Turns down claims and pretends they did not receive the explanation of benefits. They do not respond well to out of network providers - which is where many of the good Dentists are. They make their higher premium package sound attractive; however, few dentists are in-network for their extended annual limit. In short, they are manipulative, heartless, bean-counting, scum.

Signed up last December. Got a first bill... What a bait and switch. This is a summary of the Explanation of Benefits:

    SUBMITTED AMOUNT--------------196.00
    APPROVED AMOUNT--------------132.00
    ALLOWED AMOUNT-----------------97.00
    PLAN COVERAGE %-----------------40%
    DELTA DENTAL PAYS---------------38.80

    PATIENT PAYS-----------------------108.20

Delta paid 40% of the ALLOWED amount, but I have to pay 60% of the APPROVED amount. WTF! So in effect, Delta paid 29.23% of what the amount I had to pay from. Is this not a scam? I have to scrap my treatment plan until I can get a real plan.

Various problems occurred getting set up with the Delta Dental USA plan from healthcare.gov; a lot of phone calls and frustration. I did not realize when we chose the plan that we were so limited by the choice of dentists. We chose one of the dentists recommended. The staff was unable to get through to Delta for weeks to obtain our benefits since the info was not in the Delta binder. Finally, the information was obtained and appointments were scheduled for my husband and myself. My husband, for some reason, was not linked to the dentist so I had to call Delta to correct that. Three days before my husband's appointment on June 19, 2014, the dental staff called saying they were cancelling because they could not service the influx of required patients by Delta.

After four months of payments, we do not have a dentist and have not received care. Spoke with Delta Dental representative on 6/20/2014 to cancel the insurance, which he did. I asked for a refund of $183.92 (45.98 x 4 months). He said that he was unable to refund our money. I asked to speak with his supervisor. Senior Rep said that there would be no refund because we were insured for those 4 months but did not use the insurance. I explained that it was out of our control and we were not able to use the insurance due to the many delays by Delta Dental. He suggested that we submit a grievance.

We are unhappy with the choice of dentists in this plan. We would like to receive a refund of $183.92. We are not wealthy and paid this premium in good faith and have not received any services in return. We are sure that this $183.92 will not hurt Delta Dental's bottom line and that it would be much better for the company to refund this amount than to possibly face a class action lawsuit, since we're sure we are not the only people who have had this problem.

Myself and my family of 5 have dental coverage through delta under retired gov't workers plan. We pay $200 every month to these people and now I have started getting the bills. They will not cover anything but preventative care. I was informed that I and my wife did not wait the required time after cleaning to have additional service, i.e. cap on crack broken tooth and a cavity filled in. Was told that must wait 3 months to be treated for a broken tooth (silly me, I thought that would be a priority). When trying to resolve the problem, I was put on a loop of incompetent operators, who kept sending me back to other departments. This is the worst child-like behavior from an insurance company and will lead to our termination. Met-life has a package which includes ortho. I will also inform every person I know retiring about their lack of quality.

I was in bad need of scaling and root planing as my gums were diseased, and my plan stated this procedure is partially covered. Delta refused to pay any of it the first go round, asking for more information as to why I needed the procedure (basically a novel). My dentist and I basically put together a large case with x-rays, etc. to appeal. It took months for them to even review it, meanwhile I had to continue the work being done (which would not be covered until the original procedure was approved). After three and a half months they finally denied the claim, saying that the work was preventative.

I purchased DeltaCare insurance from the healthcare.gov website on May 5, 2014 before the insurance starts on June 1st, and they deducted my account for it. On the effective date June 1st, I have not received the insurance card yet, so I was worried and called Delta customer service line. They said they have not received my information and to call healthcare.gov. I called HC.gov and they opened a case and said it might take them 30 days to fix it! I asked them what happens if it takes more than 30 days will I get a refund for the month that I paid but did not get coverage, they said to talk to Delta insurance about it. I asked if I could cancel my delta insurance but they said I would have to cancel my medical insurance altogether which I needed!

I am furious at how they mishandled my information and how slow their system is! And their representatives are not helping at all, they're scripted! I need a dentist to take a look at my molar that has a tooth decay and I need a long-overdue cleaning, and knowing how long it takes they actually book an appointment with a dentist, it seems that they don't care!

