RH, US of Corte Madera, Calif., is recalling about 3,400 children’s chairs and stools sold in the U.S., and Canada.

Paint on the chairs and stools contains levels of lead that exceed the federal lead paint ban, and can cause adverse health effects if ingested by young children.

No incidents or injuries are reported.

This recall involves Vintage Steel Play Chairs and Stools sold in distressed red under the following model names: “Play Chair”, “Tall Play Chair”, “Play Stool”, “Tall Play Stool” and “Step Stool.”

The chairs have a curved, tubular back frame and a single vertical slat with an embossed seat.

The stools have a curved frame and an embossed seat with a hand grip. “

Made in India” and one of the following SKU numbers are printed on a green label located under the chair seat or stool step: 103480DRED (play chair); 103481DRED (tall play chair); 103521DRED (tall play stool); 103522DRED (play stool); and 111081DRED (step stool).

The lock nut that connects the arm rest to the chair can loosen, posing fall and injury hazards.

This recall involves West Marine Deck chairs and Comfort Plus Deck Crew chairs.

The folding deck chair has metal legs, light-colored wood armrests and blue back-pack style straps. “West Marine” is printed on the side of the wood armrest. The corners of each leg have white plastic feet. The seat and back are covered in light blue and navy blue nylon fabric. The back can be adjusted to various angles.

This recall involves Skip Hop’s Tuo convertible high chairs with charcoal gray or silver/white with clouds fabric.

The chairs can break at the seat or back welded support rod connecting the back legs of the chair, posing a fall hazard.

The firm has received three reports of the chairs breaking, including one minor injury reported.

This recall involves the Everyday Yoga Backless Yoga Chairs and Everyday Yoga Tall Backless Yoga Chairs.

Both are metal folding chairs primarily used for restorative yoga, with a welded support rod on the rear legs (none in the front) and no back support.

The Tall Backless Yoga Chair, item number 8162640, is about 44 inches high and 19 inches wide. The Backless Yoga Chair, item number 8132666, is about 37 inches high and 18 inches wide.

The chairs were sold in the following colors: aqua, black, white, plum, and peacock.

The chairs were also sold under the brand name Sporti Studio and have the brand’s sticker on the back of the seat.

NEMO Equipment of Dover, N.H., is recalling about 7,500 Stargaze recliner chairs. Some 15,500 were recalled in February 2018.

The plastic joint supports attached to the legs of the chairs can break, posing a fall hazard.

The firm has received 14 reports of the joint supports breaking. No injuries have been reported.

This recall involves the Stargaze Recliner, Stargaze Recliner Low, and Stargaze Recliner Luxury lifestyle camping chairs.

The portable swinging and reclining outdoor chairs have an aluminum frame and black and gray monofilament mesh seat, and weigh between five and seven pounds. They were sold in four colors: birch leaf green, graphite, verdigris (teal) and Sedona (red).

They come in a black, padded carrying case, and are used as a portable seat for camping and outdoor activities. The model name is printed on the pocket on the inside right side of the seat. The NEMO name and logo is attached to the back of the seat as a stitched logo.

The three manufacturing lots included in this recall are marked with November 2017, December 2017, March 2018 or May 2018 date codes. The date code can be found on the plastic hub located on the inside edge of the leg strut.

The date code is in a clock format: The numbers around the circle correspond to the 12 months of the year, the arrow points to the month of manufacture and the numbers on either side of the arrow represent the last two digits of the year.

This recall involves Human Touch’s Perfect Chair, model PC-610 with serial numbers between B613315034 and B614215154. The power reclining chair was sold in a walnut, dark walnut or chestnut wood finish base with a leather pad set that came in 33 different colors.

The chair measures about 43 inches long, 31 inches wide and 47 inches high and has a head pillow at the top and a joystick controller on the left armrest. The chairs can collapse unexpectedly, posing a fall and injury hazard.

The company has received 15 reports of injuries from collapsing chairs. Injuries included back and tailbone injuries, one report of a fractured finger, three reports of stitches to fingers and reports of cut, bruised or swollen fingers.

