In my opinion The Federal Gov't has a very short list as shown for worst Nrsg Homes.  This one has workers without name tags including Administrator.  Appearance is great in hallways, not so in rooms.  Aids are untrained, uncaring and annoyed when asked to attend to needs of rehab pt.

Pt. arrived from Med.facility unable to move about, placed in a bed without electricity cannot raise head. A prisoner in bed.I had to chase admin.and staff several hours to get a working bed. My pt.upon discharge got a urinary tract infection due to poor use of cleansing her and returned to a medical facility. This facility is a poor rehab and a place from Hell.There no real care given if there is not a person present to defend and protect them. As for rehab it is hit or miss. Mostly miss. A visit should be surprised and not just look at aethetics, evaluate pt. areas first.
