    No punctuality. No communication. I've been reaching out for weeks and instead a phone call or response to an email I get a delay in benefits letter when everything needed had been turned in. When I first signed up took nearly two months to receive benefits.

    Benefits were stopped despite medical documentation. Decisions relied on non-medical reasoning and selective record review. Poor communication and lack of accountability caused unnecessary hardship. Very disappointing claims handling.

    I worked for the same company for eight years. Then my 13 year old daughter, who is also autistic and nonverbal, was involved in a tragic accident and air-lifted to a children's hospital. I applied for an FMLA and it was handled through Matrix, and approved for 12 weeks ONLY. My daughter's hospital stay was for 35 days and during that time she needed multiple surgeries, blood transfusions and intensive treatment. At the end of my 12 week absence period, she was out of the hospital, but she was still using a wheelchair and wearing a neck extrication collar. The doctors wouldn't clear her to return to school until the neck collar was removed. However, Matrix denied my request for an extension and gave me an ultimatum that I had to return to work within 48 hours or I would be terminated for "No Call, No Show".

    I had been in communication everyday with my store manager, so technically I was not a NCNS. But Matrix, made me choose between a company or my child. Of course, I will choose my family each and every time. That's what my employment came down after being there for eight years. I included a picture of my daughter that was taken during her first 24 hours after being admitted from the ER.

    I am submitting this complaint to document significant issues with how my leave case was handled by Marisa. Throughout the process, I submitted all required provider-completed forms and supporting medical documentation promptly and in full, including materials provided prior to the Christmas break. Despite this, I experienced repeated delays, lack of confirmation of receipt, and minimal substantive communication.

    More concerning, I was informed that documentation had not been received or was delayed, which does not align with the timeline of my submissions. I maintained consistent responsiveness and followed all instructions provided, yet progress on my case stalled without clear explanation. Managing a medical condition while navigating this process was unnecessarily stressful and caused further health issues due to the lack of follow-through, unclear guidance, and mischaracterization of my compliance. Ultimately, I was forced to request escalation and reassignment in order to move my case forward. I am sharing this feedback so the organization can review internal processes, communication standards, and case management practices to prevent similar experiences for others who rely on timely, accurate support during medical leave.

    I have been disabled for 22 years with very rare conditions that have no treatment options. I’m more disabled now than I’ve ever been with severe exhaustion and chronic pain. My Case Manager from MATRIX called me a week before Christmas to tell me that my case has been canceled because their doctors feel I’m no longer disabled with no other explanation. They said they were sending me paperwork a week ago for the appeal process. I’ve called my Case Manager several days in a row now for him to contact me to email me a copy in the meantime and he has not responded. This is after a letter from Social Security a few months ago said they were going to do a medical review, but decided they are NOT going to bother because I’m so disabled it’s not worth wasting their time!

    What is the point fighting for months getting all the paperwork together just for a company to say an ADA is just a suggestion and deny it. This company is full of heartless people who don't care about the individuals coming to them.

    Everything any of these reviews say reminds me of the exact same thing my son is talking about. The healthcare provider had done what they thought was correct and then more hoops to jump through and no way to get in touch with them or get a response from multiple requests for information. The people on leave need to heal and the frustration and worry does not help at all.

    I went on for about 2 months due to my mental health. I was unable to do my job and kept making mistakes and was in a very dark place. Matrix kept losing paperwork and made the process as complicated as possible. They denied my STD claim, giving reasons that were the exact opposite of what my providers put down on their forms. With my appeal, they've been trying to request full session notes from my therapist. A diagnosis and plan of action should be all they need, they do NOT need every private piece of info my therapist writes down.

    The fact that they think they know better for me than my providers is disgusting, and them saying there was no suicidal ideation when my therapist wrote down that there was is a HUGE problem. If I were to have acted on any thoughts while they made my mental health worse and caused me more stress while trying to recooperate, they would have been left with a massive lawsuit and since they clearly only care about keeping money, you'd think they'd be better at trying to avoid lawsuits. Businesses, if you care at all about your employees, please do not use this company, as they make every single thing harder and use fax machines to conveniently lose paperwork all the time.

    If I could give them a -100 score I would. I submitted the form 6 times and no one could locate my claim or even find my information in their system. They transferred me to a ton of different people, for days and no one helped. Emailed several people and still that didn’t really help as well. Submitted claim form with all supporting documents 21 days ago! It was assigned to some to review it last Friday. And right now seems like they are trying to find reasons now to pay out my claim. Very disgusted with this company.

    Dealing with Brandy ** is ridiculous. She never answers the phone or emails and is constantly out of the office. All I want is them to release me to work and I can’t even get her to follow up on her end. Negative stats are in order here.

    Absolutely awful customer service. They make you do all the leg work. I don't understand the point of their services. They insist on the use of fax machines and your claim will be stuck pending. Suddenly they email you with another hurdle that you must quickly jump through. I feel like they lack compassion and empathy for people going through a difficult time. Faxing of information???? It's not 1987 folks!