This recall involves T.J. Maxx and Marshalls foldable weatherproof lounge chairs. The chairs are made of an acacia wood frame and striped fabric in two styles: a natural oiled wood frame with red and white stripe fabric or a white gloss frame with blue and white stripe fabric.

The chairs measure about 30 inches high by 42 inches long when unfolded. The style number is printed on the hang tag attached to the chair. “MADE IN VIETNAM” is printed on a label on the bottom of the chair frames.

This recall involves Noble House’s Louigi and Tate counter stools with solid wood legs and a metallic kick pate.

The Louigi black leather counter stool has a backrest and measures about 18 inches wide by 26 inches deep by 41 inches tall. The backless Tate counter stools were sold in white or brown and measure about 16.5 inches wide by 16.5 inches deep by 27 inches tall.

The seat plate weld can break, posing a fall hazard.

The company has received eight reports of the seat plate weld cracking or breaking. No injuries have been reported.

This recall involves Crawley II executive chairs. The black, high-back, adjustable chairs have a black base with five wheels.

The affected chairs have Office Depot SKU number 493822 or OfficeMax item number 23324118 and “US REG. No. PA 25917(CN)” printed on a label located on the underside of the seat cushion.

The recalled bean bag chairs have two zippers, including an outer zipper that does not have a pull tab and another zipper directly underneath that zipper.  Although the outer zipper on the bean bag chair does not have a pull tab, children can open it. Once they have opened the outer zipper, they can open the inner zipper which contains the foam beads and crawl inside.

Children can suffocate or choke on these foam beads. The voluntary standard requires non-refillable bean bag chairs to have closed and permanently disabled zippers. Ace met the voluntary standard’s requirement for a warning label.

The two deaths involved a 13-year old boy from McKinney, Texas died and a 3-year-old girl from Lexington, Ky. who suffocated from lack of air and inhaling the chair’s foam beads. Both children were found inside the chairs.

The recalled chairs with zippers that can be opened were sold in a variety of sizes, shapes, colors and fabrics. They include round or L-shaped, vinyl or fabric, and are filled with polystyrene foam beads.

They were sold in a variety of colors, including purple, violet, blue, red, pink, yellow, Kelly green, black, port, navy, lime, royal blue, turquoise, tangerine and multi-color.  The round bean bag chairs were sold in three sizes, 30, 32 and 40 inches in diameter. The L-shaped bean bag chair measures 18 inches wide by 30 inches deep by 30 inches high. “ACE BAYOU CORP” is printed on a tag sewn into the bean bag chair’s cover seam. They were made in China.

Office Depot will pay a $3.4 million civil penalty to settle Consumer Product Safety Commission (CPSC) staff’s charges that the company knowingly failed to report -- as required by federal law -- defects and an unreasonable risk of serious injury concerning the Quantum and the Gibson models of office chairs.

The seatbacks of both models failed, resulting in multiple back and other injuries.

Failure to report

Federal law requires parties like Office Depot to report to CPSC immediately (within 24 hours) about a consumer product containing a defect that could create a substantial product hazard or presenting a risk of serious injury.

Office Depot received dozens of reports of seatback failures and related injuries involving both the Quantum chairs and the Gibson chairs. It never reported the Quantum chair hazard to CPSC, and reported the Gibson chair hazard only after receiving a request from staff.

By the time the Quantum and Gibson chairs were recalled, Office Depot had received 33 and 153 reports – respectively -- of seatback detachment from the chairs, and 14 and 25 reports – respectively -- of injuries, some of which required medical attention.

Office Depot sold about 150,000 Quantum chairs nationwide between May 2006, and August 2009, for about $350 each, and about 1.4 million Gibson chairs nationwide between 2003, and 2012, for about $40 each.

Settlement terms

In addition to paying the $3.4 million civil penalty, Office Depot has agreed that the company has, and shall maintain, a compliance program designed to ensure compliance with the Consumer Product Safety Act and a related system of internal controls and procedures. The program must include written standards and policies designed to convey information obtained from sources such as complaints, parts requests, and incident reports to personnel responsible for CPSC compliance.

The compliance program also must address:

• confidential employee reporting of compliance concerns to a senior manager;
• effective communication of compliance policies and procedures, including training;
• senior management responsibility for, and general board oversight of, compliance; and
• requirements for record retention.

