEIVED A RETURN CALL from them. I called the next day, during business hours and I got the same "answering service" and same lecture. AGAIN, l left my name and phone number and, AGAIN, no return call. The next day, I emailed them (they gave their email in their two-minute phone lecture). STILL, NO RETURN EMAIL FROM THEM. On 8/28/2013, I went in to their lab and I told them what happened and I told them that their customer service SUCKED! I did FINALLY get my question answered and the cost of that particular service was OUTRAGEOUS!!! They said that the cost of a CBC with them was $61.00. I told them that American Lab only charges $25.00 and I walked out. They are RIPPING people (and insurance companies) OFF and their customer service SUCKS!!!!!!!

    I have always received bills in the mail from labs after they have been sent to my insurance company. This is fair, as I have a $6000 deductible and have no idea what the negotiated rate will be once the insurance company processes it. Blood tests are very expensive before they have been discounted. And since my deductible is $6000, they are still very expensive even after they have been discounted. When I receive the bill, sometimes they are hundreds of dollars. I pay whatever I can, and sometimes must pay monthly payments of $50 a month until I have paid them off.

    Quest is requiring that we give them our credit card number so that they can automatically receive their money immediately once the insurance company processes it. First of all, there are high interest rates associated with using credit cards. If I receive a charge of hundreds of dollars on my credit card, not only do I not have the money to pay the credit card off immediately, but I will also incur 15% + interest on top of it. I do not just turn over my credit card information to anybody unless I know exactly what the charges are and have signed for it.

    Due to exorbitant insurance premiums, I pay Anthem monthly to maintain a $6000/year deductible. I have to make payments on all my doctor's bills from my checking account, sometimes $50/month. That's just the way it is. I only make so much income. What if the negotiated rate is $500-$600? Are they just going to add that onto my credit card? I cannot afford to pay that at the end of the month and, therefore, will incur interest charges on my credit card.

    They are taking away our rights by this Easy Pay system. Don't be fooled. You will pay more in the long run on your credit card. It is Easy only for them. It will cost you. Don't give away your rights. Refuse to give your credit card information for them to use however they wish.

    On May 14, 2013, I had an appointment to have blood work done. Ms. Regina was the phlebotomist. It wasn't until it was all over that I became upset. I ended up with a nice sized hematoma which I have never had. Being 57 years old, I have had blood drawn many times. In looking back, there were a few things that disturbed me. The first thing was her demeanor. Not to distract from Regina’s setting up for the drawing, I remained quiet. After a time and before drawing blood, I would start with small talk asking, “How long have you been working here?” She would reply, “A while.” It wasn't just the curt answer but the monotone, dismissive voice she would use. It was obvious she wasn't going to be talking to me. I was feeling very uncomfortable to say the least, and we hadn't even started drawing the blood.

    Second, she put the tourniquet on. I read it should be 3-4 inches above the selected site and on only for 2 minutes. She did neither. She had it high up my arm and on for a very long time until the time came and she was unable to draw more for some reason or another. After applying the tourniquet, she took a cotton ball, adding alcohol and swiped 2 or 3 times to the general site. I am reading online exactly what they are supposed to do. If there is supposed to be a circular fashion, beginning at the site and working out, then she did neither. She swiped two or three times at the general area. I say “general area” because she never used her finger to trace the path of the veins, nor did she ask me to make a fist. She just injected the needle. I found this strange in that it was a quick and careless manner done, or it at least felt that way.

    A nurse friend of mine told me that you could see at what angle she took the blood and that she had even come at the wrong angle, coming perpendicular to the vein rather than more parallel to it. I took a photograph of the site a day later to show the damage. What really angers me is going online and finding where people have had serious damage to their bodies from such people - lifelong nerve problems in the arm and hands. The pain actually started when she began probing. She had drawn some blood, but as my friend was telling me, sometimes the phlebotomist is unable to continue because the body wants to naturally coagulate after some time.

    I saw Regina wiggle around and I would winch in pain. Not ever experiencing anything like this, I thought it’s normal. She would say she couldn't get a vein. She had had a vein. My friend told me that when this happens, you switch arms and do not go probing as she did. Yes, she did ask me if I was okay. I told her that I was. I assumed she meant apart from the obvious wincing from pain. The next day, I got a phone call from the office asking me to return. She had done some blood work incorrectly. This time it was done on the other arm since this one was traumatized.

    I had blood work taken at Omni Medical in Newburgh, NY. Weeks after, I received a $500 lab bill from Quest Diagnostics! It was simple blood work. They didn't even run all the tests that my doctor requested (lucky for me in hindsight)! I was dumbfounded, and they have refused to work with me on the pricing. I have no insurance, by the way. I have heard nothing but bad things about Quest Diagnostics and now, I see first-hand why!

