Excalibur Crossbow US of Canada is recalling nearly 19,000 C2 Crank cocking aids for crossbows in the U.S., and Canada.

The cocking hooks can break during use, causing flying plastic hook projectiles, posing impact injury and laceration hazards.

The firm has received 538 reports of the C2 Crank cocking aid hooks breaking during use, resulting in plastic projectiles flying and hitting consumers, including one injury where a consumer reportedly suffered a permanent eye injury.

This recall involves Excalibur Crossbow’s C2 Crank sold as a cocking aid for crossbows.

The C2 Crank, sold in black, has two plastic cocking hooks attached to the string of the C2 Crank.

The “Excalibur Crossbow” logo is on the front of the unit, with part number 2199 and the UPC number 626192021991 located on the packaging.

The C2 Crank, manufactured in Canada, was sold at Bass Pro Shops, Cabela’s, and other stores nationwide and online at excaliburcrossbow.com, ebay.com and amazon.com from January 2008, through December 2021, for about $190.

What to do

Consumers should immediately stop using the recalled C2 Crank and contact Excalibur Crossbow for instructions on receiving free replacement rope and hooks.

Primal Kitchen is recalling approximately 2,060 cases of Primal Kitchen Avocado Oil.

The glass bottle may be prone to breakage, causing the product to spill.

No consumer injuries or illnesses have been reported to date.

The recalled 750 mL bottles of Primal Kitchen Avocado Oil with an individual package UPC of 8-5523200719-4, case UPC of 108552320071910000 and a “Best When Used By” date of 19 August through 21 August 2025, were sold in the eastern U.S., California and online nationally.

What to do

Consumers who purchased the recalled items should dispose of them and contact Primal Kitchen at (888) 774-6259 from 10 a.m. to 6:30 p.m. (EST) Monday through Friday.

Johnson Health Tech North America of Cottage Grove, Wis., is expanding its earlier recall of Matrix T1 and T3 series commercial treadmills.

The firm is adding about about 9,700 treadmills to the 19,900 that were previously recalled.

The power cord can become loose from the treadmill’s power socket, posing a fire hazard.

Matrix Fitness has received one report of a fire in a fitness facility since the last recall, in addition to the 58 reported in the earlier recall, of the treadmill’s power cord and socket sparking, smoking, and/or melting, including seven reports of fire; two resulting in property damage.

No injuries have been reported.

This recall involves the Matrix Fitness T1xe, T3x, and T3xm treadmills.

The treadmill has a running platform that measures 60 inches long by 20 inches wide, a 33-inch-wide console, and a straight power cord with no power cord bracket.

Matrix is printed on the treadmill console masts. The model name and the serial numbers, which may be found here, are located on the connecting bar at the bottom of the machine near the power socket.

The serial number is composed of the letters TM, a three-digit model number, the manufacture date in YY/MM format followed by a five-digit serial.

The treadmills, manufactured in China and Taiwan, were sold at Johnson Health Tech North America and Johnson Fitness & Wellness stores nationwide from March 2018, through January 2024, for between $3,400 and $8,900.

What to do

Exercise facilities should inspect their treadmills to confirm whether they have a power cord bracket installed on the unit. If the bracket is not already installed, facilities should ensure that the power cord is completely inserted into the power socket on the treadmills prior to each use, until repaired.

Tesla is recalling 3,878 model year 2024 Cybertrucks.

The accelerator pedal pad may dislodge and become trapped by the interior trim.

A trapped accelerator pedal can cause the vehicle to accelerate unintentionally, increasing the risk of a crash.

What to do

Tesla service will replace or repair the accelerator pedal assembly free of charge.

Letters notifying owners of this recall are expected to be mailed in June 2024.

Oregon Spice Company of Portland, Ore., is recalling 5-oz containers of Johnny’s Parmesan Garlic Seasoning.

The product may contain sesame, an allergen not declared on the label.

No illnesses have been reported to date.

The recalled product, which comes in a plastic spice bottle with a green cap with BB 04MAR26 0644 19:48 to 21:41 on the bottom of the bottle, was sold at Fred Meyer, QFC, Albertson’s and Safeway stores in Washington, Oregon, Idaho, Montana, Nevada, North Dakota, Utah, Wyoming, and Alaska, as well as Amazon.com and online orders placed through http://www.johnnysfinefoods.com.

What to do

Customers with a sesame allergy or sensitivity who purchased the recalled product should not consume it, but dispose of it or return it to the place of purchase for a full refund.

Intimidator of Batesville, Ark., is recalling about 19,264 Intimidator and Mahindra branded utility vehicles (UTVs).

The universal joint can fail and pierce and sever the brake line, posing a crash hazard.

The firm has received 40 reports of incidents, including 12 involving a brake line impact. No injuries have been reported.

This recall involves Intimidator and Mahindra branded models 750, 800, 1000, TGB, GC1K and electric UTVs sold in a variety of colors.

“Intimidator” or “Mahindra” is displayed on the front grill and the model series is printed on the rear-side panels. The serial number can be found on a label inside the glove box, inside the hood compartment and below the driver’s seat.

