Recalls in September 2022

Custom Elevator of Plumsteadville, Pa., in cooperation with the Consumer Product Safety Commission (CPSC) is recalling certain of the 15,200 residential elevators manufactured by Custom Elevator since 2003.

Young children can become trapped in the space between the exterior landing (hoistway) door and the interior elevator car door or gate if there is a hazardous gap between the doors, and suffer serious injuries or death when the elevator is called to another floor.

A 7-year-old child died in a vacation home in North Carolina in July 2021, after becoming trapped in a residential elevator manufactured by Custom Elevator and installed and serviced by a third-party installation company.

In fact, CPSC is aware of 41 deaths associated with elevators between 2018 and 2021. Between 2020 through 2021, there were more than 19,000 ER-treated injuries in the U.S.

"Residential elevators pose a deadly risk to children, said CPSC Chair Alex Hoehn-Saric. "It's long past time for all homeowners to address the hazard and ensure that children cannot get trapped between elevator doors."

To address the entrapment hazard, a total of 117,100 residential elevators have been recalled in the the U.S. in the past year

Custom Elevators sold the units to elevator contactors nationwide from 2003, through August 2022, for between $10,000 and $25,000, excluding installation costs charged by third party installation companies.

What to do

Consumers should keep young children away from the recalled residential elevators and contact Custom Elevator for free space guards to eliminate any hazardous gaps.

Custom Elevator will provide consumers with instructions on how to measure for space guards to correct any hazardous gap.

Kia America is recalling 28,065 model year 2016 Sedonas and Sorentos.

The vehicles may have been manufactured with faulty automatic transmission shift lever assembly components, preventing the shift lever from locking.

This can allow the vehicle to roll away, increasing the risk of a crash and injury.

MotorBiscuit, a website offering automotive news, reviews and buying advice, reports there are nearly 150 deaths and 2,000 injuries each year due to rollaway accidents.

What to do

Dealers will inspect and replace the automatic transmission shift lever assembly components -- as necessary -- free of charge.

Owner notification letters are expected to be mailed October 7, 2022.

Russell Brands of Bowling Green, Ky., is recalling about 13,400 Spalding Momentous EZ Portable Basketball Goals sold in the U.S. and Canada.

The welds in the arm attaching the backboard to the pole can fail and cause the backboard to fall, posing an impact injury hazard.

The firm has received 26 reports of weld failures including three backboards separating from the pole.

One injury involving the backboard hitting a consumer on the shoulder/upper torso and one injury hitting a consumer on the head have been reported.

This recall involves Spalding Momentous EZ Assembly Portable Basketball Goals.

The goals have a 50 inch or 54-inch acrylic clear backboard with a visible “H-frame” providing support behind the backboard, or a 54-inch acrylic clear backboard with aluminum trim, but no H-frame.

All backboard styles are attached to a black metal pole with a black metal arm. “Spalding” is printed on the front of the black base and “6E” is molded on the back.

The numbers 1010, 1011 or 1012 are printed to the right of the CAUTION statement on the front of the backboard.

The portable basketball goals, manufactured in China, were sold at Academy and Scheels stores nationwide and online at Spalding.com and Amazon.com from May 2021, through April 2022, for between about $400 and $600.

What to do

Consumers should immediately stop using the portable basketball goals and contact Spalding to arrange for a technician to remove the original arm and install a new one on the goal system.

The brake fluid reservoir cap may not vent properly, creating a vacuum inside the brake fluid reservoir.

This could lead to a brake fluid leak resulting in a partial or total loss of brake function, posing a crash risk.

This is an expansion a recall conducted in late 2021.

What to do

Dealers will replace the brake fluid reservoir cap and update the software to provide a new warning message for low brake fluid free of charge.

Owner notification letters were expected to be mailed September 24, 2022.

Toufayan Bakery of Plant City, Fla., is recalling Publix GreenWise Animal Crackers.

The product may contain coconut, an allergen not declared on the label.

The problem was caused by a temporary breakdown in the production and packaging processes.

The recalled product, which comes in an 8-oz pouch with UPC 0-41415-12009-9 on the back and an expiration date of February 5, 2023, was sold at Publix Supermarkets in Florida, Alabama, Georgia, South Carolina, North Carolina, Virginia and Tennessee.

What to do

Customers with an allergy or severe sensitivity to tree nuts who purchased the recalled product should not consume it, but return it to their local store for a full refund

Nissan North America is recalling 5,904 model year 2017-2019 Rogue Hybrids.

The hydraulic brake booster assembly may overheat internally, resulting in a loss of power brake assist.

A loss of power brake assist can extend the distance required to stop the vehicle, increasing the risk of a crash.