    I went into Quest Lab to have blood drawn for a routine thyroid check. I have never had any problems giving blood because I have a large vein in my right arm. The nurse stabbed the needle straight down and hit a nerve in my arm. It was an immediate sharp, burning pain that radiated into my thumb. I now have a burning and tingling sensation in my thumb. It has been days now and the pain is not subsiding. I am writing this because I don't want anyone to experience what has happened to me! If your insurance covers another lab, don't walk... run to the other one. This happened at the Chevy Chase location!

    Avoid these clowns if you can. The local office posts incorrect hours online. The phone help says it will give hours and doesn't. The several offices I've visited are a completely disorganized incompetent shambles and everybody has an angry unsatisfactory experience. My last visit, a whole waiting room crammed with people were screaming at the crap employees. How often are places so bad that you see that?

    My husband was sent to Quest for bloodwork. They made copies of his insurance cards and then asked for a payment for $83 for the blood work. I contacted Blue Shield when he returned with the credit card receipt for $83. Blue Shield rep asked if they had a sign posted on the wall that will collect fees upfront. I didn't go to the appointment with my husband. Furthermore, Blue Shield rep informed me that it is not illegal for Quest to ask for payment upfront. He put me on hold for 20 minutes then came back on the line to advise me that Quest has a new policy that they will be collecting the estimated amount due for services provided at the time of service, because Blue Shield was taking too long to pay them. Furthermore, if we overpaid over for the blood work above the preferred contracted rates Blue Shield allows, then Quest will be mailing a refund for overpayment.

    So now the members have to wait and see if they overpaid Quest once Blue Shield and Quest go back and forth and review claims submitted? What kind of customer service is this for Blue Shield not to even notify their members that they will have to pay Quest upfront for services rendered?

    I went to the lab at the request of my physician, had blood work and a urinalysis done and was charged nearly $440. My sister who works in the health care setting was angry that they would charge a self-pay patient that much when they would not have received that much from any insurance company. Not even Medicare would have paid that much. They had the nerve to charge $45.76 for a urinalysis when they would have only have received at the most $4.48 from Medicare. Another test that they performed was for a Chem test 14. If Medicare had paid, that would have only been $15 and they had the nerve to charge $60. When she asked for a discount for me, she was told that the pricing was set by corporate. She asked for the 800 #. She called and spoke to a supervisor who would not budge and stated they do not do any type of cash pay discount for self-pay patients. It's such a sad state and then they wonder why the people feel the way they do. This is such a rip off!

    Standard blood test invoice: My insurance received incorrect claim information, again. This has happened several times, and I got an invoice from Quest. Then I spent hours on hold, trying to get details on why the claim was not covered. After hours and unfriendly customer service at Quest, it got resolved and covered 100%. But I got no details as to what went wrong.

    We are covered under medicare and Tricare for life. Our doctor ordered labs and we had them done by Quest. All of our insurance information was given to Quest before the labs were drawn. Shortly after labs were drawn, we received a bill for a couple thousand dollars. I contacted Quest customer service and spoke to the supervisor Roger who told me Quest did not submit to insurance companies and this was my responsibility. This took many months for me to get medicare and then Tricare to accept my claims but meanwhile Quest has turned us into the collection agency without any letter notifying us of this action. I found out when I phoned the customer service number and this was on their recording when I put in our invoice number. I continue to wait for the two insurance companies to process the claim. This is a needless waste of time on my part. If Quest would submit the claim, as most businesses would, this would have been settled quicker and more professional. The only thing I get out of this is to never use Quest Diagnostics ever again.

    My husband and I are senior citizens. We have had several blood tests with more than one doctor. We didn't know that each blood test is given a different account name and you have to pay on each one. Some are several hundred dollars and we can't pay in full. I have canceled checks for 2 payments in October and they turned the account over to a collection agency on Nov. 4th. If I charged at Macy's in San Fran, Dallas, Vegas, Atlanta and New York, when I got home, I would have one bill, one total. I'm 78 years old and I cannot keep up with several accounts at each place. This is archaic bookkeeping if I ever heard of it.

    On December 23, 2009, I received a bill from Credit Collection Services in the amount of $241.15 for services from Quest Diagnostics on the date of May 13, 2008. At the time of service, I was covered under GoldenRule Health care. Until the date of service, I have never received a bill in the mail from Quest Diagnostics. The reason why is because Quest had been sending the bill to **, Philadelphia, PA, an address that I hadn't lived at since 2006. Quest claimed that they had been given all of my information from my doctor's office of Michael ** and Kathleen ** of **, Philadelphia PA.