Medical insurance denied the claim because they wanted a more detailed operative report. Both insurance companies kept referring back to the other. We finally received a final denial from our insurance company but now Delta Dental is saying the claim is too late! They have created multiple claims on the same surgery and now they can't even decipher their own notes! I was told the claim has been pushed to the Grievance office who do not answer their phones. They left a message on my phone saying they won't discuss this with me since my son didn't submit an authorization form for them to discuss his procedure with me! All the other multitudes of people I have spoken with over the months and no one ever brought this up.

I am in shock that this company gets to manipulate their customers this way. I will end up filing a grievance with the insurance commission since I don't have 2,000 dollars to pay on something I was assured was covered.

In January of 2013 Delta Dental faxed my Orthodontist an explanation of coverage for treatment. Delta Dental stated I was eligible for $1,500 in orthodontic treatment. I entered a legal contract with my orthodontist under the understanding that I would be legally and financially responsible for any amount exceeding $1,500. After Delta Dental's multiple denials to honor claim submitted by my orthodontist, I decided to contact Delta Dental myself.

Five months into treatment, May of 2013, Delta Dental confirmed via telephone that I was eligible for $1,500 in orthodontic treatment. Within an hour the same Delta Dental employee returns my call and state she made an error and I was not covered for orthodontic treatment under my insurance plan. I asked to speak to a Supervisor who advised me it was ultimately my responsibility and I was financially responsible. Supervisor explained my only option was to file a grievance on Delta Dental's website. This is a huge mistake committed by Delta Dental NOT my orthodontist or me.

Shortly after my grievance was denied and I was forced to involve my husband's employer. After a year Delta Dental reviewed telephone conversations and determined it was a glitch in their system. My husband's employer notified me I would be receiving reimbursement shortly. It is now February 28, 2014 and I have yet to receive ANY official statement from Delta Dental claiming responsibility nor any reimbursement. I am alarmed by Delta Dental's ethics and conduct while handling this matter. Most importantly it's disturbing and irresponsible for Delta Dental to attempt to, and in some cases, defraud their consumers.

To conclude, all insurance companies will attempt to deny payment for services. It's up to the consumer to verify denial is accurate and always request a response in writing. In addition, it appears Delta Dental believes their consumers should spend valuable time away from their families to perform the duties of their employees. A very shady individual once told me, "Always get it in writing!".

During my bi-annual periodic exam and prophylaxis in June 2013, my dentist noted significant damage to the back (distal) of my first molar (DO19) and to the top of my second molar (MO18). He advised me that it needed to be repaired. I made an appointment for July 2013. I had no expectation that my dentist would use anything but composite material for the filling/repair work needed. Having had a number of fillings over the past 20 years, that's all any dentist I've been treated by has used. Upon receiving the EOB for this service, which was functionally a denial with a plan payment of $32.00 on charges of $478.00, I reviewed my Delta Dental PPO Plus Premier policy outline and noted the following:

    1) Predetermination of Benefits - If the total estimated charges for a recommended dental treatment plan are expected to exceed $300, the dental treatment plan should be submitted to Delta Dental for review before treatment begins.

Like any other medical procedure, given the above parameters, I would assume that it would be the doctor or dentist's office responsibility to submit the proposed treatment plan to Delta Dental for pre-approval. At no time prior to the procedure on was I advised of a potential cost exceeding $300.00. Given that there was almost a month between diagnosis and treatment, there was plenty of time for the dentists staff to submit the proposed treatment for pre-approval and to have advised me of the projected expense. The only time became aware of the actual cost of the procedure was as I was checking out of the dentist's office just after the procedure was completed. I also checked my dentist's website and noted the following:

"Composite fillings, also known as white or invisible fillings, are the restorative material of choice in all of our dental offices. A composite restoration is used to repair a tooth that has been affected by decay, fractures or to improve the cosmetics of discolored or stained teeth. Reasons for composite fillings are:

    1. Cavities
    2. Cosmetic Bonding
    3. Repair of Fractures and Chips
    4. Acid Erosion and Wear from Grinding

    5. Repair of Root Cavities and Lesions at the gum line

Composite fillings offer several advantages to our patients. First, the ability of white restorations to bond to the tooth allows our dentists to remove only the decayed portions of the tooth. Silver restorations, which were held into the tooth by mechanical retention, often necessitated larger preparations and more aggressive procedures. Secondly, white fillings are less likely to cause a fracture of the surrounding tooth. Large amalgam restorations are notorious for fracturing the clinical crown of the tooth, often resulting in the need for additional, more involved dental procedures such as root canal therapy, full coverage restorations (caps) and even extraction if the tooth is not restorable."