Office Depot neither admits nor denies CPSC staff’s charges.

Cost Plus Management Services of Oakland, Calif., is recalling about 125,000 twist swivel stools.

The weld joint attaching the stool seat to the center post can break, posing a fall hazard.

The firm has received 12 reports of the stool’s joint breaking. No injuries have been reported.

This recall involves twist swivel stools with a light brown-finish wood seat and base with four black metal legs. The recalled power reclining furniture involves three series of the PowerReclineXRw: P16, 32P and 39P. The P16 series is a single-seat recliner; the 32P series is a loveseat recliner; and the 39P series is a loveseat with a middle console.

The furniture’s back and leg rest are raised and lowered electronically by pressing buttons built into the furniture’s side or on an optional attached controller. All models are upholstered in a variety of fabrics or leather products.

The light blue seat of the chair has a navy blue fabric seat cushion and seat back with red buttons. “FLY BOY” is printed on the top frame of the seat back with a printed picture of a red airplane.

This recall includes Moon model 3-in-1 high chairs which adjust to a newborn, high and junior chair with a design consisting of a base with two feet supporting a single post that holds up the seat in a clear shell. The shell has a white inner seat with a removable seat pad in white, camel or black. The high chair measures about 3 feet tall when in its highest position.

There is a “Mima” logo where the metal post attaches to the base to the seat. There is also a “Moon” logo on the inside of the feet that form the base of the high chair. The serial and model numbers are located on a sticker on the inside of one of the two feet that form the base of the high chair.

This recall involves GCI Outdoor BleacherBack stadium seats made of a powder-coated steel frame, a stitched fabric backrest and a fabric-covered padded bottom. The backrest has a mesh pocket on the back side.

The seat bottom has a strap that runs from one side through two D rings on the upper part of the backrest and back down to the other side. The strap has a yellow buckle. The backrest and seat bottoms were sold in the colors black, green, maroon, navy blue, red and royal blue.

When open, the seats measure 16 inches wide x 12 inches deep x 17.5 inches high. The backrest folds forward to a height of 4 inches.

The chair can tip or recline too quickly, posing a fall hazard.

The company has received 5 reports of falls occurring in Ross Stores, 3 of which resulted in minor injuries.

This recall involves 3 colors of 4Seasons folding lounge chairs, which are made of steel tube frames and textilene fabric with attached head cushion in orange, turquoise or brown. The chairs are approximately 3 feet, 6 inches tall.

The affected product SKU numbers are 400085136029 (bronze fabric and frame), 400085136036 (turquoise fabric and a white frame), and 400085136043 (orange fabric and a white frame). SKU numbers can be found on the retailer’s hang tag attached to one of the arms of the chair.

There’s a separate manufacturer’s tag sewn on the back of the chair that says “Weight limit – 250 lbs.” and “Made in China.”

The lounge chairs, manufactured in China, were sold exclusively at Ross Stores nationwide between April and June 2013, for about $40.

Consumers should stop using the chairs and destroy them by carefully cutting out a 5 inch by 5 inch piece of the fabric on the seat of the chair, rendering the chairs unusable, and removing the manufacturer’s tag from the back of the chair. 4Seasons will provide a full refund to all consumers who present the cut-out fabric and tag.

Dimension Industries Company of Taiwan is recalling about 18,000 sling fabric cafe sets.

The chairs can break due to a missing metal washer plate, posing a fall hazard to consumers.

No incidents or injuries have been reported.

The three-piece sling fabric cafe sets include two aluminum swivel chairs with medium brown-colored stretched fabric seats and backs, a round aluminum base and aluminum arm rests, and a rectangular table with a tabletop made from porcelain tile with a natural stone look.

The chairs are about 44 inches high x 26 inches long x 30 inches wide. The table is about 29 inches high x 30 inches long x 26 inches wide. Costco Item #966710 is printed on the hangtag, label and on the packaging.

The cafe sets, manufactured in China, were sold exclusively at Costco Wholesale stores nationwide from December 2013, through May 2014, for about $400.