    When I spoke with the doctor's office, they informed me that they had my most recent address which was **, Philadelphia PA, and that this information was given to Quest at the time of service. I have spoken with several people from Credit Collection Services, Quest Diagnostics and Golden Rule regarding this issue. On December 24, Ginny ** from Golden Rule told me that Quest Diagnostics submitted the claim incorrectly by sending it to United Health Care, rather than sending it to Golden Rule. She instructed me to have Quest re-file the claim and submit proof of timely filing.

    I then contacted Quest and explained the situation to Damon **. On March 12, 2010, I called Golden Rule and they said they had not received the claim, so I called Quest and asked that they send the claim and proof of timely filing directly to me. On April 1, 2010, I received a claim from Quest but it was not the original claim as I had requested. It was a updated version of the bill. I called again on May 5 and was told by a customer service rep. that there was nothing Quest could do because it was past timely filing limit.

    My son (Michael **) was sent home from school with flu-like symptoms on August 31, 2009. I took him to the doctor where they ran tests and started him on Tama Flu. Fortunately, his test showed he had the Type A flu and not the dangerous H1N1 flu. I received a bill from Quest Diagnostics for $318.00 which should have been paid by my insurance company.

    When I called my insurance company (BCBS), they said they sent a request to Quest Diagnostics, stating they needed the correct procedure code or a description of the services before the claim could be paid.

    I sent Quest the information. I received the bill again. So this time, I called and spoke with customer service who went on about them not resubmitting claims. I informed her my insurance company will pay this bill, and that they just need the correct code or either a description of the service. Again, she stated they do not resubmit claims. I informed her I could not pay this bill out of pocket. That's why my insurance company is paid $891.00 a month to cover my children's medical expenses.

    In spite of the help I have received (documentation in hand) by the Nevada Governor's Office Consumer Health Assistance Program in getting a promise by Quest Laboratories to stop the harrassing and unlawful billing practices that Quest Diagnostics has forced upon me with threats of collection actions and credit damage along with their repeated failure to apply valid insurance to incurred charges (including Medicare as secondary for which no citizen can submit charges, forcing me to abandon Medicare coverage for which I pay every month); they have now unlawfully submitted me to Collections. I am submitting the Collections Letter back to Consumer Health Assistance for further help; however, it appears clear that Quest is thumbing their corporate nose at the State of Nevada also by continuing the practice which they already agreed to abandon. Because this behavior by Quest continues for quite some time, it is apparent that Question does not plan to abide by law in billing practices. Because they are a large national laboratory and because my primary insurer (CIGNA) has them on the list of provides, I feel compelled to continue using them -- I feel this makes CIGNA complicit in the maltreatment I am being forced to endure at the hands of Quest Diagnostics.

    My wife had contacted Quest by phone asking if they accept Horizon Blue Cross insurance and we were told yes. We went to Quest presented our cards and they took the information. After 3 blood works we have have been billed for the blood work. After contacting Quest, we were told they have not accepted Horizon since June of 2007 and we have only been on the plane since December of 2007.

    I went to the Doctor for a blood test. I have abnormal bile duct enlargement. I received a bill from Quest Diagnostics. I told them I have Blue Cross Blue Sheild PPO. They said that the insurance company would not pay the claim. They could not give me specifics. The date of service was Nove 30, 2005. Blue Cross said the claim was paid. They would not give me specifics. Today, 9/8/2006, I managed to get online at Blue Cross. I was able to find all the claims made by Quest Diagnostics.

    Totaling $4942.60
    I find that Blue Cross paid a total of $615.97 on the claims (all for the one day of service.)

    I now receive a collection letter for $735.30

    my son had lab work done 2-12-02, a claim with my insurance company cigna. cigna paid all claims. they were paid 2-15-02, cigna verified this information. But I am still receiving documents saying I owe 993.05 amd 99.30 for svc paid, balance is 0.

    On Apri,21st 2001 my doctor sent the blood sample for testing for my physical check up to quest diagnostics as quest diagnostics participate in my health insurance plan ( Medical Mutual ) Quest diagnostics submitted the bill to insurance company for $482.65.

    Insurance company dsid not pay the bill and Quest started sending me the letters for collection. I was talking to the insurance company so I sent a check for $43.25, so that I amy not be sent for collection. Later insurance Paid the claim under check # 9007569 dated 8-21-01 as per their calculation, which was toatl $96.13 in place of $482.65. I kept on getting the threatening letters from collection agencies and ultimately I paid 439.40 under my personal check # 1523 dated 2/11/02.

    I did not know the claim sattlement has been done by the insurance company till the time I wrote the check. As soon as I came to know about that, I called and asked for refund of my money $482.65 which I paid thru my check. I never heard from quest diagnostics. Please get my money back. Thanks.
