Recalls in November 2023

Worthington Industries of Columbus, Ohio, is recalling about 121,100 Balloon Time mini helium tanks.

Compressed helium from the tank can escape and cause the plastic cap to be released into the air unexpectedly, posing an injury hazard due to projectiles striking users and bystanders.

No incidents or injuries are reported.

This recall involves Balloon Time mini helium tanks that are used to fill up to 20 balloons with helium. They measure about 13.5 inches tall by 3.5 inches in diameter. The helium tanks have a red label and purple cap and hold 5.8 cubic feet of helium/air mixture.

The mini helium tanks, manufactured in the U.S., were sold at Meijer, Cool Kat Party Warehouse, Bradley Party and Variety, Ultimate Party Super Store, RSVP Food and Party Outlet, Party Center and select Party City stores nationwide, and online at Amazon.com from April through October 2023 for between $30 and $48.

What to do

Consumers should immediately stop using the recalled mini helium tanks, point the tank away from themselves and others, and carefully remove the plastic safety seal and cap.

Release all helium in a well-ventilated area and then clearly write their name, "RECALLED" and destruction code on the front of the tank with a non-erasable marker.

Consumers should register online at www.recallrtr.com/miniheliumtank and upload a photo of their product for a full refund.

After receiving a refund, consumers should discard the product.

Subaru of America is recalling 3,732 model year 2024 Crosstrek & Imprezas, model year 2023-2024 Foresters, and model year 2023 WRXs.

The front driveshaft assemblies' outer race may develop cracks and break.

A broken driveshaft can cause a loss of drive power while the vehicle is being driven or vehicle rollaway when it's in park without the parking brake engaged.

Either of these can increase the risk of a crash.

ZLINE Kitchen and Bath, of Reno, Nev., is expanding the remedies available to owners of about 30,000 ZLINE 30-inch, 36-inch and 48-inch RG gas ranges that were recalled earlier in 2023.

Remedies now include a replacement range or a refund. Consumers initially were offered a repair.

The ZLINE 30-inch, 36-inch and 48-inch RG gas ranges were recalled because the oven can emit dangerous levels of carbon monoxide (CO) while in use, posing a serious risk of injury or death from carbon monoxide poisoning.

Before the recall, ZLINE received 44 reports of carbon monoxide emission, including three reports of consumers seeking medical attention.

After the recall, ZLINE received 131 reports that the repair was not completed successfully and that the ranges were still emitting dangerous levels of carbon monoxide.

This recall involves the oven compartment of ZLINE gas ranges with model numbers RG30, RGS-30, RGB-30, RG36, RGS-36, RGB-36, RG48, RGS-48 and RGB-48.

Only ZLINE gas ranges with these model numbers are included in this recall.

The ranges were sold in various door colors including black matte, blue gloss, blue matte, DuraSnow, red gloss, red matte and white matte and multiple finishes including stainless steel, black stainless steel and DuraSnow, a cloudy steel finish.

The recalled ranges were sold in three sizes, 30 inches, 36 inches and 48 inches.

The ZLINE logo is printed on the bottom of the door. The model number is printed on a label on the back of the range. For ranges purchased after 2020, the model number is also printed on a label underneath the right side of the range top, visible when the oven door is open.

The ranges, manufactured in China, were sold at Best Buy, Lowe’s, The Home Depot and The Range Hood Store stores nationwide and online at www.bestbuy.com, www.build.com, www.costco.com, www.homedepot.com, www.therangehoodstore.com, www.overstock.com and www.wayfair.com.

The gas ranges were sold from February 2019, through December 2022, for between $2,300 and $6,900.

What to do

All consumers who have a recalled range should immediately stop using the oven compartment, even if they have already had their range repaired as part of the prior recall.

Consumers who have received a repair are being contacted directly by ZLINE with additional information and about the offer of an inspection, a replacement range or a refund.

Consumers who have not yet scheduled a repair as part of the recall should contact ZLINE for information regarding available remedies.

Consumers may continue to use the range tops which are unaffected by the issue.

KinderFarms is recalling all lots of KinderMed Infants’ Pain & Fever (oral suspension) and KinderMed Kids’ Pain & Fever (oral suspension).

This recall  recall is due to acetaminophen instability. Testing of sample batches indicated some product lots were no longer in specification and may cause abdominal pain, nausea, vomiting or jaundice at higher doses.

There are no reports of serious adverse events to date.

All lots of the following products, sold nationwide in drug/pharmacy, supermarkets, direct delivery and national retailers, and online through major ecommerce sites, are being recalled:

Subaru of America is recalling 95,830 model year 2021 Crosstreks, model year 2022 Foresters, and model year 2021-2023 Legacys, and Outbacks.

An insufficient weld may allow water to enter the inhibitor switch, causing it to fail.

An inoperative inhibitor switch may prevent the reverse lights from illuminating and the rearview camera image from displaying, increasing the risk of a crash.

Twin-Star International of Boca Raton, Fla., is recalling about 14,670 Style Selections 42-inch-wide infrared quartz electric fireplaces.

The electric fireplaces can overheat, posing a fire hazard.

The firm and Lowe’s have received 24 reports of overheating, fire or smoke incidents. These include two reports of property damage.

No injuries have been reported.