The serial range of the recalled Intimidator UTVs is 000000 to 014034. The serial range of the recalled Mahindra UTVs is 000000 to 010545.

The UTVs, manufactured in the U.S., were sold at Intimidator and Mahindra authorized dealers nationwide from August 2013, through November 2023, for between $11,000 and $21,500.

Kia America is recalling 48,232 model year 2022-2024 EV6s.

The Integrated Charging Control Unit (ICCU) may become damaged and stop charging the 12-Volt battery, which can result in a loss of drive power.

A loss of drive power increases the risk of a crash.

What to do

Dealers will inspect and replace the ICCU and its fuse as necessary. In addition, dealers will update the ICCU software. All repairs will be performed free of charge.

Owners should receive letters notifying them of this recall around April 29, 2024.

Lovey & Grink of Los Angeles, Calif., is recalling about 23,700 sets of children’s two-piece pajamas and lounge dresses.

The garments violate the flammability regulations for children’s sleepwear, posing a risk of burn injuries.

No incidents or injuries are reported.

This recall involves four types of children’s sleepwear garments: girl’s two-piece pajama sets, girl’s nightgowns, boy’s two-piece pajama sets, and unisex two-piece pajama sets.

They were sold in the following prints: Sushi, S’mores, Heart Pops, Rainbow Sky, Popcorn, Puppy Love, Race Car, Sports, Beach Day, Summer Treats, Pasta, Cherry Berry, Love Potion, With the Band, Recycle, Halloween, Birthday Treats, Build-a-Block, Blue Gummy Bears, Gingerbread, Helicopters, Hanukkah Penguins, Rainbow Hearts and Vacation Vibes.

The pajama sets were sold in sizes 12 Months to 9/10, with the size, “Made In Indonesia”, “RN64664”, “48% Cotton/47% Modal/5% Spandex”, and washing instructions printed on the neck label.

No side seam label is found on the product.

Consumers who purchased the product will be asked to destroy the garments by cutting them in half and disposing of them in accordance with local and state recycling laws.

Kia America is recalling 427,407 model year 2020-2024 Telluride SUVs.

The intermediate shaft and right front driveshaft may not be engaged fully, which can result in vehicle roll away while in PARK.

A vehicle roll-away increases the risk of a crash or injury.

What to do

Owners are advised to engage the emergency parking brake prior to exiting the vehicle until the recall remedy is performed.

Dealers will update the electronic parking brake software and replace any damaged intermediate shafts -- as necessary -- free of charge.

Walmart Stores of Bentonville, Ark., is recalling about 51,750 Mainstays Electric Mini Choppers.

The chopper’s blade can operate unexpectedly during assembly or when not enclosed in the container, posing a laceration hazard.

The company has received five reports of lacerations, two of which required medical attention, including stitches. Injuries occurred during assembly of the product or during cleaning/handling of the product.

This recall involves Mainstays Electric Mini Choppers, model number MS14100094536S1.

The choppers are rechargeable and cordless, and have a green lid, white body, and clear plastic bowl. They have one blade attachment, which consists of three curved blades on one white plastic spindle.

Model number MS14100094536S1 is on the label attached to the bottom of the clear plastic bowl.

The choppers, manufactured in China, were sold exclusively at Walmart stores nationwide and online at Walmart.com from August 2022, through October 2023, for between $10 and $15.

What to do

Consumers should immediately stop using the recalled choppers, and contact Walmart to receive a full refund. Consumers may bring the chopper to their nearest Walmart store for a refund.

Essential Medical Supply of Orlando, Fla., is reannouncing the recall of about 272,00 Endurance Hand Bed Rails.

Users can become trapped within the bed rail or between the rail and the side of the mattress, posing a risk of death by asphyxiation.

The previous recall was issued in December 2021, following the entrapment death of an 86-year-old man in California, reportedly from the Endurance Hand Bed Rail (model P1410).

Since then, two additional entrapment deaths have been reported. In December 2022, an 86-year-old man died at an assisted living facility in Connecticut, and in August 2023, a 99-year-old man died at his home in California.

This recall involves the Hand Bed Rail (model P1410), Hand Bed Rail with Pouch (model P1410-P), Hand Bed Rail with Floor Support (model P1411), and Hand Bed Rail with Floor Support with Pouch (model P1411-P).

The name “Essential Medical Supply, Inc.” and the model number are printed on a label located on the grip handle of the bed rails.

The rails are made of either chrome or white metal tubing and include black padding on the grip handle and under-bed frame. They measure approximately 17 inches to 21 inches high by 20 inches wide.

Only bed rails that do not have model numbers ending in “R” are included in this recall.

The bed rails were sold at at medical supply stores nationwide and online at www.amazon.com and www.walmart.com from October 2006, through December 2021, for between $36 and $98.

What to do

Consumers should immediately stop using the recalled bed rails and contact Essential Medical Supply to receive a full refund.

The firm is only providing refunds on bed rail models sold or imported on or after November 1, 2015.

The amount of the refund will be pro-rated based on the age of the bed rail.
