    Soft Play recalls Sky Wheels

    The Sky Wheels can become detached from the overhead rail on the play set

    Soft Play of Huntersville, N.C., is recalling about 3,500 Sky Wheels.

    The Sky Wheels can become detached from the overhead rail on the play set, posing a fall hazard and risk of injury to children.

    The firm has received 15 reports of the Sky Wheel detaching from the rail of the play set, including three reported injuries involving lacerations to the scalp.

    This recall involves Sky Wheels, an individual active play component manufactured for and sold to businesses. They can be incorporated into a larger indoor soft-contained play system.

    The Sky Wheel is comprised of a 14-inch diameter 4-spoke metal wheel attached to a downward facing milled and threaded shaft which is supported from an overhead bar within the play system framing structure.

    The Sky Wheel, manufactured in the U.S., was sold to McDonalds, YMCA, Lifetime Fitness, Burger King, Crunch Fitness, LTF Club Organizations and other fitness and recreational play areas for use by children from June 2009, through July 2022, for about $624.

    What to do

    Businesses should contact Soft Play to schedule a free inspection. If needed, a repair will be made.

    Soft Play has contacted all known businesses/operators directly. Businesses have been advised to communicate to customers to not use the Sky Wheel until it is inspected and -- if necessary -- repaired.

    Rich Products of Buffalo, N.Y., is recalling 15 cases of Member’s Mark Breaded Mozzarella Sticks.

    The products may contain egg and soy, allergens not declared on the label.

    No illnesses have been reported to date.

    The recalled product, sold in a cardboard box package with a net weight of 5 lbs, Best If Used By Date of 12/28/2024 stamped on the side panel and a UPC number 078742226880 on the back of the package, was sold at Sam’s Club retail stores in Kentucky, Ohio, Pennsylvania, Virginia and West Virginia.

    What to do

    Customers with an allergy or sensitivity to egg and soy who purchased the recalled product should not consume it, but return them to the place of purchase for a full refund.

    Mazda recalls 227,000 Mazda3s and CX-3s

    The rearview image may not display properly

    Mazda North American Operations is recalling 227,335 model year 2014-2018 Mazda3s and model year2016-2021 CX-3s.

    The rearview camera image may not display properly by showing a flickering or distorted image.

    An improper rearview camera image can reduce the driver's rear view, increasing the risk of a crash or injury.

    What to do

    Dealers will inspect and replace the rearview camera as necessary. In addition, dealers will install a wiring harness fastening seal.

    All repairs will be performed free of charge.

    Owner notification letters are expected to be mailed September 12, 2023.

      Salewa USA recalls Wild Country Superlight Rocks cable wire chocks

      The cable chocks can break, posing a fall hazard

      Salewa USA is recalling about 3,700 Wild Country Superlight Rocks cable wire chocks sold in the U.S., and Canada.

      When exposed to seaside conditions, corrosion can occur and weaken the cable chocks and they can break, posing a fall hazard to climbers.

      No incidents or injuries have been reported in the U.S.

      This recall involves Wild Country brand Superlight Rocks (model number 40-RSL) and Superlight Rocks Set 1-6 (model number 40-RSLSET) single cable wire chocks used for climbing.

      Superlight Rocks were sold as a set of six and individually in six different sizes and colors including: size 1 purple, size 2 green, size 3 silver, size 4 gold, size 5 blue and size 6 red.

      The brand name, product name, and batch code are located on the swage on the chock.

      The cable wire chocks, manufactured in Taiwan, were sold at REI, MEC, and other specialty outdoor stores nationwide and online at www.wildcountry.com, www.backcountry.com, and www.campsaver.com from January 2017, through February 2023, for between $14 and $16 each and about $80 for a set of six.

      What to do

      Consumers should immediately stop using the recalled Wild Country Superlight Rocks and contact Wild Country to coordinate free shipment for a free replacement.

      Consumers may choose to have their recalled Superlight Rocks replaced immediately with the Wild Country Rocks 40-RWA or the new Superlight Rocks (available in September 2023).

      Tesla recalls 15,000 model year 2021-2023 Model S and Model X vehicles

      The seat belt may not restrain an occupant properly

      Tesla is recalling 15,869 model year 2021-2023 Model S and Model X vehicles.

      The front-row seat belts may not be connected properly to the pretensioner anchors, which can cause them to detach.

      A detached seat belt may not properly restrain an occupant during a crash, increasing the risk of injury.

      What to do

      Tesla Service will inspect the front-row seat belts and connect the seat belts to pretensioner anchors, or replace the seat belt assemblies -- as necessary -- free of charge.

      Letters to owner notifying them of the recall are expected to be mailed September 12, 2023.

      CUPKIN recalls 346,000 stainless steel children’s cups

      The cups contain excessive levels of lead

      CUPKIN of Bothell, Wash., is recalling about 346,000 double-walled stainless steel children’s cups.