This recall involves Twin-Star Style Selections 42-inch-wide, 5,200 BTU, Black Infrared Quartz Electric Fireplace with model No. 42HF295FGT manufactured before January 2023 and identified with the following date codes: 0521; 0322; 0422; 0522; 0622; 0722; and 0822.

“Twin-Star International, Inc.,” and the model number are printed on the top panel of the fireplaces. The date code and serial number are printed on the back panel.

The fireplaces, manufactured in Vietnam, were sold at Lowe’s stores nationwide and online at Lowes.com from September 2021, through January 2023, for about $200.

Kilitch Healthcare India Limited is recalling various eye drop products with expiration dates ranging from November 2023, to September 2025, a list of which may be found here.

Food and Drug Administration investigators found insanitary conditions at the place of manufacture, posing a potential risk of eye infections or related harm.

Kilitch Healthcare's action follows FDA warnings regarding the the eye drops. FDA is warning consumers not to purchase and to immediately stop using 26 over-the-counter eye drop products due to the potential risk of eye infections that could result in partial vision loss or blindness.

The recalled products were sold nationwide by retailers including CVS, Rite Aid and Walmart.

Mercedes-Benz USA (MBUSA) is recalling 42,742 of the following vehicles:

The vehicles may not be equipped with an inspection gauge to check the wear of the rear brake pads.

Mazda North American Operations is recalling 4,252 model year 2024 CX-90 plug-in hybrid electric vehicles (PHEVs).

Because of a software error, the engine and electric motor may shut down unexpectedly, which can result in a loss of drive power.

Dealers will update the powertrain control module and engine control module software free of charge.

IKEA Supply of Switzerland is expanding its earlier recall of IKEA LETTAN Mirrors to include about 26,000 sold in the U.S. and Canada. 

The plastic fittings that attach the mirror to the wall can break, causing the mirror to fall, posing a laceration hazard.

The firm has received ten incident reports globally in addition to the 55 reported in the previous recall, and one report of the plastic fittings coming loose and the mirror falling off the wall in the U.S. in addition to the one in the previous recall.

No injuries have been reported.

This recall involves all sizes of LETTAN flat mirrors which are frameless, about 38 inches high and sold in 23, 31, 39 and 47 inches wide.

LETTAN mirrors affected by this recall for repair now include: all LETTAN mirrors with a date stamp before and including 2105 (YYWW), and LETTAN mirrors with supplier number 21944 and a date stamp before and including 2325 (YYWW).

The date stamp is in (YYWW) format where the first two digits represent the year, and the last two digits represent the week of manufacture, and is located on the back of the mirror.

The mirrors, manufactured in China, were sold at IKEA stores nationwide and online at IKEA.com from December 2019, through June 2023, for between $23 and $56.

What to do

Consumers should immediately stop using the recalled mirrors and contact IKEA to order a free set of replacement wall fittings and instructions.

Alternatively, customers may return the mirror to any IKEA store for a full refund. Proof of purchase (receipt) is not required to receive a full refund.

The garments fail to meet the flammability standards for children’s sleepwear, posing a risk of burn injuries.

This recall involves four styles of iMOONZZZ-branded children’s nightgowns sold in sizes 3-4T, 5-6 years, 6-7 years, 7-8 years, 8-9 years, and 10-12 years.

“iMOONZZZ,” “Made in China,” the size, the fiber content and the washing instructions are printed on a sewn-in, side-seam label.

Yoocaa Direct of China is recalling about 4,140 Yoocaa baby loungers.

The loungers fail to meet the safety requirements of CPSC’s Infant Sleep Products Rule, creating an unsafe sleeping environment for and posing a suffocation risk and fall and entrapment hazards.

The failures include not having a stand and not meeting the requirements for side height and fabric-sided openings.

The product’s sides are shorter than the minimum side height limit; the sleeping pad’s thickness exceeds the maximum limit; and an infant could fall out of an enclosed opening at the foot of the lounger or become trapped.

No incidents or injuries are reported.

This recall involves baby loungers made of a cloth cover with a foam sleeping pad and padded bumper.

They were advertised as “baby lounger” and “baby nest” and sold in different printed fabrics and colors, including animal, blue star, cloud, feather, flower, forest, leaf, pink star, polar bear and zoo.

“SLEEPING SET” is printed on a tag sewn on the interior of the cover.

The recall applies to products purchased on or after June 23, 2022; consumers should review their Amazon purchase history to determine their purchase date.

Consumers who still have the packaging may also review the packaging for a label that states the date of manufacture, marked in the format “LOT: YYYYMMDD.”

This recall applies to products manufactured with “LOT” markings on the packaging of 20220623 (representing June 23, 2022) or later dates.

The baby loungers, manufactured in China, were sold exclusively online at Amazon.com from January 2021, through May 2023, for between $30 and $55.

Because of a software error, the rearview, front, and/or side camera images may not appear on the display.

Kia America is recalling 6,262 model year 2015-2019 Soul EVs, expanding and replacing a recall issued in July 2020.

The parking mechanism may be damaged, which can result in unintended vehicle movement when the gear shift lever is in the Park 'P' position and the parking brake is not applied while the vehicle is powered on.

Unintended vehicle movement can increase the risk of a crash.

Dealers will update the electronic control unit software free of charge.

Vehicles previously repaired under the earlier recall will need to have the new remedy completed