    HORRIBLE!!! I've been on leave for 4 weeks & still no pay!! No one will respond to my emails or phone calls! They do not care! I've been begging them to contact me since they have had my disability paperwork since DAY 1 & I cannot pay my bills!! This leave was supposed to HELP me but omg it’s making everything WORSE!!! I have no money to pay my bills & credit is going to get hit but do they care? Absolutely not.

    Negative stars. I was released to work 10 days ago, but work needs clearance from Matrix. I have called and emailed dozens of times a day, but cannot get a response. Even messages to managers and supervisors go unanswered. I am unable to return to work until Matrix tells my work I can, so I am not getting paid and have no ways to pay bills.

    I'd give this company a negative star review if possible. Since day one of needing their assistance there have been multiple issues. Originally when my company started using them they told me I had to use Reliance for my ADA Accommodations. I call them they tell no its an HR issue. So days of going back and forth to finally find out reliance is supposed to take care of this.

    Fast forward to requesting reduced hours due to a high risk pregnancy. The individual who over sees my case leaves for vacation for a week and no one does anything. I explain to her (Megan) prior to her vacation that this is time sensitive due to the risk of preterm labor. My requests are ignored. Finally my midwife and I decided it was best for me to go on leave to alleviate the additional stress. Provide all requested documentation just for Megan to tell me she didn't receive it. It was in the same email chain. Then, they make my provider and I jump through tons of hoops to try and get my short term disability approved. She never actually requested my midwife send any documents then I get a letter saying they're going to deny my claim as it had been over 30 days but I'm reality it wasn't even close to 30 days. I've been on leave for almost a month, zero payments received.

    She emailed me saying she received the notes from my midwife and that upon review it does show i need to be on leave but then asks for my psychiatrists information (that she already has) because she needs more info. Why does she need additional information if what she's received already shows I'm eligible. She then emails back telling me my medical info that received from my Midwife telling me that info doesn't support my claim. WHICH IS IT MEGAN?! I've had the displeasure of working with these people since May. It's been an absolute nightmare. Stay far far away!

    They are awful. They literally exist to find ways to deny your claim. They intentionally make things obtuse and challenging so you or your physician will make mistakes that leads to them being able to deny your claim. It is cruel. They intentionally lose paperwork, fax to the incorrect physician, or flat out lie about your claim. Hire an attorney immediately if you find out they are the administrator for your disability. Day #1 call an attorney.

    These people will minimally help you through the disability process and will not inform you if you are eligible in receiving supplemental health benefits that you are entitled to. You have to be proactive, research the site from top to bottom and no people or have good HR at work in order to utilize eligible benefits from a place that is supposed to be a 'life preserver' for employees in times of strife. Both Employers as well as their employees pay premiums for the subpar service that is received in return. I can only say it's been a huge epic fail for this company. I do not know how they have managed to stay in business as long as they have.

    If this company could receive zero stars from me, I would gladly do so. This company is hands down one of the most terrible companies I have ever had to interact with when it comes to disability accommodations and taking time off from work. Their incompetence knows no bounds. It is crystal clear to me that they have absolutely no regard for your health and well-being. I am currently facing a difficult health situation, and instead of providing support, they only succeed in making my life more difficult due to their lack of professionalism and effectiveness. It is obvious that their priorities lie with money, not with the people they are supposed to serve.

    The company is horrible! I wouldn't refer anyone to the company. When filing a claim, the process was easy. But when it's time for payments to be issue is horrible. Payments are late being issue to you. They inform you your payment was issue on that Friday but when looking in the portal it doesn't show release payment. So not accurate information given, so to look at it you don't get paid on that Friday you want receive money until the next following week. But if they were smart processing, the payment or payroll have to be done by Monday or Tuesday to have your payment on that Friday.

    It really puts people in a bind because they are not working and relining on this source of income. But they didn't have any problem receiving they payments out of your check every pay period. Then I'm missing a payment from the pay period 8/15/25. It's no way you can track payment and when you call customer services, they don't know nothing, no tracking number and it take forever for the claim adjuster to contact you back.

    This is just horrible. I have bills to pay for and can't be waiting on my money. But back to the lost check I have to wait until Tuesday to see if I finally receive it. If not they're going to cancel it and reissue my payment. But finally with direct deposit. First of all, I should never receive any checks because when I started the process of doing my claim for leave of absence I filled out my direct deposit form. When I did question the claim adjuster, she explained I never filled it out. I told her yes, I did. So, she went to look at all my documents that was uploaded and realize I did fill out the inform. Now I feel that when you're a claim adjust it's no way you should make mistakes. This can cause people to go in debt. Overall, I'm just over it, not able to pay my bills and will look into further legal action.

    Gone 6 weeks without getting my claim approved despite sending them all the paperwork that was requested. All they do is delay and deny at every step, they deny getting faxes from the doctors that I know were sent and have some of the most unhelpful employees anywhere!