Ross Procurement of Dublin, Calif., is recalling about 840 metal bistro chairs

The chair’s seat can fail to attach to the frame when the chair is unfolded, causing the chair to collapse. This poses a fall hazard to consumers.

The company has received three reports of minor injuries from the chairs collapsing, including a back strain and minor bruises.

This recall involves orange metal folding bistro chairs with perforations on the seat. The chairs measure about 36 inches tall by 15 inches wide by 16 inches deep. SKU 400101018858 is found on the product hangtag.

The chairs, manufactured in China, were sold exclusively at Ross stores nationwide from February 2014, through April 2014, for about $30.

Consumers should immediately stop using the recalled chairs with seats that do not attach to the frame and return them to any Ross store for a full refund.  

Ace Bayou of New Orleans, La., is recalling about 2.2 million bean bag chairs following the deaths of two children.

The zippers on the bean bag chairs can be opened and children can then crawl inside, get trapped and suffocate or choke on the bean bag chair’s foam beads. The voluntary standard requires non-refillable bean bag chairs to have closed and permanently disabled zippers.

A 13-year old boy from McKinney, Texas, died and a 3-year-old girl from Lexington, Ky., died after suffocating from lack of air and inhaling the chair’s foam beads. Both children were found inside the chairs.

The recalled bean bag chairs have two zippers that can be unzipped and opened, including one of the exterior cover and other directly underneath that zipper.

The recalled chairs with zippers that open were sold in a variety of sizes, shapes, colors and fabrics. They include round or L-shaped, vinyl or fabric, and are filled with polystyrene foam beads. They were sold in a variety of colors, including purple, violet, blue, red, pink, yellow, Kelly green, black, port, navy, lime, royal blue, turquoise, tangerine and multi-color.

The round bean bag chairs were sold in three sizes, 30, 32 and 40 inches in diameter. The L-shaped bean bag chair measures 18 inches wide by 30 inches deep by 30 inches high. “ACE BAYOU CORP” is printed on a tag sewn into the bean bag chair’s cover seam.

The recalled bean bag chairs, manufactured in China, were sold at Bon-Ton, Meijer, Pamida, School Specialty, Wayfair and Walmart stores and online at Amazon.com, Meijer.com and Walmart.com before July 2013 for between $30 and $100.

Consumers should check their bean bag chairs for any zippers that can open, take those that can open away from children immediately and contact Ace Bayou for a free repair kit to permanently disable the zippers so that they cannot be opened.

There are no reports of any incidents or injuries.

The recall includes Dream On Me Dinah high chairs made with a steel, powder coated tubing open-framed base with white plastic foot grips and black with white trim or red with white trim fabric seats. The chair has a white plastic tray, plastic footrest and a white fabric five point adjustable safety strap. “Dream On Me” is printed on a label attached to the front of the tray.

There is a color-coordinated black and white or red and white fabric storage bin attached to the bottom of the chair legs. The high chair can be folded for storage.

The high chairs, manufactured in China, were sold exclusively online by Walmart.com and Amazon.com from November 2012, to November 2013, for about $60.

The set includes two brown powder coated outdoor metal resin wicker chairs and a round table with aluminum top with brown resin wicker trim. The chairs measures about 23 inches wide by 23 inches deep by 32 inches high. The table measures about 24.75 inches in diameter. The chairs were sold with a hang tag with item number RT043SET.

The chairs, manufactured in China, were sold at Christmas Tree Shops, andThat!, and Christmas Tree Shops andThat! stores nationwide in March 2014 for about $60 for the entire set.

Brooks Furniture of Tazewell, Tenn., is recalling about 350 Brooks Furniture glider rockers.

The spindles in the glider base can separate while rocking, posing a fall hazard to the user.

The firm has received 29 reports of the chair’s spindles separating or loosening on the base of the glider rockers. No injuries have been reported.

The recall includes two styles of Brooks Furniture glider rockers: 1529P and 1529V-LM. The 1529P glider rocker has a maple wood frame with blue fabric upholstery. The 1529V-LM glider rocker has a maple wood frame with beige vinyl upholstery and a locking mechanism to disengage the glider.

Both recalled chairs have the style number and manufacture date between 1/1/2011 and 12/31/2012 printed under the chair’s seat. Brooks Furniture is printed on a label attached to the seat cushions.