      The cups contain levels of lead that exceed the federal lead content ban. Lead is toxic if ingested by young children and can cause adverse health effects.

      No incidents or illnesses are reported.

      This recall involves improperly manufactured 8-oz. and 12-oz. models of CUPKIN double-walled stainless steel children’s cups sold in pairs.

      Both sizes were sold in 12 different color combinations including a matching straw: blue and green, pink and purple, blue and gray, peach and teal, black and white, coral and yellow, green and pink, polignac and potpourri, brown and peach, rust and salmon, aqua and periwinkle, and cobalt and mint.

      "Cupkin" is printed on the front bottom of the cups.

      The cups, manufactured in China, were sold online at Amazon.com and Cupkin.com from January 2018, through March 2023, for about $20.

      What to do

      Consumers should immediately take the cups away from children and stop using them. Contact CUPKIN for a full refund.

      This recall involves Yamaha personal transportation vehicles sold in white, blue, green and orange.

      The model’s name is found on the left and right side of the vehicle and the serial number can be found on the frame under the operator’s seat.

      The personal transportation vehicles, manufactured in Japan, were sold at Yamaha Golf car dealers and distributors nationwide from August 2022, through March 2023, for between $6,200 and $9,700.

      Toyota recalls 43,000 RAV4 Primes and Lexus PHEVs

      The DC-DC converter may fail and cause a short circuit

      Toyota Motor Engineering & Manufacturing is recalling 43,442 model year 2021-2022 RAV4 Primes and model year 2022 Lexus NX450h+ plug-in hybrid electric (PHEV) vehicles.

      The electric current rectifying module contained within the DC-DC converter may have been damaged during production, which can result in an electrical short-circuit.

      A short circuit can cause the DC-DC converter to overheat, increasing the risk of a vehicle fire.

      What to do

      Owners should not charge their vehicles when the ambient air temperature is below or expected to drop below 41 degrees Fahrenheit (5 degrees Celsius).

      Dealers will replace the DC-DC converter, free of charge.

      Owner notification letters are expected to be mailed August 7, 2023.

      Fire risk leads Electrolux to recall Frigidaire gas laundry centers

      Lint may accumulate in the dryer, creating a fire hazard

      Electrolux Home Products of Charlotte, N.C., is recalling about 13,600 Frigidaire gas laundry centers.

      The felt seal on the dryer drum can be folded inward which can lead to lint accumulation, creating a fire hazard.

      The company has received 23 reports of fire and one report of a smoke inhalation injury.

      This recall involves all Frigidaire gas laundry centers with model numbers FFLG4033QW and FFLG4033QT, and serial numbers FFLG4033QWX and 4E44308482 through 4E81603380.

      The laundry centers were sold in white and titanium colors and have “FRIGIDAIRE” written below the dryer door.

      The serial number is located on a white sticker inside the dryer door.

      The laundry centers, manufactured in Mexico, were sold at Lowe’s, Home Depot, and other independent appliance stores nationwide from October 2014, through April 2018, for about $1,400.

      What to do

      Polaris Industries is recalling 22,218 model year 2020 Slingshot GTs, model year 2021-2023 Slingshot Ss & S Techs, model year 2020-2023 Slingshot Rs, & Slingshot SLs, and model year 2022-2023 Slingshot SLRs updated with Ride Command Software, version SLS7AB.M32304A.

      After shifting out of reverse, the rearview camera image may freeze and display a blank screen if shifted into reverse again.

      The loss of the rearview camera image while in reverse can reduce the driver's rear visibility, increasing the risk of a crash.

      What to do

      The remedy is currently under development.

      Notification letters are expected to be mailed to owners July 28, 2023.

      Husqvarna recalls residential snow blowers

      Husqvarna Professional Products of Charlotte, N.C., is recalling about 7,100 Residential 300 Series Snow Blowers sold in the U.S. and Canada.

      The snow blowers can unexpectedly go into drive mode and collide with a consumer, posing an injury hazard.

      The firm has received 22 reports where the auger pulley bolt loosened and the drive unexpectedly became engaged. No injuries have been reported.

      This recall involves Husqvarna-branded gas-powered residential 300 Series Snow Blowers, model numbers ST 324, ST 327 and ST 330.

      The snow blowers are orange and black with the Husqvarna logo painted on the front plastic cover. 

      The product is about 46 inches tall, 54 inches deep and 24 to 30 inches wide, depending on the model.

      The model, product and serial numbers are located on the right side of the unit behind the tire.

      The snow blowers, manufactured in the U.S., were sold at authorized Husqvarna dealers and various hardware stores nationwide and online at www.husqvarna.com from July 2022, through February 2023, for between $1,350 and $1,800.