If the only type of filling your dentist does is composite, are you to forego having your tooth filled, or find a new dentist so that it will be covered? In doing my research for this appeal, I wasn't able to locate a dentist in this area who does anything but composite fillings. I asked that this claim be reviewed in light of the above information. Got no response so had to send the appeal again. Got a poorly worded denial stating among other things: "Many of our dentists have begun using composite fillings." Seriously? "Begun" - no, they've been using them for years - only the medicaid dentists still use amalgam. "Allowing composites would mean raising premiums." Really? Like you don't every year as you cover less and less? "Restorative filings are considered routine and a pre-treatment estimate is not required." Apparently you don't read your own plan as that's not what it says - it's any treatment exceeding $300... What bunch of ** wagons!

I got an unexpected $400 bill after I did what I reasonably could to ensure that I know exactly how much of the procedure would be covered and how much I would need to pay out of the pocket. Thanks to Delta. I needed to do root planing. I have two dental PPO insurances. Delta is 'secondary', meaning they pay after the 1st insurance paid. Okay, fine. I requested the dentist to provide confirmation letters from both insurances to make sure I know what, if anything, I would have to pay out of pocket.

The procedure cost $1,500 in our area. Delta confirmed they cover around $1,200 and pay after first insurance pay. First insurance confirmed they will cover $750 (as dentist is not contracted with them and their rates generally are lower for this procedure). Cool then, right? Not so. 1st insurance paid $750 as promised. Well Delta paid $268, leaving me with $400 bill from the dentist. How come? According to Delta (never mind their own pre-authorization), since 1st insurance rates with their contracted dentists are lower than what Delta would normally pay to even Delta own contracted dentists (not to mention independent dentists), then Delta is not going to pay by their rates but by the same rates the 1st insurance would pay if the dentist would be in the contractors network.

If you did not get it from the first time, I would not blame you. It’s not designed to be easily understood. Basically, Delta says that it pays $1,200 for this type of procedure. But since I have another insurance, and the other insurance pays for this procedure $750, then Delta would also consider the cost of procedure to be $750. Obviously, that would not be a Delta point of view if 1st insurance rates were higher than those of Delta. On top of this, Delta does not pay by these rates for the remainder of the bill. It only pays for what I would be obligated to pay if dentist was on a contract with the 1st insurance and were obligated to charge what 1st insurance would restrict him to charge and what would remain after 1st insurance paid - which is $260, not $750.

Good job Delta! From $1,200 to $260 in a single sweep. Because I’m paying for two separate insurances (mind you, regular price), Delta figures it should not pay more than 20% of what it would pay if I had only one insurance (and never mind the pre-authorization letter - means nothing. Who cares about them?). Makes sense to anybody? Besides Delta that is. For Delta, it makes perfect sense - $260 is certainly less than $1,200 and less is better (when paying). It looks to me only a sophisticated criminal mind can concoct something like that. Not only Delta would pay anyway less than full charge for the procedure, it does not even want to pick-up the remainder of charges. Can’t tell if I should admire their shrewdness or shame them for fraudulent practices. Unfortunately, for now I’m just stuck with $400 bill.

I contacted my wife and she confirmed this for me with Shavon at Delta Dental. So basically, I am paying $24.08 per month, plus a $50.00 deductible (24.08 x 12 + 50 = 338.96) to receive $161.04 worth of benefit from Delta Dental. That's insane and I even wonder how it is legal to charge more for insurance than you are willing to pay out in benefit to the consumer. It leads me to wonder how they sleep at night, but then again most con artists sleep just fine knowing they are benefiting at the expense of others.

In October 2011, we applied for a AARP Delta Dental for a dental policy. About six months later, we discovered that when attempting to make an appointment for some dental work, we discovered we had never received a dental policy from Delta. Checking our bank records, we found that Delta Dental had withdrawn monthly premiums from our account at least five times. They also admitted that they would have continued withdrawals had we not canceled the withdrawal option. We called Delta Dental regarding our not having received our policy or an ID card. They said they mailed it by USPS first class mail and were not responsible for its delivery failure. We then were complied to call our bank to cancel the Delta account. This action cost us an additional $30.00 expense.