The rockers, manufactured in the U.S., were sold exclusively through Kaplan Early Learning Co. nationwide from October 2011, to December 2012, for about $390 to $540 depending on style.

The mounting plate weld can break and separate the seat from the base of the chair, posing a fall hazard to consumers.

The company has received about 153 reports of the seat plate weld cracking or breaking, including 25 reports of contusions, abrasions and reports of injuries to the head, neck and a fractured back and hip which required medical attention.

This recall involves Office Depot Gibson Leather Task Chairs. The black, armless, adjustable chairs have a black base with five wheels. The affected chairs have the SKU number 105479 and the registration numbers PA 27248 (CN), PA 25498 (CN), PA 25276 (HK) or CA 35181 (RC) that can be found next to the words “Made in China” which is printed on a label located on the underside of the seats.

The chairs, manufactured in China were sold exclusively at Office Depot retail stores and online from 2003 through 2012 for about $40.

The legs of the chairs can bend and break, posing a fall hazard to the user.

The company has received three reports of the chair legs bending or breaking and causing falls, including one consumer who bumped his head when falling.

The recall includes the chairs sold with the Fairview 7-piece woven patio dining set. The set includes an 80-inch long by 42-inch wide by 29-inch high rectangular table with a tan with white border porcelain tile table top and black metal framed base and six chairs.

The chairs have a black metal frame with brown woven wicker seats and seatbacks, and measure 24 inches wide by 27 inches deep by 40inches high. “7 piece Woven Dining Set” and “Imported by Costco Wholesale” is printed on the product packaging. The item number is printed on the instructions sold with the set and reads “ITM./ART. 966630.”

The sets, manufactured in China, were sold exclusively at Costco Wholesale stores nationwide from January 2014, to March 2014, for about $1,300.

This recall involves black wooden accent chairs with a shaped wood top rail and center back splat. The chairs have a black and white chevron print seat cushion. Item number 5218300 and PO number 9099294 are printed on the chair’s hang tag and a tag affixed to the underside of the chair.

The base of the chair can break, posing a fall hazard. The company has received 41 reports of the chairs’ base breaking, including three from consumers who reported falling out of the chairs with one receiving a scratched leg and bump to the back of the head.

The office chairs with unpadded arms have a five-wheel base and come in two colors: gray and burgundy. A tag on the bottom of the seat cushion has the SKU and item numbers printed on it: the gray SKU is 924204 with item number 21084; and the burgundy SKU is 924203 with item number 21083.

The recalled Anywhere Lounger bean bag chairs are 100% polyester or 100% cotton and measure about 51 inches in height with a 43 inch wide base. This recall includes Eat model high chairs in red, black, green, purple, navy, orange, and brown. The model number BH2104 is located on a label on the back of the high chair. The word “babyhome” is printed on one leg of the chair and the word “eat” is printed on the opposite leg of the chair.

The high chairs have a nylon fabric seat with a plastic tray and metal frame and measure about 36 inches high and 24 inches wide. There is a printed white “babyhome” logo shaped like a backwards letter “h” on the seat back The recalled high chairs have lot numbers: BH00301/01-2012, BH00303/07-2012, BH00304/09-2012 and BH00304/09-2012.

The lot numbers are located on a sticker affixed to the bottom of the footrest.

The front openings between the tray and seat bottom and on the side openings of the high chair between the armrest and seat bottom can allow a child's body to pass through and become entrapped at the neck. This poses a strangulation hazard to young children if the belt is not engaged. In addition, exposed springs between the seat and armrest on both sides of the high chair can create a pinch hazard to the child.

There are no reports of incidents or injuries.

This recall involves all Bistro high chairs with model/style number "255" printed on a tag attached to the back of the seat. The high chair was sold in blue or pink. The fabric on the seat has a polka-dot design and "Dream On Me" is printed on a label attached to the front of the white tray. The recalled high chair was manufactured in July 2011 and has a date code printed on a label on the back of the chair.

Trend Lab of Burnsville, MN, is recalling about 16,850 children's upholstered toddler chairs because staples in the binding on the back of the chair may come loose, posing a laceration or choking hazard if swallowed.