In a campaign bulletin, the automaker notes that frequent braking in a short period of time, combined with high engine room temperatures the hydraulic brake booster motor can fail.

As a result, the brake warning light (with buzzer) will illuminate and the brake pedal will be hard to depress.

What to do

A remedy is under development, with owner notification letters scheduled to be sent in September 2022.

Mr. Maria of Miami Fla., is recalling about 1,400 Miffy First LED Lights.

The recalled lights can overheat during use, posing a burn hazard.

No incidents or injuries are reported.

This recall involves Miffy First Lights with a blue color label on the bottom.

The lights are about 12 inches tall and are made from silicone and have a white exterior in the shape of a rabbit. The LED light has a step dimmer and USB charger.

The model number Mr LED 11 or Mr. LED 11A is printed on the blue-colored label on the bottom of the light.

The recalled lights were produced in 2017 and 2018.

The production year is embossed in a circle on the bottom right foot of the light; a “7” on the right side of the arrow represents 2017 and an “8” on the right side of the arrow represents 2018.

Only models with a blue-colored label on the bottom of the unit are included in this recall.

The lights, manufactured in China, were sold at baby boutiques, gift shops and children’s stores nationwide, including MoMA, Clic Gallery and Albee Baby, all of New York City; Aldea Home, San Francisco; Modern Art Museum, Fort Worth, Texas; and Tabletop, Potomac, Maryland from July 2018, through September 2019, for about $100.

What to do

Consumers should immediately stop using the recalled Miffy First Lights and visit https://www.mrmaria.com/bluemodule for instructions on how to determine which LED module is being recalled.

Consumers who purchased the Miffy First LED Light at a local store, should take it to the store to have the module replaced free of charge.

Audi is recalling 28,624 model year 1998 A8s and model year 1997-1998 A4s equipped with Non-Azide Driver airbag Inflators (NADI) that do not contain phase stabilized ammonium nitrate (PSAN) propellant.

The NADI inflator may absorb moisture because of a manufacturing issue.

As a result, the inflator could explode during airbag deployment with metal fragments striking the driver or other occupants.

Additionally, an underinflated airbag may not protect the occupant properly.

In either case an occupant could be injured or killed.

What to do

Dealers will replace the driver's airbag inflator with an inflator of a different design free of charge.

Owner notification letters were sent September 21, 2021.

The vehicles in this recall were mistakenly omitted from a recall issued in 2020.

These owners have already been notified by mail and informed that the remedy is available.

Ford Motor Company has another recall in the works – the 56th this year.

This time the automaker is recalling 86,050 model year 2022-2023 Bronco Sports and model year 2022 F-150s.

The parking lights may flicker when the headlights are activated, which can reduce visibility, increasing the risk of a crash.

So far this year Ford has recalled more than 7.5 million vehicles, according to the National Highway Traffic Safety Administration, more than triple the 2 million General Motors has recalled.

"Ford’s numerous recalls and quality issues in the past year seem to be attributable to last-minute changes prior to new model launches," according to Grant Feek, CEO of Tred, an online person-to-person used car marketplace.

In addition, Feek told Consumer Affairs, "Supply chain issues, which often necessitate making last-minute changes to parts, likely also have played a role in Ford’s quality issues."

Regarding problems with the supply chain, Feek noted, "Every time a tried and tested part is unavailable, manufacturers have to find an alternate source or make a design change. Because this usually happens at the last minute (and without time for proper testing), there’s a much higher risk of problems."

Last week Ford reported supply chain problems would cost the automaker an additional $1 billion in the current quarter.

In the case of the current recall, dealers will replace or reprogram the LED Driver module, free of charge.

World Variety Produce is recalling select lots of Melissa’s Dutch Red Potatoes with Dijon Mustard Sauce.

The product contains egg, an allergen not declared on the label.

People with this allergy or a severe sensitivity to egg who consume the recalled product have a risk of a serious or life-threatening allergic reaction.

No illnesses have been reported to date.

The recalled product, which comes in a 1.1.70 lb clear overwrap tray in cardboard sleeve with UPC Code 0-45255-15221-0 and “best by” dates running through early October 2022, was sold in the produce departments of grocery stores in Arizona, California, Florida, Iowa, Idaho, Kansas, Louisiana, Mississippi, Montana, New Jersey, Oklahoma, Texas, Utah and Washington.

Customers with an  allergy or a severe sensitivity to egg who purchased the recalled product should not consume it, but discard it.

Four separate companies are recalling a total of 7,100 pool and spa drain covers.

All of the recalled drain covers fail to conform to the entrapment protection standards of the Virginia Graeme Baker Pool and Spa Safety Act (VGBA), and pose an entrapment hazard to swimmers and bathers.