We have asked for a refund and have been refused several times the return of nearly $500.00 collected by Delta. We had sent an application, money and allowed Chase to allow Delta withdrawals from our bank account in good faith. We expect Delta Dental to demonstrate the same good faith and return the unearned deposit and withdrawals. It's also interesting that when we reported not receiving their policy, they did not even offer replacing the lost one. They then pressured us to cancel the non-existent policy. As suggested in my complaint, the money should be sent to the Red Cross for victims of Sandy. As noted in my complaint, time is of essence.

However, after the procedure was done, nothing was paid. I spoke to two different representatives at Delta Dental and called the Lockheed Martin employee service center. All insisted that it was not paid due to a non-duplication clause for my group. The non-duplication clause does not apply in this case because MetLife (my wife's dental plan through her employer) paid $0.00 for this procedure so there would be no duplication if this claim were paid. The pre-treatment estimate clearly shows how much is supposed to be paid and in the Notes Section, it acknowledges that the benefit is secondary to another plan and benefits have been adjusted based on the non-duplication of benefits. The last Delta Dental representative I talked to confirmed that the insurance company is uncoordinated in accounting for primary insurance in the pre-treatment estimate.

I took out a policy starting March 1, 2010 and paid $37.00 per month until August 31, 2011. I never needed to see a dentist until May 2011. My dental office filed a claim for general cleaning. Delta Dental denied the claim. When I called, I was told it was because they were not my primary insurance. Then in July 2011, I was told by my dentist that I would need two major root canals. And the office called Delta Dental to find out how much they would pay, so I could figure out what my part would be.

The next week, I received an e-mail stating they were canceling me, because the plan was for someone that did not have any dental insurance. I told them at that time that they knew I had other insurance, but the first insurance would not pay for major work. That was the sole reason I got another policy and paid out of pocket. I talked with Jennifer **. After our conversation was over, she said she saw no reason to cancel the policy and re-instated it. Then, when I made the appointment to have the work done on my two teeth, the dental office again said Delta Dental said they would not pay for anything. So, I called my bank and stopped payments for monthly premiums. I tried to talk to Jennifer stating that I was paying for a plan that would not pay out anything. And the minute they knew they had to pay a claim, they wanted to drop me.

I called on Tuesday, September 13, 2011, and spoke to Jennifer. I told her since they misrepresented my plan, I asked to cancel the plan and request a full reimbursement of what I paid into the plan. It was a total of 17 payments at $37.00 a month, a total of $629.00. Jennifer sent an e-mail on 9/13/2011 stating my policy was canceled and I would not receive any reimbursement. I then sent an e-mail back to her stating that this was wrong, and they took my money and I wanted it back. Otherwise, I would seek legal council and file a suit. I would like to resolve this matter but will stand by and pay for a plan that never intended to pay out any money back to me.

Please help me resolve this issue so both sides benefit. I don't want to file suit and don’t want to go to the media, but will if this is not resolved. It is the best interest of both of us that they pay me back what I paid in premiums. And they should have never written the policy for me, knowing I was employed with the US Dept .of Labor and had insurance. Many people have additional insurance polices to enhance what they already have. Consumer, please be alert about this insurance company. I do want an attorney to contact me or I will seek one myself. I will sue in the court of law and contact media to get the word out.

For over a month, I have been attempting to pay my bill. I have as of now spent over 3 hours on the phone with Delta Dental's service representative, never the same representative twice. We have three different residences; Hawaii, Washington State and New Westminster, BC. I used my wife's Visa card which lists both the New Westminster, BC and Seattle address for credit card usage. Several times I was told by Delta Dental that the bank did not recognize the address. Each time, my wife phoned the bank and was told that the bank had authorized payment but that Delta Dental had cancelled the transaction at their end.

Frustrated, I phoned Delta Dental to pay the bill and received the same runaround about the address. My wife once again phoned the bank and was assured everything was OK at their end, but that it appeared that Delta Dental did not have software to check for both a primary and secondary address. We phoned Delta Dental again to inform them that their software failed to search for a secondary address. We were informed that we could send a check or money order, but they could do no more than that. When asked to speak with a supervisor, we were unable to get someone who could solve the problem. This has not been an issue with several other agencies or companies. There are probably many senior AARP insurers who have multiple residences. This company is failing its customers and its payment practices are antiquated.