The recalled children's upholstered toddler chairs in Club style and Mod style are 24" wide by 18" tall by 17" deep.

The Club style chairs come in three fabrics and colors:

1. suede in brown, pink, avocado green, red and turquoise blue;
2. velour in blue, green, pink and red; and
3. print fabric in chocolate dot on pink or blue.

The Mod style chairs come in dark pink or zebra printed velour in chocolate on blue or pink; or on printed fabric of chocolate stripes on pink or blue.

This recall involves three-piece patio sets sold in a white, cut-out rose pattern. The set consists of a table and two chairs made of cast iron and aluminum. The table is about 26 inches high and measures about 24 inches in diameter. Each chair is about 33 inches high with an oval-shaped seat that measures about 19 inches long and about 16 inches wide. L G Sourcing, Inc., of North Wilkesboro, NC, is recalling about 16,400 Patio Bistro Sets sold at Lowe's stores nationwide from August 2011 through February 2012 for about $98 for the set. 

When the chair support bar is not fully engaged, the chair poses a fall hazard to a consumer who sits in the partially engaged chair.

There have been 13 reports of consumers who fell from partially opened chairs, resulting in reports of back injuries, contusions and scrapes. 

The recall involves folding chairs sold with three-piece patio bistro sets. Each set contains two folding chairs and a folding table. The chairs have a black steel frame and dark stained wooden seats and backs. 

Model #S658-01, item # 0355053 and "Garden Treasures" is printed on the cover of the instruction manual sold with the patio bistro sets. 

Consumers should stop using the recalled chairs immediately and contact Midas Lin, the manufacturer, for a new set of warning labels and supplemental instructions for the chairs. Do not return the product to the store.

Downeast Concepts Inc., doing business as Backyard and Beyond, of Yarmouth, ME, is recalling about 15,400 children’s folding beach chairs. The recalled chairs have exposed, sharp metal rivets, posing a laceration hazard. The company says it is aware of one injury -- a 21-month-old girl who fell on the chair's metal rivets and cut her forehead and had to have stitches. 

The recalled children's folding beach chairs, which were made in China, have white aluminum tube frames and pink, yellow, blue or purple fabric seats and chair backs with fish, palm trees or mermaid decorations. The chairs measure 13 inches wide by 18 inches high by 20 inches deep. 

They were sold at Home Goods and other stores nationwide from June 2011 through June 2012 for between $13 and $25. 

Consumers should stop using the recalled beach chairs immediately and return them to Downeast Concepts for a full refund.

Artsana USA Inc., of Lancaster, PA, is recalling  about 455,000 Chicco Polly high chairs in the United States and 30,690 in Canada. 

Children can fall on or against the pegs on the rear legs of the high chair, resulting in a bruising or laceration injury. The firm is aware of 21 reports of incidents in which a child fell against the peg and received injuries -- including four laceration injuries requiring medical closure (stitches, tape or glue) and one scratched cornea. 

This recall involves a range of Chicco Polly high chairs with pegs on the back legs intended for tray storage. The high chairs have a folding metal frame for storage and a reclining seat. The recalled high chairs can be identified by the model number and date code printed on a label on the underside of the seat, close to the footrest. 

The date code is in the format DDMMYYYY or YYYY-MM-DD. High chairs included in this recall were manufactured prior to October 13, 2010 and have one of the following model numbers on the label:

Bluestem Brands, Inc., of Eden Prairie, Minn., is recalling about 400 recliners. The surface paint on the legs of the recliner contains excessive levels of lead which is prohibited under federal law.

The recalled adult-sized recliner chairs have wooden legs. The chairs include a reclining function and foot rest extension. The fabric is jacquard in a paisley design. The phrases: "Manufactured By Xiamen Xinzhjheng Foam Products Co., LTD Xiang-Bei Industrial Area, Xiang' An District, Xiamen" and "Made in China" are printed on a label located on the underside of the recliner chair.

The chairs were sold through Fingerhut catalogs, Fingerhut.com and Gettington.com from July 2011 through January 2012. They were made in China.

West Marine Products is recalling about 1,200 Folding Deck Chairs. The chair cannot support the stated weight capacity. This poses a collapse hazard to consumers.