    Its seems as though there is a common pattern here of incompetent claims examiners, & their rude over talking supervisors. They purposefully lie to delay claims, purposefully don't process claims in a timely fashion when having all documents necessary to do so. I also see I'm not the only one who had a claims examiner go on vacation without processing your claim or making sure it's processed by someone else while they're away. Seems as though they have no empathy for anyone who is ill & are blatantly rude. Brittany ** & her supervisor Rusty, this one is for you specifically!

    I have been dealing with these clowns for weeks. Getting no where. I have received very little pay and no help. My girlfriend and I are being evicted now I don't have enough gas to get to work. I haven't eaten in 2 days. They don't care. They mysteriously misplaced my checks??? All they keep saying is they are escalating it, escalating it, escalating it. With nothing. I am beyond livid with this horrible terrible company.

    I started my claim on July 19th in anticipation for my surgery on July 31st. Since then, it’s been an horrible experience! They kept wanting to put my claim through as FMLA, which I stated several times, I wanted Short Term Disability. They kept saying they were waiting on my Drs Certification and then they would be able to approve. The first person I was assigned to the next day, they went on vacation for a month with no out of office with a covering persons name, then when going through their ridiculous phone system that puts you on hold to just make you call you back anyways, I was sent right to the person who was on vacations voicemail. Had to go through that game how many times because it goes by your social security number. After weeks of this. My surgery was about to happen and they tell me the Drs paperwork needs to be signed after my surgery to be approved. So this happens, I get told I should be paid out on such and such date.

    That date passes and now I reach out again and now being told that I’m missing some questionnaire I was never given or maybe I missed it, not sure but wish I was told weeks ago. So I’m back to leaving voicemails and emails with no responses. This place has an obvious intention and that’s not to pay anything out and to make the process impossible to do so. I’m a week out of surgery. I should not have to be dealing with this!!

    Yeah, it would have been way less stress free to have just lived in chronic pain instead of getting the surgery I've needed for years. Somehow working with UHC was less painful. I can never reach my claims person, and she does not return mine or my doctor's calls. She called me once later in the day on a Friday after the doctor's office was closed, and I told her they had sent her paperwork already. She said it was incomplete, even though what was sent to her was what is legally acceptable in the state of Wisconsin.

    The phone system is terrible! The number they give you for your claims adjuster isn't even their number, just a basic one where you have to waste your time going through automated bs. I'm afraid that I won't get approved for my fmla or short term disability at this point, and my employer is NOT lenient with missing days. I would agree with other reviewers, stay far, far away from any company that uses Matrix. I'm thinking at this point that I'm probably screwed seeing how things turned out for others. I need my paycheck to survive, and these people seriously don't give any amount of **.

    Where do I even start. I’m on paid parent bonding leave given to me from my job thru Reliance Matrix. My daughter was born July 4th and I submitted proof of birth just hours after she was born. It’s now July 28th and my claim is still not approved and nobody answers the phone or calls back. I’ve left 5+ voicemails and never get a response back. This is not a good experience to say the least and this is the 1st 1 star I’ve ever left on a company. This is causing financial burden on me and my family and affecting my bills. I am very irritated at how Reliance Matrix handles everything. Don’t ever bother trying to go on leave or get disability because good luck waiting however long until they feel like approving or responding to anything. I’d give a 0 star if I could.

    One of the worst companies out here. I had 2 claim within 1 year. Had to burn through 14 vacation days and then missing over 180 hours of pay. I just believe what they take you through in still missing over 4000 worth of paychecks and was giving wrong information. Might lose my job that I have been on almost 10 years.

    I would give no stars if possible. Horrific company. Examiner denied all claims after filling out exactly how she said to fill it out! My company has stopped my pay because of the denials. Do not use this company, they do not care, nor will they help you! I have reached out to Chenelle ** who is supposed to be my claims person, a joke. Her boss, Emma ** who did nothing but talk over me and finally waiting on her boss to call Julie **, it's been 2 days and nothing. I have reached out to the CEO Michael Askins to see if he knows what type of employees he has and also tried the contact center to see if someone will return my call. I am not getting paid and my job is on the line because of this!

    Worst company to deal with! They send you to an independent medical exam with a chiropractor to assess if my husband can go back to work after a stroke took out 2/3 of the left side of his brain even after telling them twice it needed to be neuro to test memory, understanding. He also has moderate aphasia. Exam lasted 13 minutes, dr says speech is fine and he is good to go (works on pacemakers when they fail. Works with the FDA.). Now they denied him. Their response to not setting up the appropriate Dr??? We’re not perfect…. I shouldn’t have to coordinate testing on my health insurance's dime because they are incompetent.

    This company is the worst I have ever dealt with. I would recommend staying away from any employer that uses this company. Matrix will lie and deceive to find way of denying your claim. My doctor sent requested multiple times even had the fax confirmation but Matrix still denied response from Doctor.