No incidents or injuries have been reported regarding any of the recalled products.

Angzhili

The first of these involves 1,000 eight-inch drain covers imported by Angzhili that are sold for use in pools and in-ground spas.

The top grate is 7.23inches in diameter and the bottom mounting plate is eight inches in diameter. They are white and made of ABS plastic.

“Use this grate only with Hayward drains dna (and) frames of correct size” is printed on the outer ring of the grate.

The covers, manufactured in China, were sold exclusively at www.amazon.com from June 2021, through June 2022, for about $12.

Tesla is recalling 1,096,762 model year 2017-2022 Model 3, model year 2020-2021 Model Y, and model year 2021-2022 Model S & Model X vehicles.

The window automatic reversal system may not react correctly after detecting an obstruction.

As a result, a closing window may exert excessive force by pinching a driver or passenger before retracting, increasing the risk of injury.

Tesla will perform an over-the-air (OTA) software update of the automatic window reversal system free of charge.

This is Tesla's 14th recall of 2022, affecting more than 3.3 million vehicles

Cervélo USA of Aliso Viejo, Calif., is recalling about bout 2,000 Model Year 2022 Cervélo R5 and Caledonia-5 bicycles sold in the U.S and Canada, and 182 Cervélo ST31 LT replacement stems.

The handlebars on the bikes can slip in the stem and cause the rider to lose control, posing a fall hazard.

The firm has received 13 reports of handlebar slippage. One injury has been reported, resulting from a fall.

This recall involves model year 2022 Cervélo R5 and Caledonia-5 bicycles and Cervélo ST31 LT replacement stems.

Cervélo and R5 or Caledonia-5 are printed on bicycle’s frame. The ST31 LT replacement stems were sold only in black.

The bicycles and replacement stems, manufactured in China, were sold at Cervélo dealers nationwide from May 2021, through July 2022, for between $5,000 and $13,000 (bicycles) and about $300 (ST31 LT replacement stems).

The manufacturing process of the eMMC memory module in the infotainment system may cause the failure of the rearview camera image to display.

A rearview camera that does not display an image reduces the driver's rear view, increasing the risk of a crash.

VW has recalled more than a-half million vehicles thus far in 2022.

Valley International Cold Storage Acquisition of Harlingen, Texas, is recalling approximately 22,061 pounds of frozen Korean-Style Beef.

The recalled products are labeled as Korean-Style Beef, but contain a chicken sausage and pepper product containing milk, an allergen not declared on the label.

There have been no confirmed reports of adverse reactions.

The following item, produced on July 22, 2022, is being recalled:

• 9.25-oz. cartons labeled as “Healthy Choice POWER BOWLS Korean-Style Beef” with lot code “5246220320” and a “best if used by” date of 04-18-2023.

The recalled product, bearing establishment number “34622” on the end flap of the carton, was shipped to retail locations nationwide.

Jaguar is recalling 5,297 model year 2021-2022 F-PACE vehicles.

The rear turn signals may not illuminate properly as incorrect tail lights may have been installed.

Improperly illuminated rear turn signals can reduce vehicle visibility to other drivers and increase the risk of a crash.

Jaguar dealers will inspect and replace the tail lights -- as necessary -- free of charge.

Family Dollar is recalling various over-the-counter (OTC) medical devices and pharmaceutical products that were stored and inadvertently shipped to stores outside of labeled temperature requirements.

A component inside the fuel pump may break, causing the fuel pump to fail.

Failure of the fuel pump can cause an engine to stall, increasing the risk of a crash.

Baby Trend of Fontana, Calif., is recalling about 310 Cityscape Travel Jogger Strollers, model TJ75B12A.

The parking brake on the strollers can fail, posing fall and injury hazards to children.

There are no incidents or injuries reported to date.

This recall involves Cityscape Travel System strollers sold “Moonstone” (gray) as part of a travel system including a car seat.

Baby Trend is printed on the stroller’s frame, and a label with Model TJ75B12A, 1-800-328-7363, CITYSCAPE JOGGER TRAVEL SYSTEM, Baby Trend, Inc. 13048 Valley Blvd, Fontana, CA 92335, and the manufacture date and lot information is located on the inside of the side stroller frame.

The recalled lot number is 111444 0122, and the date of manufacture of the recalled strollers is 10 28 2020.

The strollers, manufactured in China, were sold online at Amazon.com and Babytrend.com from January 2021, through May 2021, for about $175.

What to do

Consumers should stop using the recalled strollers immediately.

Baby Trend and Amazon are contacting all purchasers directly with instructions for how to return the strollers at no cost in exchange for either a $175 e-voucher for use at Baby Trend’s online store or a full refund.