West Marine is aware of six incidents of the chair collapsing. No injuries were reported.

The recalled item is a West Marine Deluxe Deck Chair. It is a folding deck chair with metal legs and light-colored wood armrests. It has plastic feet on the corners of each leg. The seat and back are covered with blue and gray nylon fabric. The back can be adjusted to various angles. Model/SKU number “13110341” is printed on a coated paper tag attached to the chair frame.

The chairs were sold exclusively at: West Marine stores nationwide, online at westmarine.com and in the West Marine catalog from February 2012 to May 2012 for about $70. They were made in China.

Evenflo is recalling about 35,00 convertible high chairs. The activity tray on the high chair can unexpectedly detach and allow an unrestrained child to fall, posing a risk of injury to the child.

Evenflo has received 18 reports of trays that detached, including eight reports of children who fell from the high chair and sustained bumps and bruises.

This recall involves Evenflo high chairs that convert from a high chair to toddler-size table and chair. The convertible high chair can be identified by the model names and numbers listed below. Model numbers are located on a label on the lower portion of one of the high chair's legs.

Toys "R" Us and Walmart stores nationwide and online at Walmart.com and Wayfair.com between December 2011 and June 2012 for about $40. They were made in China.

About 200 Stoneberry kitchen table sets are being recalled. The chairs can collapse during normal use, causing a fall hazard.

This recall involves three-piece oak table and chair sets. The table is square-shaped with four straight, rectangular legs. The chairs have a white padded cushion seat and a slatted backrest. Both the table and the wooden parts of the chairs are a light pine color. The set has the style number 404285 however this number does not appear anywhere on the set.

The kitchen table sets were sold through the Stoneberry catalog and website from January 2012 and March 2012 for about $160. They were made in China.

Raynor Marketing is recalling about 150,000 Quantum desk chairs sold exclusively at Office Depot. The bolts attaching the seatback on the recalled chairs can loosen and detach, posing a fall and injury hazard to consumers.

Raynor has received reports of 33 seatback detachments and 14 injuries involving bumps and bruises.

The recall involves the Quantum Realspace PRO 9000 Series Mid-Back Multifunction Mesh Chair SKU # 510830 and the Quantum Realspace PRO 9000 Series Mesh Chair with Headrest SKU # 690690. The Realspace PRO Mesh Guest Chair is not involved in this recall.

The chairs were sold at Office Depot stores nationwide and on the Web at www.OfficeDepot.com from May 2006 through August 2009. The mid-back chairs sold for about $300 and the chair with headrest for about $350. They were made in China.

Ross Stores has received one report of a consumer who suffered shoulder pain after her chair collapsed.

The folding wooden patio chairs are made of eucalyptus. SKU 400037791757 and manufacturer style number KTOC-1638-ROS are printed on a hangtag attached to the chair.

The chairs, made in Vietnam, were sold at Ross Stores nationwide in February 2009 for about $50.

Fisher-Price is recalling about 24,000 3-in-1 high chairs. The seat can fall backwards from high chair frame if the booster seat release is unlatched while the child is in the product. Also, the seat back can detach if not fully snapped in place, posing a fall hazard and risk of serious injury to young children.

The firm has received one report of a seat back detaching and child falling out, resulting in a skull fracture.

This recall involves the 3-in-1 High Chair to Booster, which converts from a high chair to a toddler booster seat. It includes a removable tray, height adjustment and folds for storage. The product number (P5369) is printed on the side of the seat, on a label on the seat pad, and on the products packaging.

The high chairs, made in Mexico, were sold exclusively at: Target stores nationwide from December 2008 through March 2009 for about $100.

Teknion is recalling about 60,000 Contessa office chairs. The bolts holding the seat to the frame can loosen and separate, posing a fall hazard to consumers.

The company has received two reports of separation of the chairs seat from the frame. No injuries have been reported.

This recall involves all Contessa chairs shipped prior to June 30, 2008. Each Contessa chair has a label indicating the date of shipment which is located on the bottom of the chair seat.

The chairs, made in Japan, were sold by authorized Teknion dealers nationwide from January 2004 through June 2008 for between $1,470 and $1,625.

Consumers should inspect the chair by checking the four bolts that attach the seat bottom to the chair frame. If any of the four bolts are loose or missing, consumers should stop using the chair immediately and contact a Teknion dealer for a free repair.

The chair can break when in the reclined position, posing a fall hazard to consumers.

L G Sourcing, Inc., the importer, has received seven reports of the chairs breaking when in the reclined position, resulting in minor injuries such as bruising to the back, hips, shoulders, arms and a torn ligament.

This recall involves the Garden Treasures Cloud 9 Beyond outdoor lounge chairs. The chairs are tan in color, have a light steel frame, cloth seat, a pillow at the headrest and an adjustable sun visor.

The chairs were sold at Lowe's retail outlet stores nationwide from December 2006 through April 2007 for about $60. They were manufactured in China.

Commend Co. is recalling about 600 of its HD Designs 5-piece Dining Sets. The wooden chairs can break, posing a fall hazard to consumers.

Commend has received one report of a chair breaking. In this report, there was a fall and minor injury.

The chairs and table are sold unassembled as part of a boxed 5-piece dining set. The words HD Designs 5 Piece Dining Set and item number BFR3917HO are written on the box. The chairs and dining table are honey oak wood and the seats have beige vinyl padding.

The chairs were sold at: Fred Meyer stores in Idaho, Washington, Oregon and Alaska; Frys stores in Arizona; and Smiths Marketplace stores in Utah from April 2004 to October 2004 for about $200.

About 18,000 executive office chairs sold at Staples are being recalled. The legs on the base of the chair can break, posing a risk of injury to the user.

The recalled executive office chairs include the 795-0115 model with black leather and the 795-0228 model with black fabric. Underneath the seat cushion the name Novimex Fashion Ltd. can be found on the large label and the model numbers and date codes can be found on a smaller label. Only chairs with a date code prior to April 1, 2004, are included in this recall.

Staples stores nationwide sold the chairs from March 2004 through July 2004 for about $100.

Cosco Inc. is voluntarily recalling about one million Options 5 High Chairs. The high chairs are adjustable and have both recline and upright positions. In the recline position, the seats can separate from the frame and fall to the ground. In the upright position, the seats can slip from their set height position to the lowest position or can fall to the ground.

Additionally, some seats were sold with a metal restraint anchor that can slip through the back of the seat allowing the child to fall to the ground. Infants and toddlers can suffer head, face and bodily injuries.

The recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC).

Cosco and CPSC have received 168 reports of incidents involving the high chairs' seats or restraints, including 57 injury reports, primarily to the head and face. Two children reportedly suffered slight concussions; twelve children were monitored for head injuries; and others suffered swollen eyes, bloody noses and lips, bumps and bruises.

The Cosco Options 5 High Chairs being recalled have five options for use: high chair, infant feeding, youth chair, play chair and booster seat. The chair's seat can be used in a reclined infant position or adjusted into seven upright height positions for toddlers. It has four tray positions, a vinyl seat pad and a removable footrest. "Cosco" is on the chair's leg support bar, tray and on a sticker on the bottom of the seat. The restraint system consists of a waist/crotch belt and a plastic T-bar attached to the tray. The recalled chairs can be identified by model number 03-286 and manufacture date, which are located on the bottom of the seat.

The Options 5 High Chairs subject to this recall were manufactured from December 1, 1997 through August 11, 2000. Mass merchandise, juvenile products and major discount department stores nationwide sold the high chairs for about $40 to $50.

Cosco is offering consumers two types of repair kits, depending on the date their high chair was manufactured. To prevent seat separation, all kits will contain a redesigned handle that stays out and fits over the frame each time the seat is used in the recline position. In many incidents in the recline position, the handle was not over the frame of the high chair.

Consumers should stop using these high chairs immediately, and order a free repair kit from Cosco by calling (800) 221-6736 between 8 a.m. and 4:30 p.m. ET Monday through Friday. Consumers also may visit the company's web site to order a free repair kit and to obtain instructions for proper assembly and use. When contacting Cosco, consumers should have the chair in front of them to provide the model number and four-digit manufacture date, which are located on the bottom of the seat.
